RESUMEN

The nitrate content of a wide variety of vegetables, fruits and meat purchased in Singapore were determined by high-pressure liquid chromatography. From the 1982 statistical figures of retained imports and local production of these items the average per capita daily consumption of nitrates was calculated and found to be 215 mg, one of the highest reported figures in the world. Two vegetables are not so popular with the Malays and Indians who comprise 21% of the population. If their consumption of these vegetables is ignored then the per capita daily nitrate consumption of the Chinese population amounts to 250 mg and those of the Malays and Indians to 113 mg. Although these may be overestimates they do appear to show an association with the male gastric cancer incidence rates (age-adjusted) of 44.8 for the Chinese, 10.3 for Malays and 19.4 for the Indians. This could, however, be a chance association.

RESUMEN

50 unselected cases of bleeding duodenal ulcer were treated by vagotomy and pyloroplasty with suturing of the bleeding vessels. The operative mortality was 8% of which recurrent haemorrhage accounted for 6% of the patients. The operative mortality was related to advanced age, severity of bleed and associated medical illnesses. Vagotomy-pyloroplasty with suture ligation is technically easy to perform for bleeding duodenal ulcer and has a low incidence of recurrent haemorrhage.

RESUMEN

The racial distribution and pathology of 115 colorectal polyps from 72 patients were analysed. There were 88 (77%) adenomas. 90.9% of these were from Chinese and none from Malay patients. 79% were found in the sigmoid colon and rectum. The racial and site distributions of adenomas reflect the patterns of carcinoma seen in Singapore. The management of adenoma was discussed. Annual surveillance for high-risk and three yearly surveillance of low-risk patients up to an arbitrary age of 75 years is recommended.

RESUMEN

115 cases of acute calculous cholangitis were analysed for their presentations, treatment and complications. There were slightly more Chinese males than females (1.4:1) and the mean age was 62 years old. Though pain was the commonest symptom (95.7%), only 35.7% of patients had complete Charcot's triad. 21.7% presented in septicaemic shock. E. Coli and Klebsiella were the commonest organisms isolated. All patients had cholecystectomy and exploration of common bile ducts. Twenty patients had additional choledochontrostomy performed. 50.4% of patients developed complications and the overall mortality was 16.5%. Patients over 60 presented in shock carried a significantly higher mortality (40%).
