ABSTRACT

The study consists in studying the state of oral hygiene and periodontal tissues in elderly patients during dynamic follow-up for 12 months. Patients aged 61-74 years with inflammatory periodontal diseases and partial absence of natural teeth in both jaws were under observation. The study showed a significant improvement in oral hygiene and the condition of periodontal tissues in the elderly when using therapeutic and prophylactic products of domestic production for individual intraoral hygiene and professional oral hygiene at least four times a year.

ABSTRACT

Most patients over 50 years of age are diagnosed with diseases of the dentoalveolar apparatus, in particular, bone tissue disorders, which leads to a decrease in the survival rate of implants. Identification of the causes, as well as the development of a methodology for predicting survival by minimally invasive methods, is relevant. The aim of the study was to study the markers of tight junctions in the buccal epithelium (BE) in people of different ages. BE scrapings were taken before and after implantation in patients divided into 5 age groups, from young to centenarians. Immunocytochemical method was used to study markers for tight junction proteins - claudin-1, -7 and 10. It has been shown that with age there is a decrease in the intensity of expression of adhesion molecules, in particular claudin -1, -7 and -10 in the mucous membranes, the minimum values were recorded in the group of centenarians. The study found that after dental implantation, there was a decrease in the expression of claudin-1 and -10 and an increase in the expression of claudin-7. Changes in the expression of claudins may indicate the development of a pathological process in the body, including the success of implantation, especially in people of older age groups.

ABSTRACT

Competence-based approach plays an important role in the training of a modern doctor. Considering that the strategy of dental care for older age groups is determined not only by the demographic situation and its development, but also by the justified need for effective dental care, it is important to implement the teaching of gerontostomatology in a medical university based on the results of evidence-based medicine and a personalized approach. The aim of the work was to develop a methodology for teaching geriatric aspects according to the curriculum of a specialist in the discipline of maxillofacial surgery and surgical dentistry. Based on the study of scientific works on various aspects of gerontostomatology, the author's position is indicated in the work and methodological foundations for modern teaching of the gerontostomatology section are presented according to the curriculum of a specialist in the main areas of training in the field of maxillofacial surgery and surgical dentistry. Particular attention is paid to the methodology of teaching pyoinflammatory diseases and injuries of the maxillofacial region, salivology, pathology of the temporomandibular joint, masticatory muscles and oncostomatology.

ABSTRACT

Traumatic damages to the oral mucosa are the main predisposing factor for the development of the oncostomatological pathology among the old people. The aim of the investigation was to study the occurrence of traumatic lesions of the oral mucosa among people of old age groups and estimate the effectiveness of their treatment using modern domestic remedies. 967 people (569 males and 398 females) from 19 to 82 years old were examined to determine the occurrence of traumatic oral mucosal pathology. Value of effectiveness while treatment of 83 elderly patients using modern domestic agents «ASEPTA® adhesive balm for gums¼ and «ASEPTA® gel for gums with propolis¼ was assessed in comparison with vinylin for internal use. It is found that 9,41 and 9,52% of cases are traumatic lesions of the oral mucosa respectively among elderly and senile people respectively. The cause of them in 96,7-97,1% cases are traumatic factors while using removable dental prostheses; the untimely treatment of these factors in 7,9% leads to the formation of prosthetic fibroms. A comparative evaluation of the treatment effectiveness of prosthetic-etiology-traumatic lesions of the oral mucosa showed the advisability of using modern domestic agents like «ASEPTA® adhesive balm for gums¼ during the first 2 days from the start of therapy and then «ASEPTA® gel for gums with propolis¼ until complete healing of traumatic injuries, compared to the using of vinylin for internal use for the same purposes, which efficiency remains high. Timely detection and treatment of the traumatic defeats of oral mucosa are an important factors of prevention of oncostomatological pathologies among elderly and senile persons.

ABSTRACT

The strategy of dental care for older age groups is based not only on the priorities determined by the demographic situation, guided by its development, epidemiologically justified need for dental care, but also by its effectiveness, which should be conditioned and based on the results of evidence-based medicine and a personalized approach, in the implementation of which it is important a competence-based approach plays a role in the process of teaching gerontostomatology students at a medical university. The aim of the work was to create a modern methodological basis for teaching the section of gerontostomatology in accordance with the modern curriculum of a specialist in a medical university. Based on the study of scientific works devoted to various aspects of gerontostomatology, the work presents the methodological foundations for modern teaching of the section of gerontostomatology on a specialist in a medical university in the main areas of specialist training, taking into account the modern curriculum in the field of cariesology, non-carious dental lesions, endodontics, periodontology, diseases of the mucous membrane of the cavity and the basics of providing orthopedic dental care to elderly and senile people.

ABSTRACT

The increase in the proportion of elderly and old people in the structure of the population is a feature of the present time, which covers almost the entire world. The aging of the population has changed the scope of tasks of doctors of various specialties, including dentists, making improving the quality of dental care and the life of older people a top priority. Bioregulation is important in maintaining the health of people of older age groups, which is a multiple complex of intercellular interactions aimed at maintaining homeostasis and responsible for the course of such important physiological processes as cell differentiation and division, tissue regeneration, aging of the body, exchange and reproduction of genetic information, and much other. Bioregulatory therapy is aimed at eliminating homeostasis disorders in order to normalize the functions of damaged cells, tissues and organs. The purpose of this work is to investigate the main directions of use, achievements and prospects of bioregulatory therapy in the field of gerontostomatology. The main results of the use of bioregulatory therapy in gerontostomatology in the treatment of periodontal diseases, oral mucosa, temporomandibular joint, odontogenic purulent-inflammatory diseases and foci of stomatogenic chronic infection, as well as the prospects for their further use are presented.

The article presents data on the use of peptide bioregulators of cell homeostasis and the protective functions of the body in various dental diseases and pathology of organs and tissues of the maxillofacial region. Issues related to the immunopharmacological effect and clinical use of natural and synthetic peptide thymomimetics (thymalin, thymogen, vilon, epithalamin, cortexin) and peptide bioregulators from cartilaginous (sigumir, chondrolux) and other tissues in case of trauma, as well as inflammatory, inflammatory and other pathological processes of tissues of the oral cavity and maxillofacial region. Particular attention is paid to their use in the complex treatment of dental diseases in the elderly.

ABSTRACT

Based on the evaluation of the condition of the organs and tissues of the masticatory apparatus 275 (69 men and 206 women) people aged 25 to 86 years, including the 74 the elderly and old people suffering from diabetes mellitus type 2, chronic nonspecific inflammatory bowel disease, alcoholism, chronic kidney disease using the methods of electrodontometry, estesiometry, electro-gustometry, assessment thresholds and functional mobility of taste receptors of the tongue, studied age-related physiological and pathophysiological features of the masticatory apparatus. It was found that with age, the thresholds of pain sensitivity of teeth, oral mucosa, tongue and taste sensitivity increase, as well as there is a violation of the functional mobility of taste receptors. The presence in the elderly and old people of psychosomatic pathology, especially type 2 diabetes, chronic kidney disease, chronic alcoholism in older age groups exacerbates the existing age-related changes in pain, taste sensitivity, and also leads to severe violations of the directed reaction of taste receptors. Presented in the article indicators characterizing some age physiological and pathophysiological features of the masticatory apparatus of elderly and old people can be used as a starting point for studying the degree of functional disorders of the masticatory apparatus, as well as to take into account the degree of normalization of impaired functions in the treatment of people of older age groups.

ABSTRACT

A study related to the disease in 39 (7 men and 32 women) people aged 61-86 years, suffering from paresthesia oral mucous membranes (POMM). To determine the type of relationship to the patient's illness was used clinical test method which employs a clinical-psychological typology of relationship to the patient's illness. It was found that for patients with middle and old age (69,2 %), suffering from severe and moderate POMM severity of pathology characteristic intrapsychic focus of personal response to the disease, is caused due to the presence of a constant burning sensation in the mouth, often accompanied by the syndrome of «dry mouth¼ violation of social adaptation of patients. For the older age groups suffering POMM, which proceeded in a light, at least moderate disease severity (28,2 %) is characteristic orientation interpsychic personal response to the disease, also causes disturbances of social adaptation of patients. The obtained information about the type of relationship the patients of elderly and senile age, suffering POMM show that to achieve a positive therapeutic outcome and successful rehabilitation is necessary to change their existing inadequate response to disease, it is possible by attracting clinical pharmacologist and (or) the therapist.

ABSTRACT

The article presents the results of pre-clinical studies of the possibility of optimization of reparative osteogenesis in postoperative bone cavities of the jaws in old rats with a peptide bioregulator of Kartalaks. Using histological and morphometric methods in the experimental study, it is shown that the use of Kartalaks positive effect on the course of the process of reparative osteogenesis of the mandible in old rats. The features of the Kartalaks drug investigated for optimization of reparative osteogenesis jaw during aging. It is found that by using Kartalaks in old animals to 30 days before the surgical intervention, the process of reparative osteogenesis standard defect of mandible occurs at early stages (before 30 days), similar to the control. The deadlines for 60-120 days from the beginning of the experiment using the peptide bioregulator Kartalaks, noted the formation of a complex regenerate tissue containing a smaller proportion of connective tissue and cartilage, as well as more advanced remodeling of newly formed bone tissue compared with the control group, and older animals who received the drug after performing surgery.

ABSTRACT

On the basis of analysis of primary medical records of 9 158 (3 329 males and 5 829 females) patients of average, elderly and old age of outpatient clinics the prevalence of pain and paresthesys of the oral mucosa and tongue was studied. Profile of outpatient medical institutions, where people suffering from painful phenomena and paresthesys of the oral mucosa pass examination and treatment was defined. The causal factors that play a role in the origin and development of this disease in patients of older age groups were established.

ABSTRACT

The author presents the results of the surveillance of medical rehabilitation of 22 elderly patients suffering from chemical stomatitis combined with medication sialadenopathys that occurred after combined treatment of malignant tumors of oropharyngeal area. It is shown that the inclusion in the conventional regimen chemical stomatitis peptide bioregulator Vezugen and dietary supplement Algiklam enhances the effectiveness of the treatment gerontostomatologic patients suffering from chemical stomatitis combined with medication sialadenopathys.

This article presents the results of medical reports retrospective analysis of 605 patients that came to multisectoral maxillofacial hospital with acute odontogenic periostitis of maxilla and jaw by emergency or according to urgent indications in 2008-2011. The age-related features of prevalence and principal reasons of development and clinical aspects of localization of odontogenic periostitis of maxilla and jaw within the people of different age are shown.
