ABSTRACT

PIP: The authors report their experiences with 80 early vacuum aspirations (mini-interruptions) performed at the 1st Dept. of Gynecology and Obstetrics of the University Hospital in Kosice during 1980-81. The procedure was performed between days 36-42 in 15% of the women, between days 43-49 in 52.7% of the women, between days 50-56 in 27.5%, and between days 57-60 in 5% of the women. Interruption of pregnancy did not require general anesthesia or hospitalization of the patients. Complications occurred in 10% of the women, 6.25% due to residua and 2.5% to inflammation. Further experience should reduce the occurrence of complications. The authors consider the method to be advantageous and prospective. (author's modified)^ieng

ABSTRACT

PIP: As mini-abortions or vacuum aspirations (VA) are almost always considered painful procedures, effective analgesia and anesthesia have to be provided. The most effective form of anesthesia was studied. Hypnomide (ethyl-1-phenylethyl-1-H-imidazole-5-carboxydate) made by the Jansen firm was administered iv to 106 patients in doses of 0.2-0.3 mg/kg. In the first 5 cases where only plain anesthetic was given, strong spontaneous muscle contraction was observed. As the efforts to neutralize the side effects caused by diazepam proved ineffective, fentanyl was later used for the preparation of anesthesia and proved to be the most effective. An average dose of 0.05-0.0525 mg fentanyl and 10-12 mg Hypnomidate was given iv and the optimum time of operation was 70-80 seconds after administration of the preparation. Adequate verbal response was observed after an average of 10 minutes and adequate motor response and clear consciousness after an average of 13 minutes. Spontaneous muscle twitching also occurred in 7% of the cases, however, this did not interfere with the progression of the procedure. No one found the procedure painful. The combination of Hypnomidate-fentanyl was considered the most effective, as it was less traumatizing psychologically, it could be applied on an out-patient basis, and since the cessation of its effect was fast, it would not prolong the operation.^ieng
