RESUMO

Actually, in Russian health care, the organization of community-based medical care of patients with mental disorders is considered as one of requested problems to be resolved. The in-patient substituting forms of specialized primary medical and social care for mental patients create optimal conditions for treatment and rehabilitation of this group of patients without separation from familiar social environment, that in many ways reduces "personality stigmatization" and promotes returning of individual to full-fledged social life, decreasing the load on the most expensive hospital care. One of such organizational forms implemented in Moscow under the new model of provision of mental care is out-patient mental module that provides specialized primary medical and social care and is tested in conditions of basic medical organization. Hence, the purpose of the study is to develop system of indices for continuous information support of management of implemented innovations.

RESUMO

The article analyzes topical issues of organization of patronage medical care of patients with restricted mobility in Moscow, considering ongoing organizational measures at various stages of implementation of new organizational form of medical sanitary care of patients of different age groups. The results of analysis of effectiveness of its functioning are presented.

RESUMO

The purpose of study. To analyze problems and perspectives of development of primary medical sanitary care according the principle of general practitioner practices in the Russian Federation. The article presents historical aspects of main stages of development of service of general practices in our country. Also article considers characteristics of development of general practitioner practices in various regions. In particular, from 2011, number of departments of service increased up to 2.5 times and number of rural out-patient clinics increased up to 1.6 times. The experience of implementation of system of general practitioner practices demonstrates that after corresponding training general practitioner is able to undertake up to one third-one fourth of all visits of patients to specialists. In spite of particular achievements, nowadays no real reform of primary health care occurred mainly because of complicity of problems of reforming not only primary health care but factually all health care services, resources inadequacy and shortcoming of system of professional training of personnel and insufficient development of legal and financial base.

RESUMO

The prevalence of habitual miscarriage varies depending on territories of the Russian Federation and other countries. The rate of premature delivery in the Moscow Zelenograd administrative okrug in 2013-2015 in average made up to 4.5%-4.7%. The percentage of habitual miscarriage made up to 3.6% that is a rather low indicator as compared with other Moscow okrugs and national average indices. The article considers three-level model of evaluation of medical, economic and social efficiency of obstetric gynecologic care support of population at the out-patient stage.

AIM: To develop a new instrument able to identify pathological states and assess their changes during medication treatment. We aimed to study the typical practice of using alimemazine (teraligen) in patients with the diagnosis of autonomic nervous system disorder and to test the Russian version of @The Four-Dimensional Symptom Questionnaire@ (4DSQ) for measuring distress, depression, anxiety and somatization. MATERIAL AND METHODS: We examined 3053 patients (mean age 42.09 ± 11.71 years) who received teraligen in doses gradually increasing from 5 to 15 mg per day. The observational program was carried out in over 600 outpatient clinics of the Russian Federation. The 4DSQ was administered before treatment and 4 weeks after treatment. The Clinical Global Impression (CGI) scale was used before, during (after 2 weeks) and after (4 weeks) treatment with teraligen. RESULTS AND CONCLUSION: There was a significant improvement of patient's state assessed both by physicians (CGI scale) and by patients (96 and 98%, respectively). The 4DSQ was sensitive to the parameters of response to treatment with teraligen: parameters obtained at baseline and 4 weeks after the beginning of treatment differed significantly demonstrating a significant decrease in distress, anxiety and somatization.
