RESUMO

The present study examines the effect of the organic loading rate and the configuration of a semi-pilot modular microbial electrolysis cell (MEC) on the energy consumption during domestic (dWW) wastewater treatment. The MEC reactor consisted of twin tubular units hydraulically connected in series and was able to reduce up to 85% of the chemical oxygen demand (COD) concentration of the influent dWW at a relatively low energy consumption (1.6 kW h kg-COD(-1)). Hydrogen production was limited by the reduced amounts of organic matter fed into the reactor and the poor performance of the cathode. Overall, the results identified both an organic loading rate (OLR) threshold that makes the use of MECs for dWW treatment feasible in terms of energy consumption and COD removal efficiency and an OLR threshold that justifies the operation of two MECs in series to provide the required degree of COD removal.

AIMS: To know the characteristics and prevalence of non-tuberculous mycobacterial infection infection in patients with non-cystic fibrosis bronchiectasis. PATIENTS AND METHODS: A retrospective descriptive study of NCFB adult patients whose disease had been followed-up for at least two years was performed. RESULTS: A total of 68 subjects were included, 50 females (73.5%), with mean age of 63.31± 16.2 years. The most frequent etiology of the non-cystic fibrosis bronchiectasis was COPD in 28 cases (41.2%) with a light-moderate pulmonary involvement and Pseudomonas aeruginosa (P. aeruginosa) colonization (70.6%). Seven patients (10.3%) had MNT infection, six of whom had Mycobacterium avium complex (7.35%). Four patients (57.14%) were treated. In the infected patients, P. aeruginosa and the use of inhaled steroids were observed with less frequency. There were no significant differences between the infected and non-infected patients in relation to spirometric values. CONCLUSIONS: The non-cystic fibrosis bronchiectasis could be considered a risk factor for non-tuberculous mycobacterial infection.

BACKGROUND AND OBJECTIVES: In Spain, epidemiological studies of the prevalence of diffuse interstitial lung disease (ILD) in rheumatoid arthritis (RA) are limited. Our objective was to estimate the prevalence of symptomatic ILD in RA and its characteristics in our area. MATERIALS AND METHODS: In our hospital's interdisciplinary rheumatology and pulmonology clinic, a prospective longitudinal observational study was designed in which we included RA with respiratory symptoms and ILD confirmed by high resolution computed tomography. RESULTS: Of the 2729 people with RA in our area, 47 had symptomatic ILD, estimating a prevalence of symptomatic ILD in RA of 1.72% (95% CI 1.26-2.29) with an age at diagnosis of RA of 57.3 ±â€¯13.3 years. It was more frequent in men, 60.6% had a history of smoking, and 84.3% and 84.7% had rheumatoid factor (RF) and anti-cyclic citrullinated peptide (Anti-CCP) antibodies, respectively. The most frequent pattern was usual interstitial pneumonitis (UIP), appearing in 28 (31.1%). Nonspecific interstitial pneumonia (NSIP) was more frequent in women, while the combined pulmonary fibrosis-emphysema (CPFE) syndrome presented exclusively in men. CONCLUSIONS: We have analysed the prevalence of symptomatic RA-ILD in our area, which is lower than expected, probably in relation to the definitions used. We have also described that the UIP pattern is the most frequent in RA in our environment, followed by the NSIP. Lastly, we have analysed the prevalence of CPFE in RA, which reaches 13%, for the first time.

Leishmania major promastigotes were found to avoid activation of mouse bone marrow-derived macrophages (BMM0) in vitro for production of cytokines that are typically induced during infection with other intracellular pathogens. Coexposure of BMM0 to the parasite and other microbial stimuli resulted in complete inhibition of interleukin (IL) 12 (p40) mRNA induction and IL-12 release. In contrast, mRNA and protein levels for IL-1(alpha), IL-1(beta), tumor necrosis factor (TNF) alpha, and inducible NO synthase (iNOS) were only partially reduced, and signals for IL-10 and monocyte chemoattractant protein (MCP-1/JE) were enhanced. The parasite could provide a detectable trigger for TNF-alpha and iNOS in BMM0 primed with interferon (IFN) gamma, but still failed to induce IL-12. Thus IL-12 induction is selectively impaired after infection, whereas activation pathways for other monokine responses remain relatively intact. Selective and complete inhibition of IL-12(p40) induction was observed using BMM0 from either genetically susceptible or resistant mouse strains, as well as IL-10 knockout mice, and was obtained using promastigotes from cutaneous, visceral, and lipophosphoglycan-deficient strains of Leishmania. The impaired production of the major physiological inducer of IFN-gamma is suggested to underlie the relatively prolonged interval of parasite intracellular survival and replication that is typically associate with leishmanial infections, including those producing self-limiting disease.

Veterinary pharmaceuticals, widely used in intensive livestock production, may contaminate surface waters. Identifying their sources and pathways in watersheds is difficult because i) most veterinary pharmaceuticals are used in human medicine as well and ii) septic or sewer wastewater treatment plants (WWTP) can release pharmaceuticals into surface water, even in agricultural headwater watersheds. This study aimed to analyze the spatiotemporal variability of animal-specific, mixed-use, and human-specific pharmaceuticals, from agricultural headwaters with intensive livestock production and a WWTP to a watershed used for Water Framework Directive monitoring. Grab sampling was performed during one hydrological year upstream and downstream from a WWTP and at three dates in seven nested watersheds with areas of 1.9-84.1km2. Twenty pharmaceuticals were analyzed. Animal-specific pharmaceuticals were detected at all sampling dates upstream and downstream from the WWTP and at concentrations higher than those of human-specific pharmaceuticals. The predominance of animal-specific and mixed-use pharmaceuticals vs. human-specific pharmaceuticals observed at these sampling points was confirmed at the other sampling points. Animal-specific pharmaceuticals were detected mainly during runoff events and periods of manure spreading. A large percentage of mixed-use pharmaceuticals could come from animal sources, but it was difficult to determine. Mixed-use and human-specific pharmaceuticals predominated in the largest watersheds when runoff decreased. In areas of intensive livestock production, mitigation actions should focus on agricultural headwater watersheds to decrease the number of pathways and the transfer volume of veterinary pharmaceuticals, which can be the main contaminants.

BACKGROUND AND AIMS: To specify the prevalence of patients diagnosed with CF at age of ≥60 year-old and to analyze their characteristics. PATIENTS AND METHODS: Observational study of CF patients which were diagnosed at age ≥60 year-old. The analyzed variables were: age, sex, nationality, lung function parameters, conditions present at diagnosis, microbiological characteristics and genetic findings. RESULTS: eight patients were included. 7 patients were female (87.5%) with a mean age of 70.6 years (median 71.5 years, range 60-78 years). The most important findings were: sweat test >60 mEq/l; heterozygotes F508del; bronchiectasis in CT; methicillin-sensitive Staphylococcus aureus (50%) in sputum. The most patients presented a normal or mild obstructive lung function. CONCLUSIONS: CF must also be considered a disease diagnosed in adulthood, incorporating the sweat test within the usual techniques of differential diagnosis in patients with different diseases associated with CF, because genetic counselling is esencial.

BACKGROUND: Bone loss in the distal femur is a common problem in knee revision surgeries. The problem is exacerbated in the context of an active infection. In extreme cases this bone loss could compromise the feasibility of a two-stage exchange protocol using dynamic spacers due to the inherent instability of this type of spacers. Use of a hip prefabricated spacer in a "reverse" way forming a ball-and- socket joint is a therapeutic option in cases of massive bone defect and infection. MATERIAL AND METHODS: A retrospective review was performed of our institutional database to identify all cases of massive distal femoral defect in which this technique was used from January 2010 to December 2013. A record was made of the epidemiological data, characteristics of the infection (clinical and microbiological), and adverse event during the spacer stage. The main end-point was the infection eradication rate (minimum: 18 months of follow-up). The complications associated with the technique were also assessed. Finally, each patient completed a visual analogue pain scale, and a satisfaction questionnaire (SAPS). RESULTS: This technique was successfully used in six cases so far, controlling the infection in all cases. Mean femoral defect was 117cm (range: 32-191cm). Mean time with spacer was 7.6 months, with no major complications. All but one patient reached second stage reconstruction with a mega-prosthesis, and mean time since second stage was 34.7 months. All patients stated high levels of satisfaction with the technique employed, as well as and low pain scores (mean visual analogue pain scale: 1; range: 0-4). CONCLUSION: A reproducible and safe technique is described. Patients report a high level of satisfaction with the procedure, and there were no cases of recurrence of the infection after a minimum follow-up of 18 months.

The purpose of this study is to develop a system capable of performing calculation of temporal gait parameters using two low-cost wireless accelerometers and artificial intelligence-based techniques as part of a larger research project for conducting human gait analysis. Ten healthy subjects of different ages participated in this study and performed controlled walking tests. Two wireless accelerometers were placed on their ankles. Raw acceleration signals were processed in order to obtain gait patterns from characteristic peaks related to steps. A Bayesian model was implemented to classify the characteristic peaks into steps or nonsteps. The acceleration signals were segmented based on gait events, such as heel strike and toe-off, of actual steps. Temporal gait parameters, such as cadence, ambulation time, step time, gait cycle time, stance and swing phase time, simple and double support time, were estimated from segmented acceleration signals. Gait data-sets were divided into two groups of ages to test Bayesian models in order to classify the characteristic peaks. The mean error obtained from calculating the temporal gait parameters was 4.6%. Bayesian models are useful techniques that can be applied to classification of gait data of subjects at different ages with promising results.

This prospective study evaluated the relationship between inflammation and oxidative stress in a group of dialysis patients just before and 3 months after kidney transplantation and compared the results with a control group of healthy subjects. The oxidative stress markers determined were different F2-isoprostane isomers. The inflammatory markers included C-reactive protein, interleukin-6, tumor necrosis factor-alpha, and pregnancy-associated plasma protein A. Forty-three patients were the study group and 50 healthy subjects from a hospital blood bank as controls. The results showed levels of inflammatory and oxidative stress markers to be higher in the dialysis patients than in the control group, although they improved following kidney transplantation. Finally, significant correlations were observed between F2-isoprostane isomers and inflammatory markers.

Samples of three common commercial gutta-percha endodontic filling points (Hygienic, Mynol, and Maillefer Pink) and a sample of natural gutta-percha were submitted to thermomechanical analysis under pressures ranging from 0.01 to 0.2 N. Samples of Mynol and Maillefer filling points were thermally treated and then submitted to thermomechanical analysis in parallel with differential scanning calorimetric analysis. The results show that thermomechanical analysis gives results distinct from those obtained by classic dilatometry and that it is a technique well suited to the study of the thermoplastic properties of gutta-percha. An analysis of the results shows that the amount of the inorganic component used in a commercially available endodontic point has a strong influence on its thermomechanical properties. Thermodynamical properties and the "thermal history" of the gutta-percha are also important. Both temperature and force should be controlled in order to assess the thermomechanical properties of endodontic filling points, while the latter have not yet been codified in clinical procedures.

Seven-week-old female kids of the Murciana-Granadina breed naturally infected with coccidia were superinfected with a multispecific Eimeria inoculum (300,000 oocysts) or treated to control the coccidial infection with Amprolium (50 mg kg-1 liveweight day-1; 4 days every 14 days); 80 days later both animal groups received 2500 third-stage larvae of a sheep-derived mixture of Trichostrongylus colubriformis (50%), Teladorsagia circumcincta (40%) and Haemonchus contortus (10%). Kids experimentally superinfected with Eimeria showed reduced food intake at the early patency period, higher numbers of oocysts passed at weaning time and lower liveweight gain than the Amprolium-medicated kids. In the Eimeria superinfected kids the trichostrongyle infection caused a tendency to result in chronic coccidial infections. Further, these animals had higher numbers of nematode eggs in their faeces, lower dressed weights on slaughter and inferior meat quality.

Sixteen- to eighteen-week-old lambs were infected with 2500 3rd stage larvae (L-3) of Haemonchus contortus or kept as uninfected controls. Two months later all animals were challenged with 5000 L-3 of this parasite. Soluble antigens of H. contortus L-3 and adult worms were analyzed by SDS-PAGE and Western blotting during experimental infection and challenge. Antigens from both sources, particularly of low molecular weight under reducing conditions, were recognised by the pooled sera of infected lambs. No single L-3 antigen was recognised by all infected lambs, whereas two peptides having around 25 and 26 kDa from adults were recognised by infected animals during the patency and could be of potential use in the diagnosis of lamb haemonchosis.

A preliminary trial on the extent of cross-antigenicity among the sheep strongylids Haemonchus contortus, Trichostrongylus colubriformis, Teladorsagia circumcincta and Nematodirus battus in 2.5- to 4-month-old lambs has been carried out using ELISA and Western blotting (WB). Cross antigenicity was tested using soluble extracts from adult and third stage larvae (L3) of H. contortus as antigenic source probed with sera from lambs with monospecific heterologous infections. There was cross-antigenicity between L3 of H. contortus and Trichostrongylus colubriformis in ELISA and WB. Immunodetection results with adult H. contortus antigen showed a closer relationship to Teladorsagia circumcincta. Certain heterologous sera reacted with H. contortus antigens more strongly than the homologous one, but sera from the H. contortus-infected animals had reactivity around the 25 kDa region from adult antigens which could have potential diagnostic use.

OBJECTIVE: To analyze depression in women who have suffered perinatal loss in the present study. METHODS: The level of depression was studied, by means of the Beck Depression Inventory (BDI), in two groups of women, mothers who suffered perinatal loss and received psychological intervention for 1 year and mothers with live-birth babies. The BDI was recorded immediately after delivery and at 6 and 12 months postpartum. A third group of women with perinatal loss who received no intervention were studied only 12 months postpartum. RESULTS: At the time of delivery, women who suffered perinatal loss showed higher levels of depression, as measured by higher scores on the BDI than women experiencing a live-birth. At 6 months postpartum the intervention group showed improvement (lower BDI scores), but as a group they endorsed more depressive symptoms than the live-birth group. At 12 months the perinatal loss group who received the 1-year intervention was less depressed than the group who did not, and scored very similar to the live-birth group. CONCLUSION: Women who experience perinatal loss endorse more depressive symptoms than mothers of live-births, and these depressive symptoms can be ameliorated by a psychological intervention.

RESUMO

Adenoid cystic carcinoma is an infrequent tumor, but not a rare one. Its primary location is most often the salivary glands, although it can be found in many other organs, including the lung, which is the most frequent site of remote metastasis. The tumor's two main features, which indicate a need for long-term monitoring, are 1) its high level of local aggressivity and rate of recidivism in spite of radical excision, with recurrence sometimes taking place long after the primary tumor appears, and 2) its slow natural history even when disseminated disease is present. Type of tumoral tissue is an important prognostic factor.

We studied four indirect radiological measures of shoulder balance, their correlation with true shoulder balance, and measurement error of each method. Thirty-three consecutive patients with idiopathic scoliosis requiring surgical treatment were selected. Actual shoulder level was determined from a line in the top margin of the acromion clearly visualized in the wide chart. Indirect references used to assess shoulder balance were tilting of cephalic end-plate of T1, inclination of a line through superior border of both first ribs, the level of top margin of coracoid processes, and the level of two specific points where the clavicle intersects the rib cage. All radiographs were assessed by three independent observers. Correlation coefficients between each measurement and true shoulder level were 0.54 for T1 tilting (p = 0.001), 0.63 for first rib inclination (p < 0.001), 0.96 for coracoid process (p = 0.0001), and 0.93 for clavicle-rib cage intersection (p = 0.0001).

Lithium-based ceramics have been proposed as tritium breeders for fusion reactors. The lithium aluminate (gamma phase) seems to be thermally and structurally stable, the damages produced by neutron irradiation depend on the absorbed dose. A method based on the measurement of neutron activation of foils through neutron capture has been developed to obtain the neutron absorbed dose in lithium aluminates irradiated in the thermal column facility and in the fixed irradiation system of a Triga Mark III Nuclear Reactor.

RESUMO

A case of posterior interosseous nerve paralysis at the level of the Frohse arcade, caused by an intramuscular myxoma, is reported. Review of the literature shows this to be an exceptional presentation of this tumor as a cause of a compressive peripheral neuropathy. Twelve months after surgical treatment, total neurological recovery was observed.

RESUMO

Al oral monophasic contraceptive formulation containing gestodene (75 mcg) and ethynilestradiol (30 mcg) is evaluated concerning its safety, efficacy and cycle-control. Sixty-seven healthy women were included, allowing a total of 574 evaluable cycles. This formulation showed a high degree of efficacy, with no pregnancies after its utilization. Side affects were similar or lower to other reported with another hormonal contraceptive oral formulations. Hematologic, biochemical or urinary parameters were not altered. An acceptable menstrual pattern was maintained. In conclusion, this formulation constitutes an oral contraceptive with a high efficacy rate, a well degree of acceptability and with a side effects profile similar or lower than those produced by other oral contraceptives.

The influence of applied voltage and hydraulic retention time on the performance of a semi-pilot modular tubular wastewater-fed microbial electrolysis cell (MEC) with high scalability was investigated. A chemical oxygen demand (COD) removal efficiency of 80%, as well as an energy consumption of 0.3-1.1 Wh g-COD(-1) removed, were achieved. Hydrogen production was limited by the reduced amounts of organic matter fed into the reactor, the poor performance of the cathode, and COD consuming by non electrogenic microorganisms. The presence of COD consuming microorganism that do not contribute to electrogenic metabolism severely affected the MEC performance.
