RESUMEN

OBJECTIVE: To investigate the clinical features, radiologic scores and clinically relevant risk factors prognosis of secondary interstitial lung disease (ILD) in patients with systemic lupus erythematosus (SLE). METHODS: In this study, 60 SLE patients in Department of Rheumatology of the First Affiliated Hospital of Baotou Medical College and Taizhou First People's Hospital from January 2015 to March 2019 were retrospectively analyzed. All of those 60 patients with SLE underwent lung high resolution computed tomography (HRCT) examination. We used a 1 ∶1 case-control study. There was a matching of age and gender between the two groups. Thirty patients with SLE related ILD (SLE-ILD) were in the case group, and 30 patients with SLE without ILE (SLE non-ILD) were in the control group. The clinical features, pulmonary function test, radiologic characteristic of SLE patients were collected and were used to analyze SLE-ILD. RESULTS: In this study, we reached the following conclusions: First, there were statistically significant differences in chest tightness/shortness of breath, Raynaud's phenomenon, and Velcro rale between SLE-ILD and SLE non-ILD patients (both P < 0.05); Second, hemoglobin (Hb) and albumin (ALB) in the patients of SLE-ILD had a significant decrease compared with the patients of SLE non-ILD. Blood urea nitrogen (BUN), erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) increased in SLE-ILD patients compared with SLE non-ILD patients, the difference had statistical significance (P < 0.05); Third, for SLE-ILD patients, the most common type was non-specific interstitial pneumonia (NSIP), followed by usual interstitial pneumonia and lymphocytic interstitial pneumonia; Fourth, there was no significant difference in clinical-radiology-physiology scores between the different ILD types (P>0.05), similarly, the lung HRCT score and lung function between different ILD types had no significant difference (P>0.05); Fifth, multivariate Logistic regression analysis showed that decreased albumin and chest tightness/shortness of breath might be the risk factor for SLE-ILD. CONCLUSION: There are statistically significant differences between the SLE-ILD group and SLE non-ILD group in terms of chest tightness/shortness of breath, Velcro rale and Raynaud's phenomenon. Decreased albumin and chest tightness/shortness of breath in SLE patients should be alerted to the occurrence of ILD. NSIP is the most common manifestation of SLE-ILD.

OBJECTIVE: To construct sphingosine 1-phosphate receptor-1 (S1P1)-small interfering RNA (siRNA) lentiviral vectors and infect human salivary gland cells (HSG), and to investigate its possible therapy on Sjogren's syndrome. METHODS: HSG cells were divided into blank group, empty vector group, scramble-siRNA group and S1P1-siRNA group. The lentiviral vectors expressing siRNA against S1P1 and the pLL3.7 were respectively transfected into 293T cells with pMD2.G, pMDL g/p RRE, pRSV-REV to produce virus, and then infect HSG cells. The efficiency was observed by flow cytometry after the transfection for 48 h. The expression levels of S1P1 mRNA of HSG were detected by real-time RT-PCR and the expression of S1P1 protein was detected by immunohistochemistry method. The expression levels of interferon-γ (IFN-γ) and interleukin (IL)-17 in the supernatant of the cells were detected by ELISA method. RESULTS: (1) The scramble-siRNA, S1P1-siRNA lentiviral vector was successfully constructed, and the lentivirus titer was about 3.5×108 TU/mL. (2) The level of S1P1 mRNA was lower in S1P1-siRNA group than those in the blank group, empty vector group, and scramble-siRNA group 48 h after infection, there were significant differences between them (P<0.05). (3) The expression of S1P1 protein was lower in S1P1-siRNA group than those in blank group, empty vector group, and scramble-siRNA group 48 h after transfection, there were significant differences between them (P<0.05). (4) The levels of IL-17 were lower in S1P1-siRNA group than those in blank group, empty vector group, and scramble-siRNA group 48 h after transfection, there were significant differences between them (P<0.05). (5) The levels of IFN-γ in S1P1-siRNA group were lower than those in blank group, empty vector group, and scramble-siRNA group 48 h after transfection, there were significant differences between them (P<0.05). CONCLUSION: The lentiviral vector targeting S1P1 was successfully constructed. S1P1 siRNA could suppress the levels of S1P1 mRNA and protein, and decrease the expression of IL-17 and IFN-γ. S1P1 siRNA could infect HSG cells stably and inhibit the expression of S1P1 gene specifically and efficiently, and reduce the levels of inflammatory cytokines.

The effect of ACTH on the expression of Fos in rat adrenal glands was investigated immunocytochemically at both light and electron microscopic levels. An approximately 4-fold increase in the number of Fos-positive cortical cells per unit area of the adrenal cortex was found 45 min following a single injection of ACTH (i.v., 2 IU/kg b.w.), as compared with the control animals. The number of immunoreactive cells reached a maximum 90 min after ACTH injection and remained high 150 min after the injection. However, 5 h following the injection it declined significantly. At the ultrastructural level, the ACTH-induced Fos-like immunoreactivity was localized in the nuclei of cortical cells, exclusively confined to the nuclear regions associated with the euchromatin. The result that ACTH enhances Fos-like immunoreactivity suggests that ACTH is involved in c-fos induction in adrenal cortical cells. The characteristic intracellular localization of Fos-like immunoreactivity implies that Fos, once synthesized, may be rapidly translocated into the nuclei of the cells, where it participates in transcriptional regulation of genetic events.

C-fos expression was studied immunocytochemically in sympathetic neurons of the rat superior cervical ganglion (SCG). Fos-like immunoreactivity was confined to the principal neurons of the ganglion and was exclusively localized within their nuclei. In 2-month-old rats, the immunoreactivity was detected in 1.2% of the principal neurons with a density of 4.95 Fos-positive cells/mm2 of ganglion area. This proportion increased with age and reached a value which was 6.5-fold higher in the 26-month-old rats than that in the young adult. A density of 24.5 Fos-positive cells/mm2 of ganglion area was seen in the 26-month-old animals. The age-enhanced c-fos expression suggests that Fos may be involved in regulation of the genetic events associated with the adaptive changes in neuronal activity of the sympathetic ganglion during aging.

Weight-for-height and weight changes over a 2-year period among 335 institutionalized elderly adults at a geriatric care facility are described. Weight-for-height at admission was compared with several current standards for elderly persons. Mean relative weight and proportions of underweight and obese patients varied with the standards used. Although changes in group mean weights were slight, 73% of the patients gained or lost 4.5 kg or more in 2 years. Patients who lost 4.5 kg or more in the interval had a lower 4-year survival rate (p less than .05) than patients who gained weight or remained stable. Fluctuations in weight may be more prevalent among institutionalized elderly persons than previously thought, or than are apparent in analyses of group trends. Monitoring an individual's weight is simple and may be more meaningful than comparison to a standard, especially because weight loss is associated with mortality, until age appropriate standards are developed.
