OBJECTIVE: To explore the relationship between age, education, sex, and ApoE4 (+) status to brain volume among a cohort with amnestic mild cognitive impairment (aMCI). METHOD: One hundred and twenty-three participants were stratified into Hispanic (n = 75) and White non-Hispanic (WNH, N = 48). Multiple linear regression analyses were conducted with age, education, sex, and ApoE4 status as predictor variables and left and right combined MRI volumes of the hippocampus, parahippocampus, and entorhinal cortex as dependent variables. Variations in head sizes were corrected by normalization with a total intracranial volume measurement. RESULTS: Bonferroni-corrected results indicated that when controlling for ApoE4 status, education, and age, sex was a significant predictor of hippocampal volume among the Hispanic group (ß = .000464, R2 = .196, p < .01) and the WNH group (ß = .000455, R2 = .195, p < .05). Education (ß = .000028, R2 = .168, p < .01) and sex (ß = .000261, R2 = .168, p < .01) were significant predictors of parahippocampal volume among the Hispanic MCI group when controlling for the effects of ApoE4 status and age. One-way ANCOVAs comparing hippocampal and parahippocampal volume between males and females within groups revealed that females had significantly larger hippocampal volumes (p < .05). Hispanic females had significantly larger hippocampal (p < .001) and parahippocampal (p < .05) volume compared to males. No sex differences in parahippocampal volume were noted among WNHs. CONCLUSIONS: Biological sex, rather than ApoE4 status, was a greater predictor of hippocampal volume among Hispanic and WNH females. These findings add to the mixed literature on sex differences in dementia research and highlight continued emphasis on ethnic populations to elucidate on neurodegenerative disparities.

Recent research outcomes suggest that social and economic conservatism show divergent associations with external criteria. In this article, we present a quantitative study with 534 Colombian participants. We found that, although positively correlated, these two dimensions of conservative ideology also exhibit suppression effects and differentiated psychological profiles regarding moral foundations and moral absolutism. Specifically, controlling for economic conservatism increased the positive association between social conservatism and the binding moral foundations and moral absolutism, while controlling for social conservatism changed the signs of the initial positive associations between economic conservatism and these variables. Our results suggest the need to use independent measures for each dimension of ideology and test for suppression when evaluating their respective interaction with other psychological constructs.

The increasing prevalence of AD among Hispanics calls for a need for examining factors that affect cognitive functioning and risk of AD among Hispanic older adults. The current study examined cognitive functioning among older Hispanic adults living in the U.S. from two Hispanic regions, South America and the Caribbean, in relation to the country where education was obtained. Participants (n = 139) were stratified into groups based on Hispanic education region and diagnostic categories: cognitively normal and amnestic MCI (aMCI). Results of Pearson correlations showed that among Hispanic Americans in general, there were significant positive correlations between the country of education to performance on measures of episodic, verbal, and word list tests. When examined separately by region and diagnosis, only cognitively normal (CN) South Americans showed significant relationships between country of education and cognitive functioning in these areas. Results of general linear models controlling for education identified differences in neuropsychological performance between groups with the CN groups demonstrating better performance than the aMCI groups within each region. Overall, it was evident that relationships between years of education obtained outside of the U.S. and cognitive functioning were not similar among individuals from these two disparate Spanish speaking regions. This is the first study to examine the country where education was obtained among individuals from countries located in different regions with different cultures that may influence their education and cognitive development throughout life. Findings contribute to the cross-cultural neuropsychological literature in understanding factors that are unique to Hispanic older adults at risk for developing AD.

BACKGROUND: Following the 2016 Peace Agreement with the Fuerzas Armadas Revolucionarias de Colombia (FARC), Colombia promised to reincorporate more than 13,000 guerrilla fighters into its healthcare system. Despite a subsidized healthcare insurance program and the establishment of 24 Espacios Territoriales de Capacitación y Reincorporación (ETCRs-Territorial Spaces for Training and Reintegration) to facilitate this transition, data has shown that FARC ex-combatants access care at disproportionately lower rates, and face barriers to healthcare services. METHODS: Semi-structured interviews were conducted with FARC health promoters and healthcare providers working in ETCRs to determine healthcare access barriers for FARC ex-combatants. Analysis was completed with a qualitative team-based coding method and barriers were categorized according to Julio Frenk's Domains of Healthcare Access framework. RESULTS: Among 32 participants, 25 were healthcare providers and 7 self-identified as FARC health promoters. The sample was majority female (71.9%) and worked with the FARC for an average of 12 months in hospital, health center, medical brigade, and ETCR settings. Our sample had experiences with FARC across 16 ETCRs in 13 Departments of Colombia. Participants identified a total of 141 healthcare access barriers affecting FARC ex-combatants, which affected healthcare needs, desires, seeking, initiation and continuation. Significant barriers were related to a lack of resources in rural areas, limited knowledge of the Colombian health system, the health insurance program, perceived stigma, and transition process from the FARC health system. CONCLUSIONS: FARC ex-combatants face significant healthcare access barriers, some of which are unique from other low-resource populations in Colombia. Potential solutions to these barriers included health insurance provider partnerships with health centers close to ETCRs, and training and contracting FARC health promoters to be primary healthcare providers in ETCRs. Future studies are needed to quantify the healthcare barriers affecting FARC ex-combatants, in order to implement targeted interventions to improve healthcare access.

OBJECTIVES: To assess the safety and effectiveness of percutaneous endobiliary radiofrequency ablation (EB-RFA) in the management of refractory benign biliary strictures. MATERIALS AND METHODS: Percutaneous EB-RFA was performed in 15 individuals (M/F = 8/7; median age: 57 [33-84]) for benign biliary strictures resistant to traditional methods (transhepatic cholangioplasty and biliary drains). All patients underwent ≥1 unsuccessful cholangioplasty session and upsizing of their transhepatic biliary drains pre-procedure. Technical and clinical success were defined as luminal gain with enhanced flow and a lack of clinically evident recurrent stricture on follow-up after drain/stent removal, respectively. RESULTS: A total of 16 EB-RFA procedures were performed. Technical success rate was 100% (16/16). Procedure-related complications occurred in 1/16 cases (drain leakage with subsequent cellulitis). Clinical success rate was 87% (13/15) with a median follow-up of 17 (2-24) months. Drain/stent was not removed in one case (1/16) as the patient was lost to follow-up immediately post-procedure. The 1-year patency rate was 100%. A significant reduction was observed in the median number of IR visits (8 [1-51] to 1 [0-9]; p = 0.003) and drain insertion/exchange procedures (5 [1-45] to 0 [0-6]; p = 0.003) pre- and post-EB-RFA with a median follow-up of 18 (0-26) months. CONCLUSION: Percutaneous EB-RFA can safely and effectively treat refractory benign biliary strictures. However, larger prospective studies with extended follow-ups are needed to gather more robust data. ADVANCES IN KNOWLEDGE: This study contributes to the limited evidence on the role of EB-RFA in addressing refractory benign biliary strictures, enhancing the understanding of its clinical utility.

INTRODUCTION: Poorer baseline functioning is associated with long-term cognitive decline among Hispanic older adults, but little is known about associations of these factors with Alzheimer's disease (AD) neuroimaging biomarkers. METHODS: A total of 461 Hispanic and White non-Hispanic (NHW) older adults who are cognitively normal (n = 76), had impaired cognition without mild cognitive impairment (MCI) (n = 41), or carried a diagnosis of MCI (n = 253) or dementia (n = 91) completed neuropsychological and functional assessment, genetic testing, and brain magnetic resonance imaging (MRI). Structural equation modeling (SEM) was used to examine predictive associations between functional and cognitive measures of AD neuroimaging biomarkers. RESULTS: MRI volumes significantly predicted functional limitations in both groups. Sex and amyloid load significantly predicted functional limitations among the Hispanic group only. Years of education and MRI regional volume were the strongest predictors of cognition among both groups. DISCUSSION: Results indicate that functional performance is associated with early AD biomarkers among Hispanic older adults. Clinical implications are discussed. Highlights: The current study addresses health disparities in Alzheimer's disease (AD) and related dementia assessment among Hispanics by identifying measures sensitive to early AD biomarkers.Associations of functional measures with AD genetic and neuroimaging biomarkers revealed that similarities in these associations exist between Hispanic and White non-Hispanic individuals, but biological sex and amyloid load significantly predicted functional limitations among the Hispanic group only.These results have clinical implications for physicians who treat Hispanic AD patients and indicate that when compared to traditional diagnostic assessments, functional assessments may better aid in AD diagnostic precision among Hispanics.

PURPOSE: This study aimed to present the institutional experience and algorithm for performing biliary interventions in liver transplant patients using the modified Hutson loop access (MHLA) and the impact of percutaneous endoscopy via the MHLA on these procedures. METHODS: Over 13 years, 201 MHLA procedures were attempted on 52 patients (45 liver transplants; 24 living and 21 deceased donors) for diagnostic (e.g., cholangiography) and therapeutic (e.g., stent/drain insertion and cholangioplasty) purposes. The most common indications for MHLA were biliary strictures (60%) and bile leaks (23%). Percutaneous endoscopy was used to directly visualize the biliary-enteric anastomosis, diagnose pathology (e.g., ischemic cholangiopathy), and help in biliary hygiene (removing debris/casts/stones/stents) in 138/201 (69%) procedures. Technical success was defined as cannulating the biliary-enteric anastomosis and performing diagnostic/therapeutic procedure via the MHLA. RESULTS: The technical success rate was 95% (190/201). The failure rate among procedures performed with and without endoscopy was 2% (3/138) versus 13% (8/63) (P = 0.0024), and the need for new transhepatic access (to aid the procedure) was 12% (16/138) versus 30% (19/63) (P = 0.001). Despite endoscopy, failure in 2% of the cases resulted from inflamed/friable anastomosis (1/3) and high-grade stricture (2/3) obstructing retrograde cannulation of biliary-enteric anastomosis. Major adverse events (bowel perforation and injury) occurred in 1% of the procedures, with no procedure-related mortality. CONCLUSIONS: MHLA-based percutaneous biliary intervention is a safe and effective alternative to managing complications after liver transplant. Percutaneous endoscopy via the MHLA improves success rates and may reduce the need for new transhepatic access. Level of Evidence Level 4.

Autism spectrum disorder (ASD) involves social communication difficulties and repetitive behaviors, and it has a growing prevalence worldwide. Symptoms include cognitive impairments, gastrointestinal (GI) issues, feeding difficulties, and psychological problems. A significant concern in ASD is food selectivity, leading to nutrient deficiencies. Common GI issues in ASD, such as constipation and irritable bowel syndrome, stem from abnormal gut flora and immune system dysregulation. Sensory sensitivities and behavioral challenges exacerbate these problems, correlating with neurological symptom severity. Children with ASD also exhibit higher oxidative stress due to low antioxidant levels like glutathione. Therapeutic diets, including ketogenic, high-antioxidant, gluten-free and casein-free, and probiotic-rich diets, show potential in managing ASD symptoms like behavior, communication, GI issues, and oxidative stress, though the evidence is limited. Various studies have focused on different populations, but there is increasing concern about the impact among children. This review aims to highlight the food preferences of the ASD population, analyze the effect of the physicochemical and nutritional properties of foods on the selectivity in its consumption, GI problems, and antioxidant deficiencies in individuals with ASD, and evaluate the effectiveness of therapeutic diets, including diets rich in antioxidants, gluten-free and casein-free, ketogenic and essential fatty acids, and probiotic-rich diets in managing these challenges.

Natural cyclodextrins (CDs) can be formed by 6, 7, or 8 glucose molecules (α-, ß-, and γ-, respectively) linked in a ring, creating a cone shape. Its interior has an affinity for hydrophobic molecules, while the exterior is hydrophilic and can interact with water molecules. This feature has been used to develop active packaging applied to food, interacting with the product or its environment to improve one or more aspects of its quality or safety. It also provides monitoring information when food is optimal for consumption, as intelligent packaging is essential for the consumer and the merchant. Therefore, this review will focus on discerning which packaging is most appropriate for each situation, solubility and toxicological considerations, characterization techniques, effect on the guest properties, and other aspects related to forming the inclusion complex with bioactive molecules applied to packaging.

With the Peace Agreement between Fuerzas Armadas Revolucionarias de Colombia (FARC), Colombia promised healthcare to 13,000 'reincorporating' FARC ex-combatants. Shortages of healthcare workers in reincorporation camps means this promise is in danger of going unfulfilled. More information is needed to determine incentives, disincentives, and recruitment of healthcare providers to address this shortage. Semi-structured interviews were conducted with healthcare providers across FARC reincorporation camps, and a multidisciplinary team conducted analysis in NVivo12 using a team-based coding method. Twenty-four healthcare professionals from 15 camps participated, of which 75% were female. Incentives to work with FARC included improved clinical skills, professional advancement, increased comfort with FARC, and contributing to the peace process. Disincentives included poor living conditions, lack of support, biases, familial commitments, and sacrificing career opportunities. Three-fourths of the sample recommended working with FARC, and 92% reported a shortage of healthcare workers. Recruitment strategies included improved resources and specialised career development for healthcare workers, facilitating interactions between FARC and healthcare professionals outside clinical scenarios, and integrating medicine for vulnerable populations into health education. This study shows the impact that working with FARC ex-combatants can have on healthcare providers and tangible suggestions for increasing provider participation to address the healthcare worker shortage.

INTRODUCTION: Globally, medical students have demonstrated knowledge gaps in emergency care and acute stabilization. In Colombia, new graduates provide care for vulnerable populations. The World Health Organization (WHO) Basic Emergency Care (BEC) course trains frontline providers with limited resources in the management of acute illness and injury. While this course may serve medical students as adjunct to current curriculum, its utility in this learner group has not been investigated. This study performs a baseline assessment of knowledge and confidence in emergency management taught in the BEC amongst medical students in Colombia. METHODS: A validated, cross-sectional survey assessing knowledge and confidence of emergency care congruent with BEC content was electronically administered to graduating medical students across Colombia. Knowledge was evaluated via 15 multiple choice questions and confidence via 13 questions using 100 mm visual analog scales. Mean knowledge and confidence scores were compared across demographics, geography and prior training using Chi-Squared or one-way ANOVA analyses. RESULTS: Data were gathered from 468 graduating medical students at 36 institutions. The mean knowledge score was 59.9% ± 23% (95% CI 57.8-62.0%); the mean confidence score was 59.6 mm ±16.7 mm (95% CI 58.1-61.2). Increasing knowledge and confidence scores were associated with prior completion of emergency management training courses (p<0.0001). CONCLUSION: Knowledge and confidence levels of emergency care management for graduating medical students across Colombia demonstrated room for additional, specialized training. Higher scores were seen in groups that had completed emergency care courses. Implementation of the BEC as an adjunct to current curriculum may serve a valuable addition.

OBJECTIVES: In the 2016 Peace Accord with the Fuerzas Armadas Revolucionarias de Colombia (FARC), Colombia promised to reincorporate 14,000 ex-combatants into the healthcare system. However, FARC ex-combatants have faced significant challenges in receiving healthcare, and little is known about physicians' abilities to address this population's healthcare needs. METHODS: An electronic questionnaire sent to the Colombian Emergency Medicine professional society and teaching hospitals assessed physicians' knowledge, attitudes, and experiences with the FARC ex-combatant reincorporation process. RESULTS: Among 53 participants, most were male (60.4%), and ∼25% were affected by the FARC conflict (22.6%). Overall knowledge of FARC reincorporation was low, with nearly two-thirds of participants (61.6%) scoring in the lowest category. Attitudes around ex-combatants showed low bias. Few physicians received training about reincorporation (7.5%), but 83% indicated they would like such training. Twenty-two participants (41.5%) had identified a patient as an ex-combatant in the healthcare setting. Higher knowledge scores were significantly correlated with training about reincorporation (r = 0.354, n = 53, P = 0.015), and experience identifying patients as ex-combatants (r = 0.356, n = 47, P = 0.014). CONCLUSION: Findings suggested high interest in training and low knowledge of the reincorporation process. Most physicians had low bias, frequent experiences with ex-combatants, and cared for these patients when they self-identify. The emergency department (ED) serves as an entrance into healthcare for this population and a potential setting for interventions to improve care delivery, especially those related to mental healthcare. Future studies could evaluate effects of care delivery following training on ex-combatant healthcare reintegration.

Histoplasmosis is a fungal disease usually occurring in endemic areas that can affect immuno-impaired patients in whom pulmonary involvement is the rule. We present the case of an 18 year-old immunocompetent, male patient, resident of the State of Florida, who showed signs of mononucleosis syndrome that included odynophagia, cervical adenomegaly, sporadic fever and rash; however, no pulmonary involvement or visceromegaly were present. Faced with this atypical and unexpected clinical picture, histoplasmosis infection was eventually diagnosed following cervical lymph-node biopsy. Disseminated histoplasmosis may have unexpected manifestations, as is pointed out in the case described below.

BACKGROUND: Patients undergoing liver transplant have worse outcomes in the presence of pulmonary hypertension. Correlation between echocardiography and catheterization derived pressures in this population is not well studied. Our study's aim is to show the relationship between pulmonary artery systolic pressure derived from transthoracic echo (ePASP) with pulmonary artery systolic pressure measured during right heart catheterization (cPASP). METHODS: Single center retrospective study, patients being evaluated for liver transplant (n = 31) who had an interpretable Doppler signal for ePASP and had right heart catheterization (RHC) measurements within 3 months constituted the study group. Control group (n = 49) consisted of patients who did not have liver disease. RESULTS: There was modest correlation between ePASP and cPASP (R = 0.58, p < 0.001) in LT candidates (n = 31) compared with the control group (R = 0.74, p < 0.001, n = 49). The 95% limits of agreement by Bland-Altman analysis ranged from +33.6 mmHg to -21.7 mmHg. Using receiver operating characteristic analysis, ePASP cut-off > 47 mmHg was 59% sensitive and 78% specific to diagnose pulmonary artery (PA) hypertension (mean PA pressure > 25 mmHg) in the LT candidates, while a similar cutoff performed well in the control group (cutoff > 43 mmHg, n = 47, 91% sensitive, 100% specific). CONCLUSIONS: Compared with other disease states, ePASP correlates modestly with cPASP in patients with advanced liver disease. A higher ePASP cutoff should be used to screen for pulmonary hypertension. A multi-center prospective study with simultaneous transthoracic echocardiography and RHC measurements is required to determine the best cut-off in this population.

Introducción. La baja agudeza es un problema de salud pública en el mundo, que aumenta año tras año, relacionada con factores comportamentales poco saludables como el sedentarismo. La baja agudeza afecta especialmente a escolares, en quienes la visión representa uno de los sistemas más importantes para su desarrollo, tanto para el aprendizaje como para las relaciones sociales. El objetivo es establecer asociación entre baja agudeza visual y características sociodemográficas y comportamentales en una población escolar de la ciudad de Bucaramanga, Colombia. Metodología. Estudio observacional de corte transversal basado en datos recolectados de encuestas a estudiantes de una institución educativa de Bucaramanga entre los años 2006 y 2017, sobre las cuales se realizó un análisis bivariado entre características sociodemográficas y comportamentales con baja agudeza visual. Resultados. La prevalencia de baja agudeza visual fue de 33.57%, con mayor prevalencia de baja agudeza visual en mujeres y aquellos con mayor índice de masa corporal. La agudeza visual baja no tuvo asociación estadísticamente significativa con las otras variables estudiadas. Discusión. Las mujeres tienen una mayor prevalencia de baja agudeza visual. Parece existir una relación entre la actividad física y la agudeza visual. Conclusiones. Las diferencias encontradas según el sexo pueden deberse a patrones comportamentales diferentes entre hombres y mujeres, como la actividad física y el sedentarismo. Hacen falta estudios que evalúen la causalidad de la asociación entre los problemas de agudeza visual y los hábitos de vida poco saludables. Introduction. Low visual acuity is a public health problem worldwide, which is increasing year by year, and it is associated with unhealthy behavioral factors such as sedentary lifestyle. Low visual acuity particularly affects schoolchildren, whom eyesight is one of the most important abilities for their development, both in terms of learning and social interactions. The objective of this study is to establish an association between low visual acuity, sociodemographic, and behavioral characteristics of a student population in Bucaramanga, Colombia. Methodology. This was a cross-sectional observational study based on data collected from surveys of students within an educational institution sited in Bucaramanga between 2006 and 2017. A bivariate analysis was conducted between sociodemographic and behavioral characteristics and low visual acuity. Results. The prevalence of low visual acuity was 33.57%, with a higher prevalence of low visual acuity in women and those with a higher body mass index. However, low visual acuity didn't have a statistically significant association with the other variables studied. Discussion. Women have a higher prevalence of low visual acuity. There appears to be an association between physical activity and visual acuity. Conclusions. The encountered differences according to sex may be due to different behavioral patterns between males and females, such as physical activity and sedentary lifestyle. Introdução. A baixa acuidade é um problema de saúde pública no mundo, que aumenta a cada ano, relacionada a fatores comportamentais pouco saudáveis, como o sedentarismo. A baixa acuidade atinge especialmente as crianças em idade escolar, para as quais a visão representa um dos sistemas mais importantes para o seu desenvolvimento, tanto para a aprendizagem quanto para as relações sociais. O objetivo é estabelecer uma associação entre baixa acuidade visual e características sociodemográficas e comportamentais em uma população escolar na cidade de Bucaramanga, Colômbia. Metodologia. Estudo observacional transversal baseado em dados coletados em pesquisas com estudantes de uma instituição de ensino de Bucaramanga entre 2006 e 2017, nos quais foi realizada uma análise bivariada entre características sociodemográficas e comportamentais com baixa acuidade visual. Resultados. A prevalência de baixa acuidade visual foi de 33.57%, com maior prevalência de baixa acuidade visual em mulheres e naqueles com maior índice de massa corporal. A baixa acuidade visual não teve associação estatisticamente significativa com as demais variáveis estudadas. Discussão. As mulheres apresentam maior prevalência de baixa acuidade visual. Parece haver uma relação entre atividade física e acuidade visual. Conclusões. As diferenças encontradas segundo o sexo podem ser decorrentes de diferentes padrões comportamentais entre homens e mulheres, como atividade física e sedentarismo. Estudos são necessários para avaliar a causalidade da associação entre problemas de acuidade visual e hábitos de vida pouco saudáveis. Resumen La hernia diafragmática congénita es un defecto en la formación del diafragma con una alta mortalidad y morbilidad para el recién nacido. La ubicación en el lado derecho corresponde a solo el 10% de todos los casos y se asocia con una menor supervivencia, especialmente cuando hay una herniación hepática. No se conoce con precisión el porcentaje de casos de hernia diafragmática congénita derecha que no implican una herniación hepática. Aun no hay certeza del tiempo estimado para obtener el mayor beneficio de la corrección quirúrgica, en nuestro caso realizado con éxito a los 2 días de vida, logrando previamente una adecuada estabilidad hemodinámica y ventilatoria. El manejo perioperatorio en unidad de cuidado intensivo neonatal, es fundamental para reducir la morbimortalidad asociada. Se presenta un caso de hernia diafragmática congénita derecha tratado con éxito y se exponen los aspectos más relevantes del manejo médico-quirúrgico de esta patología. MÉD.UIS.2021;34(3): 71-7.


Abstract Congenital diaphragmatic hernia is a congenital defect in the formation of the diaphragm with high mortality and morbidity for the newborn. The location on the right side corresponds to only 10% of all cases and is associated with a lower survival, especially when there is a liver herniation. The percentage of cases of right congenital diaphragmatic hernia that do not involve liver herniation is not known with precision. An estimated time to obtain the greatest benefit from surgical correction is not known with certainty, in our case it was carried out successfully at 2 days of life, previously achieving adequate hemodynamic and ventilatory stability. Perioperative management in the neonatal intensive care unit is essential to reduce the associated morbidity and mortality. A case of congenital right diaphragmatic hernia treated successfully is presented and the most relevant aspects of the medical-surgical management of this pathology are exposed. MÉD. Resumen Introducción: En Colombia, en el periodo 2007-2011, se estima que se diagnosticaron alrededor de 1.500 casos de cáncer de cavidad oral y que por esta causa fallecieron 500 personas, en su mayoría eran hombres. La etiología de esta enfermedad está relacionada con el virus del papiloma humano y, el consumo de alcohol y de cigarrillo. Este estudio buscó cuantificar el efecto en la incidencia del cáncer de cavidad oral del cambio hipotético en la prevalencia de factores de riesgo como consumo de alcohol y, de cigarrillo e infección por virus del papiloma humano en Colombia para el periodo 2015-2050. Métodos: Modelo de simulación macro basado en datos secundarios. Resultados: Según los escenarios simulados, al eliminar el consumo de cigarrillo, alcohol e infección por virus del papiloma humano en la población colombiana probablemente sería posible evitar del 27,6% al 82,1% de los casos nuevos de cáncer de cavidad oral en el país en el periodo 2015-2050. Conclusiones: Bajo los supuestos de un escenario basado en las expectativas de la OMS, las estimaciones del modelo elaborado prevé un posible aumento de los casos de cáncer de cavidad oral asociados al consumo de alcohol y un bajo impacto en la prevención con las medidas actuales, por lo que puede ser necesario que se pongan en marcha acciones para disminuir el consumo de alcohol, así como de cigarrillo. Aunque la dirección de los cambios está bien demostrada en los resultados, es importante tener en cuenta que un modelo de simulación depende de muchos supuestos y por ende tiene limitaciones.


Abstract: Introduction: It is estimated that in the period 2007-2011, around 1500 new cases of oral cavity were diagnosed in Colombia, and that around 500 people died of this cause, the majority men. The etiology of this disease is related to the human papilloma virus, alcohol and cigarette use. This study tries to quantify the effect on oral cancer incidence of a hypothetical change in the prevalence of these risk factors in the period 2015-2050. Methods: macrosimulation models based on secondary data. Results: According to the simulated scenarios, elimination of cigarette and alcohol use and elimination of infection by the human papillomavirus in the Colombian population, would probably prevent 27.6 % to 82.1 % of all new cases of oral cavity cancer expected to occur in the period 2015-2050. Conclusions: according to the projections produced by the World Health Organization and the models we specified, there will be a potential increase in the number of new patients diagnosed with oral cavity cancer associated with the use of alcohol and a low impact of the interventions currently in place. It will be necessary to implement measures to descrease alcohol consumption and sustain current efforts to reduce cigarette use. It is important to take into account the limitations of the simulation models.

Introducción La obesidad afecta diversos aspectos físicos, psicológicos y sociales. Recientemente se ha comenzado a estudiar su relación con la función sexual y el efecto de las intervenciones dirigidas a reducir el peso. Objetivo Describir los cambios inducidos por la cirugía bariátrica en la función sexual en un grupo de pacientes obesos. Métodos Estudio observacional prospectivo de pacientes con obesidad mórbida, intervenidos con cirugía bariátrica. Se evaluó la función sexual en hombres con el cuestionario abreviado de función sexual (SHIM) y la encuesta de autoestima y relaciones (SEAR), y en mujeres con el Índice de función sexual femenina (IFSF). La calidad de vida se evaluó con el cuestionario SF-12. Se utilizaron pruebas estadísticas no paramétricas. Resultados Se incluyeron 19 pacientes, 13 mujeres con un promedio de edad de 45,3 años (22­59) y 6 hombres con un promedio de 41 años (36­54). El seguimiento promedio fue de 19 meses (13,1­25,9). Para el IFSF, todos los dominios demostraron mejoría, aunque la variable deseo fue la única en la que se obtuvo una diferencia estadísticamente significativa (p = 0.027). Los puntajes obtenidos con la escala SHIM, demuestran disminución en la severidad de la disfunción eréctil de 16,8 (8­21) a 22,16 (18­25) (p = 0,059). En calidad de vida, tanto mujeres como hombres reportaron un incremento estadísticamente significativo (p = 0.036 y p = 0.002, respectivamente). Conclusiones La cirugía bariátrica tiene un impacto altamente favorable en la calidad de vida y cambios positivos en todos los dominios de la función sexual de hombres y mujeres. Es posible que esos cambios se vean influidos por variables físicas y psicosociales que deben medirse con investigaciones futuras.


Introduction Obesity affects physical, psychological and social aspects. Recently, researchers have begun to study the relationship between sexual function and the effect of interventions aimed at reducing weight. Objective To describe the changes induced by bariatric surgery in sexual function in a group of obese patients. Methods Prospective observational study of patients with morbid obesity and bariatric surgery. Sexual function was evaluated in men with the abbreviated sexual health inventory for men questionnaire (SHIM) and the self-esteem and relationship questionnaire (SEAR), and in women with the Female Sexual Function Index (IFSF). Quality of life with the SF-12 questionnaire. Nonparametric statistical tests were used. Results 19 patients were included. 13 women with an average age of 45.3 years (22­59) and 6 men with an average of 41 years (36­54). The average follow-up was 19 months (13.1­25.9). For IFSF, all the domains showed improvement, although desire variable was the only one with statistically significant difference (p = 0.027). The scores obtained with the SHIM scale show a decrease in the severity of erectile dysfunction from 16.8 (8­21) to 22.16 (18­25) (p = 0.059). In quality of life, women and men reported a statistically significant increase (p = 0.036 and p = 0.002, respectively). Conclusions Bariatric surgery has a highly favorable impact on quality of life and positive changes in all domains of sexual function of men and women. It is possible that these changes are influenced by physical and psychosocial variables that should be measured with future research.

Introducción: La malaria es una enfermedad infecciosa vectorial de predominio en regiones tropicales y subtropicales. Existen 5 serotipos de Plasmodium, en Colombia se encuentran dos serotipos para malaria endémica, P. vivax y P. falciparum. Norte de Santander es una zona endémica para P. vivax. Objetivo: Presentación de un caso de malaria grave importada por P. falciparum, con una breve descripción de los aspectos fisiopatológicos de la malaria grave y los advenimientos de las nuevas terapias antipalúdicas. Presentación del caso: Paciente masculino de 45 años procedente de una región endémica para P. falciparum, que ingresó por cuadro febril inespecífico, trombocitopenia severa, alteración de la función renal y hepática con deterioro de su estado general. Es trasladado a la unidad de cuidados intensivos como urgencia dialítica, se diagnóstica malaria grave por P. falciparum, es tratado con antimaláricos y se reporta posible coinfección para el virus del dengue por inmunoglobulina M positiva (IgM), recibe terapia de reemplazo renal. Se contextualiza bajo un círculo vicioso en la disfunción de órganos, estructurado entre la insuficiencia renal y la insuficiencia respiratoria aguda con incremento de la permeabilidad vascular e hipoxemia refractaria, pese al esfuerzo terapéutico fallece por falla orgánica múltiple, por malaria grave. Conclusiones: La malaria es un problema en el área de salud pública, en nuestro caso corresponde a una malaria importada ya que en el departamento de Norte de Santander no se ha identificado dicho serotipo. Introduction: Malaria is a vector-borne infectious disease which is predominant in tropical and subtropical regions. There are 5 serotypes of Plasmodium, in Colombia there are two serotypes for endemic malaria, P. vivax and P. falciparum. North of Santander is an endemic area for P. vivax. Objective: To show a case of severe malaria caused by P. falciparum, with a brief description of the pathophysiological aspects of severe malaria and the advent of new antimalarial therapies. Case Presentation: A 45- year-old male patient from an endemic region with P. falciparum, who was admitted due to nonspecific febrile symptoms, severe thrombocytopenia, impaired renal and hepatic functions with deterioration of his general condition. He is transferred to the intensive care unit as a dialytic urgency. Severe malaria due to P. falciparum was diagnosed, he is treated with antimalarial medication, and a possible coinfection is reported for the dengue virus due to a positive immunoglobulin M (IgM) result, so he receives a renal replacement therapy. The case is contextualized in a vicious circle of organ dysfunction, which is structured between renal failure and acute respiratory failure with an increased vascular permeability and refractory hypoxemia; despite the therapeutic effort, the patient dies due to multiple organ failures, and severe malaria. Conclusions: Malaria is a problem in the public health area. This case corresponds to imported malaria because this serotype has not been identified yet in the department of North of Santander. Introdução: A malária é uma doença infecciosa vetorial predominantemente nas regiões tropicais e subtropicais. Existem 5 sorotipos de Plasmodium, na Colômbia existem dois sorotipos para malária endêmica, P. vivax e P. falciparum. O estado de Norte de Santander é uma região endêmica para P. vivax. Objetivo:Apresentação de um caso de malária grave importado por P. falciparum, com uma breve descrição dos aspectos fisiopatológicos da malária grave e o advento das novas terapias anti-maláricas. Apresentação do caso: Paciente do sexo masculino de 45 anos de uma região endêmica do P. falciparum, admitido com sintomas febris não específicados, trombocitopenia grave, insuficiência renal e função hepática, num estado geral deteriorado. Ele é transferido para a unidade de cuidados intensivos como emergência de diálise, é diagnosticada a malaria grave causada pelo P. falciparum, ele é tratado com antimaláricos e se reporta uma posível co-infecção pelo vírus da dengue pela imunoglobulina M (IgM) positiva, recebe então, tratamento de reposição renal. É contextualizado num círculo vicioso na disfunção dos órgãos, estruturado entre insuficiência renal e insuficiência respiratória aguda com aumento da permeabilidade vascular e hipoxemia refractária, apesar do esforço terapêutico morre devido à falência múltipla de órgãos, gerada pela malária grave. Conclusões: A malária é um problema na área da saúde pública, no nosso caso corresponde a uma malária importada, já que no departamento de Norte de Santander este serótipo não foi identificado.
