RESUMEN

In this study about the relationships between structural and microwave electrical properties of KTa(1-x)NbxO3 (KTN) ferroelectric materials, a KTN thin film was deposited on different substrates to investigate how KTN growth affects the microwave behavior. Interdigital capacitors and stubs were made on these films through a simple engraving process. Microwave measurements under a static electric field showed the importance of the substrate on the circuit behavior and, notably, on the tuning factor.

RESUMEN

Quantitative PCR and visual monitoring of Mycosphaerella graminicola epidemics were performed to investigate the effect of curative and preventative applications of azoxystrobin in wheat field crops. A non-systemic protectant and a systemic curative fungicide, chlorothalonil and epoxiconazole, respectively, were used as references. PCR diagnosis detected leaf infection by M graminicola 3 weeks before symptom appearance, thereby allowing a clear distinction between curative and preventative treatments. When applied 1 week after the beginning of infection, azoxystrobin curative activity was intermediate between chlorothalonil (low effect) and epoxiconazole. When applied preventatively, none of the fungicides completely prevented leaf infection. There was some indication that azoxystrobin preventative treatments may delay fungal DNA increase more than epoxiconazole at the beginning of leaf infection. Both curative and preventative treatments increased the time lapse between the earliest PCR detection and the measurement of a 10% necrotic leaf area. Azoxystrobin only slightly decreased the speed of necrotic area increase compared with epoxiconazole. Hence, azoxystrobin activity toward M graminicola mainly resides in lengthening the time lapse between the earliest PCR detection and the measurement of a 10% necrotic leaf area. Information generated in this way is useful for optimal positioning of azoxystrobin treatments on M graminicola.

BACKGROUND: Forty percent of patients with autoimmune hepatitis (AIH) present with acute jaundice/hepatitis. Such patients, when treated promptly, are thought to have a good prognosis. OBJECTIVES: The objective of this study was to describe the natural history of AIH in patients presenting with jaundice/hepatitis and to determine whether the diagnosis could have been made earlier, before presentation. METHODS: This study is a retrospective review of 2249 consecutive patients who presented with jaundice to the Jaundice Hotline clinic, Truro, Cornwall, UK, over 15 years (1998-2013) and includes a review of the laboratory data over a 23-year period (1990-2013). RESULTS: Of the 955 patients with hepatocellular jaundice, 47 (5%) had criterion-referenced AIH: 35 female and 12 male, the median age was 65 years (range 15-91 years); the bilirubin concentration was 139 µmol/l (range 23-634 µmol/l) and the alanine transaminase level was 687 IU/l (range 22-2519 IU/l). Among the patients, 23/46 (50%) were cirrhotic on biopsy; 11/47 (23%) died: median time from diagnosis to death, 5 months (range 1-59); median age, 72 years (range 59-91 years). All 8/11 patients who died of liver-related causes were cirrhotic. Weight loss (P=0.04) and presence of cirrhosis (P=0.004) and varices (P=0.015) were more common among those who died. Among patients who died from liver-related causes, 6/8 (75%) died less than 6 months from diagnosis. Cirrhosis at presentation and oesophageal varices were associated with early liver-related deaths (P=0.011, 0.002 respectively). Liver function test results were available in 33/47 (70%) patients before presentation. Among these patients, 16 (49%) had abnormal alanine transaminase levels previously, and eight (50%) were cirrhotic at presentation. CONCLUSION: AIH presenting as jaundice/hepatitis was mainly observed in older women: 50% of the patients were cirrhotic, and liver-related mortality was high. Some of these deaths were potentially preventable by earlier diagnosis, as the patients had abnormal liver function test results previously, which had not been investigated.

RESUMEN

This article focuses on improving hand hygiene compliance using a multidisciplinary approach. Historically, hand hygiene compliance among health care workers and physicians has been far below an acceptable level. The facility discussed in this article uses an ongoing "Hand Hygiene" campaign, which is multidisciplinary and addresses numerous barriers to compliance.

The aim of this study was to analyse the variations of the metabolic and technical parameters during a maximal 400-m freestyle event. Seven trained male swimmers swam a maximal 400-m freestyle as if in competition (255.8 +/- 6.9 s). Intermediate time and stroke rate (SR) were recorded at each length (25 m). To estimate the changes in metabolic parameters during the 400-m event, they swam a 300-, 200-, and 100-m test set from each length of the 400-m event results, resting 90 min between each test. The exact speed at each length was given with a visual light pacer. Oxygen uptake (VO(2)) and blood lactate concentration ( [Lac]) were measured before and immediately after each test. VO(2) and [Lac] were stable during the 100-, 200-, and 300-m test but significantly higher (p < 0.05) during 400-m test. The estimated contribution of anaerobic metabolism (EsCANA ) during the first 100-m and the 400-m represented 45 % and 20 % of total energy output, respectively. Speed decreased significantly (p < 0.05) after the first 100-m and remained stable until the end. SR decreased significantly after the first 100-m, then increased until the end, while stroke length (SL) decreased linearly throughout the 400-m. During the first or the last 100-m, EsCANA was not correlated with the changes in V, SR, or SL between the second and the first 100-m, and between the fourth and the third 100-m, respectively. To conclude, this study showed that the swimmers were not able to maintain stable SL during the 400-m event. Thus, to sustain stable velocity and to compensate for the decrease in SL, swimmers increased SR throughout the last 300-m.
