RESUMEN

The development of various enzyme-linked immunosorbent assays (ELISAs) coupled with surface-enhanced Raman scattering (SERS) detection is a growing area in analytical chemistry due to their potentially high sensitivity. A SERS-based ELISA with horseradish peroxidase (HRP) as an enzymatic label, an o-phenylenediamine (oPD) substrate, and a 2,3-diaminophenazine (DAP) enzymatic product was one of the first examples of such a system. However, the full capabilities of this long-known approach have yet to be revealed. The current study addresses a previously unrecognized problem of SERS detection stage performance. Using silver nanoparticles and model mixtures of oPD and DAP, the effects of the pH, the concentration of the aggregating agent, and the particle surface chloride stabilizer were extensively evaluated. At the optimal mildly acidic pH of 3, a 0.93 to 1 M citrate buffer, and AgNPs stabilized with 20 mM chloride, a two orders of magnitude advantage in the limits of detection (LODs) for SERS compared to colorimetry was demonstrated for both DAP and HRP. The resulting LOD for HRP of 0.067 pmol/L (1.3 amol per assay) underscores that the developed approach is a highly sensitive technique. We suppose that this improved detection system could become a useful tool for the development of SERS-based ELISA protocols.

The Operationalized Psychodynamic Diagnostic of Children and Adolescents (OPD-KJ-2) in Everyday Clinical Practice with the Plämobox: Applicability and Interrater Reliability Abstract: Objective: The OPD-CA2 manual for assessing psychodynamic aspects in children and adolescents is well established in clinical practice. However, publications regarding its reliability and validity are limited to (1) adolescents, (2) the structure of the first version of the manual and not to the comprehensive revision of the OPD-CA2, (3) the axes "structure" and (partly) "conflict" but not the axis "relationship," and (4) missing applicability in everyday clinical practice. Methodology: The present study comprised 42 children aged 6-12 years (age level 2 of the OPD-CA2), with and without mental illness, and assessed them using two randomly assigned raters. We assessed them using a low-structured diagnostic symbol game with miniature figurines and objects based on videotapes. We also tested the interrater reliability of the OPD-CA2 axes. Results: The overall assessment of structure and the assessment of the four subdimensions succeeded with good to very good agreement. We could also determine the presence of relevant conflict dynamics with very high agreement, while not recognizing specific conflicts in the clinical sample. Our assessment of the items of the relationship axis shows a low level of agreement. Conclusions: Overall, we can confirm the reliability of the OPD-CA2 for everyday clinical assessment in the younger age groups. Finally, we discuss which factors contribute to the heterogeneous picture.

PURPOSE: To determine the toric intraocular lens (IOL) axis by using internal astigmatism axis obtained from Optical Path Difference (OPD) Scan III (Nidek Co.) in non-dilate eyes. METHODS: The eyes of patients who underwent toric IOL implantation for astigmatic correction were investigated. Patients who have ocular surface disorder, keratoconus, posterior capsule opacification were excluded. The IOL axis measured in non-dilated eyes in mesopic conditions by OPD scan III device and the IOL axis measured by using classical slit lamp biomicroscopy method in dilated eyes were detected at postoperative 1st and 6th months. Results were compared with correlation and linear regression analysis. RESULTS: Totally, 26 eyes of 18 patients were included. The difference between biomicroscopic IOL axis and OPD internal astigmatism axis was 4.96 ± 4.41 degrees at the 1st month and 3.62 ± 3.5 degrees at the 6th month. There was a significant and high correlation between biomicroscopic IOL axis and OPD internal astigmatism axis at both 1st month (r = 0.992 p < 0.001) and 6th month (r = 0.995 p < 0.001). According to regression analysis, the results of two measurement methods were significantly compatible with each other at 1st month (R Sq = 0.984 p < 0.001) and 6th month (R Sq = 0.990 p < 0.001) and there was a close to ideal linear (R Sq = 1) relationship between two methods. CONCLUSIONS: In eyes with toric IOL implantation, the IOL axis and IOL rotation according to target IOL axis can be detected easily and effectively in a short time by OPD scan internal astigmatism axis without the need to dilate the pupil.

A novel determination method for protein biomarkers based on on-chip flow rate measurement was developed using a microchip with organic photodiodes (OPDs). This quantitative method is based on the flow rate measurement of an ink solution pushed out by oxygen gas generated through catalase reaction. The amount of oxygen gas generated in the sample reservoir is dependent on the concentration of the analyte; therefore, the flow rate of the ink solution is also dependent on the concentration of the analyte. The concentration of the analyte can thus be estimated by measurement of the ink solution flow rate. The ink solution flow rate was estimated by measuring the migration time of the ink solution between two points using two OPDs placed below the microchannel. The principle of this method was demonstrated by the measurement of catalase using the microchip. In addition, the developed method was applied to the determination of C-reactive protein (CRP), a biomarker of inflammation, based on a catalase-linked immunosorbent assay (C-LISA). The limit of detection for CRP was 0.20 µg/mL. The method was also applied to the determination of CRP in human serum, and the quantitative values obtained by this method were in excellent agreement with those obtained by the conventional enzyme-linked immunosorbent assay (ELISA) method. The developed method does not require a photodetector with high sensitivity and is thus capable of downsizing; therefore, this will be useful for on-site analyses such as point-of-care testing and field measurements.

Objectives: Personality organization or functioning describes biographically acquired characteristics for the regulation of psychological processes. Limitations correlate with symptom load. This study examines whether significant improvement in personality functioning can be achieved in the framework of a multimodal psychodynamic treatment and its influence on the psychological symptoms. Methods: In this naturalistic study design (N = 318) personality organization was measured with the OPD-SQS and the symptoms were obtained using PHQ-9, GAD-7, SCL-9 and mini- Spin. Changes in the functioning levels were calculated using a t-test. The associations between the functional and symptom improvements were calculated using hierarchical regressionmodels. ANOVAs for dependent samples were used to calculate the association of the personality organization changes on symptom reduction Results: Treatment resulted in significant improvement in personality structure.The greater the changes, the lower the symptom burden at the end of treatment. Patients with lower personality structure benefited equally well from treatment. Conclusions: Personality functioning improves with multimodal psychodynamic therapy and is accompanied by reduction of psychological symptoms. Structural changes proceed equally in high and low structured patients.

Objective: The study aims to assess interrater reliability of the revised Operationalized Psychodynamic Diagnosis 3 (OPD-3) system. Three OPD axes were included in the reliability analysis: the Levels of Structural Integration Axis (OPD-LSIA), the Conflict Axis (OPD-CA), and the Interpersonal Relations Axis (OPD-IRA). Methods: A heterogeneous sample of n = 80 patients was diagnostically assessed with SCID-I and SCID-II as well as with an OPD interview. Two raters of a group of five independently rated each patient's OPD interview according to OPD-3. Results: Interrater reliability was good to excellent for OPD-LSIA [ICC1,1 = .874 - .920], and moderate to good forOPD-CA [ICC1,1 = .547 - .764] as well as forOPD-IRA at the aggregated level of Agency and Communion [ICC1,1 = .575 - .793], except for raters' self-perceived tendencies to act towards the patient. Conclusion: The revised OPD-3 can be used as a reliable instrument for a psychodynamic diagnosis of conflicts, levels of structural integration, and dysfunctional relationship patterns.

PURPOSE: To investigate the relationship between lens morphology and aberrations in patients with myopia. METHODS: This cross-sectional study included 155 patients with myopia in their right eyes. Spherical power and cylindrical power were achieved by cycloplegic autorefraction. The eyes were divided into three groups for analysis based on their spherical equivalent (SE) values. The 4 mm and 6 mm ocular and internal aberrations were measured using the OPD-scan III. Lens parameters were measured using CASIA2, including lens thickness (LT), radius of anterior/posterior lens surface curvature (RAL/RPL), lens decentration (DEC), and lens tilt (TILT). The differences of lenticular parameters and aberration parameters among the three groups analyzed with ANOVA or Kruskal Wallis test. Pearson correlation or Spearman correlation analysis was performed to evaluate the relationships between the lens parameters and aberrations. A p value < 0.05 indicated statistical significance. RESULTS: The difference in LT, RAL, DEC and TITL among the three groups was statistically significant (p < 0.05). And there were differences among differences in internal high-order aberrations, spherical aberration, and coma aberration(p < 0.05).Spherical power was positively correlated with LT and TITL (p < 0.05) and negatively correlated with DEC, RAL, and RPL (p < 0.05). Cylindrical power was positively correlated with LT (p < 0.05) and negatively correlated DEC (p < 0.05); The lenticular parameters (LT, RAL, DEC, and TILT) were mainly correlated with the ocular and internal spherical aberration. LT and DEC were correlated with ocular and internal higher-order aberrations and coma aberration. CONCLUSION: DEC and LT were the main factors affecting aberrations in patients with myopia.

In this study, we present a highly integrated design of organic optoelectronic devices for Point-of-Need (PON) nitrite (NO2-) measurement. The spectrophotometric investigation of nitrite concentration was performed utilizing the popular Griess reagent and a reflection-based photometric unit with an organic light emitting diode (OLED) and an organic photodetector (OPD). In this approach a nitrite concentration dependent amount of azo dye is formed, which absorbs light around ~540 nm. The organic devices are designed for sensitive detection of absorption changes caused by the presence of this azo dye without the need of a spectrometer. Using a green emitting TCTA:Ir(mppy)3 OLED (peaking at ~512 nm) and a DMQA:DCV3T OPD with a maximum sensitivity around 530 nm, we successfully demonstrated the operation of the OLED-OPD pair for nitrite sensing with a low limit of detection 46 µg/L (1.0 µM) and a linearity of 99%. The hybrid integration of an OLED and an OPD with 0.5 mm × 0.5 mm device sizes and a gap of 0.9 mm is a first step towards a highly compact, low cost and highly commercially viable PON analytic platform. To our knowledge, this is the first demonstration of a fully organic-semiconductor-based monolithic integrated platform for real-time PON photometric nitrite analysis.

BACKGROUND: Studies show that 16% to 77% of psychotherapy patients abandon therapy within the first sessions. The aim of this study is to examine how patient personality variables, specifically the patients' structural integration and the personality traits dependency and self-criticism, are associated with symptomatic change and therapy dropout. METHOD: We analysed data from 96 patients (age: M = 30.56, SD = 11.39; 78.5% women; 44.6% students, 28.3% employees). A hierarchical logistic regression analysis was carried out to determine whether patients' structural integration (assessed via the OPD-SQ) and their level of dependency and self-criticism (DEQ) can predict therapy dropout. In addition, a multiple regression was used to analyse how these variables affect symptomatic change (OQ-45.2 symptom subscale). RESULTS: The interaction of structural integration level and dependency best predicts therapy dropout. For the prediction of symptomatic change, both structural integration and dependency were significant. However, their interaction showed no significant results. DISCUSSION: The patient's structural integration was associated to both symptomatic change and dropout. Therapists' training should include techniques addressing patients' structural integration and degree of dependency to prevent patient dropout from therapy.

Reliability and validity of the OPD-CA Conflict Questionnaire in an adolescent inpatient sample Abstract. Abstracts: The OPD-CA Conflict Questionnaire (OPD-CA-CQ) assesses intrapsychic conflicts according to the Operationalized Psychodynamic Diagnosis in Childhood and Adolescence (OPD-CA-2) in a simple and time-efficient way. The questionnaire exists in two versions: as a self-report measure for adolescents and as an informant-report measure for therapists. We examined the psychometric criteria of the questionnaire (internal consistency, construct validity, convergent validity) in a sample of adolescent inpatients. Overall, results were heterogeneous: Some scales of the questionnaire seem to be suitable for assessing the corresponding intrapsychic conflicts, while others require a critical review. Particularly the conflict scales closeness vs. distance and submission vs. control need revision.

Psychodynamic conflicts: Development of a questionnaire for the assessment by patients and their therapists Abstract. The OPD-CA conflict questionnaire represents a means of recording modes of coping with unconscious conflicts according to the Operationalized Psychodynamic Diagnostics (OPD-CA) for patients and their therapists. To select the items for the conflict questionnaire, we combined the expert ratings and psychometric quality criteria using a sample of 427 adolescents (53.6 % patients) and 44 therapists. The short version contains 28 items, which include 7 intrapsychic conflicts and their active and passive coping modes (total ICC = .855; p < .001). We found expected differences between healthy and clinically abnormal adolescents with a higher level of conflicts among outpatients. A comparison of the therapist's version with two different methods (assessment according to the sheet in OPD-CA and the conflict questionnaire) produced consistent assessments of the same patient both in the expert assessment (total ICC = .861; p < .001) and from the point of view of 52 participants in two OPD-KJ training courses (total ICC = .825; p < .001).

The development and application of conflict-specific story completion tasks as part of OPD diagnostics in childhood Abstract. The study combines use of the MacArthur Story Stem Battery (MSSB) with the Operationalized Psychodynamic Diagnostics in Childhood and Adolescence (OPD-CA-2). It describes the development and application of conflict-specific story completion tasks, which are meant to enable a systematic evaluation of the conflict axis and the structure axis of Age Group 2. It determines interrater reliability for the conflict axis and the structure axis and checks further exploratory correlations between conflict axis and structure axis using the SDQ (Strengths and Difficulties Questionnaire). The diagnostic method presented here enables a reliable evaluation of the conflict axis and the structure axis. Moreover, correlations occur between the OPD-CA-2 axes and the SDQ.

Changes in the level of structure and conflict are, in addition to alleviating of symptoms, central goals of psychodynamic therapies. This study examines this question with a focus on intrapsychic development-impairing conflicts in the sense of the OPD-CA on a sample of 121 adolescents and their 46 therapists. At the beginning, in the middle and at the end of the M = 73.27 hours (SD = 17.40) therapy, they assessed the level of conflict and structure from the patients' and the therapists' perspectives. The results show large effect sizes with regard to the reduction of the overall conflict level from the point of view of the therapists (η2 = .33). While the therapists reported higher initial values in almost all intrapsychic conflicts and significant linear decreases over time, the therapy tended to sensitize the patients to the existence of an intrapsychic development-impairing conflict. With regard to the structural level, which was already quite good at the start of treatment, there were no significant changes over time. However, the overall structural level and the conflict scales contributed to the explanation of the variance in the severity of symptoms at the end of therapy.

Population variation of several microscopic structures used in age-at-death estimation was assessed for three different population samples. The aim of the study was to determine if the need exists for population-specific standards when dealing with individuals of African and European origin. A total sample 223 bone sections from the anterior cortex of the femur (n = 99 black South Africans, n = 94 white South Africans and n = 30 Danish individuals) were analysed using a stereological protocol. Variables assessed included the average number of osteons per grid area (OPD), osteon size and Haversian canal size. ANCOVA was employed for assessment of statistically significant differences. The results indicated that OPD differed significantly between the three groups, but that osteon size was similar for all individuals. Haversian canal size showed unpredictable changes with age and high levels of variation, making it unsuitable to use for age estimation as a single factor. As there are conflicting opinions in the literature on whether to use population-specific equations for the estimation of age-at-death or not, this paper provided additional insight into the use of specific variables and its related variation between groups.

BACKGROUND: Financial conflicts of interest (COI) have been shown to affect the interpretation of scientific findings. Publications with unreported COI tend to be more favorable to industry. Since 2014 industry payments to United States (US) physicians are publicly reported in the Open Payments Database (OPD). Several studies show high levels of unreported COI in medical literature; however, there is no research examining COI reporting at surgical conferences. We hypothesized that compliance with the COI disclosure requirement would be high at the 2018 SAGES meeting. However, we expected to find significant discrepancy between speaker-reported and OPD-reported COI. A secondary aim was to characterize the amount, source, and variation in industry payments to invited speakers. METHODS: We reviewed all available presentations from SAGES 2018 as recorded and publicly available on YouTube™ for the presence of COI disclosure and the disclosed industry relationships. For US physicians we searched the OPD and recorded all industry payments > $500. We compared the self-disclosed COI for each speaker with OPD records. Presentation topics were divided into ten groups to determine which topics received the most funding. RESULTS: Of the 526 invited presentations, 479 (91%) videos were available. Disclosures were reported by 414 presenters (86.4%). There were 420 unique presenters of which 315 were listed in the OPD. Speaker-reported disclosures were fully concordant with the OPD in 38.3% (121/315) of cases with 39% (123/315) under-reporting disclosures. Of presenters listed in OPD, the median payment was $992 ($0-$374,502) with a total of $6,389,097 paid in 2017. SAGES speakers failed to disclose $2,049,535 worth of industry payments with an average undisclosed payment of $16,662.88 (± $40,733.19). The largest financial contributor was Intuitive Surgical with $1,981,169 paid. Among topics, robotics and hernia received the most funding with $2,593,925 (40.6%) and $2,591,671 (40.5%) paid, respectively. CONCLUSIONS: Overall compliance with SAGES disclosure rules is high. There remains a discrepancy between speaker- and industry-reported disclosures, including a number of undisclosed payments, some of which are substantial. Adjustments to disclosure rules to include the relative amount of compensation may be warranted.

Interferometry sensors are frequently analyzed by applying the Fourier transform because the transformation separates all frequency components of its signal, making its study on a complex plane feasible. In this work, we study the relation between the optical path difference (OPD) and poles location theoretically and experimentally, using the Laplace transform and a pole-zero map. Theory and experiments are in concordance. For our study, only the cosine function was considered, which is filtered from the interference pattern. In experimental work, two unperturbed low-finesse Fabry-Pérot interferometers were used. First, a Fabry-Pérot interferometer that has a cavity length of ~1.6 mm was used. Its optical path difference was 2.33 mm and the poles were localized at points ±i12. rad/nm. Secondly, a Fabry-Pérot interferometer with a cavity length of ~5.2 mm was used, and its optical path difference was 7.59 mm and the poles were localized at points ±i40.4 rad/nm. Experimental results confirmed the theoretical analysis. Our proposal finds practical application for interferometer analysis, signal processing of optical fiber sensors, communication system analysis, and multiplexing systems based on interferometers.

Background: User-generated content shared in the online health communities (OHCs) is becoming a valuable resource for researchers to understand patients' decision-making behaviors in the management of their health. Many studies have focused on how to obtain useful information from online reviews in OHCs. Introduction: This study focuses on a telemedicine service called Online Private Doctor (OPD), which is offered by a leading Chinese physician review website (PRW). OPD reviews have not received much attention. By data mining the reviews, our goal is to determine what patients are talking about when they use the OPD service and whether they are satisfied with the service or not. Materials and Methods: We used a Python web crawler to collect 41,029 reviews and 84,510 short reviews (labels) of all 5,645 physicians who offered the OPD service on a PRW (haodf.com) in China. Mixed methods (i.e., a literature review, topic discovery, annotation, and a sentiment analysis) were used to determine the information that the OPD reviews are meant to express. Results: We discovered that the OPD reviews can be categorized into four subjects: competence (35.1%), communication (29.4%), treatment (26.0%), and convenience (9.5%). In terms of previously discovered topics, we found that competence, communication, and treatment have been discussed before, but convenience is an emerging subject. The sentiment analysis indicated that 93.67% of the reviews indicated positive emotions, and the area under the receiver operating characteristic (ROC) curve is 0.64. Furthermore, the labels indicated that only 0.72% (603/84,570) of reviews were negative toward the OPD service. The subjects of the labels were distributed according to competence (34.7%), communication (23.8%), treatment (33.5%), and convenience (8.0%). Discussion: The findings of our study suggest that patients who ever used OPD have been quite satisfied with the service. From their reviews, we discovered that OPD has its special characteristics and is convenient. However, it still has some shortcomings, for example, the quality of the phone connection. In terms of both the platform and the doctors, more efforts should be made to make the OPD better and more regulated. Conclusion: OPD is an emerging telemedicine service that still needs more time and space to evolve. For patients, it helps reduce problems such as scheduling and queuing. Therefore, it brings more convenience to people's daily lives. In the future, more attention should be paid to this service, as it is helpful in reducing the uneven distribution of medical resources.

Level of personality functioning (OPD-2) and the symptom severity of posttraumatic stress disorder - a cohort study Objectives: To investigate the affirmed correlation between the level of personality functioning (OPD-2, Axis IV "Structure") and the symptom severity of posttraumatic stress disorder based on clinical routine data was the main goal of this study. Method: Cross-sectional data was acquired between 2013 and 2016 using random samples of standardized questionnaires in the trauma outpatient clinic of the Department of Psychosomatics and Psychotherapy, University Hospital in Münster, Germany. The German version of the PDS (Ehlers et al. 1996) measured the symptoms and the severity of PTSD, level of personality functioning was assessed by the OPD-SFK (Ehrenthal et al. 2015) Results: Level of personality functioning in OPD-SFK and the severity of posttraumatic symptoms are significantly correlated to each other. The less the functioning of the psychic structure the more the symptom severity shows up. The results were confirmed with different sets of predictors in our statistical models. Conclusions: The level of the personality functioning (OPD-2) is related to the severity of posttraumatic symptoms. Whether a low-level personality functioning is a disposal for PTSD or more likely a result of a traumatic life event, should be discussed.

OBJECTIVE: The aim of the study is to increase the effectiveness of the treatment of exacerbation of COPD group B GOLD II with the use of combined therapy of the combined drug PulmoBRIZ containing two components - ambroxol and acetylcysteine and the course of halotherapy. PATIENTS AND METHODS: Materials and methods: We observed 60 patients with COPD B, GOLD II. They were divided into two groups: the first - the main (n=30) - patients receiving basic therapy, mucolytic therapy - a combination of Ambroxol and acetylcysteine - 200/30 to 1 tabl. 2 times a day, number 7 days and, from the 3rd day - sessions of halotherapy 1 time per day № 10. The second group, the control group (n=30), followed only basic therapy, did not take mucolytics and halotherapy sessions. RESULTS: Results: Patients receiving therapy with the combination of ambroxol and acetylcysteine and halotherapy sessions experienced a significant increase in FEV1 by 8.3% (p <0.05); the Tiffon index was 7.2% (p<0.05), reactive anxiety levels (RA) and manifestations of autonomic dysfunction decreased, whereas in patients in the control group these indicators did not improve significantly. CONCLUSION: Conclusions: The proposed complex therapy of COPD patients with the inclusion of the combined drug ambroxol and acetylcysteine and halotherapy sessions contributes to the improvement of the quality of life of patients.

BACKGROUND: Knowing the factors for patient satisfaction is an important and direct indicator of quality of health care which is essential for providers to fill their gaps. Although few studies have been conducted on patient satisfaction in Ethiopia; but there is limited evidence for comparing patient satisfaction and associated factors in the public and private wing of the health services. Thus, this study aimed to investigate factors of patient satisfaction in adult outpatient departments in the private wing and regular services at public hospitals of Addis Ababa, Ethiopia. METHODS: A comparative institution based cross-sectional study was conducted from March to April 2018. A total of 955 systematically selected patients were interviewed by using an interviewer-administered structured questionnaire. Binary logistic regression analysis was performed. In the multivariable logistic regression analysis p value < 0.05 and adjusted odd ratio (AOR) with 95% confidence interval (CI) were used to identify the associated factors. RESULTS: The overall patient satisfaction was 89.3% (95% CI: 87.2-91.2). At the regular and private wings of outpatient departments it was 88.3% (95% CI: 85.4-91.2) and 90.4% (95% CI: 87.6-93), respectively. At regular service OPD, patient satisfaction was affected by female sex (AOR: 7.78; 95% CI: 2.89-20.93), long waiting time (AOR: 0.22; 95% CI: 0.07-0.73), information on the prevention of recurrent illnesses (AOR: 14.16; 95% CI: 4.58-43.83), and information on drug use and side effects (AOR: 0.22; 95% CI: 0.08-0.63). In private wing, it was affected by being in the age group of 38 to 47 years (AOR: 22.1; 95% CI: 2.39-203.6), attended elementary school (AOR: 4.69; 95% CI: 1.04-21.26), availability of drugs (AOR: 0.14; 95% CI: 0.04-0.58), and the accessibility of latrines (AOR: 6.56; 95% CI: 1.16-37.11). CONCLUSIONS: Patient satisfaction at the private wing and regular adult OPDs' of public hospitals had no statistically significant difference. Female sex and information on the prevention of recurrent illnesses were factors positively affected patient satisfaction at regular services, whereas at private wing OPDs' age, attended elementary school, and accessibility of latrines were factors that positively affected patient satisfaction.
