RESUMO
Two techniques using molded autografts on intact ossicular chains were compared in 159 patients: some patients received molded autografts alone while in the others it was reinforced by a second fragment of temporal aponeurosis inserted behind the manubrium of the malleus. Fixation of these two grafts prevented ascension of the graft into the canal. Results obtained showed slight superiority for the double graft over the single one.

A 5-year follow up study of functional results of transposition of the anvil was conducted in 114 patients. Audiometric gains obtained could be rated as satisfactory since closure of more than half of the Rinne was obtained in 70% of cases and the gain was maintained over long periods.

The most important publications concerning the complications following transnasal ethmoid surgery are reviewed by the authors who underline the dangers in this type of surgery: meninges, the orbit and orbital elements especially the muscles, the extra-orbital portion of the optic nerve and the internal carotid artery. A great prudence during surgical procedure is emphasized as well as the necessity of a perfect knowledge in scannographic and endoscopic anatomy of ethmoid to avoid the inflation of these complications.
