RESUMEN
The primary surgical treatment of injured soft tissues of the maxillofacial area is performed with special reference to anatomo-physiological features of the injured area, mechanism and character of the trauma. Complete closure of all wound surfaces is a necessary condition of the primary surgical treatment of wounds of the face with elements of primary plasty when necessary. It gives the best functional and esthetic results and a more valuable preparation of the injured area for the following reconstructive operations.

Differential choice of methods of treatment was used for the surgical treatment of patients with the syndrome of the first and second branchial arches depending on the disturbance of the face shape, supporting the function of the temporo-mandibular articulation and the kind of bite. In patients with different disorders of bite, supporting function of the temporo-mandibular articulation and displacement of the chin to the damaged side the authors used different means of osteogenic correction of the position of the mandible directed to a simultaneous improvement of the face shape, bite and creation of a stop in the temporo-mandibular articulation.

The examination of 50 patients aged from 6 to 35 has first revealed a pronounced disorder of the indices of immunity and biochemical composition of blood (decreased level of aminotransferases, cholesterol, increased content of alkaline phosphatase) in patients with otomandibular dysostosis.

Six months is believed to be optimal age for primary chiloplasty; the most effective method is a combined chiloplasty after Mirault---Limberg---Obukhova---Anamenski resulting in primary union in 99% of cases. In cleft palate the operation is possible at the age of 6--7 years. The article describes specific features of the surgical treatment of clefts involving part of the hard palate, complete uni- and bilateral cleft palate and alveolar processes of maxilla.
