ABSTRACT: We evaluated the quality of care provided by the nursing team within specialized care services (SCS) from the perspective of persons with HIV (PWH). A cross-sectional study was conducted in 25 SCS selected by a single-stage cluster sampling in 21 municipalities in Brazil. Systematic sampling was performed between October 2019 and March 2020 in the reception areas of SCS locations to choose the study population ( N = 377). The adapted and validated Quality of Care Through the Patient's Eyes-HIV (QUOTE-HIV) questionnaire was used to measure the quality of perceived care, and the Mann-Whitney test to compare the scores between men and women. Of the 377 PWH, most were women (57.71%), Black or Brown (85.9%), heterosexual (67.1%), and cisgendered (96.0%); the mean age was ≥30 years (76.1%). Users' evaluation of the care provided by SCS according to scores of importance and performance of the QUOTE-HIV instrument ranged from 5.60 to 8.95 and 0.03 to 0.76, respectively. The perception of PWH about the quality of care provided by the nursing team in SCS suggests that there are opportunities to improve.

RESUMO

It will be presented the main academic contributions of Professor Ana Marlúcia Oliveira (AMO) (in memoriam), nutritionist, professor at the School of Nutrition at the Federal University of Bahia, Ph.D. in epidemiology and CNPQ Researcher level A, from 1980 to 2021. Professor Ana accumulated, throughout her academic career, scientific articles published in national and international journals; book and book chapters authored by her; papers presented at scientific events, in addition to guiding scientific projects, dissertations and theses. She has coordinated several research projects in the field of food and nutrition in public health, with a focus on nutritional epidemiology. The scope of the subjects addressed in her scientific production expressed the concern that mobilized her around the production of knowledge to face the complex health and nutrition problems in Brazil. Her way of being in the world, welcoming and caring for people who approached her seeking qualification opportunities, her example, words and teachings influenced, and still influence, the trajectory and training of nutritionists, professors and researchers at ENUFBA and other national and international institutions. She was a Brazilian researcher and intellectual committed to the health of the most vulnerable populations and the fight against malnutrition and hunger in our country. Her wide and fruitful work left us a legacy to be remembered and continued. Some of her friends, colleagues and collaborators pay this tribute to her memory, to her example and to the legacy she left for all of us and future generations.


São apresentadas neste artigo as principais contribuições acadêmicas da Profa. Marlúcia Oliveira (AMO) (in memoriam), nutricionista, docente da Escola de Nutrição da Universidade Federal da Bahia, Doutora em Epidemiologia e Pesquisadora A do Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPQ), no período de 1980 a 2021. Food service work is hazardous due to the intense rhythm of food production, and the working conditions can cause discomfort, fatigue, and occupational accidents and illnesses. For the perception of exposure to occupational hazards, workers must participate in continuing education programs. This study aimed to verify the perception and exposure to occupational risks at school food services (SFS) in Bahia, Brazil. This cross-sectional study was conducted in SFS from public schools in Bahia/Brazil. Researchers identified sociodemographic variables, occupational characteristics, and the Perception of Exposure to Occupational Risks by SFS food handlers. Also, anthropometric assessment (weight, height, and waist circumference), the presence of comorbidities, and the identification of exposure to occupational risks and measures of environmental comfort were evaluated. Most workers were female (98.6%; n = 140), mean age of 46.85 y/o, working as SFS food handlers between 1 and 5 years (50.7%; n = 72) but with no training on occupational risks (52.8%; n = 75). This lack of training is not associated with demographic or other occupational variables. The majority of the food handlers present a fair or good perception of exposure to occupational risk. These food handlers are also mostly overweight, and higher BMI was associated with hypertension and edema. The SFS were classified as of high occupational risk (mean of 31.24% of adequacy) environments. Ergonomic Risks had the lowest percentage of adequacy (7.69%, very high risk) regarding occupational risks, followed by chemical risks (31.5% of adequacy, high risk), accident risk (32.19%, high risk), and physical risk (36.89%, high risk). The excess of activities associated with precarious physical structure, insufficient number of equipment and utensils (in inadequate conservation) favors the exposure to occupational risk in SFS.

This cross-sectional study aimed to identify factors that interfere with the adoption of good hygiene practices in public school food services (SFS) in Bahia, Brazil. The search was conducted in public schools in Bahia/Brazil. Data collection included (i) evaluation of the adoption of Good Practices in school food services through visual observation and registration in the checklist in Good Hygienic Practices in School Food Services; (ii) Identification of schools' foodservice physical areas and environmental comfort measures; (iii) identification of sociodemographic and occupational characteristics and assessment of attitudes and level of knowledge in food hygiene. The minimum sample of 158 schools (confidence level of 95% and an error of 5%) was stratified considering the total number of districts (areas) and the schools' number per area. The data were analyzed using the Statistical Package for Science-SPSS® in version 26.0. The categorical variables were described as frequencies and percentages, and the quantitative variables as mean and standard deviation. Chi-square, Kruskal-Wallis with Tukey's post-hoc tests were used to examine qualitative variables. Independent Student t-test and ANOVA with Tukey's post-hoc tests were used to examine quantitative variables. Categorical variables were evaluated by Pearson Chi-squared tests. The Pearson test was used to analyze the correlation between Attitude and knowledge. The classification of the sanitary status was of moderate risk in 74.8% (n = 119) of the SFS (51-75% of compliance) and 25.2% (n = 40) at high risk (26-50% compliance). The average percentage of compliance for the 159 SFS in the municipality was 50.23%, obtaining a high-risk classification (26-50% compliance). In the SFS, the absence of dry goods' storage, meat preparation area, and storage of residues in more than 98% of schools was observed. Accessing attitudes, 1.4% (n = 2) of the food handlers were classified as unsatisfactory (0 to 49% of correct answers), 8.5% (n = 12) as satisfactory with restriction (50-69% of correct answers) and 90.1% (n = 128) as satisfactory. There was no correlation between Attitude and Knowledge level (p = 0.394). Considering the knowledge level, the item with the highest and lowest number of hits were: "To avoid food contamination, I wash and disinfect my hands before preparing food" and "Contaminated food will always have some change in color, smell, or taste". There was a significant difference in the level of knowledge considering mean wage (p = 0.000), time working in school food service (p = 0.001), weekly workday (p = 0.000), and participation in food hygiene training (p = 0.000). Therefore, factors that interfered in adopting good practices in the SFS were: inadequate physical structure, absence of areas in the SFS, and absence/low number of equipment to control the production process in the cold and hot chain. Food handlers showed satisfactory attitudes and level of knowledge. However, the physical structure of the SFS compromises the adoption of good practices. It risks the safety of the food served to students at the evaluated public schools.

Vitamin A supplements have been distributed during National Immunization Days since 1983, and Brazil has been a pioneer in this kind of strategy. The current study evaluated the National Program from 1995 to 2002, from a structure-process-results perspective. The methodology involved document research, interviews with health services managers, and a case study in 44 municipalities in Bahia State, resulting in 1,344 household interviews. In relation to structure, from 1998 on the program operated without a normative instrument due to the extinction of the National Institute of Nutrition. Improvements in the processes of vitamin supply procurement and distribution had a positive impact on coverage, reaching 3.5 million capsules in 2002 (72% coverage). For the 2,546 children studied in Bahia, vitamin A distribution was irregular, with annual coverage varying from 8% to 26%. Program managers reported time constraints and insufficient human resources, consistent with the hypothesis that this type of strategy does not help inform the population about the importance of vitamin A. These findings point to the need for systematic linkage between different levels of government in order to fully implement and monitor the program.

RESUMO

Diet is a vital health question for workers. This review of the related literature sought to identify the elements that could contribute to an examination of the topic and help ascertain how it has been approached from a historical perspective. Our bibliographic research was based primarily on the Scientific Electronic Library On-line, from which pertinent articles were selected for analysis, without regard to year of publication. Among other findings, studies on worker health proved scarce; moreover, the few studies that have explored nutrition focus on the Worker's Food Program, primarily from a quantitative perspective and without taking the category of 'work process' into account in their analyses.

RESUMO

Empreende uma revisão de literatura sobre a alimentação do trabalhador no Brasil, questão importante para a saúde dos trabalhadores. Pretende oferecer elementos que permitam reflexão acerca do tema, procurando entender, sob uma perspectiva histórica, o tratamento que lhe foi dado. A pesquisa bibliográfica baseou-se sobretudo na Scientific Electronic Library On-line, cujos artigos sobre o tema foram analisados sem restrições quanto ao ano de sua publicação. Constatou-se, entre outros resultados, que o tema é rarefeito nos estudos de saúde do trabalhador, e que na nutrição as investigações são poucas, concentradas no Programa de Alimentação do Trabalhador, predominantemente de natureza quantitativa e desconsiderando a categoria 'processo de trabalho' nas análises.

Com o objetivo de avaliar a qualidade de óleos e/ou gorduras utilizados para fritura por imersão em bares, restaurantes e lanchonetes, foram coletadas 180 amostras de óleo e/ou gordura, sendo 90 antes e 90 após fritura. Aplicou-se um questionário para avaliar as técnicas de fritura e a infra estrutura dos estabelecimentos. Foram realizados os métodos analíticos para avaliar a alteração dos óleos e gorduras a saber, compostos polares totais (%), acidez (mgKOH/g) e índice de peróxido (mEq/kg). Das 90 amostras coletadas antes da fritura, 18,18 % demonstraram valores de compostos polares superiores ao limite estabelecido (5%), predominantemente nas amostras coletadas de lanchonetes. Quanto aos compostos polares totais, acidez e índice de peróxidos para óleos e gordura de fritura, 7,78%, 12,22 % e 2,22 % das amostras, respectivamente, apresentaram valores acima dos limites estabelecidos para efetuar o descarte. A avaliação das técnicas de fritura demonstrou dados insatisfatórios, tais como falta de controle da temperatura, tipo de óleo e equipamentos empregados. Os resultados obtidos indicam a necessidade de ações educativas no setor, assim como efetiva fiscalização por parte dos órgãos competentes.


Aiming at assessing the quality of oils and fats employed for frying food in bars, restaurants and snack bars, 180 samples were analyzed, from which 90 were collected before use and 90 after the frying process. A questionnaire was used in order to collect information on food frying procedures and on the respective food-serving places infrastructure. The analysis on alterations in oil and fat samples was performed by using the following analytical methods: total polar compounds (%), free fatty acid (mg KOH/g) and peroxide index (meq/kg). Among the 90 samples collectedbefore the frying procedure, 18.18% showed higher values of polar compounds over the established limit (5%), predominantly in samples collected from snack bars. In 7.78%, 12.22% and 2.22% of the analyzed frying oil and fat samples, the total polar compounds, free fatty acids and peroxide indices, respectively, were in a higher rate than those limit values established for discarding them. The inadequacy on the employed frying procedure was observed as this process was performed without control over the frying temperature, type of oil, and employed equipment.These findings indicate that the food-service locations require close surveillance supervisions.

O estudo avaliou o Programa de Alimentação do Trabalhador (PAT) entre 1995 e 2002, sob a ótica da estrutura-processo-resultado. A metodologia consistiu em pesquisa documental e estudo de caso em 45 municípios na Bahia, resultando em 2.389 questionários domiciliares. Em termos de estrutura analisou-se a evolução normativa do programa até 2002. Quanto ao atendimento das necessidades nutricionais dos trabalhadores, o programa passou de uma insuficiente adequação calórica nos anos 80 para uma associação positiva entre sobrepeso e trabalho em empresa com PAT. Na Bahia a cobertura foi insuficiente entre os 5.120 adultos maiores de 20 anos investigados. Houve diferença significativa no acesso a benefícios alimentares entre trabalhadores do interior (26,1 por cento) e da capital (6,1 por cento). Contudo, a focalização foi adequada: todos os beneficiados no interior e 92,4 por cento na capital tinham renda menor que cinco salários mínimos. Faz-se necessária uma maior divulgação do PAT junto à população alvo, de forma a conscientizar os trabalhadores sobre os seus direitos e as ações desenvolvidas pelo Programa.


This study evaluated the Workers' Nutrition Program in Brazil from 1995 to 2002, from a structure-process-results perspective. The methodology involved documental research and a case study in 45 municipalities in the State of Bahia, resulting in 2,389 household interviews. In relation to structure, we analyzed the program's normative evolution until 2002. As for nutritional recommendations, the program shifted from insufficient calorie supply in the 1980s to a positive association between overweight and employment in companies adopting the Workers' Nutrition Program. In Bahia, overall program coverage was insufficient among the 5,120 adults 20 years or older who were interviewed. A significant difference was observed in access to food benefits among workers in the interior of the State (6.1 percent) as compared to the State capital, Salvador (26.1 percent). However, targeting was adequate: all workers benefiting from the program in the interior and 92.4 percent of those in Salvador earned less than five times the minimum wage (approximately U$950/month). It is necessary to improve the program's coverage in the target population in order to raise workers' awareness about their rights and the actions developed by the program.
