Eccentric, compared to concentric exercise, is proposed to involve different neuro-motor processing strategies and a higher level of mental demand. This study compared eccentric and concentric cycling at matched perceived effort and torque for the mental demand and related-cortical activation patterns. Nineteen men (30 ± 6 years) performed four different 5-min cycling conditions at 30 RPM on a semi-recumbent isokinetic cycle ergometer: (1) concentric at a moderate perceived effort (23 on the CR100® scale) without torque feedback; (2) concentric and (3) eccentric at the same average torque produced in the first condition; and (4) eccentric at the same moderate perceived effort than the first concentric condition. The conditions two to four were randomized. After each condition, mental demand was monitored using the NASA Task Load Index scale. Changes in oxy-(O2 Hb) and deoxy-(HHb) hemoglobin during exercise were measured over both prefrontal cortices and the right parietal lobe from a 15-probe layout using a continuous-wave NIRS system. Mental demand was significantly higher during eccentric compared to concentric cycling (+52%, p = 0.012) and when the exercise intensity was fixed by the torque rather than the perceived effort (+70%, p < 0.001). For both torque- or perceived effort-matched exercises, O2 Hb increased significantly (p < 0.001) in the left and right prefrontal cortices, and right parietal lobe, and HHb decreased in the left, and right, prefrontal cortices during eccentric compared to concentric cycling. This study supports that acute eccentric cycling, compared to concentric cycling, involves a higher mental demand, and frontoparietal network activation.

PURPOSE: Isometric exercise (IE) and isometric wall squat (IWS) training have been shown to be effective methods of reducing arterial blood pressure. However, most IE interventions require methodologies and equipment that could present a barrier to participation. Therefore, this study aimed to examine the effectiveness of an accessible RPE prescribed IWS intervention. METHODS: Thirty normotensive and pre-hypertensive adults were randomly assigned to a control group or one of two 4-week home-based IWS intervention groups: the first group conducted IWS exercise where intensity was prescribed and monitored using RPE (RPE-EX), whilst the other used a previously validated HR prescription method (HR-EX). Resting and ambulatory heart rate (HR) and blood pressure (BP) were measured pre- and post-intervention. RESULTS: Minimum clinically important differences (MCID; - 5 mmHg) in SBP and/or DBP were shown in 100% of intervention participants. Statistically significant reductions were shown in resting seated BP (RPE-EX: SBP: - 9 ± 6, DBP: - 6 ± 4, MAP: - 6 ± 3 mmHg; HR-EX: SBP: - 14 ± 6, DBP: - 6 ± 4, MAP: - 8 ± 4 mmHg), supine BP (RPE-EX: SBP: - 8 (- 5), DBP: - 8 (- 7), MAP: - 8 (- 4) mmHg; HR-EX: SBP: - 5 (- 4), MAP - 5 (- 4) mmHg), and ambulatory SBP (RPE-EX: - 8 ± 6 mmHg; HR-EX: - 10 ± 4 mmHg) following the interventions. There were no statistically significant differences between intervention groups in the magnitude of BP reduction. CONCLUSION: RPE prescribed IWS exercise can provide an effective and more accessible method for reducing BP at home, providing reductions comparable to the current HR-based prescription method.

PURPOSE: Using exercise protocols at a fixed rating of perceived effort (RPE) is a useful method for exploring the psychophysical influences on exercise performance. However, studies that have employed this protocol have arbitrarily selected RPE values without considering how these values correspond to exercise intensity thresholds and domains. Therefore, aligning RPE intensities with established physiological thresholds seems more appropriate, although the reliability of this method has not been assessed. METHODS: Eight recreationally active cyclists completed two identical ramped incremental trials on a cycle ergometer to identify gas exchange threshold (GET). A linear regression model plotted RPE responses during this test alongside gas parameters to establish an RPE corresponding to GET (RPEGET) and 15% above GET (RPE+15%GET). Participants then completed three trials at each intensity, in which performance, physiological, and psychological measures were averaged into 5-min time zone (TZ) intervals and 30-min 'overall' averages. Data were assessed for reliability using intraclass correlation coefficients (ICC) and accompanying standard error measurements (SEM), 95% confidence intervals, and coefficient of variations (CoV). RESULTS: All performance and gas parameters showed excellent levels of test-retest reliability (ICCs = > .900) across both intensities. Performance, gas-related measures, and heart rate averaged over the entire 30-min exercise demonstrated good intra-individual reliability (CoV = < 5%). CONCLUSION: Recreationally active cyclists can reliably replicate fixed perceived effort exercise across multiple visits when RPE is aligned to physiological thresholds. Some evidence suggests that exercise at RPE+15%GET is more reliable than RPEGET.

This study aimed at determining the effect of a passive exoskeleton on local perceived discomfort, perceived effort and low back muscles' activity. Thirteen volunteers performed two simulated working tasks with and without the exoskeleton. In the static task, the exoskeleton decreased the lumbar perceived discomfort, the perceived effort and the level of low back muscles' activity (â¼10%) while increasing discomfort in the chest and feet. The percent decrease in EMG amplitude was correlated with the percent increase in perceived effort with exoskeleton. For the dynamic task, the exoskeleton increased the discomfort in the chest and decreased the level of back muscle activity (â¼5%). Current findings suggest exoskeleton is effective in reducing the back load while increasing the perceived discomfort at non-targeted body regions in both working tasks. The concurrent increase of discomfort in non-targeted areas probably led to a higher perceived effort despite the reduction of low back muscle activity. Practitioner summary: This study provided insights into exoskeleton effects on local discomfort, perceived effort and muscle activity. Overall, the potential benefits of passive exoskeleton should be considered alongside its adverse effects on the non-targeted body regions that can lead to an increase of perceived effort despite the reduction of back muscle activity.

Music has been shown to be capable of improving runners' performance in treadmill and laboratory-based experiments. This paper evaluates a generative music system, namely HEARTBEATS, designed to create biosignal synchronous music in real-time according to an individual athlete's heartrate or cadence (steps per minute). The tempo, melody, and timbral features of the generated music are modulated according to biosensor input from each runner using a combination of PPG (Photoplethysmography) and GPS (Global Positioning System) from a wearable sensor, synchronized via Bluetooth. We compare the relative performance of athletes listening to music with heartrate and cadence synchronous tempos, across a randomized trial (N = 54) on a trail course with 76 ft of elevation. Participants were instructed to continue until their self-reported perceived effort went beyond an 18 using the Borg rating of perceived exertion. We found that cadence-synchronous music improved performance and decreased perceived effort in male runners. For female runners, cadence synchronous music improved performance but it was heartrate synchronous music which significantly reduced perceived effort and allowed them to run the longest of all groups tested. This work has implications for the future design and implementation of novel portable music systems and in music-assisted coaching.

Aquatic cycling is a program of physical exercises performed with immersed stationary bikes. Few studies have provided evidence about the intensity control during its practice. Therefore, the primary aim of this study was to examine the concurrent and construct validity of a new scale for rating perceived exertion (RPE) during aquatic cycling in young men. Thirty physically active, healthy young men performed a load-incremented aquatic cycle ergometer protocol. Concurrent validity was established by correlating the Aquatic Cycling Scale (ACS) with oxygen uptake, pulmonary ventilation (VE), heart rate (HR), and blood lactate concentration (BL) responses to the maximal load-incremental test. Construct validity was established by correlating RPE derived from the Aquatic Cycling Scale (0-10) from the Borg Scale (6-20). RPE-overall, maximal oxygen uptake (VO2max), oxygen uptake indexed to body weight (VO2), VE, HR, and BL were measured during each exercise stage. The range of exercise responses across the incremental test were VO2max = 1.07-3.55 L/min, VO2 = 14.26-46.89 ml/Kg/min, VE = 23.17-138.57 L/min, HR = 99.54-173.31 beats/min, BL= 1.18-11.63 mM, ACS RPE-overall = 1.11-9.33. Correlation/regression analyses showed ACS RPE as a positive linear function of VO2max (r = 0.78; p < 0.05), VO2 (r = 0.87; p < 0.05), VE (r = 0.86; p < 0.05), HR (r = 0.77; p < 0.05), and BL (r = 0.85; p < 0.05). RPE-ACS distributed as a positive linear function of the RPE-Borg Scale (r = 0.97; p < 0.05). ANOVA indicated that an incremental pedalling cadence of 15 revolutions per minute (rpm) provoked significant differences (p < 0.05) regarding previous stages in the majority of the variables analysed. The Aquatic Cycling Scale is an appropriate tool for monitoring exertion intensity during aquatic cycling in fit men. A brief increment in aquatic pedalling cadence of 15 rpm increases the intensity of the aquatic pedalling exercise.

This study aimed at evaluating the effects of mental and muscle fatigue on table tennis performance. Mental fatigue (MF) was induced by completion of 90 minutes of the AX-CPT; muscle fatigue was induced by completion of an eccentric exercise performed with the elbow flexors (biceps fatigue, BF) or the knee extensors (quadriceps fatigue, QF). The control condition consisted of watching a movie. Stroke parameters (speed and accuracy of the ball), as well as feelings of fatigue and force production capacity of the elbow flexors (BF, MF and control conditions) and knee extensors (QF condition), were assessed pre and post fatigue protocols. Feelings of fatigue increased post fatigue protocols. Force production capacity decreased only in the BF and QF conditions. BF and MF induced a decrease in accuracy. This decrease in accuracy was associated with an increased ball speed in the BF condition, and a decreased ball speed in the MF condition. QF had a negligible effect on stroke performance. Our results suggest that both mental fatigue, and muscle fatigue, significantly impair table tennis performance and therefore coaches should take into account both the physical and mental state of table tennis players to optimize performance.

The present study aimed to assess the relationship between rating of perceived exertion (RPE) and percentage of peak oxygen uptake ([Formula: see text]) during three head-out water-based aerobic exercises. In addition, the RPE at the second ventilatory threshold (VT2) was also compared among them. Twenty young women performed head-out water-based maximal tests for the exercises stationary running (SR), frontal kick (FK) and cross-country skiing (CCS). RPE was monitored during the tests and the values corresponding to VT2 and training zones corresponding to 50-59%, 60-69%, 70-79%, 80-89% and [Formula: see text] were determined. Regression analysis, descriptive statistics and ANOVA with repeated measures were used. Significant relationships were observed between the RPE and [Formula: see text] (r = 0.858-0.893; P < 0.001) for all head-out water-based aerobic exercises. Average RPE ranged from 12.1-12.7 in the training zone corresponding to 50-59%, from 13.7-14.8 to 60-69%, from 15.8-16.4 to 70-79%, from 17.3-18.1 to 80-89% and from 18.5-18.9 to [Formula: see text]. No significant differences were found among the three head-out water-based aerobic exercises at VT2 (P > 0.05; SR: 16.1 ± 0.9, FK: 16.7 ± 1.5, CCS: 15.9 ± 1.3). The results support the use of RPE to control the relative intensity of training during head-out water-based aerobic exercises and indicate values near to 16-17 when targeting VT2 intensity for young women.

This study examined the perceived effort, success rates and kinematics for three push strategies in a simulated lateral patient transfer (horizontal slide). Thirteen healthy subjects (four males) completed three repetition pushing loads of 6, 10 and 14 kg in random order; with a spontaneous push strategy, then with a straight-back bent-knees (squat) strategy and the preparatory pelvic movement ('rockback') strategy in random order. Perceived effort and kinematic parameters measured at the onset of movement and at maximum push excursion were compared between strategies and between loads with repeated measures ANOVA. The spontaneous and 'rockback' strategies achieved the pushing task with less perceived effort across all loads than the squat push (P < 0.001). Only 3/13 participants were successful on all attempts at pushing the 14 kg load using a squat strategy, which contrasted with 12/13 participants when the spontaneous strategy or the 'rockback' strategy was used. Forward movement of the pelvis and forward trunk inclination may be positively associated with lower perceived effort in the push task. Practitioner Summary: In a manual-handling task that simulated a lateral patient transfer (horizontal slide), perceived effort and success rates of three push strategies were compared. A straight-back bent-knees push (squat) strategy demonstrated greater perceived effort and lower success rates than a spontaneous push strategy, or a push strategy with preparatory 'rockback' pelvic movement.

BACKGROUND: The multidimensionality of the academic self-concept in terms of domain specificity has been well established in previous studies, whereas its multidimensionality in terms of motivational functions (the so-called affect-competence separation) needs further examination. AIM: This study aims at exploring differential effects of enjoyment and competence beliefs on two external validity criteria in the field of mathematics. SAMPLE: Data analysed in this study were part of a large-scale longitudinal research project. Following a five-wave design, math enjoyment, math competence beliefs, math achievement, and perceived math effort expenditure measures were repeatedly collected from a cohort of 4,724 pupils in Grades 3-7. METHOD: Confirmatory factor analysis (CFA) was used to test the internal factor structure of the math self-concept. Additionally, a series of nested models was tested using structural equation modelling to examine longitudinal reciprocal interrelations between math competence beliefs and math enjoyment on the one hand and math achievement and perceived math effort expenditure on the other. RESULTS: Our results showed that CFA models with separate factors for math enjoyment and math competence beliefs fit the data substantially better than models without it. Furthermore, differential relationships between both constructs and the two educational outcomes were observed. Math competence beliefs had positive effects on math achievement and negative effects on perceived math effort expenditure. Math enjoyment had (mild) positive effects on subsequent perceived effort expenditure and math competence beliefs. CONCLUSION: This study provides further support for the affect-competence separation. Theoretical issues regarding adequate conceptualization and practical consequences for practitioners are discussed.

Objective: This study aims at comparing the energy expenditure (EE) and heart rate (HR) data from Ring Fit Adventure (RFA) with those from indirect calorimetry (COSMED) and a heart rate monitor (Polar FT7). A secondary goal is to evaluate self-reported enjoyment and perceived effort levels. Materials and Methods: Thirty participants (age = 21.8 ± 2.2; body mass index = 31.8 ± 4.4) were recruited for two laboratory visits. The first visit involved baseline measurements. In the second visit, participants performed a 55-minute ad libitum exercise session with the RFA in adventure mode with moderate difficulty. During this session, EE, HR, perceived effort, and enjoyment of physical activity were recorded. Results: Although no statistically significant overestimation of EE was found between the RFA and the metabolic cart, two-way analysis of variance results show a main effect of condition (RFA vs. Polar FT7) on HR (122.8 ± 20.1 bpm and 129.0 ± 18.6 bpm; P = 0.007; ᶯp2 = 0.235). Based on comprehensive statistical evaluations, including the mean absolute percent error, intraclass correlations, typical error of measurement, and limits of agreement, the data suggest that the RFA provides reliable estimates for EE and HR. Overall, participants enjoyed the game considerably (71.3 ± 5.9/80 arbitrary units), and their reported perceived exertion was low. Conclusion: This study underscores that the RFA values are relatively accurate and precise, and thus it can be safely suggested for individuals with overweight and obesity to adopt an active lifestyle.

Using a think aloud approach during fixed perceived effort exercise is a unique method to explore the decision-making processes that guide the self-regulation of perceived effort during endurance-based activity. In a two-part study, authors investigated the attentional focus and self-regulatory strategies associated with: Part A - perceived effort corresponding to (RPEGET) and above gas exchange threshold (RPE+15%GET); Part B - between experienced and inexperienced cyclists during fixed perceived effort cycling tasks. Eighteen (15 male, 3 female) healthy, active individuals completed three visits (visit 1 - ramped incremental test and familiarisation, visit 2 and 3-30-min fixed perceived effort cycling). During which, power output, heart rate, lactate, think aloud, and perceptual markers were taken. Random-intercepts linear mixed-effects models assessed the condition, time, and condition × time interactions on all dependent variables. Power output, heart rate, lactate and instances of internal sensory monitoring (t195=2.57,p=.011,ß=0.95[0.23,1.68]) and self-regulation (t195=4.14,p=.001,ß=1.69[0.89,2.49]) were significantly higher in the RPE+15%GET versus RPEGET trial. No significant differences between inexperienced and experienced cyclists for internal sensory monitoring (t196=-1.78,p=.095,ß=-1.73[-3.64,0.18]) or self-regulatory thoughts (t196=-0.39,p=.699,ß=-1.06[-6.32,4.21]) were noted but there were significant condition × time interactions for internal monitoring (t196=2.02,p=.045,ß=0.44[0.01,0.87]) and self-regulation (t196=3.45,p=.001,ß=0.85[0.37,1.33]). Seemingly, experienced athletes associatively attended to internal psychophysiological state and subsequently self-regulate their psychophysiological state at earlier stages of exercise than inexperienced athletes. This is the first study to exhibit the differences in attentional focus and self-regulatory strategies that are activated based on perceived effort intensity and experience level in cyclists.

The aim was to evaluate the concurrent validity and reliability of the Resistance Intensity Scale for Exercise [RISE], which uses verbal descriptors, to quantify the intensity in velocity-based training with elastic bands. Eighteen trained volunteers performed parallel squats at maximum speed at 40%, 55%, 70%, and 85%1RM in four sessions, two for familiarization and two for reliability. Each set was stopped at a 10% intra-set velocity loss. Participants reported the perceived effort (easy-low-moderate-hard-maximal) at the first and last repetition. The concurrent validation was conducted with external load (i.e., mean propulsive velocity, weight, repetitions, and maximum power) and internal load parameters (i.e., heart rate). Participants' relative strength was calculated to assess its influence on the dependent variable. Acceptable concurrent validity and reliability (ICC>0.77, CV<21%) were observed, with the perceived effort being appropriate to differentiate between intensities and not being influenced by the participants' relative strength (p = 0.88). A categorical linear regression showed significant (p < 0.001) associations between the RISE scores and the weight, repetitions, and mean propulsive velocity (r = 0.43-0.63). The findings certify the usefulness of the perceived exertion for quantifying the intensity during velocity-based training with elastic bands. The perceived exertion of the first and last repetition favors a proper dosage of the training load.

BACKGROUND: Post-stroke fatigue can manifest as both physical and mental fatigue. The Fatigue Scale for Motor and Cognitive Functions (FSMC) evaluates fatigue on the motor and cognitive domains separately, however, the psychometric properties of this measure in stroke have not been reported. OBJECTIVE: To determine the internal consistency, test-retest reliability, and concurrent validity of the FSMC in chronic stroke. METHODS: Thirty-four participants with chronic stroke (55.26±12.27 years of age; 59.53±89.21 months post-stroke) completed the FSMC on two separate visits. Internal consistency and reliability of the FSMC were examined using Cronbach's alpha and two-way mixed effects intraclass correlation coefficients (ICC), respectively. Correlation between the FSMC and the Fatigue Severity Scale and Visual Analog Scale-Fatigue was used to assess concurrent validity. RESULTS: Internal consistency was excellent (Cronbach's alphaâ>â0.9) and reliability was moderate to good (ICCâ=â0.72-0.81) for all FSMC scores. The FSMC demonstrated moderate to good concurrent validity with the Fatigue Severity Scale (ρ=â0.66-0.72) but only fair concurrent validity with the Visual Analog Scale-Fatigue (ρ=â0.37-0.44). CONCLUSION: The FSMC is a valid and reliable measure of post-stroke fatigue and may be a useful tool to examine physical fatigue and cognitive fatigue in chronic stroke.

Background: Sit-to-stand (STS) is a mechanically demanding task. Little is known about the energy expenditure (EE) and the perceived effort of patients with stroke during STS. Objectives: The objectives of our study were to assess the perceived effort and EE of patients with stroke when moving from STS and to determine whether an association between actual energy expended and patient-perceived effort exists. Method: This descriptive cross-sectional pilot study assessed participants' EE and perceived effort during STS, with a triaxial accelerometer and the modified Borg scale (MBS), respectively. Results: The team screened 428 individuals for potential inclusion, with nine participants (n = 5 female, 55.5%) meeting the criteria for our pilot study. Participants had a mean age of 52.77 (standard deviation [SD] ± 11.33) years, the majority had a haemorrhagic stroke (n = 6, 66.6%) and left hemiplegia (n = 6, 66.6%), and they were assessed 9.11 (SD ± 6.57) days post-stroke. The mean EE during STS was 2.82 (SD ± 1.9) kCal. Most participants (n = 7, 77.77%) perceived STS as more than a 'moderate' effort on the MBS. The correlation coefficient between the metabolic equivalent of task (METs) and MBS was r = 0.34 (p = 0.38). Conclusion: Our study found a fair positive correlation between METs and MBS for patients with stroke during STS. Clinical implications: The increased EE shown can be a key point for rehabilitation to lessen the extent of EE during STS. Further research is warranted.

We previously showed that perceived effort during visually guided reaching was altered as task demand varied. Further, self-reported subjective fatigue correlated with perceived effort and reach performance under visually guided conditions. Memory-guided reaching often leads to performance deterioration and can provide insights about the planning and control of reach actions. It is unclear how perceived effort changes during memory-guided reaching and whether self-reported subjective fatigue is associated with perceived effort of memory-guided reaching. Twenty-three young adults performed reach actions under visually- and memory-guided conditions. Perceived effort, reaction time, and endpoint error increased significantly from the visually- to the memory-guided condition. Self-reported subjective fatigue was associated with perceived effort and reach distance error during memory-guided reaching; those with higher levels of fatigue reported greater perceived effort and tended to reach farther when visual information was not available. These findings establish a foundation to examine relationships between subjective fatigue, perceived effort, and reach control.

Purpose: To determine whether swimming while listening to fast (140 bpm) or moderate tempo (120 bpm) music enhances physical performance compared to a non-music condition. Methods: Sixteen healthy university students (21.5 ± 2.3 years) with a minimum of one year of experience swimming front crawl were recruited. All completed four testing sessions. In the first session, a graded exercise treadmill test was performed to establish baseline fitness. The next three visits were at the swimming pool and participants were asked to swim as far as possible in 12-minutes (i.e., Swimming Cooper Test) under each of three randomly assigned conditions: 120 bpm (M120), 140 bpm (M140) and a non-music condition (NM). Results: No significant differences were found between conditions for either heart rate (p > .05) or rating of perceived exertion (p > .05). However, differences were found on distance covered (p = .014) between M120 (305.7 ± 19.7 m) and M140 (321.2 ± 19.4 m; p = .035), and on stroke frequency (p = .009) between M120 (48.4 ± 1.8) and M140 (51.6 ± 1.9; p = .028). Conclusion: These results suggest that distance covered and the frequency of strokes per minute were greater when participants were exposed to fast tempo music (M140) compared to moderate tempo music (M120) and a non-music condition (NM) in a 12-minute swimming test.

The perception of effort is elementary for the self-regulation of exercise intensity in sports. The competence for rating perceived effort (RPE) seems to be related to physical and cognitive development. Children accurately rate perceived effort during incremental exercise tests when loads progressively increase, but it remains unclear how children perform when they participate in sports games, which are characterized by complex tasks with varying intensity profiles. The present study investigates children's competencies for rating perceived effort and producing predetermined intensities during soccer training. Twenty-five children aged 11-13 years performed two similar training sessions. In the first session, the children trained without intensity instructions and continuously rated their effort. In the second session, the children were instructed to produce predefined intensities. Before the first training session, executive functions were assessed by cognitive performance tests and a self-report measure. RPE correlated significantly with heart rate measures (R2 = 0.27, p < 0.001). As confirmed by factor analysis, individual differences in these correlations were related to the outcomes of the cognitive tests and the self-report measure. RPE in training session 2 differed from RPE in training session 1 (d = 1.22, p < 0.001), although the heart rate data did not differ significantly between training sessions (d = -0.19, p = 0.780). Thirteen-year-old children performed significantly better than eleven-year-old children (d = 1.69, p = 0.027). The results suggest that children are able to rate perceived effort during soccer training and that this ability is related to executive functions. Conversely, children may not be able to alter their intensities in response to instructions, although their ratings suggest that they have largely succeeded in doing so.

Cognitive-motor training could be used to improve open-skill sport performances, increasing cognitive demands to stimulate executive function (EF) development. Nevertheless, a distributed training proposal for the improvement of EFs is increasingly difficult to combine with seasonal sport commitments. This study aimed to investigate whether a massed basketball training program enriched with Fitlight training can improve EFs and motor performance. Forty-nine players (age = 15.0 ± 1.5 yrs) were assigned to the control and Fitlight-trained (FITL) groups, which performed 3 weeks of massed basketball practice, including 25 min per day of shooting sessions or Fitlight training, respectively. All athletes were tested in cognitive tasks (Flanker/Reverse Flanker; Digit Span) and fitness tests (Agility T-test; Yo-Yo IR1). During the intervention, exercise/session perceived effort (eRPE/sRPE) and enjoyment were collected. RM-ANOVA showed significant EFs scores increased in both groups over time, without differences between the groups. Moreover, an increased sRPE and eRPE appeared in the FITL group (p = 0.0001; p = 0.01), with no group differences in activity enjoyment and fitness tests. Three weeks of massed basketball training improved EFs and motor performance in young players. The additional Fitlight training increased the perceived cognitive effort without decreasing enjoyment, even if it seems unable to induce additional improvements in EFs.

Patients with sternotomy are advised to follow sternal precautions to avoid the risk of sternal complications. However, there are no standard recommendations, in particular to perform the supine-to-sitting postural change, where sternal asymmetrical force may be applied. The aim of this study was to compare the rotational movement and the use of a tied rope (individual device for supine-to-sitting, "IDSS") to perform the supine-to-sitting postural change. A total of 92 patients (26% female) admitted to a rehabilitative post-surgery ward with sternotomy were assessed for sternal instability. Levels of pain and perceived effort during the two modalities of postural change and at rest were assessed. Patients reported higher values of pain and perceived effort (both p < 0.0005) during rotational movement with respect to the use of the IDSS. Moreover, patients with sternal instability (14%) and female patients with macromastia (25%) reported higher pain than those stable or without macromastia (both p < 0.05). No other risk factors were associated with pain. Thus, the IDSS seems to reduce the levels of pain and perceived effort during the supine-to-sitting postural change. Future studies with quantitative assessments are required to suggest the adoption of this technique, mostly in patients with high levels of pain or with sternal instability.
