RESUMO

In Argentina there are no multicenter studies evaluating the management of patients with acute pancreatitis (AP) nationwide. OBJECTIVES: The main objective of this study is to know how the patients with AP are treated in Argentina. The secondary objective is to assess whether the results comply with the recommendation of the American College of Gastroenterology Guide. MATERIAL AND METHODS: Twenty three center participated in the study. They include in a database hosted online consecutive patients with acute pancreatitis from june 2010 to june 2013. RESULTS: 854 patients entered the study. The average age was 46.6 years and 495 (58%) belonged to the female sex. The most common cause (88.2%) of AP was biliary. Some prognostic system was used in 99 % of patients and the most used was Ranson (74.5%). Were classified as mild 714 (83.6%) patients and severe 140 (16.4%). Systemic complications occurred in 43 patients and local complications in 21. 86 patients underwent dynamic CT scans and 73 patients had pancreatic and / or peripancreatic necrosis. Mortality was 1.5%. There was no difference in mortality in relation to the size, complexity or affiliation of the center. The comply of key recommendations of the American College of Gastroenterology Guide was over 80%. CONCLUSIONS: The diagnosis and treatment of patients with AP in 23 health centers located throughout the country was optimal. The management complied with most of the recommendations of the American College of Gastroenterology Guide.

Intra-abdominal infections (IAIs) are an important cause of morbidity and mortality in hospital settings worldwide. The cornerstones of IAI management include rapid, accurate diagnostics; timely, adequate source control; appropriate, short-duration antimicrobial therapy administered according to the principles of pharmacokinetics/pharmacodynamics and antimicrobial stewardship; and hemodynamic and organ functional support with intravenous fluid and adjunctive vasopressor agents for critical illness (sepsis/organ dysfunction or septic shock after correction of hypovolemia). In patients with IAIs, a personalized approach is crucial to optimize outcomes and should be based on multiple aspects that require careful clinical assessment. The anatomic extent of infection, the presumed pathogens involved and risk factors for antimicrobial resistance, the origin and extent of the infection, the patient's clinical condition, and the host's immune status should be assessed continuously to optimize the management of patients with complicated IAIs.

RESUMO

La hemorragia digestiva (HDA) es relativamente frecuente y su manejo varía de acuerdo a los distintos centros. El objetivo de este trabajo es evaluar nuestra incidencia de HDA comparando factores de riesgo con hallazgos endoscópicos e histopatológicos. A su vez el tratamiento recibido y el seguimiento posterior de los pacientes.

El Absceso del psoas (AP) es una infrecuente y servera entidad que presenta consideraciones diagnósticas difíciles por sus síntomas no específicos, su evaluación rutinaria de laboratorio y radiológica convencional poco útiles, que hacen del mismo un desafío clínico y terapéutico donde el drenaje quirúrgico y la antibiótico-terapia específica cumplen un rol fundamental. RESUMO

Se presentan 9 casos de utilización de transección gástrica con exclusión del tumor y gastroyeyunoanastomosis sobre muñón proximal, como procedimiento quirúrgico paliativo para tumores oclusivos irresecables de región antro pilórica, operados entre el 1-1-1976 y el 31-12-1985. El procedimiento resultó efectivo en los 7 casos de adenocarcinomas primarios del estómago, no siéndolo en 1 caso de linfoma gástrico y otro de cáncer de vesícula propagado al duodeno y región antro pilórica. En los primeros se solucionó el problema obstructivo, mejorando el estado general, con disminución de la anemia y recuperación del peso corporal. Si bien la sobrevida promedio fue sólo ligeramente más prolongada que la referida luego de utilización de otros procedimientos quirúrgicos paliativos, la calidad de vida ofrecida resultó sensiblemente superior

Antecedentes: El trauma es la primera causa de muerte entre el año de vida y los 35 años, para luego paulatinemente, ocupar la tercera y cuarta causa de mortalidad en edades superiores. La prevención primaria es una estrategia fundamental para el control de esta enfermedad. Objetivo: Presentar la experiencia en el diseño y aplicación del proyecto Intersocietario de Trauma (P.I.T) en el cual participan la AAC, SAP, ACACI, y la UNMP-INE. La meta general apunta a la transferencia de conocimientos de las entidades científicas a las ciudades participantes mediante un proceso de autogestión comunitaria, destinado a generar programas de prevención primaria y secundaria en trauma. Lugar de aplicación: Cinco ciudades de Argentina con pobalción estable menor de 130.000 habitantes. Diseño: Análisis prospectivo de investigación y aplicación epidemiológica. Material y Método: Se desarrolló un Sistema de Información para el estudio sobre epidemiología y morbimortalidad de las lesiones traumáticas tanto en niños como en adultos. Se utilizó la Clasificación Internacional de Enfermedades, (CIE-10) para codificar las causas externas de morbilidad y mortalidad. La información fue codificada, validada e incorporada a una base de datos utilizando el paquete EPI-INFO. Se estableció un cronograma para el proyecto. Resultados: La fase de preparación del proyecto culminó con la firma del Acta del Convenio el 22 de abril de 1996 entre los representantes de cada entidad científica...
