RESUMEN

BACKGROUND: Dental caries is one of the most common microbial infectious diseases. The important method for as-sessment of oral health status is DMFT (Decayed, Missing, and Filled Teeth). Recent studies have shown conflicting results regarding the relationship of antibodies with dental caries. This study aimed to investigate the salivary and serum IgA and IgG levels in adult caries. METHODS: This cross-sectional study was conducted on 96 patients with dental caries who were referred to the specialized dental clinic in Yasuj. Based on the DMFT index, patients were divided into three groups. DMFT index in group 1, 2, and 3 were 0, 1 - 3, and more than 3, respectively. Salivary and serum levels of IgA and IgG were measured by nephelometric method. Data were analyzed using SPSS software, chisquared, and paired t-test. RESULTS: The mean salivary IgA level in group 1, 2, and 3 were 0.259 ± 0.118, 0.264 ± 0.175, and 0.169 ± 0.106 mg%, respectively (p = 0.001). Also, the mean salivary IgG level in group 1, 2, and 3 were 1.360 ± 316, 1.320 ± 0.350 and 1.320 ± 0.370 mg% (p = 0.929). The mean serum IgA level were 1.443 ± 0.433, 1.805 ± 0.518 and 1.790 ± 0.700 gr% in group 1, 2 and 3, respectively (p = 0.363). Also, the mean serum IgG level in group 1, 2, and 3 were 9.275 ± 1.899, 10.257 ± 1.658 and 10.589 ± 3.113 gr% (p = 0.451). CONCLUSIONS: This study showed that by decreasing the level of salivary IgA, the rate of dental caries and DMFT index increase and this indicates the protection mechanism against dental caries by IgA.

RESUMEN

Glomus tumors are rare benign neoplasms usually occurring in the upper and lower extremities. However, oral cavity involvement is rare, with only a few case reports. We present a 37-year-old male patient with a chief complaint of an exophytic lesion in the left buccal mucosa for one year referred to our center. At the time, he did not have any pain or lymphadenopathy. The patient underwent surgery using a high-power CO2 laser. His histopathological examination revealed a glomus tumor. After 2 years of follow-up, no evidence of recurrence was detected. Glomus tumors should be taken seriously when patients have a painless exophytic bluish lesion in the buccal mucosa.

Myxofibrosarcoma (MFS) is a fibroblastic soft tissue sarcoma that is extremely rare in the maxillofacial region. Due to its non-specific clinicoradiographic findings and challenging histopathological features, the diagnosis is difficult. Here, we present a case of MFS which was first diagnosed as nodular fasciitis. The initial examination of the incisional biopsy showed a benign-appearing proliferation of fibroblasts without features of malignancy. The patient returned with recurrence four months after surgical excision of the primary lesion. The second histologic study revealed a high-grade spindle cell sarcoma with myxoid features most compatible with MFS. Definitive diagnosis of MFS was confirmed by these histopathologic features and supportive immunohistochemical stains. Unfortunately, the patient died of disease 3 months later.

BACKGROUND: Candida species have an influence in the pathogenesis of denture stomatitis. The current study aimed to investigate the efficacy of indocyanine green (ICG)-mediated photodynamic therapy (PDT) in combination with nystatin mouthwash (PDT + nystatin) for the treatment of denture stomatitis in comparison with routine antifungal therapy with nystatin alone. METHODS: In this double-blind randomized clinical trial, 66 patients were randomly assigned into PDT + nystatin (n = 33) and nystatin (n = 33) groups, both groups were treated 3-times a day (15 days) with nystatin mouthwash, and PDT was performed twice once a week for the PDT + nystatin group. Briefly, ICG was applied on the palatal lesion and laser irradiation was performed using a diode laser (810 nm, 56 J/cm2). Nystatin group was also treated with sham laser in order to eliminate the possible psychological effects. The clinical and mycological evaluations were carried out at the baseline, during treatment, and the end of follow-up. Patients who completed the treatment and follow-up were eligible for statistical analysis (each group 28 cases). RESULTS: Patient treatment with nystatin or PDT + nystatin reduced the lesions extension. Candida species were isolated from all patients and the number of Candida CFU in both groups showed a significant reduction at each post-treatment visit; however, the mean reduction achieved in the PDT + nystatin group was significantly higher than nystatin alone. CONCLUSIONS: ICG-mediated PDT in combination with nystatin mouthwash can improve the clinical feature of denture stomatitis with no adverse effects; therefore, it could be used as an alternative to the currently available antifungal therapy using nystatin alone.

STATEMENT OF THE PROBLEM: Due to the close contact with patients during dental treatments, dentists and those affiliated to the dental profession are at higher risk for various infections. Infection prevention in dentistry is an important topic that has gained more interest in recent years. PURPOSE: We aimed to evaluate the protective role of front-closed and front-open gowns against staphylococcus aureus contamination of dental students before and after restorative treatments. MATERIALS AND METHOD: Sixty male dental students performed the restorative treatments on the teeth of the patients in the front-closed and front-open gowns groups. Before and after the treatment, the wet sterile swab samples were collected from the students' neck and anterior part of the chest. The samples simultaneously cultured on the blood agar and the Staphylococcus-specific medium using spread plate method. Finally, the colonies were counted within 24-48 hours. RESULTS: The mean of the total colony count increased in both groups of front-open (787.8 ± 88.91) and front-closed gowns (630 ±122.7), but the changes were significant only in the front-open gown group (p≤ 0.001). Compared to the front-closed gown group (430±71.08), the total colony count of staphylococcus aureus in the front-open gown group (490.3±62.5) was increased significantly (p≤ 0.001). CONCLUSION: We confirmed that dental students occupationally exposed to the bacterial agents and even simple minor changes in gown could considerably decrease the contamination. Education about bacterial transmission, as well as infection prevention and control measures is necessary for dental students, especially when they participate in clinical practice.
