RESUMEN

This paper addresses the problem of three-dimensional speaker orientation estimation in a smart-room environment equipped with microphone arrays. A Bayesian approach is proposed to jointly track the location and orientation of an active speaker. The main motivation is that the knowledge of the speaker orientation may yield an increased localization performance and vice versa. Assuming that the sound produced by the speaker is originated from his mouth, the center of the head is deduced based on the estimated head orientation. Moreover, the elevation angle of the head of the speaker can be partly inferred from the fast vertical movements of the computed mouth location. In order to test the performance of the proposed algorithm, a new multimodal dataset has been recorded for this purpose, where the corresponding 3D orientation angles are acquired by an inertial measurement unit (IMU) provided by accelerometers, magnetometers and gyroscopes in the three-axes. The proposed joint algorithm outperforms a two-step approach in terms of localization and orientation angle precision assessing the superiority of the joint approach.

BACKGROUND: The health benefits of the Mediterranean diet (MedDiet) have been linked to the presence of beneficial gut microbes and related metabolites. However, its impact on the fecal metabolome remains poorly understood. OBJECTIVES: Our goal was to investigate the weight-loss effects of a 1-y lifestyle intervention based on an energy-reduced MedDiet coupled with physical activity (intervention group), compared with an ad libitum MedDiet (control group), on fecal metabolites, fecal microbiota, and their potential association with cardiovascular disease risk factors. METHODS: A total of 400 participants (200 from each study group), aged 55-75 y, and at high cardiovascular disease risk, were included. Dietary and lifestyle information, anthropometric measurements, blood biochemical parameters, and stool samples were collected at baseline and after 1 y of follow-up. Liquid chromatography-tandem mass spectrometry was used to profile endogenous fecal metabolites, and 16S amplicon sequencing was employed to profile the fecal microbiota. RESULTS: Compared with the control group, the intervention group exhibited greater weight loss and improvement in various cardiovascular disease risk factors. We identified intervention effects on 4 stool metabolites and subnetworks primarily composed of bile acids, ceramides, and sphingosines, fatty acids, carnitines, nucleotides, and metabolites of purine and the Krebs cycle. Some of these were associated with changes in several cardiovascular disease risk factors. In addition, we observed a reduction in the abundance of the genera Eubacterium hallii group and Dorea, and an increase in alpha diversity in the intervention group after 1 y of follow-up. Changes in the intervention-related microbiota profiles were also associated with alterations in different fecal metabolite subnetworks and some cardiovascular disease risk factors. CONCLUSIONS: An intervention based on an energy-reduced MedDiet and physical activity promotion, compared with an ad libitum MedDiet, was associated with improvements in cardiometabolic risk factors, potentially through modulation of the fecal microbiota and metabolome. Schwanomas originating from the brachial plexus, although rare, may be mistaken for another type of tumour. A 40 year-old woman, who had been treated years earlier for a breast adenocarcinoma, showed in the 5-year follow-up magnetic resonance examination a localized lesion in the right brachial plexus. The presumptive radiological diagnosis was a metastasis from the primary adenocarcinoma. Following surgical access via the right axilla, a well-circumscribed mass in the brachial plexus was detected. Under intraoperative electrophysiological guidance, the lesion was observed to depend on the ulnar nerve and its complete resection was possible without compromising nerve function. Histological findings indicated a schwannoma thus ruling out the presence of metastasis. The postoperative development was uneventful and six years after surgery, the patient is to date tumour-free. To the best of our knowledge, this is the first report of a brachial plexus schwannoma arising from the ulnar branch. Intraoperative electrophysiological monitoring is essential for a good surgical outcome.

CONTEXT: Obesity is a multifactorial disorder, that is, a disease determined by the combined effect of genes and environment. In this context, polygenic approaches are needed. OBJECTIVE: To investigate the possibility of the existence of a crosstalk between the CALPAIN 10 homologue CALPAIN 5 and nuclear receptors of the peroxisome proliferator-activated receptors family. DESIGN: Cross-sectional, genetic association study and gene-gene interaction analysis. SUBJECTS: The study sample comprise 1953 individuals, 725 obese (defined as body mass index > or = 30) and 1228 non obese subjects. RESULTS: In the monogenic analysis, only the peroxisome proliferator-activated receptor delta (PPARD) gene was associated with obesity (OR = 1.43 [1.04-1.97], p = 0.027). In addition, we have found a significant interaction between CAPN5 and PPARD genes (p = 0.038) that reduces the risk for obesity in a 55%. CONCLUSION: Our results suggest that CAPN5 and PPARD gene products may also interact in vivo.

OBJECTIVE: To evaluate the impact of a multifactorial intervention to promote bisphosphonate deprescription after over 5 years of use (BF5y) in a health care organisation (HCO) in Gipuzkoa (Spain) and to compare it with the standard intervention in other HCOs in the Basque Health Service-Osakidetza. METHODS: An 8-month follow-up study (results from before and after) to assess the impact of two interventions. All patients from Osakidetza receiving BF5y treatment (electronic prescription) in July 2013 were included. The standard intervention (9 HCOs) consisted of mailing a consensus statement on BF5y deprescribing and facilitating patient identifiers with BF5y prescription for review by the primary care physician. The multifactorial intervention (Gipuzkoa) also included a local consensus with leading specialists and training sessions in health centres. RESULTS: 18,725 patients were included; 94.7% were women. Standard intervention deprescribing rates ranged from 26.4% (Bilbao) to 49.4% (Araba), being 37.2% overall. The multifactorial intervention deprescribing rate was 44.6%, 7.4% (p <0.0001; 95% confidence interval [95%CI]: 5.4-9.4) higher than standard intervention. Changes to other treatments were less common with the multifactorial intervention, with a difference of 3.7% (p <0.0001; 95%CI: -2.2 to -5.2). CONCLUSIONS: Standard and multifactorial interventions are very effective in reducing unnecessary treatments with bisphosphonates. The multifactorial intervention is more effective than the standard one, although more complex to implement.

Resumen Introducción: Tradicionalmente, los estudios de escarabajos coprófagos en los bosques secos tropicales (BST) del Caribe colombiano han aplicado metodologías diseñadas para zonas húmedas y andinas del país, lo cual podría estar incidiendo en el rendimiento y la eficiencia del muestreo. Objetivo: Con el fin de aportar a esta discusión, se analizó cómo la cantidad de cebo y el tiempo de operación de la trampa de caída inciden en la efectividad de captura de escarabajos coprófagos en un fragmento de BST en La Reserva Campesina la Flecha, San Jacinto, Colombia. Métodos: Para la captura de los escarabajos, se utilizó trampas de caída, cebadas con tres cantidades diferentes de atrayente: pequeño (34.6 g), mediano (53.8 g) y grande (114.9 g), las cuales permanecieron activas en campo durante 48 h. Cuatro muestreos fueron realizados entre marzo y septiembre de 2015, abarcando por igual la época seca y de lluvia. Resultados: Se registró un total de 4 563 individuos, agrupados en 10 géneros y 27 especies de escarabajos coprófagos. Los mayores valores de riqueza, abundancia y biomasa se presentaron en el cebo grande, a las 48 h, durante la época de lluvias. Sin embargo, el tiempo de operación de la trampa no tuvo efecto en la estimación de la riqueza, abundancia y biomasa de escarabajos independientemente de los tamaños de cebo. Los tres órdenes de la diversidad ( 0 D, 1 D y 2 D), presentaron valores similares entre las cantidades de cebo durante la época de lluvia, pero en la época seca, los cebos de mayor tamaño presentaron los valores de diversidad más altos. Por su parte con el cebo de mayor tamaño se capturó significativamente más riqueza, abundancia y biomasa de escarabajos de cuerpo pequeño y grande siendo esta situación más notaria durante la época seca. Conclusiones: Los resultados de este trabajo evidencian que para el estudio de los ensamblajes de escarabajos en el BST la utilización de un cebo de mayor tamaño contribuye a una mejor estimación de riqueza, abundancia, diversidad y biomasa, sobre todo durante la época seca, cuando las condiciones ambientales propician que los cebos pierdan su atractividad con mayor rapidez.


Abstract Introduction: Typically, research on dung beetles in the Tropical Dry Forests (TDF) of Colombian Caribbean region, have applied methodologies designed for wet or Andean areas of the country, which could be influencing the performance and efficiency of sampling. Objective: In order to contribute to this discussion, we analyzed how the bait amount and pitfall trap operating time influence the collection effectiveness of dung beetles in a TDF fragment at Reserva Campesina La Flecha, San Jacinto, Colombia. Methods: For the collection of beetles, we utilized pitfall traps baited with three different amounts of attractants: small (34.6 g), medium (53.8 g), and large (114.9 g), which remained active in the field for 48 h., 4 samplings between March and September 2015 were carried out, covering both, dry and rainy seasons. Results: A total of 4 563 individuals were recorded, grouped into 10 genera and 27 species of dung beetles. The highest values of richness, abundance and biomass were registered in the large bait, at 48 h, during the rainy season. However, the trap operating time had no effect on the estimation of beetles' richness, abundance, and biomass, regardless of bait sizes. The three diversity orders ( 0 D, 1 D y 2 D) showed similar values between the bait amounts during rainy season, but in the dry season, the largest baits displayed the highest diversity values. On the other hand, with the largest bait, significantly more richness, abundance, and biomass of small and large body beetles were registered, especially during the dry season. Conclusions: The results of this research show that, for the study of beetles' assemblages in the BST, the use of a larger bait contributes to a better estimate of richness, abundance, diversity and biomass, especially during the dry season, when environmental conditions promote a more rapidly loss of baits attractiveness.

Los neurinomas del plexo braquial son tumores infrecuentes que pueden confundirse con otras lesiones de índole tumoral. Se presenta el caso de una mujer de 40 años, tratada previamente de un adenocarcinoma de mama derecha en el pasado, que en el estudio de extensión realizado 5 años después se detectó una lesión localizada en el plexo braquial derecho. La paciente se encontraba asintomática. El diagnóstico radiológico de presunción fue metástasis de adenocarcinoma mamario. Se realizó un abordaje axilar derecho descubriendo una lesión bien delimitada en el plexo braquial. Con ayuda de la monitorización neurofisiológica intraoperatoria, se observó que la lesión dependía de la rama cubital y se pudo realizar una resección completa preservando la función de dicho nervio. El estudio anatomopatológico confirmó que se trataba de un neurinoma, descartando así la existencia de metástasis. La evolución postoperatoria fue satisfactoria. Seis años después de la intervención no existe recidiva tumoral. En nuestro conocimiento este es el primer caso publicado en la literatura de un neurinoma del plexo braquial dependiente de la rama cubital. La monitorización neurofisiológica intraoperatoria resulta fundamental para abordar este tipo de lesiones con baja morbilidad.
