BACKGROUND: In the Brazilian public health system, primary health care (PHC) is provided by the municipalities and is considered the entry level of the Unified Health System (SUS). Governmental pharmaceutical services (PharmSes) are part of the SUS, including PHC, and are the most significant way in which patients access medicine and services. Considering the diversity of the country, the municipalities have the autonomy to decide how PharmSes are implemented. Even though policies and procedures should be implemented as expected by policy makers and experts, municipality characteristics may interfere with implementation fidelity. Therefore, this study evaluated the degree to which the PharmSes in PHC were delivered as intended in Brazilian municipalities. METHODS: We analysed data from a secondary database originating from a cross-sectional nationwide study carried out by the Ministry of Health and the World Bank from 2013 to 2015. Data on 465 municipalities and the Federal District were collected from 4939 governmental PharmSes. A rating system comprising 43 indicators was developed and applied to the dataset to obtain the implementation degree (ID) of each PharmSe. Additionally, the IDs of the two PharmSes dimensions and the nine components were measured. RESULTS: Overall, the ID of the PharmSes in Brazilian PHC was evaluated as critical. The ID was critical in 81% of the municipalities (n = 369), incipient in 14% (n = 65) and unsatisfactory in 4.8% (n = 22). Regarding the PharmSes dimensions, the 'medicine management' (MM) ID was considered critical (Mean = 46%), while the 'care management' (CM) ID was incipient (Mean = 22%). In terms of the PharmSes components, the highest ID was achieved by 'forecasting' (58%). In contrast, 'continuing education and counselling' showed the lowest figure (ID = 11%) in the whole sample, followed by 'information and communication' and 'teamwork'. CONCLUSIONS: The degree to which PharmSes were implemented was critical (ID< 50%). This analysis demonstrated that PharmSes were implemented with low fidelity, which may be related to the low availability of medicine in PHC. Although the care management component requires more attention, considering their incipient ID, all components must be reviewed. Municipalities must increase their investment in PharmSes implementation in order to maximize the benefits of these services and guarantee the essential right of access to medicine.

BACKGROUND: Increasing medicines availability and affordability is a key goal of Brazilian health policies. "Farmácia Popular" (FP) Program is one of the government's key strategies to achieve this goal. Under FP, antihypertension (HTN) and antiglycemic (DM) medicines have been provided at subsidized prices in private retail settings since 2006, and free of charge since 2011. We aim to assess the impact of sequential changes in FP benefits on patient affordability and government expenditures for HTN and DM treatment under the FP, and examine their implications for public financing mechanisms and program sustainability. METHODS: Longitudinal, retrospective study using interrupted time series to analyze: HTN and DM treatment coverage; total and per capita expenditure; percentage paid by MoH; and patient cost sharing. Analyzes were conducted in the dispensing database of the FP program (from 2006 to 2012). RESULTS: FP has increased its coverage over time; by December 2012 FP covered on average 13% of DM and 11.5% of HTN utilization, a growth of over 600 and 1500%, respectively. The overall cost per treatment to the MoH declined from R$36.43 (R$ = reais, the Brazilian currency) to 18.74 for HTN and from R$33.07to R$15.05 for DM over the period analyzed, representing a reduction in per capita cost greater than 50%. The amount paid by patients for the medicines covered increased over time until 2011, but then declined to zero. We estimate that to treat all patients in need for HTN and DM in 2012 under FP, the Government would need to expend 97% of the total medicines budget. CONCLUSIONS: FP rapidly increased its coverage in terms of both program reach and proportion of cost subsidized during the period analyzed. Costs of individual HTN and DM treatments in FP were reduced after 2011 for both patients (free) and government (better negotiated prices). However, overall FP expenditures by MoH increased due to markedly increased utilization. The FP is sustainable as a complementary policy but cannot feasibly substitute for the distribution of medicines by the SUS.

INTRODUCTION: Lung cancer (LC) is a relevant public health problem in Brazil and worldwide, given its high incidence and mortality. Thus, the objective of this study is to analyze the distribution of smoking and smoking status according to sociodemographic characteristics and disparities in access, treatment, and mortality due to LC in Brazil in 2013 and 2019. METHOD: Retrospective study of triangulation of national data sources: a) analysis of the distribution of smoking, based on the National Survey of Health (PNS); b) investigation of LC records via Hospital-based Cancer Registry (HCR); and c) distribution of mortality due to LC in the Mortality Information System (SIM). RESULTS: There was a decrease in the percentage of people who had never smoked from 2013 (68.5%) to 2019 (60.2%) and in smoking history (pack-years). This was observed to be greater in men, people of older age groups, and those with less education. Concerning patients registered in the HCR, entry into the healthcare service occurs at the age of 50, and only 19% have never smoked. While smokers in the population are mainly Mixed-race, patients in the HCR are primarily White. As for the initial stage (I and II), it is more common in White people and people who have never smoked. The mortality rate varied from 1.00 for people with higher education to 3.36 for people without education. Furthermore, White people have a mortality rate three times higher than that of Black and mixed-race people. CONCLUSION: This article highlighted relevant sociodemographic disparities in access to LC diagnosis, treatment, and mortality. Therefore, the recommendation is to strengthen the Population-Based Cancer Registry and develop and implement a nationwide LC screening strategy in Brazil since combined prevention and early diagnosis strategies work better in controlling mortality from the disease and continued investment in tobacco prevention and control policies.

This study describes and compares tuberculosis (TB) data among persons deprived of liberty and the general Brazilian population, from 2007 to 2019, using the Joinpoint tool to observe changes in trends. This study focuses on women and older adults, for HIV testing, and on the number of detainees according to prison capacity. This is a retrospective, quantitative, and analytical study, which uses methods of regression of time series data from secondary data of unrestricted access collected from the Brazilian Information System for Notifiable Diseases (SINAN), Brazilian Institute of Geography and Statistics (IBGE), and from analytical reports made available by the Brazilian National Penitentiary Department (DEPEN). The results show a considerably higher increase in the prevalence of TB in persons deprived of liberty in all perspectives analyzed; increased HIV testing; and a debatable trend of stability in the number of detainees according to prison capacity. When analyzing trends in prevalence, services, and determinants, it is curious to see the temporal non-coincidence in most cases. Clearly, national policies against TB do not have the same effect within prisons and even the National Policy for Comprehensive Health Care for People Deprived of Liberty in the Prison System (PNAISP) showed restricted effects in view of the health situation herein analyzed. Despite working with secondary data of variable reliability, comparisons were reached that can impact health decisions and actions. Although lacking complete and definitive answers, it was possible to launch a new point-of-view on the evolution of questions for which reflection is essential.


Este estudo descreveu e comparou dados de tuberculose (TB) entre pessoas privadas de liberdade e população geral brasileira, de 2007 a 2019, utilizando a ferramenta Joinpoint para observação de mudanças de tendências. Apresenta um recorte para mulheres e idosos, para testagem para HIV e para número de custodiados por vaga. Trata-se de um estudo retrospectivo, quantitativo e analítico, que utiliza métodos de regressão de dados de séries temporais a partir de dados secundários de acesso irrestrito coletados do Sistema de Informação de Agravos de Notificação (SINAN), do Instituto Brasileiro de Geografia e Estatística (IBGE) e de relatórios analíticos disponibilizados pelo Departamento Penitenciário Nacional (DEPEN). Os resultados retratam aumento da prevalência de TB consideravelmente maior em pessoas privadas de liberdade em todas as perspectivas analisadas; aumento nas testagens para HIV; e discutível tendência de estabilidade na quantidade de custodiados por vaga. Ao se analisar tendências de prevalências, serviços e determinantes, é curioso ver a não coincidência temporal na maioria dos casos. Ficou claro que as políticas nacionais de combate à TB não têm o mesmo efeito dentro das prisões e mesmo a Política Nacional de Atenção Integral à Saúde das Pessoas Privadas de Liberdade no Sistema Prisional (PNAISP) mostrou efeitos restritos diante da situação de saúde aqui analisada. Apesar de trabalhar com dados secundários de confiabilidade variável, alcançaram-se comparações que podem impactar decisões e ações de saúde. Ainda que carente de respostas completas e definitivas, pôde-se lançar um novo olhar à evolução de questões sobre as quais a reflexão é imprescindível.


El presente estudio describió y comparó los datos de tuberculosis (TB) entre las personas privadas de libertad y la población general brasileña, de 2007 a 2019, utilizando la herramienta Joinpoint para observar los cambios en las tendencias. Presenta un límite para mujeres y ancianos, para las pruebas para VIH y para el número de internos por vacante. Se trata de un estudio retrospectivo, cuantitativo y analítico, que utiliza métodos de regresión de datos de series temporales a partir de datos secundarios de acceso no restringido recolectados del Sistema de Información de Agravios de Notificación (SINAN), Instituto Brasileño de Geografía y Estadística (IBGE) e informes analíticos disponibles por el Departamento Penitenciario Nacional (DEPEN). Los resultados muestran un aumento de la prevalencia de TB considerablemente mayor en personas privadas de libertad en todas las perspectivas analizadas; un aumento en las pruebas de VIH, y una tendencia discutible a la estabilidad en la cantidad de internos por vacante. Al analizar las tendencias de las prevalencias, los servicios y determinantes, es curioso ver la no coincidencia temporal en la mayoría de los casos. Quedó claro que las políticas nacionales de combate a la TB no tienen el mismo efecto dentro de las prisiones y incluso la Política Nacional de Atención Integral a la Salud de las Personas Privadas de Libertad en el Sistema Penitenciario (PNAISP) mostró efectos restringidos frente a la situación de salud aquí analizada. A pesar de trabajar con datos secundarios de confiabilidad variable, se lograron comparaciones que pueden afectar las decisiones y acciones de salud. Aunque se carezca de respuestas completas y definitivas, se ha podido dar una nueva mirada a la evolución de cuestiones sobre los que es imprescindible reflexionar.

Importance: It is estimated that, from 2023 to 2025, lung cancer (LC) will be the second most frequent cancer in Brazil, but the country does not have an LC screening (LCS) policy. Objective: To compare the number of individuals eligible for screening, 5-year preventable LC deaths, and years of life gained (YLG) if LC death is averted by LCS, considering 3 eligibility strategies by sociodemographic characteristics. Design, Setting, and Participants: This comparative effectiveness research study assessed 3 LCS criteria by applying a modified version of the LC-Death Risk Assessment Tool (LCDRAT) and the LC-Risk Assessment Tool (LCRAT). Data are from the 2019 Brazilian National Household Survey. Participants included ever-smokers aged 50 to 80 years. Data analysis was performed from February to May 2023. Exposures: Exposures included ever-smokers aged 50 to 80 years, US Preventive Services Task Force (USPSTF) 2013 guidelines (ever-smokers aged 55 to 80 years with ≥30 pack-years and <15 years since cessation), and USPSTF 2021 guidelines (ever-smokers aged 50 to 80 years with 20 pack-years and <15 years since cessation). Main Outcomes and Measures: The primary outcomes were the numbers of individuals eligible for LCS, the 5-year preventable deaths attributable to LC, and the number of YLGs if death due to LC was averted by LCS. Results: In Brazil, the eligible population for LCS was 27 280 920 ever-smokers aged 50 to 80 years (13 387 552 female [49.1%]; 13 249 531 [48.6%] aged 50-60 years; 394 994 Asian or Indigenous [1.4%]; 3 111 676 Black [11.4%]; 10 942 640 Pardo [40.1%]; 12 830 904 White [47.0%]; 12 428 536 [45.6%] with an incomplete middle school education; and 12 860 132 [47.1%] living in the Southeast region); 5 144 322 individuals met the USPSTF 2013 criteria for LCS (2 090 636 female [40.6%]; 2 290 219 [44.5%] aged 61-70 years; 66 430 Asian or Indigenous [1.3%]; 491 527 Black [9.6%]; 2 073 836 Pardo [40.3%]; 2 512 529 [48.8%] White; 2 436 221 [47.4%] with an incomplete middle school education; and 2 577 300 [50.1%] living in the Southeast region), and 8 380 279 individuals met the USPSTF 2021 LCS criteria (3 507 760 female [41.9%]; 4 352 740 [51.9%] aged 50-60 years; 119 925 Asian or Indigenous [1.4%]; 839 171 Black [10.0%]; 3 330 497 Pardo [39.7%]; 4 090 687 [48.8%] White; 4 022 784 [48.0%] with an incomplete middle school education; and 4 162 070 [49.7%] living in the Southeast region). The number needed to screen to prevent 1 death was 177 individuals according to the USPSTF 2013 criteria and 242 individuals according to the USPSTF 2021 criteria. The YLG was 23 for all ever-smokers, 19 for the USPSTF 2013 criteria, and 21 for the USPSTF 2021 criteria. Being Black, having less than a high school education, and living in the North and Northeast regions were associated with increased 5-year risk of LC death. Conclusions and Relevance: In this comparative effectiveness study, USPSTF 2021 criteria were better than USPSTF 2013 in reducing disparities in LC death rates. Nonetheless, the risk of LC death remained unequal, and these results underscore the importance of identifying an appropriate approach for high-risk populations for LCS, considering the local epidemiological context.

Introduction: The recommendation of rifampin-based shorter - and safer - regimens for tuberculosis preventive treatment (TPT) is progressively replacing monotherapy with isoniazid by different countries. The Brazilian Ministry of Health (MoH) approved the incorporation of the Rifapentine + isoniazid regimen (3HP) at the end of 2020, with free distribution in the Brazilian Unified Health System (SUS) started from the last quarter of 2021. The objectives were to describe the implementation of the IL-TB System (Information System of TPT Notification) and uptake of Rifapentine + isoniazid (3HP) and Isoniazid (6H or 9H) in Brazil. Methods: A quantitative observational and descriptive was performed using the IL-TB National System as the main data source, from January 2018 to December 2022. Results and discussion: There was a steady increase of the number of TPT prescription quarterly throughout the period, which reflects the implementation of the system itself and the progressive adherence of the health system to the non-compulsory notification of new TPT. The substitution of isoniazid (6H or 9H) by 3HP is progressing. The 3HP regimen represented less than 4% of the total administered by the end of 2021, reaching around 30% in the second half of 2022 and 40% in the last quarters of 2022. The study points not only to the need to expand TPT in the country, but also to accelerate 3HP uptake and to encourage the municipalities to notify to the IL-TB system, since there is still a high level of underreporting.

OBJECTIVE: To investigate the association between racial discrimination and three health outcomes: self-rated health (SRH), physical morbidity and depression, in the Brazilian population. DESIGN: Cross-sectional study based on data obtained from a representative national survey carried out in 2008: The Research for Social Dimension of Inequalities. PARTICIPANTS: 3,863 household heads who classified themselves as Blacks or Mullatoes regarding their race and answered the entire research questionnaire. MAIN OUTCOME MEASURES: Racial discrimination was measured through a scale of 9 domains based on a previously validated instrument and classified into two categories: no discrimination and any experience of racial discrimination. SRH was based on the question from the SF-36 quality of life instrument. Physical morbidity and depression were obtained from a list of chronic diseases from the questionnaire. Regression analysis was carried out for the three health outcomes controlling for socioeconomic, demographic, health behavior variables, and body mass index. RESULTS: Racial discrimination was negatively associated with health for all evaluated outcomes. Any experience of racial discrimination was associated with 1.37 more chance of having worse SRH, 1.55 more chance of having more physical morbidities and 1.77 more chance of having depression, even after controlling for confounders. CONCLUSION: An impact of racial discrimination on the health of the Brazilian population was found, regardless of the health indicator used, which revealed that depression was the health outcome with the most pronounced association.

This article aims to present a methodology for monitoring the procedures recommended in the care protocol for diabetic patients, based on the indicator of the ratio between supply and demand for exams, according to Brazil, macro-regions, federative units (FUs), and municipalities. The prevalence of diabetes mellitus (DM) and its complications were estimated using a multinomial model. The offer of DM procedures was obtained from the Ambulatory Information System (SIA/SUS) and the demand from the number of tests defined in the protocol as necessary per year, according to disease risk categories. Based on this, the supply-demand ratio indicator was created. The innovation here consists of analyzing the demand for diabetic care according to established parameters and the supply of health services together. The connection between the recommended treatment protocol and the existence of the service offered concerning the demand for care based on the prevalence of the disease provides a key monitoring tool. And, when analyzed together with the indicator of the ratio between supply and demand for procedures, these measures become a proxy for the quality of prevention and care for patients with the disease.


Este artigo tem como objetivo apresentar uma metodologia de monitoramento dos procedimentos preconizados no protocolo de atenção ao paciente diabético a partir do indicador de razão entre a oferta e a demanda de exames, segundo nível nacional, macrorregiões, UF e municípios. A prevalência de diabetes mellitus (DM) e suas complicações foi estimada a partir de modelo multinomial. A oferta de procedimentos para DM foi obtida a partir do Sistema de Informações Ambulatoriais (SIA/SUS) e a demanda a partir do número de exames definidos no protocolo como necessários por ano, segundo categorias de risco da doença. A partir disso foi criado o indicador de razão entre oferta e demanda. A inovação que ora apresentamos consiste em analisar conjuntamente a demanda por cuidado ao diabético segundo parâmetros estabelecidos e a oferta de serviços de saúde. A conexão entre o protocolo de tratamento preconizado e a existência do serviço ofertado em relação da demanda de cuidado baseada na prevalência da doença disponibiliza uma ferramenta chave de monitoramento. E, quando analisado conjuntamente ao indicador de razão entre oferta e demanda de procedimentos, essas medidas tornam-se proxy da qualidade da prevenção e atenção ao portador da doença.

The prevalence of emotional distress in the population attended by the teams of the Family Health Strategy is high, however the training of the Primary Care professionals in Mental Health care is inadequate. Matrix Support, provided through the Family Health Support Centers, is considered fundamental in the provision of such care. The scope of this study is to assess the training in mental health in primary care through matrix support actions in mental health. A quantitative, cross-sectional study was conducted in the secondary database of the 2012 National Program for the Enhancement of the Access to and the Quality of Primary Health Care. The associations between many matrix support activities in mental health and the development of comprehensive care strategies were analyzed. It revealed that 60% of the teams carry out Matrix Support actions in Mental Health, and qualified care strategies in mental health are performed at least twice as often when those activities are present. The most significant activities are discussion of cases, joint physician appointments, and the creation of therapeutic projects. The conclusion drawn is that this study provides quantitative evidence that the development of shared actions is decisive for the provision of mental health care in primary care.


A prevalência de sofrimento emocional na população atendida pelas equipes da Estratégia de Saúde da Família (ESF) é alta, mas a capacitação dos profissionais da Atenção Básica (AB) para o cuidado em Saúde Mental (SM) é inadequada. O Apoio Matricial (AM), através dos Núcleos de Apoio à Saúde da Família (NASF), tem sido considerado como fundamental na qualificação deste cuidado. Tem-se como objetivo analisar a qualificação do cuidado em SM na AB através das ações de Apoio Matricial em Saúde Mental (AMSM). Trata-se de estudo quantitativo (corte transversal) em base de dados secundária do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB), de 2012. Foram analisadas as associações entre a realização das diversas ações de matriciamento em SM e o desenvolvimento das estratégias de cuidado integral, avaliadas no instrumento. Verificou-se que 60% das equipes realizam ações de AMSM e que as estratégias de cuidado qualificadas em SM são realizadas pelo menos duas vezes mais quando estas ações estão presentes, destacando-se as ações de discussões de caso, consultas conjuntas e construção de projetos terapêuticos (PTS). Este estudo traz evidências quantitativas de que o desenvolvimento de ações compartilhadas é determinante para qualificação do cuidado em SM na AB.

OBJECTIVE: To analyze the impact of implementing the Contrato Organizativo de Ação Pública (COAP - Public Action Organizational Contract) on the expansion of access to primary care in the states of Ceará and Mato Grosso do Sul. METHODS: We used the interrupted time series method to analyze the effect of COAP on primary care coverage (PCCov) and on avoidable hospitalization rates. To analyze the effects of increased PCCov on avoidable hospitalizations, we used non-segmented time series models. RESULTS: The results showed that implementing COAP had a positive impact on increased coverage in both cases, with did not happen in the control states. However, this impact was not reflected in the decrease in hospitalizations due to primary care sensitive conditions (HPCSC) or for acute preventable causes. When we analyzed the effects of the increase in PCCov on avoidable hospitalizations between 2009 and 2016, we observed that coverage had a positive impact on the decrease in the rate of HPCSC only in Ceará, although hospitalizations have a significant trend to decrease in time both in this state and in Mato Grosso do Sul, except for acute respiratory infections. CONCLUSIONS: The COAP continues to be the regulatory instrument of regionalization force, however, the results obtained by adhering to it in the expansion of primary care in Ceará and Mato Grosso do Sul makes us question whether the contractual model, as predicted, is the best instrument for advancing regionalization in the Brazilian Unified Health System.

Diabetes mellitus is one of the most prevalent diseases worldwide and is among the leading causes of loss of healthy years of life, which is aggravated in Brazil by accelerated population aging. This study aims to measure the problem of diabetes mellitus and its complications and characterize healthcare for diabetics in Brazil, according to regions. Prevalence rates were estimated using a multinomial regression model, and characterization of healthcare was based on triangulation between the Brazilian National Health Survey (PNS), the National Program for Improving Access and Quality in Primary Care (PMAQ-AB), and data from the Popular Pharmacy program. Diabetes prevalence in Brazil was 9.2%, according to the multinomial model, and prevalence in the corrected PNS (self-report + altered glycated hemoglobin - HbA1c ≥ 6.5) was 9.4%. The proportion of diabetes mellitus underreporting in the country was 42.5%, reaching 72.8% in the North. Among individuals diagnosed with diabetes mellitus, half presented HbA1c ≥ 6.5. Insufficient fundus eye examination (only 40% on average), with major regional variation (North 25% - Southeast 52%), is reflected in the high prevalence of retinopathy. Insufficient examination of feet (only 30%), can lead to more amputations. About 80% of diabetics used medications, indicating a persistently high proportion still without treatment. Healthcare deficiencies for diabetics lead to greater morbidity, hospitalizations (15%), and visits to emergency departments (27%, PMAQ). The scenario in 2012, although not ideal, occurred in a context of strengthening of the Brazilian Unified National Health System (SUS). The growing prevalence of diabetes mellitus and cutbacks in public health budgeting call for serious reflection on control of the disease in the coming years.


O diabetes mellitus é uma das doenças mundialmente mais prevalentes em adultos e está entre as principais causas de perda de anos de vida saudável, o que se agrava com o acelerado envelhecimento populacional no Brasil. Este estudo visa dimensionar o problema do diabetes mellitus e suas complicações e caracterizar a atenção à saúde do diabético no Brasil, segundo regiões. As prevalências foram estimadas utilizando modelo de regressão multinomial, e a caracterização da atenção à saúde se deu a partir da triangulação entre a Pesquisa Nacional de Saúde (PNS), o Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) e dados da Farmácia Popular. A prevalência de diabetes mellitus no Brasil foi de 9,2%, pelo modelo multinomial, e a prevalência da PNS corrigida (autorreferida + alterada na hemoglobina glicosilada - HbA1c ≥ 6,5) foi de 9,4%. A proporção de subnotificação do diabetes mellitus no país foi de 42,5%, chegando a 72,8% na Região Norte. Dentre os diagnosticados, mais da metade apresentou HbA1c ≥ 6,5. A insuficiente realização de exame de fundo de olho (40%), com ampla variação regional (Norte 25% - Sudeste 52%), reflete-se na alta prevalência de retinopatia. O exame dos pés apresentou baixa realização (30%), podendo levar a mais amputações. Cerca de 80% dos diabéticos usavam medicamentos, o que indica uma alta parcela ainda sem tratamento. Deficiências na atenção à saúde do diabético levam a maior morbidade, internações (15%) e idas a emergências (27% - PMAQ). O cenário apresentado em 2012, apesar de não ser ideal, deu-se num contexto de fortalecimento do Sistema Único de Saúde (SUS). Com a crescente prevalência de diabetes mellitus e cortes no investimento em saúde pública, cabe a reflexão sobre o controle da doença nos próximos anos.

COVID-19 is an acute infectious respiratory distress syndrome (ARDS) caused by the novel coronavirus SARS-CoV-2. The disease is highly communicable and produces mild to severe symptoms, generating a high demand for intensive care and thousands of deaths. In March 2020, COVID-19 was declared a pandemic and has already surpassed five million cases and 300,000 deaths in the world. The natural history of the disease has still not been fully established, hindering the elaboration of effective clinical protocols and preventive measures. Nevertheless, the disease requires a systemic approach, since there is evidence of acute and chronic complications, in addition to the catastrophic effects on the population's mental health. This highlights the need for a methodology that more effectively captures the effect of COVID-19, considering such aspects as severity, duration, and the potential to generate chronic complications that will increase the demands on Brazilian Unified National Health System (SUS). DALYs, or disability-adjusted life years, are thus an extremely useful indictor that adds mortality, an estimate of years of life lost (YLLs), and morbidity, an estimate of years of life lived with disability (YLDs). This article discusses the relevance and difficulties of studying the burden of COVID-19 and its complications in the Brazilian context, highlighting the natural history of the disease and estimating indicators such as YLDs, considering the high burden of disease in planning strategies to deal with the consequences of COVID-19 after the pandemic. The article also discusses the future challenges to deal with the disease in the SUS and the effects on the calculation of DALYs.


A COVID-19 é uma síndrome respiratória aguda grave (SRAG) infecciosa, causada por coronavírus. A doença tem alta transmissibilidade e ocasiona sintomas leves a graves, gerando elevada demanda por cuidados intensivos e milhares de óbitos. Em março de 2020, a COVID-19 foi caracterizada como pandemia e já soma mais de 5 milhões de casos e 300 mil óbitos pelo mundo. A história natural da doença ainda não é bem estabelecida, dificultando a elaboração de protocolos clínicos eficazes e medidas de prevenção. Apesar disso, pode-se afirmar que é uma doença de abordagem sistêmica, já que há evidências de complicações agudas e crônicas, além de efeitos catastróficos na saúde mental da população. Destaca-se então a necessidade de uma metodologia que capte de forma mais efetiva os efeitos da COVID-19, considerando aspectos como sua gravidade, duração e potencial de gerar complicações crônicas que aumentarão as demandas no Sistema Único de Saúde (SUS). Nesse sentido, é de extrema utilidade o indicador DALY, ou anos de vida perdidos por morte prematura ajustados por incapacidade (DALY), que agrega a (1) mortalidade - estimativa dos anos de vida perdidos (YLL) e (2) morbidade - estimativa dos anos vividos com incapacidade (YLD). Este artigo discute a relevância e as dificuldades de estudar a carga da COVID-19 e de suas complicações, no contexto brasileiro, ressaltando a importância de caracterizar a história natural da doença e estimar indicadores como o YLD, que considerem a alta carga de morbidade no planejamento de estratégias para lidar com as consequências da COVID-19 pós-pandemia. Discute-se também os desafios futuros para o enfrentamento da doença no SUS e reflexões sobre o cálculo do DALY.

La enfermedad posee una alta transmisibilidad y ocasiona de síntomas leves a graves, generando una elevada demanda de cuidados intensivos y millares de fallecimientos. En marzo de 2020, la COVID-19 se caracterizó como pandemia y ya suma más de 5 millones de casos y 300 mil fallecimientos por el mundo. La historia natural de la enfermedad todavía no ha sido bien establecida, dificultando la elaboración de protocolos clínicos eficaces y medidas de prevención. A pesar de eso, se puede afirmar que es una enfermedad de abordaje sistémico, ya que existen evidencias sobre complicaciones agudas y crónicas, además de efectos catastróficos en la salud mental de la población. Se destaca entonces la necesidad de una metodología que capte de forma más efectiva los efectos de la COVID-19, considerando aspectos como su gravedad, duración, potencial de generar complicaciones crónicas que aumentarán las demandas en el Sistema Único de Salud (SUS). En este sentido, es de extrema utilidad el indicador DALY o años de vida perdidos por muerte prematura ajustados por incapacidad, que agrega la (1) mortalidad - estimación de los años de vida perdidos (YLL) y (2) morbilidad - estimación de los años vividos con incapacidad (YLD). Este artículo discute la relevancia y las dificultades de estudiar la carga de la COVID-19 y sus complicaciones, en el contexto brasileño, resaltando la importancia de caracterizar la historia natural de la enfermedad y estimar indicadores como el YLD, que consideren la alta carga de morbilidad en la planificación de estrategias para lidiar con las consecuencias de la COVID-19 pospandemia. This study sought to evaluate the associations between health conditions and sociodemographic characteristics in relation to quality of life (QoL) in the Brazilian population; as well as to assess the main factors associated with higher chances of a better QoL among diabetics. This is a study with data from a population-based survey conducted in Brazil in 2008. A total of 12,423 Brazilians aged ≥ 20 years were analyzed, of which 935 were diabetic. QoL outcomes were measured using the summary components of the SF-36 instrument. QoL scores were classified as "above" and "below average" to obtain binary outcomes both for the general population and diabetics. Logistic regressions were performed to obtain prevalence ratios adjusted to the odds of above average QoL by adjusting for sociodemographic and health variables. Results indicate that female diabetics aged ≥ 65 years old, with a sedentary lifestyle, belonging to D/E social class and with more morbidities had greater chances of a worse physical and mental QoL. Regarding the general population, in addition to these factors, not having a partner and being illiterate were also responsible for worse QoL. Sedentary lifestyle and D/E social class had greater influence for worse QoL among diabetics when compared to the general population.


Buscou-se avaliar as associações entre condições de saúde e características sociodemográficas em relação à qualidade de vida (QV) na população brasileira; bem como estimar os principais fatores associados às chances de uma melhor QV entre diabéticos. Trata-se de um estudo com dados obtidos do inquérito de base populacional realizado no Brasil, em 2008. Foram analisados 12.423 brasileiros, com idade ≥ 20 anos, cujos 935 eram diabéticos. Os desfechos de QV foram mensurados por meio dos componentes sumários do instrumento SF-36. Tanto para a população geral quanto para diabéticos, a QV teve seus escores agrupados em "acima" e "abaixo da média" para configurar desfechos binários. Foram realizadas regressões logísticas para obtenção das razões de prevalências ajustadas às chances de QV acima da média, controlando por variáveis sociodemográficas e de saúde. Resultados apontaram que as mulheres diabéticas com ≥ 65 anos, sedentárias, pertencentes à classe D/E e com mais morbidades apresentam maiores chances de uma pior QV física e mental. Para a população geral além desses fatores, não ter um companheiro e ser analfabeto proporcionou uma pior QV. O sedentarismo e a classe D/E apresentaram maior influência para pior QV entre diabéticos quando comparado à população total.

The objective was to measure the Quality of Life (QoL) of the patients treated in Primary Health Care in the city of Rio de Janeiro and its own association with CMD, alcohol consumption and socio-demographic aspects. This is a cross-sectional study involving 624 patients in 2012/2013, using: General Health Questionnaire, Hospital Anxiety and Depression Scale, Screening for Somatoform Symptoms, Alcohol Use Disorder Identification Test e World Health Organization Quality of Life Instrument (bref version). There were conducted a bivariate analysis and a multiple linear regressions for each domain of QOL. The QoL score for the domains, physical, psychological, social relationships and environment were: 61,2; 62,6; 66 and 50,9. In multivariate analysis, the QOL was negatively associated to the CMD, especially in the psychological domain (ß = -15,75; p-value = 0,00), and the dependence on physical (ß = -5,38; p-value = 0,05). There was a positive and significant association of the QoL with the risk consumption (ß = 5,77) and the harmful consumption (ß = 6,15) in the environment domain, and with the first in the psychological domain (ß = 7,08). CMD and alcohol dependence are associated with the loss of QOL, but other patterns of consumption, even being harmful are associated with higher QOL.


Objetivou-se mensurar a Qualidade de Vida (QV) de pacientes da Atenção Primária em Saúde do município do Rio de Janeiro e verificar sua associação com Transtornos Mentais Comuns (TMC), uso de álcool e aspectos sociodemográficos. Trata-se de um estudo transversal com 624 pacientes em 2012/2013 e aplicação dos instrumentos: "General Health Questionnaire", "Hospital Anxiety and Depression Scale", "Screening for Somatoform Symptoms", "Alcohol Use Disorder Identification Test" e "World Health Organization Quality of Life Instrument (bref version)". Realizou-se análise bivariada (Teste-t) e regressões lineares múltiplas para cada domínio de QV. Os escores QV para os domínios físico, psicológico, relações sociais e meio-ambiente foram: 61,2; 62,6; 66 e 50,9. Na análise multivariada, a QV associou-se negativamente aos TMC, principalmente no domínio psicológico (ß = -15,75; p-valor = 0,00), e a dependência no domínio físico (ß = -5,38; p-valor = 0,05). Houve associação positiva e significativa da QV com consumo de risco (ß = 5,77) e nocivo (ß = 6,15) no domínio meio ambiente, e com o primeiro no domínio psicológico (ß = 7,08). TMC e a dependência de álcool estão associados à perda da QV, porém outros padrões de consumo, mesmo sendo nocivos, se associaram a maior QV.

This article aims to describe the inappropriate use of medicines in the Brazilian urban population and to identify associated factors. We conducted a data analysis of a household survey carried out in Brazil in 2013-14. The sampling plan was done by clusters with representativeness of the urban population and large regions of the country, according to gender and age domains. For this analysis, we considered a sample of adults (≥20 years) who reported having chronic non-communicable diseases, medical indication for drug treatment and medicine use (n = 12 283). We evaluated the prevalence of inappropriate use in the domains: non-adherence, inappropriate use behaviour and inadequate care with medicines, all verified in the following groups of independent variables: demographic and socio-economic characteristics, health and pharmaceutical care, health status and use of medicines. Crude and adjusted prevalence ratios were obtained using robust Poisson regression. It was found 46.1% of people having at least one behaviour of inappropriate use of medicines. The worst results were found for the domain of inappropriate use behaviour, a situation of 36.6% of the users, which included unauthorized prescriber, inadequate source of information and indication of the medicines by non-authorized prescribers. The best result was found for the lack of medicines care, informed by only 4.6% of users who kept expired drugs at home. The inappropriate use of medicines was associated with gender (female), region of residence (Northeast), not visiting the doctor regularly or visiting more than one doctor, not having free access to medicines and using of five or more medicines. There was a high prevalence of inappropriate use, which was associated with both individual and health system characteristics pointing out the need to set priorities as for health education and public interventions.

OBJECTIVE: To evaluate trends in the use of generic and non-generic medicines to treat hypertension and diabetes under the Farmácia Popular Program (FP) and its impact on generic medicines sales volume and market share in the Brazilian pharmaceutical market. METHODS: This longitudinal, retrospective study used interrupted time series design to analyze changes in monthly sales volume and proportion of medicines sales (market share) for oral antidiabetic and antihypertensive medicines for generic versus non-generic products. Analyses were conducted in a combined dataset that aggregate monthly sales volumes from the Farmácia Popular program and from the QuintilesIMS™ (IQVIA) national market sales data from January 2007 to December 2012. The Farmácia Popular program phases analyzed included: a) 2009 reductions in medicines reference prices (AFP-II) and b) 2011 implementation of free medicines program for hypertension and diabetes, the Saúde não tem preço (SNTP - Health has no price). RESULTS: Patterns of use for FP-covered antidiabetic and antihypertensive medicines were similar to their use in the market in general. After one year of the decreases in government subsidies in April 2010, market share of antidiabetic and antihypertensive medicines experienced relative declines of -54.5% and -59.9%, respectively. However, when FP-covered medicines were made free to patients, overall market volume for antidiabetic and antihypertensive generics increased dramatically, with 242.6% and 277.0% relative increases by February 2012, as well as non-generics with relative increase of 209.7% and 279% for antidiabetic and antihypertensive medicines, respectively. CONCLUSIONS: Ministry of Health policies on the amount of patient cost sharing and on the choice of medicines on coverage lists have substantial impacts on overall generic sales volume in retail pharmacies.

ABSTRACT INTRODUCTION Lung cancer (LC) is a relevant public health problem in Brazil and worldwide, given its high incidence and mortality. Thus, the objective of this study is to analyze the distribution of smoking and smoking status according to sociodemographic characteristics and disparities in access, treatment, and mortality due to LC in Brazil in 2013 and 2019. METHOD Retrospective study of triangulation of national data sources: a) analysis of the distribution of smoking, based on the National Survey of Health (PNS); b) investigation of LC records via Hospital-based Cancer Registry (HCR); and c) distribution of mortality due to LC in the Mortality Information System (SIM). RESULTS There was a decrease in the percentage of people who had never smoked from 2013 (68.5%) to 2019 (60.2%) and in smoking history (pack-years). This was observed to be greater in men, people of older age groups, and those with less education. Concerning patients registered in the HCR, entry into the healthcare service occurs at the age of 50, and only 19% have never smoked. While smokers in the population are mainly Mixed-race, patients in the HCR are primarily White. As for the initial stage (I and II), it is more common in White people and people who have never smoked. The mortality rate varied from 1.00 for people with higher education to 3.36 for people without education. Furthermore, White people have a mortality rate three times higher than that of Black and mixed-race people. CONCLUSION This article highlighted relevant sociodemographic disparities in access to LC diagnosis, treatment, and mortality. Therefore, the recommendation is to strengthen the Population-Based Cancer Registry and develop and implement a nationwide LC screening strategy in Brazil since combined prevention and early diagnosis strategies work better in controlling mortality from the disease and continued investment in tobacco prevention and control policies.


RESUMO INTRODUÇÃO O câncer de pulmão (CP) é um relevante problema de saúde pública no Brasil e no mundo, dada sua alta incidência e mortalidade. Assim, objetiva-se analisar a distribuição do tabagismo e da carga tabágica segundo características sociodemográficas e disparidades no acesso, no tratamento e na mortalidade por CP no Brasil, em 2013 e 2019. MÉTODO Estudo retrospectivo de triangulação de fontes de dados de abrangência nacional: a) análise da distribuição do tabagismo, baseada na Pesquisa Nacional de Saúde (PNS); b) investigação dos registros de CP, via Registros Hospitalares de Câncer (RHC); e c) distribuição da mortalidade por CP, no Sistema de Informação sobre Mortalidade (SIM). RESULTADOS Verificou-se redução do percentual de pessoas que nunca fumaram de 2013 (68,5%) para 2019 (60,2%), assim como da carga tabágica (anos-maço). Esta foi observada maior em homens em pessoas de faixas etárias mais avançadas e de menor escolaridade. Em relação aos pacientes registrados no RHC, a entrada no serviço de saúde se dá a partir de 50 anos, e apenas 19% nunca fumaram. Ao passo que os fumantes na população são majoritariamente pardos, os pacientes no RHC são em maioria brancos. Quanto ao estadiamento inicial (I e II), é mais frequente em pessoas brancas e que nunca fumaram. A taxa de mortalidade apresentou variação de 1,00, para pessoas com ensino superior, a 3,36, entre pessoas sem instrução, assim como pessoas brancas têm uma taxa de mortalidade três vezes maior que a de pessoas negras e pardas. CONCLUSÃO Este artigo apontou relevantes disparidades sociodemográficas no acesso ao diagnóstico, tratamento e mortalidade do CP. Assim, recomenda-se: fortalecer o Registro de Câncer de Base Populacional; desenvolver e implementar estratégia de screening de CP no Brasil, uma vez que a realização de estratégias de prevenção e diagnóstico precoce combinadas funcionam melhor no controle da mortalidade pela doença; e investimento contínuo nas políticas de prevenção e controle do tabagismo.

INTRODUCTION: Our aims in this study were to determine the prevalence of malocclusion stemming from vertical or transversal occlusal alterations in subjects with Down syndrome (DS) and the associations with individual, socioeconomic, and behavioral factors. METHODS: A cross-sectional study was carried out with 112 pairs of mothers and their children with DS between 3 and 18 years of age attending a genetics clinic at a public hospital in Rio de Janeiro, Brazil. Data were collected with a questionnaire given to the mothers and a clinical examination of the child or adolescent. Univariate, bivariate, and multiple logistic regression (backward stepwise) analyses were conducted. RESULTS: The results showed a prevalence of malocclusion in 74% of the sample. After the adjustment of the logistic regression model, these variables--age, nail or finger biting habit, mouth posture, and cold or sore-throat episodes in the previous 6 months--were associated with the prevalence of malocclusions in these subjects. CONCLUSIONS: The data demonstrate a high prevalence of malocclusions stemming from vertical or transversal occlusal alterations in children and adolescents with DS. Age, nail or finger biting, mouth posture, and upper airway infections were related to malocclusions in these patients.

OBJECTIVE: From a perspective of comprehensive care, the purpose of the study was to evaluate factors associated to dental care provided to Down syndrome children and adolescents. METHODS: A cross-sectional study was carried out including 112 pairs of mothers/Down syndrome children aged between 3 and 18 years who attended a public hospital genetics clinic in Rio de Janeiro, Southeastern Brazil, in 2006. Dental care was not provided at the clinic. Data were collected through a questionnaire administered to the mothers and oral examinations of their children. Multiple logistic regression was used for data analysis. The dependent variable was "dental care of the Down syndrome child or adolescent" and the independent variables included demographic, socioeconomic and behavioral characteristics. RESULTS: Most children (79.5%) had had at least one dental visit (90% CI: 72.3; 87.8). Dental experience of the children was associated to the following variables: mothers who reported being advised by their children's health provider to take them to the dentist's (OR=6.1 [2.5; 15.1]); children with prior history of surgery (OR=2.5 [0.9; 7.1]); and age between 12 and 18 years (OR=13.1 [2.0; 86.9]). CONCLUSIONS: Dental care provided to Down syndrome children and adolescents was associated to advice given by their health providers, a part of comprehensive care.

Resumo: Este estudo descreveu e comparou dados de tuberculose (TB) entre pessoas privadas de liberdade e população geral brasileira, de 2007 a 2019, utilizando a ferramenta Joinpoint para observação de mudanças de tendências. Apresenta um recorte para mulheres e idosos, para testagem para HIV e para número de custodiados por vaga. Trata-se de um estudo retrospectivo, quantitativo e analítico, que utiliza métodos de regressão de dados de séries temporais a partir de dados secundários de acesso irrestrito coletados do Sistema de Informação de Agravos de Notificação (SINAN), do Instituto Brasileiro de Geografia e Estatística (IBGE) e de relatórios analíticos disponibilizados pelo Departamento Penitenciário Nacional (DEPEN). Os resultados retratam aumento da prevalência de TB consideravelmente maior em pessoas privadas de liberdade em todas as perspectivas analisadas; aumento nas testagens para HIV; e discutível tendência de estabilidade na quantidade de custodiados por vaga. Ao se analisar tendências de prevalências, serviços e determinantes, é curioso ver a não coincidência temporal na maioria dos casos. Ficou claro que as políticas nacionais de combate à TB não têm o mesmo efeito dentro das prisões e mesmo a Política Nacional de Atenção Integral à Saúde das Pessoas Privadas de Liberdade no Sistema Prisional (PNAISP) mostrou efeitos restritos diante da situação de saúde aqui analisada. Apesar de trabalhar com dados secundários de confiabilidade variável, alcançaram-se comparações que podem impactar decisões e ações de saúde. Ainda que carente de respostas completas e definitivas, pôde-se lançar um novo olhar à evolução de questões sobre as quais a reflexão é imprescindível.


Abstract: This study describes and compares tuberculosis (TB) data among persons deprived of liberty and the general Brazilian population, from 2007 to 2019, using the Joinpoint tool to observe changes in trends. This study focuses on women and older adults, for HIV testing, and on the number of detainees according to prison capacity. This is a retrospective, quantitative, and analytical study, which uses methods of regression of time series data from secondary data of unrestricted access collected from the Brazilian Information System for Notifiable Diseases (SINAN), Brazilian Institute of Geography and Statistics (IBGE), and from analytical reports made available by the Brazilian National Penitentiary Department (DEPEN). The results show a considerably higher increase in the prevalence of TB in persons deprived of liberty in all perspectives analyzed; increased HIV testing; and a debatable trend of stability in the number of detainees according to prison capacity. When analyzing trends in prevalence, services, and determinants, it is curious to see the temporal non-coincidence in most cases. Clearly, national policies against TB do not have the same effect within prisons and even the National Policy for Comprehensive Health Care for People Deprived of Liberty in the Prison System (PNAISP) showed restricted effects in view of the health situation herein analyzed. Despite working with secondary data of variable reliability, comparisons were reached that can impact health decisions and actions. Although lacking complete and definitive answers, it was possible to launch a new point-of-view on the evolution of questions for which reflection is essential.


Resumen: El presente estudio describió y comparó los datos de tuberculosis (TB) entre las personas privadas de libertad y la población general brasileña, de 2007 a 2019, utilizando la herramienta Joinpoint para observar los cambios en las tendencias. Presenta un límite para mujeres y ancianos, para las pruebas para VIH y para el número de internos por vacante. Se trata de un estudio retrospectivo, cuantitativo y analítico, que utiliza métodos de regresión de datos de series temporales a partir de datos secundarios de acceso no restringido recolectados del Sistema de Información de Agravios de Notificación (SINAN), Instituto Brasileño de Geografía y Estadística (IBGE) e informes analíticos disponibles por el Departamento Penitenciario Nacional (DEPEN). Los resultados muestran un aumento de la prevalencia de TB considerablemente mayor en personas privadas de libertad en todas las perspectivas analizadas; un aumento en las pruebas de VIH, y una tendencia discutible a la estabilidad en la cantidad de internos por vacante. Al analizar las tendencias de las prevalencias, los servicios y determinantes, es curioso ver la no coincidencia temporal en la mayoría de los casos. Quedó claro que las políticas nacionales de combate a la TB no tienen el mismo efecto dentro de las prisiones y incluso la Política Nacional de Atención Integral a la Salud de las Personas Privadas de Libertad en el Sistema Penitenciario (PNAISP) mostró efectos restringidos frente a la situación de salud aquí analizada. A pesar de trabajar con datos secundarios de confiabilidad variable, se lograron comparaciones que pueden afectar las decisiones y acciones de salud. Aunque se carezca de respuestas completas y definitivas, se ha podido dar una nueva mirada a la evolución de cuestiones sobre los que es imprescindible reflexionar.
