RESUMEN

Radiation therapy to either mediastinum or pelvis causes a rapid decrease in circulating lymphocytes of both B and T types and in addition an impairment in the function of the remaining lyphocytes, as measured by their ability to proliferate in response to mitogens. The acute depression is short-lived. Substantial recovery is apparent within 3 wk after cessation of therapy; however, most patients show a modest, chronic depression in both numbers and functional capacities of circulating lymphocytes. T cells are somewhat more sensitive than B cells, but both are affected. Irradiation of the thymus per se seems to have little influence on the acute changes which occur, as patients receiving pelvic and mediastinal (including thymic) radiotherapy show a similiar degree of lymphopenia and depression of lymphocyte responsiveness.

Attempted nonspecific immunotherapy led to acceleration rather than retardation of tumor growth. Mice given injections of Freund's adjuvant were more susceptible to transplanted syngeneic Gross virus-induced leukemic cells when Freund's complete adjuvant was administered i.p. 0 to 7 days before or 1 day after tumor; thereafter, the adjuvant had no effect. Two serum-mediated phenomeana were demonstrated in vitro: (a) sera from mice immunized with Freund's complete adjuvant and tumor facilitated killing of tumor cells by peritoneal exudate cells from nonimmune mice; (b) sera from all mice with progressive tumor blocked the cytotoxicity of a xenogeneic tumor-specific serum. Certain sera produced both effects. However, sera that either blocked or facilitated tumor killing in vitro had no effect on the growth in vivo of transplanted tumor cells.

RESUMEN

A 35 year old previously healthy physician had clinical manifestations of a mononucleosis illness complicated by arthralgia, vesicular pharyngitis and hepatitis. Initially, the patient had cytomegalovirus (CMV) viremia (predominantly in polymorphonuclear leukocytes) followed by the presence of CMV in the urine, throat and semen. He also had an antibody response to the Epstein-Barr virus which appeared to be a secondary type. During the acute phase of illness, only 7 per cent of the patient's lymphocytes formed spontaneous T cell rosettes as compared to a normal value of 65 to 70 per cent. Concurrently, evidence of abnormal delayed hypersensitivity was manifested by the loss of reactivity to mumps skin test antigen. All clinical and laboratory abnormalities except for the persistence of CMV in the pharynx, urine and semen returned to normal after resolution of the clinical illness.
