RESUMEN

Presented herein is a clinical case report regarding successful treatment of a female patient with a relapsing malignant retroperitoneal tumour complicated by disseminated thrombosis of the inferior vena cava, right atrium and right ventricle of the heart. A relapse of the malignant uterine tumour had developed 3 years after the primary operation and was represented by a large-size mass ingrowing into the infrarenal segment of the inferior vena cava, aorta, as well as jejunum. Additional examination revealed the presence of a tumorous thrombus extending from the primary tumorous node in the lumen of the inferior vena cava to the right atrium and ventricle. The procedures performed consisted in removal of the tumour with resection of the inferior vena cava, aorta, and jejunum, followed by thrombectomy from the right portions of the heart under extracorporeal circulation. The postoperative period turned out uneventful, with no complications observed. The woman was discharged on POD 15. Twelve-month postoperative follow up revealed neither relapse nor progression of the disease. Currently, the patient continues undergoing specific treatment (second-line chemotherapy). Also discussed in the article are current challenges concerning both the classification of tumour thrombosis of the inferior vena cava in retroperitoneal sarcomas, and the choice of optimal strategy and policy of treatment.

RESUMEN

Application of Clopidogrel in the postoperative period in patients after surgery on extracranial arteries was analyzed. The preparation was used in 56 patients. Uninterrupted intake of the drug took place during 5 to 12 months, 7+/-1.6 months on average. Stomach and duodenal ulcer was diagnosed in 11 patients (20%), 3 patients (5%) had chronic pancreatitis with periodical exacerbations and 2 patients (4%) had erythrocytemia. One bleeding requiring revision of the wound took place in the nearest postoperative period. During the whole period of observation there were no thrombotic or thromboembolic complication against the background of using plavix. In none of the patients granulocytopenia was revealed with continuous intake of plavix. An ultrasonic investigation of the operated arteries in the remote postoperative period failed to reveal hemodynamically significant restenoses of carotid arteries.

RESUMEN

Experimental atherosclerosis in 30 rabbits was combined with the implantation into the abdominal aorta of the synthetic prosthesis MPL-79. Formation of neointima and transformation of mesenchymal cells into the endothelial ones are studied using scanning and translucent electron microscopy and autoradiography. Neointimal hyperplasia is shown to result in significant narrowing of the prosthesis lumen up to its complete occlusion. It is important that neointima fibroplasia is particularly intense against the background of progressing atherosclerosis.

RESUMEN

The literature data and the results of the authors' experiments allow one to consider Chlamydia pneumoniae as one of the leading risk factor of development of atherosclerosis and ischemic heart disease. Features of atherosclerosis morphogenesis in the presence of Chlamydia are described. Data, showing similarity of cell reactions, characterizing immune inflammation in mLDL deposit in arterial wall and obligate parasites are described. Synergism in the action of mLDL and chlamydial infection may induce a "malignant" course of atherogenesis promoting thrombosis and ischemic heart disease excerbation.

RESUMEN

The article summarizes an experience with 179 surgical interventions on patients with pathological tortuosity of brachiocephalic arteries. The optimal described diagnostic algorithm for this pathology included ultrasonic dopplerography, transcranial dopplerography, angiography or magnetic resonance tomography in the angiography regimen. Clinical and angiographic indications to surgical treatment of this pathology were determined. The operative treatment was carried on in patients with neurological symptoms having kinks of brachiocephalic arteries. The intraoperative strategy depending on the type and localization of the disease are analyzed. Stabile regression of manifestations of vascular cerebral insufficiency after surgical correction of the pathological tortuosity of the brachiocephalic arteries was achieved in 76% of the patients.

RESUMEN

The authors have proposed and fulfilled the operation of partial ileoshunting on a 45 year-old woman. The using of endovideosurgical method made the operation less traumatic and minimized the cosmetic defect. The patient was discharged on the 6th day. The authors have made 3 more operations of this kind in patients with hypercholesterinemia.

RESUMEN

The authors share their experiences with using Solcoseryl in treatment of 158 patients with obliterating atherosclerosis of the lower extremity vessels. This treatment was found to be very effective. A scheme of the treatment is proposed after which the improved quality of life retains during not less than 6 months in 93% of the patients. The accessory maintenance therapy with minimum doses of aspirin and nicotinic acid is enough between the courses of treatment with Solcoseryl.

RESUMEN

Under study was the dynamics of formation of pseudointima in a synthetic lavsan prosthesis of the aorta of 32 rabbits, 22 of them were on the atherogenic diet. Against the background of experimental hypercholesterolemia the proliferation of cells and the obliteration of the vessel lumen were found to proceed more rapidly than in normal animals.
