RESUMEN

I have stressed the importance of economic growth and employment policies for the furtherance of women's career objectives. Legal action to remove discrimination, as well as transformation of attitudes and expectations, will prove but hollow successes if adequate career opportunities do not become available. For this reason, the employment outlook for professional women in the 1970's has been examined on the basis of three alternative views of the future. All three involve elements of uncertainty and would require adjustments in the labor market. In the first alternative, although the BLS projected an overall balance in the labor market, supply and demand were expected to be ill-matched in several professional sectors, entailing burdensome retraining for those finding themselves in the areas of excess supply. The more pessimistic variant, based either on failure to reach growth and employment goals or on deliberate pursuit of reduced growth goals-combined with population stabilization and increased productivity-would require such fundamental adjustments in the labor market as work-sharing and early retirement or deemphasis of career objectives. The optimistic variant posits economic and social needs great enough to warrant high growth rates and views an expanded labor supply as a factor enabling the economy to attain such growth rates and to extend the utilization of professional personnel in new as well as in established areas (31). The major burden posed by the optimistic variant involves the need to achieve social consensus on the goals of society and to maintain the commitment to those goals with energy and intelligence.

We evaluated the merit of a directive issued by The Tennessee Department of Human Services (TDHS) requesting a medical evaluation for all children who were closely associated with a victim of sexual assault, even if these children denied being victims themselves. In a 6-month period, 247 girls, age 12 and under, were referred for medical evaluation. Of these, 188 were primary victims of sexual abuse, and 59 girls were secondary victims, associates of victims of abuse. All examinations were performed in an office setting. A colposcope was used to magnify the findings. Of these 59 girls, 45 girls were found to have abnormal genital findings. Nonspecific findings were present in 5 girls (8%), and findings considered specific for sexual abuse were observed in 40 girls (68%). When compared with 5 girls (8%), and findings considered specific for sexual abuse were observed in 40 girls (68%). When compared with primary victims, secondary victims were more likely to demonstrate genital abnormalities suggestive of sexual abuse. (chi 2 = 13.7, p = .0011). Of the 50 secondary victims who were later interviewed by TDHS, 24 girls (48%) reported that they were also victims of assault. We strongly recommend that siblings of sexual abuse victims, as well as other children who are closely associated with them and exposed to the perpetrator, be evaluated to determine whether they have abnormalities suggestive of sexual assault.

RESUMEN

To determine the prevalence of vulvar ulcerations in girls with myelocytic leukemia receiving chemotherapy, we reviewed the records of 108 patients. At the time of diagnosis, 69 patients were prepubertal, and the remaining 39 patients were postpubertal. Genital ulcerations were seen in 11 girls (10%), all of whom were white and postpubertal. None of the prepubertal girls complained of vaginal bleeding or had vulvar or vaginal ulcers. The findings suggest that genital ulcerative lesions occur primarily in postpubertal girls, probably because of the distinctive changes that occur during puberty as a result of estrogen stimulation.
