Complications associated with the intraocular lenses (IOL) implantation can be classified into 3 groups: intraoperative, early and late complications. Some of the complications have to be treated at once, otherwise the condition may deteriorate and lead to the occurrence of various secondary complications. One of the intraoperative complications during the IOL implantation can be the impairment of the corneal endothelium which occurs due to the inadequate size of the incision. A part of the IOL loop can remain incarcerated in the corneo-scleral wound during surgery. A strong choroidal hemorrhage may occur, as well as the shallowing of the anterior chamber. Iridodialysis may appear as a consequence of the implant manipulation. IOL can also be subluxated or luxated anteriorly or below the sclera. All of these complications should be promptly attended to. Early postoperative complications that should be treated as soon as possible are the occurrence of the pupillary block, shallow anterior chamber, secondary glaucoma, choroidal detachment, corneal edema and postoperative iritis. Besides the corneal edema, late postoperative complications include the formation of secondary cataract, the occurrence of cystoid macular edema and retinal detachment. The treatment of some of these complications can be delayed for a certain period, while other require the immediate treatment and correction. In this work the author presents his results, the complications encountered in different stages of the IOL implantation, and the immediate interventions that were carried out.

RESUMO

The study is based upon the fact that aging processes inevitably affect all parts of the human body. These processes are often followed by pathological changes, whose basic cause may be some of the genetic factors or insufficient and inadequate nutrition, but they may also be intensified by the influence of adverse effects in the work place or ecologic factors. It is often impossible to completely avoid the impact of all these processes upon the human body, but the application of certain preventive measures and medicamentous therapy may diminish or slow them down. The results of the application of various medicaments with the purpose of slowing down the manifestation or progress of cataract have shown that such measures of the medicamentous prevention of cataract certainly have medical justification. It has also been found that the medicaments applied slowed down the development and progress of lens opacities and the occurrence of cataract to a higher or lesser degree, depending on the kind of preparation and its efficacy.

RESUMO

The authors deal with uveitis and cataract from two aspects. The occurrence of uveitis was analysed both before cataract surgery and postoperatively. The authors found in their material that uveitis occurred before cataract surgery in 1.8% and postoperative uveitis in 1.2% of cases. Special emphasis is put on the occurrence of postoperative complications in such eyes and the indications for the IOL implantation in uveitis patients.

Medical documentation is getting more extensive, it is not standardised and can hardly be used to provide the exact and proper feedback. The crisis of data storage, processing and uninterrupted flow of information is becoming concial in the medical institutions as well. An example of informatic model of observation of postoperative complications following cataract surgery is presented in this work. The model has a very flexible pattern, and can, therefore, with minor changes, be used in observation of all postoperative complications in ophthalmology. It is based on the use of personal computer with professional software support (data base III programme was used for creation of data base, while statistical graphic analysis was carried out by following programmes: MICROSTAT, ABSTAT, STATGRAPHIC and FRAMEWORK). It has to be pointed out that any doctor, after only a short (optional) education, could prepare such models for his own purposes, and could thus assist in overcoming the information in medicine. In conclusion, the authors state computer is simple to work with, fast, economical and precise. Computerisation of health service shall include it in one of the aspects of practical realisation of the third technological revolution.

The authors analysed and compared three groups of patients who had undergone surgery. In the first group of 72 patients with diabetes and cataract, intraocular lens implantation was carried out. In the second group of 96 patients with diabetes and cataract, cataract surgery was not followed by intraocular lens implantation. The third group of 100 nondiabetic patients, selected by random choice, had intraocular lens implanted after the cataract surgery. Retinal status, postoperative complications and visual acuity were the parameters analysed in correlation with the intraocular lens implantation. In the authors' opinion, the prognosis following cataract surgery and intraocular lens implantation in diabetic patients is good, if diabetic complications do not occur, particularly retinopathy and vitreal hemorrhage which impair the vision considerably. Intraocular lens can be implanted even in cases of maculopathy and preproliferative diabetic retinopathy, provided a thorough diagnostic evaluation has been performed (echography, fluorescein angiography). Laser photocoagulation procedure should also be carried out before surgery and repeated as long as the transparence of the lens enables it. The treatment should be resumed two weeks after the cataract surgery and intraocular lens implantation at the latest. Intraocular lens implantation is contraindicated in proliferative diabetic retinopathy, and especially iridal rubeosis, as the risk of neovascular glaucoma development is considerable.

RESUMO

This review presents the results of the clinical use of human leukocyte interferon (HLI) in certain diseases with viral etiology. Viral infections with manifestations in the eye, skin, and/or mucous membranes and in the respiratory system were treated with HLI by local application. HLI was incorporated into ointment, oil suspension, powder, dilution for inhalation, and dilution for subcutaneous and subconjunctival injections. The treatment in some of the diseases yielded statistical results that were highly significant in favor of interferon.
