INTRODUCTION: Failure to adopt a correct working posture can lead to occupational diseases. OBJECTIVES: Evaluate knowledge in relation to ergonomics about BHOP concept and its application to routine clinical practice amongst undergraduate and postgraduate dental students in the University of Valencia (Valencia, Spain). MATERIAL AND METHODS: A study based on interviews of undergraduate and postgraduate dental students in the University of Valencia (Valencia, Spain) was carried out. The information from a total of 336 interviews was used for the statistical analysis, differentiating according to gender and academic year: knowledge of ergonomics, pain prevalence and antecedents, assessment of the possible necessity for improved training in ergonomics, and evaluation of postural hygiene. RESULTS: Only 28.6% of the students were found to sit correctly in the dentist chair. Furthermore, in the opinion of the students, very few subjects during the career afforded adequate teaching in relation to ergonomics and working posture. The analysis of postural hygiene showed great variability. There were no significant differences in posture between males and females, although some incorrect postures appeared to be associated with the academic year (P<.05). However, no significant improvements in postural hygiene were noted on progressing from one academic year to the next. CONCLUSIONS: The students in our study were not familiar with the principles of ergonomics and did not sit correctly in the dentist chair. Improved training in this field is required in dental school.

BACKGROUND: Previous studies suggest that expression of hypoxia markers may be associated with response to antiangiogenic drugs. Thus, we aimed to identify predictors of sunitinib outcome in clear-cell renal cell carcinoma (ccRCC). PATIENTS AND METHODS: The expression of eight key proteins related to hypoxia (CAIX, HIF1A, HIF2A, VEGFA, VEGFR1, VEGFR2, VEGFR3 and PDGFRB) and P-glycoprotein were assessed by immunohistochemistry in 67 primary ccRCC samples from prospectively recruited patients treated with first-line sunitinib. The proteins expression, VHL inactivation and EGLN3 mRNA content were compared with the patients' response to sunitinib. RESULTS: High expression of HIF2A and PDGFRB was associated with better sunitinib RECIST objective response (P = 0.024 and P = 0.026; respectively) and increased VEGFR3 expression was associated with longer progression-free survival (P = 0.012). VEGFR3 overexpression showed a negative correlation with VEGFR3 polymorphism rs307826 (P = 0.002), a sunitinib resistance predictor. With respect to overall survival (OS), high VEGFA was associated with short (P = 0.009) and HIF2A with long (P = 0.048) survival times. High EGLN3 mRNA content was associated with shorter OS (P = 0.023). CONCLUSIONS: We found an association between several proteins involved in hypoxia and sunitinib efficacy. In addition, low VEGFR3 expression was associated with worse outcome and with VEGFR3 rs307826 variant allele, reinforcing VEGFR3 as a marker of sunitinib resistance.

BACKGROUND: Lymphadenectomy for prostate cancer (PC) is the most reliable procedure for detecting lymphatic metastases. The optimal extension of this procedure is still a topic of debate. OBJECTIVE: To analyse the diagnostic performance and complications of extended lymphadenectomy (ELD) and limited lymphadenectomy (LLD) in a series of patients with high-risk PC who underwent radical prostatectomy (RP). MATERIAL AND METHODS: A retrospective study was conducted on patients with high d'Amico risk who underwent RP with lymphadenectomy between 1999 and 2014. A comparative analysis was performed of the diagnostic capacity of lymphatic metastases of ELD and LLD and of postoperative complications at 90 days. RESULTS: Ninety-three patients were analysed, 20 (21.5%) and 73 (78.5%) of whom underwent ELD and LLD, respectively. The mean age of the series was 65.26 years (SD, 5.51). The median follow-up was 1.51 (0.61-2.29) years in the ELD group and 5.94 (3.61-9.10) in the LLD group. The median number of nodes obtained was 13 (9-23) in the ELD group compared with 5 (2-8) in the LLD group (p <.001). The percentages of patients with positive nodes in the ELD and LLD groups were 35% and 5.47%, respectively (p <.001). The overall complication rate at 90 days was 35.5% (33 patients). In the ELD group, 12 patients (60%) had complications, compared with 21 patients (28.8%) in the LLD group (p=.016), with no significant differences in severity according to the Clavien scale (p=.73). CONCLUSIONS: In our series, the detection of metastatic nodes was significantly greater with ELD. ELD increases the number of complications, with no differences compared with LLD in severity according to the modified Clavien scale.

RESUMO

A case of gangliocytic paraganglioma of the papilla of Vater in a 76-year-old man with a history of recurrent obstructive jaundice is presented. This is the first case of gangliocytic paraganglioma of the major papilla successfully resected by endoscopic ampullectomy.

This study has given evaluation of a new pneumallergen diagnostic test CIS Allergen Screen I in comparison with Pharmacia Cap System and intradermal skin test. Five allergens (Mite (DPT) D1, Mite (DF) D2, Cat E1, Dog E2, Orchard grass G3) have been studied with in vitro tests (CIS Allergen Screen I Cap System) and the results obtained gave on patient to patient comparison a sensitivity of 91%, a specificity of 100% and on allergen comparison a sensitivity of 84%, a specificity of 99% and an accuracy of 93%. Compared with intradermal skin test for two allergens (Mite (DT) D1 and Orchard grass G3), CIS Allergen Screen I have good results to G3 but less specificity and sensitivity to D1. These results could be to depend on different standardisation between allergen extracts especially for Mite.

RESUMO

A 33-year-old woman sought medical attention for a painful swelling of the left ankle. Plain radiographs revealed an osteolytic lesion involving the left distal tibia. An excisional biopsy provided the diagnosis of leiomyosarcoma in the tibia. A staging work-up was performed and an abdominal CT showed 4 liver hypodense lesions in both lobes with peripheral contrast enhancement. A liver biopsy confirmed the diagnosis of epithelioid hemangioendothelioma of the liver. No association between these two entities has been described before. This case introduces the importance of the pathological confirmation of apparent metastatic lesions in low grade sarcomas and provides a review of the literature of both tumours.

We have a bibliografical study of the composite odontomas from the diagnostical point of view. We will present two clinical cases. Once the medical revision is done, and the tumor is localized, we analyze lesions associated with the tumor and compare the diagnostic from the clinical point of view as well as the radiografical point of view.
