RESUMO

The article analyzes the development of the activity of Medical Assessment Service for people with diabetes mellitus for selected assessment categories in the assessment of health conditions from 2012 to 2016. The assessment criteria for assessing disability, the degree of dependency for care allowance and the health status for grant for special aid, and certificate of person with disability are set out in the legislation. The incidence of disability for diabetes declined by 13 % between 2012 and 2016 and by 10 % for the degree of dependency for care allowance. The assessment for grant for special aid increased by 57 %, for grant in the case when the aid is a "vehicle" decreased by 8 %. Most importantly increased assessing for certificate of person with disability by 150 %. The epidemic of diabetes in the population is not reflected in the incidence of disability and the degree of dependence. Both of these assessment categories were decreased, in spite of the fact that there account for almost 70 % of the whole assessment activity. The assessment for grant for special aid, and certificate of person with disability was increased. The cause may be due to incorrect statistical reporting, not up to date expert medical criteria, or poor patient awareness.

RESUMO

In 2012 it was applied to medical practice assessor new criteria for assessing the degree of dependence for the care allowance. After four years by the Ministry of Labour and Social Affairs proposed expansion of the existing activities and at the other activities, it is proposed to refine the text. New activities refine or complement the existing framework defining the basic needs of life and should lead to a more objective and personally assessment of the degree of dependence. With the change in legislation expected a growth in the number of applicants for assessment General assessment principles and policies remain unchanged.Key words: medical assessment service, dependence degree, basic living needs.

RESUMO

Personalised medicine in disability assessment was first implemented in 2010. It was not until two years later that the medical assessment paradigm used for dependence degree assessment for the purposes of care allowance was transformed. Since January 2012, there has been an aggregation of the 36 self-care and self-sufficiency tasks into ten basic living needs , i.e. mobility, orientation, communication, food intake, putting on clothes and shoes, body hygiene, using the toilet, health care, personal activities, and household cleaning. Until the present moment, more than 150 thousand assessment cases have confirmed that personalised assessment of functional disabilities represents a significant improvement in the individualised approach to all clients of the social security system. The increase in the number of patients with confirmed dependence degrees III and IV has demonstrated that modern functional assessment is beneficial especially for patients with the most severe health impairments. Personalised medical assessment service represents the pursuit of an improved medical assessment of clients functional disabilities.

RESUMO

The article presents the outcomes of the new disability assessment paradigm, which was running for three years and concerns patients with mental and behavioural disorders. This paradigm for assessing the state of health, decrease in the working capacity, and disability in patients with mental and behavioural disorders has been asserted since 2010. It is based on the findings of the project entitled, "Advances in medical science reflected in functional health state assessment and working incapacity in relation to International Classification of Diseases and International Classification of Functional Abilities", conducted by Czech Medical Association J. E. Purkyne. Consequently, the new paradigm proved its value and significance to the transition of medical assessment into personalised medicine.

RESUMO

The article informs about a new approach to health state assessment, rate of working capacity decline and incapacity of people who suffer from mental disorders and behaviour disorders, based on the project results "Progress of medical science advances reflected in functional health state assessment and working incapacity in relation to International classification of illnesses and International classification of functional abilities" conducted by Czech Medical Association J. E. Purkyne. The authors emphasize that a new approach to incapacity assessment is in accord with expert medical public opinions and medical science advances with focus on function ability, activity and participation.

Stroke is one of the most devastating pathologies of the early twenty-first century demonstrating 1-month case-fatality rates ranging from 13 to 35% worldwide. Though the majority of cases do occur in individuals at an advanced age, a persistently increasing portion of the patient cohorts is affected early in life. Current studies provide alarming statistics for the incidence of "young" strokes including adolescents. Young stroke is a multifactorial disease involving genetic predisposition but also a number of modifiable factors, the synergic combination of which potentiates the risks. The article analyzes the prevalence and impacts of "traditional" risk factors such as sedentary lifestyle, smoking, abnormal alcohol consumption, drug abuse, overweight, hypertension, abnormal sleep patterns, and usage of hormonal contraceptives, among others. Further, less explored risks such as primary vascular dysregulation and associated symptoms characteristic for Flammer syndrome (FS) are considered, and the relevance of the FS phenotype for the stroke predisposition at young age is hypothesized. Considering the high prevalence of known genetic and modifiable risk factors in the overall predisposition to the young stroke, the risk mitigating measures are recommended including innovative screening programs by application of specialized questionnaires and biomarker panels as well as educational programs adapted to the target audiences such as children, adolescents, and young adults.
