ABSTRACT

AIM: To investigate the bidirectional influence between periodontitis and psoriasis, using the respective experimental models of ligature- and imiquimod-induced diseases on murine models. MATERIALS AND METHODS: Thirty-two C57/BL6J mice were randomly allocated to four experimental groups: control (P- Pso-), ligature-induced periodontitis (P+ Pso-), imiquimod-induced psoriasis (P- Pso+) and periodontitis and psoriasis (P+ Pso+). Samples (maxilla, dorsal skin and blood) were harvested immediately after death. Measures of periodontitis (distance between the cemento-enamel junction and alveolar bone crest [CEJ-ABC] and the number of osteoclasts) and psoriasis (epidermal thickness and infiltrate cell [/0.03mm2]) severity as well as systemic inflammation (IL-6, IL-17A, TNF-α) were collected. RESULTS: The P+ Pso+ group exhibited the most severe experimental periodontitis and psoriasis, with the highest values of CEJ-ABC, number of osteoclasts, epidermal thickness and infiltrate cells in the dorsal skin, as well as the highest blood cytokine concentration. The P+ Pso- group presented with higher cell infiltrate (/0.03mm2) compared to the control group (p <.05), while the P- Pso+ group showed substantially higher alveolar bone loss (CEJ-ABC) than the control group (p <.05). CONCLUSIONS: Experimental periodontitis may initiate and maintain psoriasiform skin inflammation and, vice versa, experimental psoriasis may contribute to the onset of periodontitis. In a combined model of the diseases, we propose a bidirectional association between periodontitis and psoriasis via systemic inflammation.

ABSTRACT

OBJECTIVES: To conduct a systematic review on the translucency of bulk-fill (BF) composite materials, evaluating the factors influencing it and its clinical implications. MATERIALS AND METHODS: A systematic search was performed on four databases (Embase, Medline/PubMed, Scopus, and Web of Science) for articles published until October 2, 2023. Search terms were: "translucency," "translucency parameter," "contrast," "contrast ratio," "bulk," "bulk-fill," "bulk fill," "composite," "resin," "resin based," "resin-based," "restoration," and "dental." In vitro studies, written in English language, investigating BF resin-based composites translucency were included. The risk of bias was performed with the RoBDEMAT tool. RESULTS: Of 590 screened articles, 13 were included in this review. They investigated 11 high-viscosity BF and 5 low viscosity ones. The limited number of the identified studies and the heterogeneity of the extracted data did not allow to conduct a meta-analysis. CONCLUSIONS: BF materials exhibit variability in translucency due to variations in experimental designs and the heterogeneous composition. CLINICAL SIGNIFICANCE: BFs may behave differently in masking the substrate or in blending capacity.

ABSTRACT

STATEMENT OF PROBLEM: The influence of computer-aided manufacturing (CAM) parameters and settings on the outcomes of milled indirect restorations is poorly understood. PURPOSE: The purpose of this scoping review was to summarize the current CAM systems, parameters, and setting changes, and their effects on different outcomes of milled indirect restorations and aspects related to their manufacture. MATERIAL AND METHODS: The protocol of this review is available online (https://osf.io/x28ps/). Studies that used at least 2 different parameters (CAM units, number of axes, digital spacers, or protocols with different rotatory instruments, grit-sizes, milling speed, or others) for milling indirect restorations were included. A structured search up to July 2023 was performed by 2 independent reviewers for articles written in English in LILACS, MEDLINE via PubMed, EMBASE, Web of Science, and Scopus. RESULTS: Of 1546 studies identified, 22 were included in the review. Discrepancies were found between the planned and actual measured cement space, with a decreasing linear relationship impacting restoration adaptation at different points. The CEREC MC XL milling machine was the most used system in the included studies, with variations in bur types, milling modes, and number of burs uses affecting internal fit and surface trueness. The results demonstrated the better adaptation of restorations made with 5-axis over 3-axis milling machines. Lithium disilicate and zirconia were the most commonly used materials, and crowns and inlays were popular designs. Marginal and internal adaptation were the primary outcomes assessed using the various techniques. CONCLUSIONS: The study presented a comprehensive exploration of CAM systems and parameters, and their influence on indirect restorations. The planned cement space was not properly reproduced by the milling. Bur characteristics can affect restoration fit and trueness. The 5-axis units seem to result in better-adapted restorations compared with 3- and 4-axis units.

ABSTRACT

PURPOSE: To investigate the long-term implant survival rate and marginal bone loss (MBL) of implants with different variables associated over an observational period of at least 20 years. MATERIALS AND METHODS: Patients with at least one implant placed prior to 2001 were recalled and visited. Data on implant macro-design, prosthetic aspects, site distribution, and patient-related factors were collected. MBL was evaluated on intraoral X-rays and peri-implant soft tissue parameters were recorded. The patients were asked to fill out a questionnaire to evaluate their satisfaction with the treatment received. Descriptive statistics indicators were estimated. Analysis of variance and analysis of covariance models were used to investigate any differences in the MBL and peri-implant probing depth (PPD) among the variables. A chi-square analysis was performed to investigate any association between different types of prosthetic implant-supported rehabilitations and survival/success outcomes. RESULTS: Forty-one (41) patients and 174 implants were included with a mean observational period of 23.3 ± 2.8 years. The implant survival and success rates were 96.5% and 83.3% respectively, while 3.5% of early failures were detected. The lowest success rates were observed for implants supporting fixed full-arch rehabilitations (71.05%) and overdenture rehabilitations (86.11%). A mean MBL of 1.81 ± 0.71 mm and a mean PPD of 3.38 ± 1.62 mm were recorded. The interaction between the collected data with MBL and PPD did not reveal any statistically significant differences between the variables (p > 0.05). A statistically significant difference was recorded when analyzing the association between the different types of prosthetic implant-supported rehabilitations and success rates (p = 0.014), with fixed and removable full-arch rehabilitations presenting with lower success rates. Answers to the questionnaire showed a generally high level of satisfaction. CONCLUSIONS: Within the limits of this retrospective study and based on the results, an implant survival rate higher than 96% was observed after a mean observational period of 23.3 ± 2.8 years. Both the implant survival rate and MBL seemed stable after a mean observational period of 23.3 ± 2.8 years. Implants supporting fixed and removable full-arch rehabilitations seemed to present lower success rates over time. Implant rehabilitation seemed to provide patients with optimal long-term outcomes in terms of functional and psychological aspects.

ABSTRACT

OBJECTIVES: To conduct a scoping review on color stability of bulk-fill compared to conventional resin-based composites exposed to colored beverages. MATERIALS AND METHODS: In vitro studies, investigating bulk-fill resin-based composites color stability submitted to artificial staining by colored beverages were included. Studies investigating color change induced by smoke and whitening treatments as well as papers not written in English language were excluded. A systematic search, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) was performed on four databases (Embase, Medline, Scopus, Web of Science) for articles published until October 01st 2022. The study selection was then performed by two authors who screened the abstracts independently and followed the JBI approach. RESULTS: Of 717 screened abstracts, 59 were selected for full-text analysis. Finally, 19 studies were included in this review. They investigated 19 different bulk-fill composites and different artificial-staining-by-liquids-protocols including coffee, red wine, tea, coke, and others. Seven papers reported higher color change in the investigated bulk fill materials than in conventional resin-based composites, while nine studies reported the opposite. SonicFill showed the highest color stability for most of the papers. CONCLUSIONS: The available evidence suggests that bulk-fill materials show variability in color stability. This behavior can be attributed to the heterogeneity of composition and staining procedures in the selected studies. CLINICAL SIGNIFICANCE: Clinicians should keep in mind that bulk-fill materials may change color when exposed to colorant drinks or food.

ABSTRACT

OBJECTIVE: To conduct a systematic review on color stability of dental resin-based composites (RBC) exposed to conventional and electronic cigarettes. MATERIALS AND METHODS: In vitro studies reporting on the color stability of RBC exposed to conventional cigarettes or to e-cigarettes: both Tobacco Heating Systems (THS) and Electronic Nicotine Delivery Systems (ENDS). The quality of the included studies was assessed with the QUIN tool (risk-of-bias tool for assessing in vitro studies conducted in dentistry). A systematic search, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) was performed on four (n = 4) databases (Embase, Medline, Scopus, Web of Science) for articles published until March 28th, 2022. RESULTS: Of the 365 screened articles, 13 were included in this review. All the included articles analyzed conventional cigarette smoke (CS), four analyzed Electronic Nicotine Delivery Systems (ENDS) and two Tobacco Heating Systems (THS). In terms of study design, smoke exposure time, smoke flow, type and number of cigarettes a high variability was reported. CONCLUSIONS: The available evidence suggests that CS smoke significantly affects color stability. Electronic cigarettes show less color change that seems to be easily recovered under clinical acceptability thresholds, although evidence is scarce. CLINICAL SIGNIFICANCE: Clinicians should be aware, and should therefore warn their patients, that RBCs are subjected to irreversible color change if exposed to smoke. Electronic cigarettes (both ENDS and THS) induce less color change that can be recovered with repolishing or whitening procedures.

STATEMENT OF PROBLEM: The color of monolithic zirconia restorations is obtained by presintering or postsintering coloring techniques. However, studies on the differences in surface characteristics and their influence on color stability are lacking. PURPOSE: The purpose of this in vitro study was to evaluate the influence of shading and staining techniques for a zirconia ceramic on the surface characteristics and colorimetric parameters (color difference, translucency, and whiteness index) after exposure to coffee or red wine and then polishing. MATERIAL AND METHODS: Ceramic disks (N=30; Ø10×1mm) were allocated into 3 groups: preshaded-shaded by the manufacturer (IPS e.max ZirCAD MT, shade A2); manually shaded-unshaded zirconia (IPS e.max ZirCAD MT, bleaching shade-BL) colored by the brushing technique, before sintering; stained-unshaded zirconia (IPS e.max ZirCAD MT BL) colored by the staining technique, after sintering. Spectrophotometric color assessments ensured the same initial perceived color (Vita Classical A2) for specimens included in the study (∆E00<1.77 acceptability threshold). Surface characteristics were assessed by scanning electron microscopy and atomic force microscopy. The specimens were immersed in coffee (n=5) or red wine (n=5) for 12 and 24 days and subsequently polished. The data were statistically and descriptively analyzed for color differences (∆E00), translucency parameters (TP00), and whiteness indexes for dentistry (WID), considering acceptability and perceptibility thresholds. RESULTS: The shaded groups found an irregular ceramic surface with uniformly sized zirconia crystals. The stained group found a glass-covered smoother surface. Significant alterations in color parameters (∆E00, TP00, WID) were observed with immersion in pigmenting beverages (P<.001) both after 12 days, and after 24 days. The shaded specimens had greater color alterations after immersion but benefited from the polishing procedure, which reduced color differences below an acceptable threshold in comparison with the baseline. Stained specimens had lower color alterations after immersion, but the polishing protocol was detrimental as it whitened the ceramic by subsurface exposure. CONCLUSIONS: The coloring technique influences the surface characteristics of zirconia ceramic and also the color parameters after exposure to colored beverages and polishing.

STATEMENT OF PROBLEM: Intraoral scanners (IOSs) are widely used for partial-coverage adhesive restorations, but data on their performance in such preparations with complex geometries are sparse. PURPOSE: The purpose of this in vitro study was to investigate whether the partial-coverage adhesive preparation design and finish line depth affect the trueness and precision of different IOSs. MATERIAL AND METHODS: Seven partial-coverage adhesive preparation designs (4 different onlays, 2 endocrowns, and 1 occlusal veneer) were tested on copies of the same tooth placed in a typodont mounted on a mannequin. Each preparation was scanned 10 times with 6 different IOSs (total 420 scans) under the same light conditions. Trueness and precision, defined according to the International Organization for Standardization (ISO) 5725-1 standard, were analyzed with a best-fit algorithm by superimposition. The obtained data were analyzed by a 2-way analysis of variance to examine the effects of partial-coverage adhesive preparation design, IOS, and their interactions (α=.05). RESULTS: Significant differences were found among different preparation designs and IOSs (P<.05) in terms of both trueness and precision. Significant differences were also found among mean positive and negative values (P<.05). Moreover, crosslinks observed between the preparation area and the adjacent teeth correlated with the finish line depth. CONCLUSIONS: Complex partial adhesive preparation designs affect the trueness and precision of IOSs, resulting in significant differences among them. Interproximal preparations should take into account the IOS's resolution and placing the finish line close to adjacent structures should be avoided.

ABSTRACT

INTRODUCTION: Computer simulations are stimulating increased attention in dentistry. Augmented reality superimposes a virtual scenario over an existing reality and allows interaction with it. Virtual reality (VR) simulates a fully immersive situation permitting the user to experience the full environment in real time. Haptic technology provides tactile and realistic force feedback for the user to experience the immersive situation as if they were really there. Preclinical training is important to gain familiarity with difficult surgical techniques and to implement interpersonal skills. Developing a valid assessment of surgical simulation is challenging. This paper wants to present a newly realized VR simulation in endodontic microsurgery through the developmental digital workflow, the demonstration of a haptic VR scenario and student self-assessment and self-reflection feedback. METHODS: The volumes were exported in a stereolithography format to prepare and optimize in terms of shape and shade for the VR simulation. The graphics and touchable haptic solid were created using Virteasy Editor, which allows the transformation of 3D surfaces into graphical and volumetric haptic solids depending on their material (enamel, dentine, pulp and bone). Users were asked to execute the osteotomy and root-resection preparation. The assessment criteria were determined, and the feedback statements were created by a questionnaire with fixed answers. Objective and qualitative criteria for assessing the preparation were obtained from the literature. RESULTS: This study provides proof that it is possible to provide reliable and clinically relevant qualitative feedback with a VR simulator. CONCLUSION: VR simulation offers an innovative approach with all the benefits of clinical experience. It permits you to save your own progress and review the assessment at any time.

ABSTRACT

The shaping outcomes after instrumentation with rotary and reciprocating glide path and shaping systems were evaluated through micro-computed tomography (Micro-CT). Thirty extracted maxillary first molars were selected. Mesio-buccal canals were randomized into two groups (n = 15): rotary system ProGlider and ProTaper Next X1, X2 (PG-PTN) and reciprocating system WaveOne Gold Glider and WaveOne Gold Primary (WOGG-WOG). Specimens were micro-CT scanned before, after glide path and after shaping. Increase in canal volume and surface area, percentage of removed dentin from the inner curvature, centroid shift and canal geometry variation through ratio of diameter ratios (RDR) and ratio of cross-sectional areas (RA) were measured in the apical and coronal levels and at the point of maximum curvature. The number of pecking motions needed to reach the working length (WL) was recorded. One-way ANOVA and post hoc Turkey-Kramer tests were used (p < 0.05). Post-glide path analysis revealed that in the coronal third, RDR was more favorable to PG and centroid shift was lower for WOGG in the apical third. Post-shaping analysis showed a reduced removal of dentin and a more favorable RA for PTN at point of maximum curvature. The number of pecking motions up to WL resulted in different between groups both for glide path and shaping phases. Despite a higher dentin removal for reciprocating instruments at the point of maximum curvature, both systems seemed to produce well-centered glide path and shaping outcomes. Rotary and reciprocating systems seemed able to respect the original canal anatomy.

OBJECTIVES: To evaluate three-dimensional external gap progression after chewing simulation of high translucency zirconia (HTZ) and zirconia-reinforced lithium silicate (ZLS) applied on endodontically treated teeth with different preparation designs. MATERIALS AND METHOD: Endodontically treated molars were prepared with low-retentive (adhesive overlay) and high-retentive (full crown) designs above cementum-enamel junction and restored with HTZ and ZLS. Micro-computed tomography analysis was assessed before and after chewing simulation to evaluate three-dimensionally the external gap progression. Results were statistically analyzed with two-way ANOVA and post-hoc Tukey test. RESULTS: High-retentive preparation design had a significantly inferior gap progression compared to the overlay preparation (p < 0.01); ZLS exhibited a significant inferior gap progression compared to HTZ (p < 0.01). CONCLUSIONS: High-retentive preparations restored with ZLS seem to better perform in maintaining the sealing of the external margin after cyclic fatigue. CLINICAL SIGNIFICANCE: The clinician should pay attention to the proper combination of preparation designs and ceramic material selection for an endodontically treated molar restoration. HTZ seems to perform worse than lithium silicate in terms of marginal sealing, still showing lacks in resistance to cyclic fatigue when adhesive preparations are performed.

ABSTRACT

This study aimed to investigate matrix metalloproteinase (MMP) activity in human dentin using in-situ and gelatin zymography, after at-home and in-office bleaching, related to their clinical exposure times. Dentin specimens (n = 5) were treated with 35% hydrogen peroxide (50 min per session/4 sessions), 10% carbamide peroxide (180 min/21 sessions), or no treatment. All were subjected to in-situ zymography. Dentin slices were, subsequently, obtained, covered with fluorescein-conjugated gelatin, and examined with confocal laser-scanning microscopy. The fluorescence intensity was quantified and statistically analyzed using one-way ANOVA and Bonferroni tests (α = 0.05). Furthermore, gelatin zymography was performed on protein extracts obtained from dentin powder (N = 8 teeth), treated with hydrogen peroxide or carbamide peroxide, with different exposure times (10/50 min for hydrogen peroxide; 252/1260 min for carbamide peroxide). The results of the in-situ zymography showed no statistical differences between the bleached specimens and the control group, with a medium level of gelatinolytic activity expressed in the dentin tubules. The results of gelatin zymography showed an increased expression of pro-MMP-9 in carbamide peroxide groups. The expression of pro-MMP-2 decreased in all the experimental groups. The bleaching treatments performed on the enamel of sound teeth do not influence dentinal enzymatic activity. However, when unprotected dentin tissue is bleached, matrix metalloproteinases are more expressed, particularly when carbamide peroxide is used, proportional to the exposure time.

ABSTRACT

BACKGROUND: Endodontic ledge (EL) formation is the most common complication of endodontic treatment. Although various etiological factors have been identified, canal curvature is the most significant variable correlated with EL formation. The aim of this micro-computed tomographic (micro-CT) study was to evaluate EL position in the mesial canals of the lower molars in relation to the degree of canal curvature. METHODS: Forty intact mandibular first molars with independent mesial canals with 20°-40° primary mesio-distal curvature, 10°-30° buccal-lingual canal curvature and 4 < r ≤ 8 mm main curvature radius were selected. Working length was measured with a K-File #10 and a high resolution pre-operative micro-CT analysis was performed. Ledges were created at the point of maximum canal curvature using stainless steel K-Files #30-35, alternating irrigation with 5% NaOCl and 10% EDTA. A post-operative high-resolution micro-CT analysis was then completed. Pre- and post-operative images were analyzed. The angle (α) formed between the vector passing through the geometric center of the EL and the center of the original canal lumen and the line joining the centers of the mesio-buccal and mesio-lingual canal orifices was calculated, and a descriptive statistical analysis was achieved. The α angle values were analyzed in relation to canal curvature using Kruskal-Wallis and post hoc Dunn's tests. The level of significance was set at P < 0.05. RESULTS: The α angles appeared inversely proportional to canal curvatures in the buccal-lingual and mesio-distal projections. The mean α angle was 36.4° (standard deviation 10.64; 95% confidence interval 34.1-40.9). CONCLUSION: Within the limitations of this study, endodontic ledges develop in the opposite direction to the three-dimensional canal curvature and their position is influenced by the degree of curvature. Clinically, the α angle values may be related to the recommended direction to manage endodontic ledges.

ABSTRACT

OBJECTIVES: The survival rate of indirect partial adhesive restorations on vital versus endodontically treated teeth is still controversial. The hypothesis is that there may be a difference in the survival rate of partial adhesive restorations performed on non-vital teeth compared to vital teeth. MATERIALS AND METHODS: This systematic review was conducted following the PRISMA guidelines. The considered clinical studies investigated the outcomes of adhesive inlays, onlays, and overlays conducted over the past 40 years, focusing on Kaplan-Meier survival curves to calculate the hazard ratio (primary objective) and the survival rate (secondary objective) between vital and non-vital teeth. The risk of bias was assessed using the Newcastle-Ottawa Scale. Studies included in the review were identified through bibliographic research on electronic databases ("PubMed," "Scopus," "Cochrane Central Register of Controlled Trial," and "Embase"). The K agreement between the two screening reviewers was evaluated. RESULTS: A total of 55,793 records were identified on PubMed, Scopus, and other bibliographic sources, and after the application of the eligibility and inclusion criteria, eight articles were included for qualitative analysis and six for quantitative analysis. The meta-analysis of the primary and secondary outcomes demonstrated that hazard ratios (HR = 8.41, 95% CI: [4.50, 15.72]) and survival rates (OR = 3.24, 95% CI: [1.76, 5.82]) seemed more favorable for indirect partial adhesive restorations on vital teeth than for those on endodontically treated teeth. CONCLUSIONS: Within the limits of this study, these findings suggest that the risk of failure of indirect partial adhesive restorations on endodontically treated teeth is higher than on vital teeth. CLINICAL RELEVANCE: The use of partial adhesive restorations on vital and endodontically treated teeth showed different long-term clinical outcomes.

ABSTRACT

OBJECTIVES: To evaluate different direct restoration techniques on various cavity designs in anterior endodontically treated teeth (ETT). MATERIALS AND METHODS: Ninety upper central incisors (n = 90) were selected, endodontically treated, and divided into three groups (n = 30) accordingly to the cavity design: minimal endodontic cavity access (group A), endodontic access + mesial class III cavity (group B), and endodontic access + two class III cavities (group C). Three subgroups (n = 10) were then created accordingly to the restoration technique: nano hybrid composite restoration (subgroup a), glass fiber post + dual-cure luting cement (subgroup b), and bundled glass fiber + dual-cure luting cement (subgroup c). Samples underwent micro-CT scan, chewing simulation, and a second micro-CT scan. 3D quantification (mm3) of interfacial gap progression was performed; then, samples underwent fracture resistance test. Data were statistically analyzed setting significance at p < 0.05. RESULTS: Groups A and B showed significantly lower interfacial gap progression compared with group C. Subgroup b performed significantly better compared with subgroups a and c. Improved fracture strength was reported for group C compared with group A, while both subgroups b and c performed better than subgroup a. CONCLUSIONS: Cavity design significantly influenced interfacial gap progression and fracture resistance. Fiber posts significantly lowered gap progression and improved fracture resistance while bundled fibers only increased fracture resistance. A significant reduction of non-repairable fractures was recorded when fibers were applied. CLINICAL RELEVANCE: A minimally invasive approach, conserving marginal crests, should be applied whenever possible. Inserting a fiber post is indicated when restoring anterior ETT, in order to reduce gap progression, improve fracture resistance, and avoid catastrophic failures.

ABSTRACT

AIM: The aim of the study was to achieve the endodontic treatment of a second maxillary molar with abnormal anatomy through cone-beam computed tomography (CBCT) analysis and three-dimensional (3D) printing. BACKGROUND: A patient arrived referring recurrent abscesses, spontaneous pain, and the difficulty to eat properly; a diagnosis of tooth necrosis was made. The periapical X-ray showed an abnormal anatomy of the tooth. CASE DESCRIPTION: The preoperative analysis of the tooth morphology was performed through CBCT analysis. The 3D scans were reconstructed through a dedicated software application (Mimics) to visualize the abnormal anatomy and detect the root canal morphology. Moreover, a 3D printed model was created to plan the clinical stage. Canal shaping was achieved with ProGlider and ProTaper Next techniques, and the root canal filling was performed with a carrier-based technique. CONCLUSION: The limits of this case were the low definition of the CBCT owned by the patient and the difficulties during the scouting of the canals due to the abnormal endodontic space complicated by narrow and flat canals. The software reconstruction allowed a 3D high-definition preclinical analysis of the tooth anatomy. CLINICAL SIGNIFICANCE: Both digital analysis and printed models based on CBCT scans seemed fundamental to preclinically understand the abnormal endodontic anatomy.

ABSTRACT

PURPOSE: To evaluate the in vitro efficacy of surface coating sealer (SCS) in reducing the amount of water absorption and stain formation in resin composites. METHODS: To evaluate the amount of water sorption, 40 stainless steel round molds (n=10 per composite) were filled with Filtek Supreme XTE; Venus Diamond; Clearfil Majesty ES-2; Gradia Direct Anterior. Groups were further divided into two subgroups (n=5) according to the presence/absence of SCS (Easy Glaze). Their masses were measured at baseline and reweighed at 1, 4, 12, and 20 weeks. For staining evaluation, 48 resin composite discs, 2 mm-thick, were obtained with the same composites and SCS was applied on a single surface of each sample. After baseline color assessment, samples were immersed in vials containing 5 mL of six different staining solutions. Color evaluation was performed again after 1, 4, 12, and 20 weeks. Three-way ANOVA was performed to evaluate differences in weight gain and color changes (P< 0.05). RESULTS: A significant protective action of SCS (P< 0.001), but not of the composites (P= 0.366) was found. Differences in weight and color change were not influenced by the resin composite, but only by the presence of the coating on the surface of the samples. SCS are well-established materials for polishing and protecting composite restorations from water sorption and color changes. Different staining solutions can still produce different effects on resin composite. CLINICAL SIGNIFICANCE: Surface coating sealers are able to reduce water uptake and discoloration from resin composites, thus leading to less degradation of composite restorations over time.

ABSTRACT

PURPOSE: To examine the effect of CHX pre-treatment on long-term bond strength of fiber posts luted with self-adhesive resin cements. MATERIALS AND METHODS: Seventy-two single-rooted teeth were selected for root canal treatment and post space preparation. The tested self-adhesive cement/post combinations were (N = 36): 1. RelyX Fiber-Posts luted with RelyX Unicem; 2. Rebilda Posts luted with Bifix SE Cement. For both self-adhesive cements, half of the specimens (experimental groups) were luted after the application of a solution of 2% CHX, while no CHX application was performed for the remaining specimens (control groups). Luted specimens were cut and used for push-out bond strength evaluation immediately, and after storage in artificial saliva for 6 months or 1 year. Additional specimens were processed for quantitative interfacial nanoleakage analysis. RESULTS: ANOVA showed that the variable times of storage had a significant influence on the results (p < 0.05), while no influence of the luting procedure (cements with or without CHX) on the final outcome (p > 0.05) was found. Tukey's pairwise post-hoc test showed that the radicular bond strength decreased with time of storage. In particular, a significant difference was found between T0 and T1y, but not between T0 and T6m. In contrast, in terms of pretreatment, no significant reduction in push-out bond strength was observed, irrespective of the aging time. CONCLUSION: CHX pretreatment did not prevent bond strength degradation of fiber posts luted with self-adhesive cements.

ABSTRACT

The aim of this in vitro study was evaluate the effect of application of synthetic hydroxyapatite on fiber post bond strength to radicular dentine. Forty, single-root teeth were endodontically treated and an 8 mm post space was prepared. Specimens were randomly placed in four groups (n = 10 in each) and treated using the following fiber post luting procedures: group 1, 17% EDTA + Panavia SA; group 2, 17% EDTA + Teethmate Desensitizer + Panavia SA; group 3, All-Bond Universal + Duo-Link Universal; and group 4, All-Bond Universal + Teethmate Desensitizer + Duo-Link Universal. Fiber posts were luted in the post space and light-cured for 120 s using a light-emitting diode (LED) lamp. After 7 d of storage at 37°C, the teeth were cut into 1-mm-thick slices, which were subjected to a push-out test until failure using a universal testing machine. Two specimens per group were prepared for scanning electron microscopy analysis. An energy-dispersive X-ray spectroscopy detector was used for elemental analysis of the specimen surface. The results were statistically analyzed using one-way anova. The fiber post bond strength was statistically significantly increased after the application of Teethmate Desensitizer to post space walls, either with a 10-MDP-containing self-adhesive cement or with a universal adhesive. Scanning electron microscopy and EDAX analysis showed that Teethmate Desensitizer created a calcium phosphate precipitate over post space dentinal tubules, which significantly improved the bond strength of the fiber post luted with 10-methacryloyloxydecyl dihydrogen phosphate (10-MDP)-containing adhesive systems.

ABSTRACT

PURPOSE: To evaluate the influence of operator experience on adhesive technique and to assess the clinical performances of two different adhesive systems. METHODS: 90 cervical lesions in 42 subjects, with a mean age of 52.4 years (range between 32-63) were selected. Lesions were divided into two groups according to operator experience (n = 42 restorations performed by a skilled operator, n = 48 restorations performed by five unexperienced operators) and further divided into two subgroups according to the adhesive system used: three-step etch-and-rinse (Optibond FL) or one step self-etch (G-Bond). Adhesives were applied according to manufacturers' instructions. Subjects underwent follow-up at 12, 24, and 36 months and restoration retention, enamel and dentin marginal integrity, marginal discoloration, caries occurrence, post-operative sensitivity, and preservation of tooth vitality were evaluated according to USPSH criteria. ANOVA test was performed to evaluate the influence of the adhesive system, the operator experience, and aging on restorations. RESULTS: Operator experience (expert vs. inexperienced), aging time (12, 24, or 36 months), and the adhesive system (self-etch vs. etch-and-rinse) all affected the results statistically, as did the interaction between the adhesive system and operator experience.
