RESUMEN

Lumbar bone mineral density (BMD) was evaluated by dual photon absorptiometry (DPA) in 38 women with laparoscopically diagnosed and staged endometriosis. DPA was performed before and at the completion of 26 weeks of treatment with either gonadotropin releasing hormone agonist (GnRHa) or danazol. Twenty-five women received GnRHa either nasally or subcutaneously and 13 women received danazol 200 mg four times daily by mouth. During treatment amenorrhea and suppressed estradiol levels were observed in all patients. Each patient took a supplement of oral calcium 1 gm daily. DPA (reproducibility +/- 1.5%) was read by a single observer unaware of the treatment assignment of the patient. The mean (+/- standard error) of the entire group prior to treatment was 98.8% +/- 0.03% of a control population of women from the same geographic area matched for race, age, and weight. At the completion of treatment, BMD was slightly decreased in the GnRHa group and marginally increased in the danazol group but neither change was significant. Women with endometriosis have lumbar BMD in the normal range and ovarian suppression with either GnRHa or danazol produces no significant change in BMD.

A prospective, randomized trial compared hormonal changes induced with intranasal leuprolide 1.6 mg/day to danazol 800 mg/day for treatment of endometriosis. Both regimens induced anovulation and ovarian suppression in all subjects. Mean estradiol (E2) and progesterone (P) levels were suppressed with both regimens, but were lower with leuprolide. There was no difference in cumulative follicle-stimulating hormone (FSH) and luteinizing hormone (LH) levels, although at times during treatment mean levels of these hormones were lower with leuprolide. Higher P levels in the danazol group, most likely of adrenal origin, indicated a suppressive effect on adrenal steroidogenesis. Symptomatic improvement was significant in both groups. Laparoscopy after treatment also demonstrated a decrease in endometriosis scores in both groups. At 12 months after treatment, cumulative pregnancy and live birth rates were similar in both groups. Leuprolide offers an attractive alternative to danazol for the medical treatment of endometriosis.

RESUMEN

Women with TBI have been inadequately studied in relation to most aspects of pathophysiology, recovery, health and behavioral issues, and community integration. This is not entirely surprising in light of the preponderance of men with TBI but also reflects the traditional tendency of medical researchers to concentrate their efforts on men. Although most of the residual effects of TBI are gender-neutral, women may present some unique problems in relation to pain and endocrine issues, reproduction, and sexual functioning In addition, a woman's roles as wife, mother, and daughter are likely to result in a different constellation of family dynamics when TBI is introduced. Attention to enrollment of women in research studies and the increasing number of multi-institutional studies of TBI may provide enlightenment on these issues in the future.

RESUMEN

Records from 495 psychiatric patients from two neuropsychology laboratories indicated the Fingertip Number Writing test shows a significant and reliable left-hand advantage within and between laboratories. While the left-hand advantage may reflect a right-hemisphere superiority for tactile-spatial processing, it is not possible to distinguish between right hemispheric functional superiority and practice effects between hands because the standard fingertip writing task always starts with the right hand.

A 44-year-old patient with cerebrovascular accident and resulting total occlusion of the right internal carotid artery and severe cognitive impairments was seen as an inpatient. Impulsivity, general inattention, left side neglect, decreased concentration and visual-spatial-perceptual difficulties were clinically evident. A conventional approach to training this patient in wheelchair transfer skills met with little success. An individualized training program based on the patient's specific cognitive strengths and deficits was developed. The major elements of the program were (a) careful observation of task performance, (b) task analysis of the skill to be taught, (c) utilization of the patient's learning strengths to guide performance, (d) small step hierarchical programming to meet the desired goal, (e) intense repetition of the skill and (f) data collection procedures to monitor change and program adjustments. Six weeks after the program was implemented the patient was able to complete safe transfers, and was discharged home. At follow-up, two weeks after discharge, inconsistency in transfers was noted; the written program was immediately reestablished, and the patient continued safe wheelchair transfers at every subsequent check. This study suggests that treatment outcome in patients with right hemisphere damage may not be as bleak as has been previously suggested. A treatment strategy that includes an optimal utilization of a patient's verbal skills to cue and monitor performance and judgment, along with systematic data collection to guide program modifications, can lead to a more successful rehabilitation outcome.

Cognitive and personality disturbances following severe closed head injury in young adults are associated with poor rehabilitation outcome. Yet systematic programmes for dealing with these disturbances have generally not appeared. The present report briefly describes the Neuropsychological Rehabilitation Program (NRP) at Presbyterian Hospital in Oklahoma City and the initial outcome data on eighteen closed head injury patients and seventeen untreated controls. Greater improvement in neuropsychological functioning occurred in the NRP patient group on selected variables, but generally the effects were modest. Emotional distress, however, substantially decreased in treated patients. Fifty percent of the NRP patients maintained productivity 75% of the time or more following rehabilitation, compared to 36% of the controls. Treatment successes showed less personality disturbances than treatment failures and better learning and memory scores post-treatment.
