BACKGROUND: The escalating costs of healthcare had prompted countries to undertake reforms, and in recent years China had focused on overhauling its outpatient healthcare system. China implemented the outpatient mutual-aid policy which had led to a change in the costs associated with outpatient treatment from being fully self-paid by the patient to being partially self-paid. PURPOSE: This study aimed to assess the impact of the outpatient mutual-aid policy on inpatient services for oncology patients in Wuhan, China, exploring the impact that the cumbersome administration of health insurance would have on patient welfare. METHODS: 24,260 oncology patients of the health insurance reimbursement database in Wuhan spanning from January 2022 to July 2023 were included. After data processing, 12,985 patients were included in the control group and 11,275 patients were included in the experimental group. The regression discontinuity design was employed to assess the impact of the policy. RESULTS: The findings was that the implementation of the outpatient mutual-aid would result in a reduction of 1.2 days in the length of stay for oncology patients, a decrease in hospital costs by 5%, and a decline in expenditure of the health insurance reimbursement funds by 5 per cent. CONCLUSIONS: Incorporating outpatient costs into reimbursement supplanted the utilization of inpatient services, enhanced the allocation of healthcare resources, and alleviated the financial burden on oncology patients. Furthermore, it highlighted the detrimental impact of eligibility review to verify that a patient meets the reimbursement requirements of the health insurance policy on patient welfare.

Counterdrug interdiction efforts designed to seize or disrupt cocaine shipments between South American source zones and US markets remain a core US "supply side" drug policy and national security strategy. However, despite a long history of US-led interdiction efforts in the Western Hemisphere, cocaine movements to the United States through Central America, or "narco-trafficking," continue to rise. Here, we developed a spatially explicit agent-based model (ABM), called "NarcoLogic," of narco-trafficker operational decision making in response to interdiction forces to investigate the root causes of interdiction ineffectiveness across space and time. The central premise tested was that spatial proliferation and resiliency of narco-trafficking are not a consequence of ineffective interdiction, but rather part and natural consequence of interdiction itself. Model development relied on multiple theoretical perspectives, empirical studies, media reports, and the authors' own years of field research in the region. Parameterization and validation used the best available, authoritative data source for illicit cocaine flows. Despite inherently biased, unreliable, and/or incomplete data of a clandestine phenomenon, the model compellingly reproduced the "cat-and-mouse" dynamic between narco-traffickers and interdiction forces others have qualitatively described. The model produced qualitatively accurate and quantitatively realistic spatial and temporal patterns of cocaine trafficking in response to interdiction events. The NarcoLogic model offers a much-needed, evidence-based tool for the robust assessment of different drug policy scenarios, and their likely impact on trafficker behavior and the many collateral damages associated with the militarized war on drugs.

Cooperation becomes more difficult as a group becomes larger, but it is unclear where it will break down. Here, we study group size within well-functioning social-ecological systems. We consider centuries-old evidence from hundreds of communities in the Alps that harvested common property resources. Results show that the average group size remained remarkably stable over about six centuries, in contrast to a general increase in the regional population. The population more than doubled, but although single groups experienced fluctuations over time, the average group size remained stable. Ecological factors, such as managing forest instead of pasture land, played a minor role in determining group size. The evidence instead indicates that factors related to social interactions had a significant role in determining group size. We discuss possible interpretations of the findings based on constraints in individual cognition and obstacles in collective decision making.

In the European Union (EU) context, regulatory instruments and incentive schemes targeting individual farms remain the main policy instruments implemented to control diffuse pollution from agriculture. Yet, collective approaches to policy implementation have been recently developing. This article aims at assessing the potential for hybrid policy instruments relying on collective action among farmers to limit diffuse nitrate pollution from agriculture. Transaction cost economics are used to assess the potential advantages of collective action as a complement to regulatory and incentive policy tools. The conditions under which such hybrid forms of governance may be successful are identified using the Social-Ecological System (SES) framework. A review of empirical studies documenting cases of collective action for policy implementation in the EU context serves as a basis for the identification of the factors likely to affect the potential of collective approaches for water quality management in agriculture. The analysis relies more particularly on two cases: the Environmental Cooperatives in the Netherlands and the "Ferti-Mieux" operations in France. The results suggest that collective action is a relevant tool to consider for improving the effectiveness and cost-effectiveness of policies targeting diffuse pollution from agriculture. In particular, relying on farmers' cooperation for policy implementation may be associated with advantages in terms of transaction costs. However, such advantages will be effective under a number of conditions related to the characteristics of the water resource, the actors involved, the governance of cooperation and the broader economic and institutional contexts.

Given a finite set of European call option prices on a single underlying, we want to know when there is a market model that is consistent with these prices. In contrast to previous studies, we allow models where the underlying trades at a bid-ask spread. The main question then is how large (in terms of a deterministic bound) this spread must be to explain the given prices. We fully solve this problem in the case of a single maturity, and give several partial results for multiple maturities. For the latter, our main mathematical tool is a recent result on approximation by peacocks.

By Asian digital economy, we refer to high-tech developments, business and social transformations, and information-driven changes in the region's growth. We discuss its background and foundations, significance in Asia and contribution to removal of historical barriers in traditional business. We assess how new value chains are transforming country-level involvement in worldwide manufacturing and note "smiling curve theory" predictions about the global value chain in Asia for high-tech firms and their economies. The takeaway is that the digital economy in Asian nations involves revamping business processes through technology innovation, government policies for growth, and digital entrepreneurship. We analyze the "digital economy and society index", and attributes of nations, societies and economies, as a basis for framing our ideas. We consider research directions prompted by data analytics and AI, the platform economy, digital trade, fintech innovation, and societal and economic sustainability. We further highlight new issues in light of the COVID-19 pandemic.

Transaction costs and uncertainty are considered to be significant obstacles in the emissions trading market, especially for including nonpoint source in water quality trading. This study develops a nonlinear programming model to simulate how uncertainty and transaction costs affect the performance of point/nonpoint source (PS/NPS) water quality trading in the Lake Tai watershed, China. The results demonstrate that PS/NPS water quality trading is a highly cost-effective instrument for emissions abatement in the Lake Tai watershed, which can save 89.33% on pollution abatement costs compared to trading only between nonpoint sources. However, uncertainty can significantly reduce the cost-effectiveness by reducing trading volume. In addition, transaction costs from bargaining and decision making raise total pollution abatement costs directly and cause the offset system to deviate from the optimal state. While proper investment in monitoring and measuring of nonpoint emissions can decrease uncertainty and save on the total abatement costs. Finally, we show that the dispersed ownership of China's farmland will bring high uncertainty and transaction costs into the PS/NPS offset system, even if the pollution abatement cost is lower than for point sources.

During the last three decades, there has been an increased pursuit of participatory approaches to managing natural resources. In South Africa, this has been evident in the management of protected areas. In particular, land claims, which affect much of the conservation estate in South Africa, frequently result in co-management of protected areas by claimant communities and conservation agencies. This is occurring against a backdrop of declining state subsidies and growing expectations that South African conservation agencies will finance themselves while simultaneously stimulating local economic opportunities. In this context, it is important for co-management partners to understand and monitor the cost-effectiveness of management processes in achieving both the socio-economic and ecological targets of conservation management. Transaction costs are useful in gauging the cost-effectiveness of policies and institutions; however there is little methodological guidance for measuring transaction costs empirically. This study develops and tests a transaction costs model for a co-managed nature reserve in the Eastern Cape province of South Africa. Transaction costs were quantified by taking into account the total time spent in meetings annually, the daily opportunity cost of participants' time and the travel costs associated with attending such meetings. A key limitation in the development of this model was a lack of record keeping by the conservation agency. The model developed in this study offers a practical means for co-management partners in similar contexts to monitor how transaction costs change over time.

This study considers the implementation of the "Broadband China" strategy as an exogenous policy shock and examines the impact of network infrastructure construction (NIC) on the low-carbon innovation (LCI) of enterprises and its underlying mechanisms by using a progressive difference-in-difference model based on the data of Chinese listed enterprises from 2009 to 2020. This study finds that NIC can improve the LCI of enterprises. After the elimination of the sample selection bias and selection of the urban slope as the exogenous instrumental variable, the conclusions remained robust. The results of the mechanism test show that upgrading the human capital level, reducing transaction costs, and alleviating financing constraints are the three important paths through which NIC can help enterprises improve their LCI level. The heterogeneity analysis determines that NIC has considerable comparative advantages for enterprises with executives who have a financial background and enterprises with high knowledge stock. In addition, LCI improvement can further enhance enterprise value. The research conclusions can broaden the microscopic research perspective of enterprise transformation and upgrading theory and provide reliable empirical evidence for China's low-carbon economic transformation.

BACKGROUND: Hemoglobinopathies, among which thalassemic syndromes (transfusion-dependent and non-transfusion dependent thalassemias) and sickle cell disease (SCD), are the most widespread monogenic diseases worldwide. Hemoglobinopathies are endemic and spread-out all-over Italy, as result of internal and external migration flows. Nowadays, the increase therapeutic options associated to the general aging of patients with hemoglobinopathies related to the improvement in clinical management, contribute to the abnormalities in kidney function going from blood and urine test alterations to chronic kidney disease and end stage renal disease. METHODS: Here, we carried out a revision of the literature as panel of recognized experts in hemoglobinopathies with the consultancy and the revision of two nephrologists on kidney alteration and kidney disease in patients with TDT, NTDT and SCD. This is part of the action of the Italian society for the study of thalassemia and hemoglobinopties (SITE). The purpose of this "good practice (GP)" is to provide recommendations for follow-up and therapy for the management of kidney alterations in patients with TDT, NTDT and SCD. The literature review covers the period 1.1.2016 to 31.12.2022. In consideration of the rarity of these diseases, the analysis was extended from 5 to 7 years. Moreover, in the absence of relevant scientific papers in the identified time frame, we referred to pivotal or population studies, when available. Finally, in the absence of evidence-based data from prospective and randomized trials, the authors had to refer to expert opinion (expert consensus) for many topics. RESULTS: We generated question and answer boxes to offer a friendly consultation, using color code strategy and focused answers. CONCLUSIONS: The present GP will help in improving the clinical management, and the quality of care of patients with hemoglobinopathies.

Circulation has always been an important research topic in ecological agriculture. The connotation of production and marketing is not only the economic connection, but also the social interaction. Starting from the perspective of transaction costs and social capital, this study attempts to take the Karst rocky desertification management areas as the study area, and by way of the field interviews and literature methods, and through sorting, induction, conceptual cross-comparison, and repeated inferences, to summarize the circumstances and contexts of the five distribution channels of ecological products in the study areas, with the aim of discussing the correlation between transaction costs, social capital, and the market distribution models of ecological products. It is discovered that (1) the intervention of non-economic factors, which play an important role and function in economic activities and can effectively reduce transaction costs and promote the circulation of social capital, forming a closer cooperation relationship, is the key to the production and marketing of ecological products. (2) It innovatively proposes the bonding, bridging, and linking social capital types to explain the social relationship between the parties under the distribution channel of ecological products, which are divided into, based on the closeness from low to high, the bonding, linking, and bridging types. (3) In product distribution, the higher the social capital circulation is, the more transparent the information exchange of market distribution channels would be, and the lower the transaction costs for both parties would become. The study attempts to provide theoretical support for the construction of the full industrial chain of ecological products for Karst rocky desertification management in Southern China.

Aerosol insecticides are widely used in stored product insect management programs in food facilities. Previous research has shown spatial variation in aerosol efficacy within facilities, but information on how spatial patterns of aerosol droplet concentration, size distribution, dispersal, and deposition contribute to this variation in efficacy is limited. This study involved two aerosol application systems: a high-pressure cylinder containing TurboCide Py-75® with pyriproxyfen IGR (ChemTech Ltd., Des Moines, IA, USA) and a hand-held fogger containing Pyrocide 100® (MGK, Minneapolis, MN, USA) with Diacon II which contains methoprene IGR (Wellmark, Schaumburg, IL, USA). These systems were used at single or multiple application locations. The spray trials were conducted in a small-scale flour mill, Hall Ross Flour Mill (Kansas State University, Manhattan, KS, USA). The droplet size distributions were monitored at multiple positions within the room using nine aerodynamic particle sizing (APS, TSI Incorp, Shoreview, MN, USA) instruments. The APS data collected over the treatment period were summarized into a mass concentration index (MCI), which ranged from 155 to 2549 mg/m3 for Turbocide and 235-5658 mg/m3 for Pyrocide. A second parameter called the Deposition Index (Dep.Idx) was derived to estimate potential insecticide depositions on the floor and has units of g/m2. The Dep.Idx was below 5.3 g/m2 for most Turbocide applications, while the Dep.Idx was below 8.4 g/m2 for most Pyrocide applications. The MCI and Dep.Idx values varied with APS position and spray application location, with proximity to the aerosol application location and degree of obstruction between the release point and APS position contributing to this variation. We assessed the relationship between aerosol droplet parameters and insect efficacy using Tribolium confusum Jacqueline DuVal, the confused flour beetle. The adults were treated directly, while the larvae were treated two weeks later during the residual test (previously published). For Turbocide, efficacy against adults increased with MCI and Dep.Idx values, but for residual efficacy of the IGR, efficacy was high at all aerosol droplet values, so no relationship was apparent. In contrast, the relationship between Pyrocide deposition and adult insect efficacy was highly variable. But with larval insect efficacy, residual larvae control was directly related to increases in Pyrocide MCI and Dep.Idx. Contour plots of Dep.Idx values were developed, which could be used to predict areas of the mill that are not receiving an adequate application rate, and this could be used to develop more effective application strategies for aerosol insecticides in food facilities.

Introduction: Digital transformation has become an important engine for economic high-quality development and environment high-level protection. However, green total factor productivity (GTFP), as an indicator that comprehensively reflects economic and environmental benefits, there is a lack of studies that analyze the effect of digital transformation on heavily polluting enterprises' GTFP from a micro perspective, and its impact mechanism is still unclear. Therefore, we aim to study the impact of digital transformation on heavily polluting enterprises' GTFP and its mechanism, and explore the heterogeneity of its impact. Methods: We use Chinese A-share listed enterprises in the heavily polluting industry data from 2007 to 2019, measure enterprise digital transformation indicator using text analysis, and measure enterprise GTFP indicator using the GML index based on SBM directional distance function, to investigate the impact of digital transformation on heavily polluting enterprises' GTFP. Results: Digital transformation can significantly enhance heavily polluting enterprises' GTFP, and this finding still holds after considering the endogenous problem and conducting robustness tests. Digital transformation can enhance heavily polluting enterprises' GTFP by promoting green innovation, improving management efficiency, and reducing external transaction costs. The improvement role of digital transformation on heavily polluting enterprises' GTFP is more obvious in the samples of non-state-owned enterprises, non-high-tech industries, and the eastern region. Compared with blockchain technology, artificial intelligence technology, cloud computing technology, big data technology, and digital technology application can significantly improve heavily polluting enterprises' GTFP. Discussion: Our paper breaks through the limitations of existing research, which not only theoretically enriches the literature related to digital transformation and GTFP, but also practically provides policy implications for continuously promoting heavily polluting enterprises' digital transformation and facilitating their high-quality development.

We measure organizational concentration-the distribution of a patient's healthcare across organizations-to examine how firm boundaries affect healthcare efficiency. First, when patients move to regions where outpatient visits are typically concentrated within a small set of firms, their healthcare utilization falls. Second, for patients whose PCPs exit the market, switching to a PCP with 1 standard deviation higher organizational concentration reduces utilization by 21%. This finding is robust to controlling for the spread of healthcare across providers. Increases in organizational concentration predict improvements in diabetes care and are not associated with greater use of emergency department or inpatient care.

We implement an allocation strategy through a regime-switching model using recursive utility preferences in an out-of-sample exercise accounting for transaction costs. We study portfolios turnover and leverage, proposing two procedures to constrain the allocation strategies: a low-turnover control (LoT) and a maximum leverage control (MaxLev). LoT sets a dynamic threshold to trim minor rebalancing, reducing portfolio turnover, mitigating costs. MaxLev calculates dynamic adjustments to the risk aversion parameter to constrain the portfolio leverage. The MaxLev adjustments depend on the risk aversion and permitted portfolio leverage, which enables optimal strategies considering the leverage constraints. The study uses US equity portfolios, and shows that, first, models with LoT result in superior return-to-risk measures than those without it when transaction costs increase. Second, considering transaction costs, the return-to-risk measures of the models using MaxLev closely match or exceed those from the corresponding unconstrained regime-switching benchmarks. Third, MaxLev returns have lower volatility and higher return-to-risk than conventional numerically constrained benchmarks. Fourth, the certainty equivalent returns indicate that models using MaxLev and LoT outperform both single-state models and unconstrained regime-switching models with statistical significance. Introduction: Do externalities work and matter differently in a world of scarcity vs. a world of abundance? In this article, we critically examine the economic phenomena of externalities. The concept of externality, an important idea in economics and law, is useful in exploring the complex and dynamic relationships between resource supply and human flourishing within various sociotechnical systems. Methods: First, we define the basic concept and explain why it is fundamental to economic analysis of complex social environments Second, we briefly survey the intellectual history of externalities with the goal of tying together a few different strands of economic theory and providing a roadmap for a general theory of externalities. This discussion highlights a latent conflict between those who pursue and those who resist perfectibility (optimization) of social systems by internalizing externalities. Third, we compare externalities in worlds of scarcity and abundance. Results: This article develops the theoretical framework, including a brief intellectual history and notes toward the development of a general theory of externalities. As a conceptual tool, externalities enable one to identify and examine social interdependencies and to map their causes and consequences. Externalities provide evidence of social demand for governance institutions. This descriptive utility can and should inform normative analysis, the design of governance institutions, and comparative institutional analysis. We also raise a series of (mostly empirical) questions that should frame comparative institutional analysis and evaluation of different externalities in the digital networked world. Discussion: We focus on the scarcity and abundance of knowledge resources and the (technological) means for participating in the production, dissemination, and modification of such resources. In the real, necessarily imperfect world where abundance and scarcity vary across resources, people, and contexts, externalities persist, indicate social demand for governance, and inform comparative analysis and design of governance institutions. Limbal stem cells (LSCs) reside discretely at limbus surrounded by niche cells and progenitor cells. The aim of this study is to identify the heterogeneous cell populations at limbus under normal homeostasis and upon wounding using single-cell RNA sequencing in a mouse model. Two putative LSC types were identified which showed a differentiation trajectory into limbal progenitor cell (LPC) types under normal homeostasis and during wound healing. They were designated as "putative active LSCs" and "putative quiescent LSCs", respectively, because the former type actively divided upon wounding while the later type stayed at a quiescent status upon wounding. The "putative quiescent LSCs" might contribute to a barrier function due to their characteristic markers regulating vascular and epithelial barrier and growth. Different types of LPCs at different proliferative statuses were identified in unwounded and wounded corneas with distinctive markers. Four maturation markers (Aldh3, Slurp1, Tkt, and Krt12) were screened out for corneal epithelium, which showed an increased expression along the differentiation trajectory during corneal epithelial maturation. In conclusion, our study identified two different types of putative LSCs and several types of putative LPCs under normal homeostasis and upon wounding, which will facilitate the understanding of corneal epithelial regeneration and wound healing.

Type 2 diabetes mellitus (DM) patients are at a higher risk for developing lung cancer due to immune dysfunction and chronic inflammation. They also have increased morbidity and mortality related to influenza, and it is recommended that they receive an annual influenza vaccination. In this study, we evaluate whether influenza vaccination could reduce the incidence of lung cancer in DM patients. This cohort study included DM patients (≥55 years old) between 1 January 2002 and 31 December 2012 by using the Taiwan Health Insurance Database. Cox proportional hazard regression method was used to compare the relation between the influenza vaccination and lung cancer incidence after adjusting for potential confounders. Sub-group analyses were done according to vaccination status (unvaccinated, total number of vaccinations: 1, 2-3, ≥4) and evaluated the dose-dependent effects on lung cancer events. Among 22,252 eligible DM patients, 7860 (35.32%) received an influenza vaccination and 67.68% (14392) did not receive an influenza vaccination. Lung cancer incidence was significantly lower in the vaccinated group versus the unvaccinated group (adjusted HR 0.77; 95% CI 0.62-0.95, p < 0.05). Significant protective effects were observed among male sex (adjusted HR 0.72; 95% CI 0.55-0.94, p < 0.05) and 55-64 year (adjusted HR 0.61; 95% CI 0.40-0.94, p < 0.05) and ≥75 year (adjusted HR 0.63; 95% CI 0.42-0.92, p < 0.05) age groups, respectively. A dose-dependent protective effect was noted with a significant protective effect in those that received ≥4 vaccinations (adjusted HR 0.42; 95% CI 0.29-0.61, p < 0.001). In sub-group analysis, elder patients with ≥65 years of age were significantly protected from ≥4 vaccinations (adjusted HR 0.37; 95% CI 0.23-0.62, p < 0.001 in 65-74 years and adjusted HR 0.31; 95% CI 0.15-0.66, p = 0.002 in ≥75 years group, respectively). Male sex with ≥4 vaccinations had a significantly lower risk of lung cancer (adjusted HR 0.35; 95% CI 0.21-0.57, p < 0.001). Patients with comorbid conditions that received ≥4 vaccinations were also protected, and was especially significant among those with CCI ≥ 3 (adjusted HR 0.38; 95% CI 0.18-0.80, p = 0.009) as compared to 1 and 2-3 vaccination groups, including those with hypertension (adjusted HR 0.35; 95% CI 0.22-0.57, p < 0.001). This population-based cohort study demonstrated that annual influenza vaccination significantly reduced the lung cancer risk in DM patients and specifically demonstrates that a higher number of vaccinations is related with a more protective effect. Whether this is due to vaccine booster effects on anti-tumor immune regulation among DM patients still needs to be explored.

The thymus is the main lymphoid organ that regulates the immune and endocrine systems by controlling thymic cell proliferation and differentiation. The gland is a primary lymphoid organ responsible for generating mature T cells into CD4+ or CD8+ single-positive (SP) T cells, contributing to cellular immunity. Regarding humoral immunity, the thymic plasma cells almost exclusively secrete IgG1 and IgG3, the two main complement-fixing effector IgG subclasses. Deformity in the thymus can lead to inflammatory diseases. Hassall's corpuscles' epithelial lining produces thymic stromal lymphopoietin, which induces differentiation of CDs thymocytes into regulatory T cells within the thymus medulla. Thymic B lymphocytes produce immunoglobulins and immunoregulating hormones, including thymosin. Modulation in T cell and naive T cells decrement due to thymus deformity induce alteration in the secretion of various inflammatory factors, resulting in multiple diseases. Influenza virus activates thymic CD4+ CD8+ thymocytes and a large amount of IFNγ. IFNs limit virus spread, enhance macrophages' phagocytosis, and promote the natural killer cell restriction activity against infected cells. Th2 lymphocytes-produced cytokine IL-4 can bind to antiviral INFγ, decreasing the cell susceptibility and downregulating viral receptors. COVID-19 epitopes (S, M, and N proteins) with ≥90% identity to the SARS-CoV sequence have been predicted. These epitopes trigger immunity for antibodies production. Boosting the immune system by improving thymus function can be a therapeutic strategy for preventing virus-related diseases. This review aims to summarize the endocrine-immunoregulatory functions of the thymus and the underlying mechanisms in the prevention of COVID-19.

Climate change represents a serious threat to life in earth. Agriculture releases significant emissions of greenhouse gases (GHG), but also offers low-cost opportunities to mitigate GHG emissions. This paper assesses agricultural GHG emissions in Aragon, one important and representative region for agriculture in Spain. The Marginal Abatement Cost Curve (MACC) approach is used to analyze the abatement potential and cost-efficiency of mitigation measures under several scenarios, with and without taking into account the interaction among measures and their transaction costs. The assessment identifies the environmental and economic outcomes of different combinations of measures, including crop, livestock and forest measures. Some of these measures are win-win, with pollution abatement at negative costs to farmers. Moreover, we develop future mitigation scenarios for agriculture toward the year 2050. Results highlight the trade-offs and synergies between the economic and environmental outcomes of mitigation measures. The biophysical processes underlying mitigation efforts are assessed taking into account the significant effects of interactions between measures. Interactions reduce the abatement potential and worsen the cost-efficiency of measures. The inclusion of transaction costs provides a better ranking of measures and a more accurate estimation of implementation costs. The scenario analysis shows how the combinations of measures could reduce emissions by up to 75% and promote sustainable agriculture in the future.
