Bluetooth monitoring systems (BTMS) have opened a new era in traffic sensing, providing a reliable, economical, and easy-to-deploy solution to uniquely identify vehicles. Raw data from BTMS have traditionally been used to calculate travel time and origin-destination matrices. However, we could extend this to include other information like the number of vehicles or their residence times. This information, together with their temporal components, can be applied to the complex task of forecasting traffic. Level of service (LOS) prediction has opened a novel research line that fulfills the need to anticipate future traffic states, based on a standard link-based variable, accepted for both researchers and practitioners. In this paper, we incorporate BTMS's extended variables and temporal information to an LOS classifier based on a Random Undersampling Boost algorithm, which is proven to efficiently respond to the data unbalance intrinsic to this problem. By using this approach, we achieve an overall recall of 87.2% for up to 15-min prediction horizons, reaching 96.6% predicting congestion, and improving the results for the intermediate traffic states, especially complex given their intrinsic instability. Additionally, we provide detailed analyses on the impact of temporal information on the LOS predictor's performance, observing improvements up to a separation of 50 min between last features and prediction horizons. Furthermore, we study the predictor importance resulting from the classifiers to highlight those features contributing the most to the final achievements.

COVID-19 has become a major global issue with large social-economic and health impacts, which led to important changes in people's behavior. One of these changes affected the way people use public transport. In this work we present a data-driven analysis of the impact of COVID-19 on public transport demand in the Community of Madrid, Spain, using data from ticket validations between February and September 2020. This period of time covers all stages of pandemic in Spain, including de-escalation phases. We find that ridership has dramatically decreased by 95% at the pandemic peak, recovering very slowly and reaching only half its pre-pandemic levels at the end of September. We analyze results for different transport modes, ticket types, and groups of users. Our work corroborates that low-income groups are the most reliant on public transportation, thus observing significantly lower decreases in their ridership during pandemic. This paper also shows different average daily patterns of public transit demand during each phase of the pandemic in Madrid. All these findings provide relevant information for transit agencies to design responses to an emergence situation like this pandemic, contributing to extend the global knowledge about COVID-19 impact on transport comparing results with other cities worldwide.

Transport agencies require accurate and updated information about public transport systems for the optimal decision-making processes regarding design and operation. In addition to assessing topology and service components, users' behaviors must be considered. To this end, a data-driven performance evaluation based on passengers' actual routes is key. Automatic fare collection platforms provide meaningful smart card data (SCD), but these are incomplete when gathered by entry-only systems. To obtain origin-destination (OD) matrices, we must manage complete journeys. In this paper, we use an adapted trip chaining method to reconstruct incomplete multi-modal journeys by finding spatial similarities between the outbound and inbound routes of the same user. From this dataset, we develop a performance evaluation framework that provides novel metrics and visualization utilities. First, we generate a space-time characterization of the overall operation of transport networks. Second, we supply enhanced OD matrices showing mobility patterns between zones and average traversed distances, travel times, and operation speeds, which model the real efficacy of the public transport system. We applied this framework to the Comunidad de Madrid (Spain), using 4 months' worth of real SCD, showing its potential to generate meaningful information about the performance of multi-modal public transport systems.

COVID-19 has dramatically struck each section of our society: health, economy, employment, and mobility. This work presents a data-driven characterization of the impact of COVID-19 pandemic on public and private mobility in a mid-size city in Spain (Fuenlabrada). Our analysis used real data collected from the public transport smart card system and a Bluetooth traffic monitoring network, from February to September 2020, thus covering relevant phases of the pandemic. Our results show that, at the peak of the pandemic, public and private mobility dramatically decreased to 95% and 86% of their pre-COVID-19 values, after which the latter experienced a faster recovery. In addition, our analysis of daily patterns evidenced a clear change in the behavior of users towards mobility during the different phases of the pandemic. Based on these findings, we developed short-term predictors of future public transport demand to provide operators and mobility managers with accurate information to optimize their service and avoid crowded areas. Our prediction model achieved a high performance for pre- and post-state-of-alarm phases. Consequently, this work contributes to enlarging the knowledge about the impact of pandemic on mobility, providing a deep analysis about how it affected each transport mode in a mid-size city.

Bicycle sharing systems (BSSs) have established a new shared-economy mobility model. After a rapid growth they are evolving into a fully-functional mobile sensor platform for cities. The viability of BSSs is floored by their operational costs, mainly due to rebalancing operations. Rebalancing implies transporting bicycles to and from docking stations in order to guarantee the service. Rebalancing performs clustering to group docking stations by behaviour and proximity. In this paper we propose a Hierarchical Agglomerative Clustering based on an Ultra-Light Edge Computing Algorithm (HAC-ULECA). We eliminate the proximity and let Hierarchical Agglomerative Clustering (HAC) focus on behaviour. Behaviour is represented by ULECA as an activity profile based on the net flow of arrivals and departures in a docking station. This drastically reduces the computing requirements which allows ULECA to run as an edge computing functionality embedded into the physical layer of the Internet of Shared Bikes (IoSB) architecture. We have applied HAC-ULECA to real data from BiciMAD, the public BSS in Madrid (Spain). Our results, presented as dendograms, graphs, geographical maps, and colour maps, show that HAC-ULECA is capable of separating behaviour profiles related to business and residential areas and extracting meaningful spatio-temporal information about the BSS and the city's mobility.

Bicycle Sharing Systems (BSSs) are exponentially increasing in the urban mobility sector. They are traditionally conceived as a last-mile complement to the public transport system. In this paper, we demonstrate that BSSs can be seen as a public transport system in their own right. To do so, we build a mathematical framework for the classification of BSS trips. Using trajectory information, we create the trip index, which characterizes the intrinsic purpose of the use of BSS as transport or leisure. The construction of the trip index required a specific analysis of the BSS shortest path, which cannot be directly calculated from the topology of the network given that cyclists can find shortcuts through traffic lights, pedestrian crossings, etc. to reduce the overall traveled distance. Adding a layer of complication to the problem, these shortcuts have a non-trivial existence in terms of being intermittent, or short lived. We applied the proposed methodology to empirical data from BiciMAD, the public BSS in Madrid (Spain). The obtained results show that the trip index correctly determines transport and leisure categories, which exhibit distinct statistical and operational features. Finally, we inferred the underlying BSS public transport network and show the fundamental trajectories traveled by users. Based on this analysis, we conclude that 90.60% of BiciMAD's use fall in the category of transport, which demonstrates our first statement.

The purpose of this study was to develop a technique allowing a faster return to previous activities after surgical treatment of short transverse and oblique fractures of the diaphysis of the fifth metacarpal. It consisted of an anterograde and retrograde centromedullary double pinning connected. Our series included 40 patients of average age 25 years including 2 women. In 20 cases (group I), an anterograde centromedullary pinning with a 1.6- or 2-mm pin was performed followed by a 6-week orthosis and then 2-week self-education. In 20 cases (group II), an anterograde and retrograde centromedullary double pinning was performed using 2 1.5-mm pins connected by a connector (MétaHUS®, Arex™). A first pin was introduced into the canal antegrade, then the second retrograde. The 2 externalized pins were connected, and self-rehabilitation was encouraged. Mean tourniquet time was 14.75 min (group I) and 15.8 min (group II), respectively. At the last follow-up, the average pain was 0.35/10 and 0.2/10, the Quick DASH 0.68/100 and 0.57/100, the strength of the hand 94.65% and 94.35%, the active flexion MCP 98.5% and 99%, respectively. MCP active extension was 98.5% and 98.75%; the sick leave was 8.4 and 6.3 weeks. All fractures healed without recovery with 3 complications: 2 infections on pins, in each group, 1 CRPS1 (group I). Our results seem to show that the technique of anterograde and retrograde centromedullary double pinning connected in the treatment of short transverse and oblique fractures of the diaphysis of the fifth metacarpal gives results at least as good as the reference technique with a non-negligible socioeconomic advantage, a less time off work.

We use a difference-in-differences strategy to estimate the effects of JUNTOS, a conditional cash transfer program targeted to poor rural households in Peru, on use of prenatal care. Using data from the Peruvian Demographic and Health Surveys over the period 2000-2011, we find that JUNTOS increased prenatal care utilization among program-eligible women. Even more, we find positive effects of JUNTOS on the probability of receiving prenatal care delivered by a skilled professional, including specific medical checkups during the prenatal visits, and a negative effect on the probability of experiencing obstetric emergencies during labor and childbirth.

The goal of this study was to compare the results of a Stack splint compared to a dorsal glued splint in the treatment of tendinous mallet fingers. Our series included 100 patients, aged 47 years in average, among which 40 are women, with a 9-week follow-up. The first 50 patients (group I) were treated with a dorsal glued splint, and the 50 following patients (group II) were treated with a Stack splint. The average lack of active extension of the distal phalanx was 3.1° in group I and 1.74° in group II. The average pain was rated 0.22/10 in group I and 0.26/10 in group II. There were seven cases of nail dystrophies, 18 cases of macerations and five cases of swan neck deformities in group I. There were six cases of macerations and four cases of swan neck deformities in group II. The dorsal nail-glued splint gives comparable results to the Stack splint with the added advantage of leaving the fingertip free.

The aim of our study was to compare the vertical fluoroscopic view of the wrist in extension and supination (ES) to the view in flexion and supination (FS) and determine which of the two views allowed the best visualization of four selected anatomical landmarks SDLR (radial styloid, dorsal radius cortex, Lister's tubercle and distal radioulnar joint). Our case series included 50 patients who had suffered a distal radius fracture and undergone an open reduction and internal fixation procedure with a volar locking plate. For each case, two fluoroscopic views were taken: ES (wrist extension and supination) (group I) and FS (wrist flexion and supination) (group II). Ten observers had to recognize the SDLR anatomical landmarks on 100 fluoroscopic skyline views (time 1) and 15 days later (time 2). The rate of recognition of the four anatomical landmarks was 78% in group I and 66% in group II (p < 0.001). The concordance rate of recognition of the four anatomical landmarks was mediocre (κ = 0.411). In conclusion, the vertical fluoroscopic skyline view in wrist extension and supination seems to be the most adequate view to assess the quality of the fracture reduction, the distal radioulnar joint and the length of the screws in open reduction and internal fixation of distal radius fractures with volar locking plates.

BACKGROUND: The distinction of type 1 and type 2 myocardial infarction (MI) is of major clinical importance. Our aim was to evaluate the diagnostic ability of absolute and relative conventional cardiac troponin I (cTnI) and high-sensitivity cardiac troponin T (hs-cTnT) in the distinction between type 1 and type 2 MI in patients presenting at the emergency department with non-ST-segment elevation acute chest pain within the first 12 h. METHODS: We measured cTnI (Dimension Vista) and hs-cTnT (Cobas e601) concentrations at presentation and after 4 h in 200 patients presenting with suspected acute MI. The final diagnosis, based on standard criteria, was adjudicated by two independent cardiologists. RESULTS: One hundred and twenty-five patients (62.5%)were classified as type 1 MI and 75 (37.5%) were type 2 MI. In a multivariable setting, age (relative risk [RR]=1.43, p=0.040), male gender (RR=2.22, p=0.040), T-wave inversion (RR=8.51, p<0.001), ST-segment depression (RR=8.71, p<0.001) and absolute delta hs-cTnT (RR=2.10, p=0.022) were independently associated with type 1 MI. In a receiver operating characteristic curve analysis, the discriminatory power of absolute delta cTnI and hs-cTnT was significantly higher compared to relative c-TnI and hs-cTnT changes. The additive information provided by cTnI and hs-cTnT over and above the information provided by the "clinical" model was only marginal. CONCLUSIONS: The diagnostic information provided by serial measurements of conventional or hs-cTnT is not better than that yielded by a simple clinical scoring model. Absolute changes are more informative than relative troponin changes.

RESUMO

The explosion of the Internet of Things has dramatically increased the data load on networks that cannot indefinitely increment their capacity to support these new services. Edge computing is a viable approach to fuse and process data on sensor platforms so that information can be created locally. However, the integration of complex heterogeneous sensors producing a great amount of diverse data opens new challenges to be faced. Rather than generating usable data straight away, complex sensors demand prior calculations to supply meaningful information. In addition, the integration of complex sensors in real applications requires a coordinated development from hardware and software teams that need a common framework to reduce development times. In this work, we present an edge and fog computing platform capable of providing seamless integration of complex sensors, with the implementation of an efficient data fusion strategy. It uses a symbiotic hardware/software design approach based on a novel messaging system running on a modular hardware platform. We have applied this platform to integrate Bluetooth vehicle identifiers and radar counters in a specific mobility use case, which exhibits an effective end-to-end integration using the proposed solution.

The minimally invasive flexor carpi radialis approach can be used for volar locking plate fixation of distal radius fractures. After 15-mm incision on the lateral aspect of the FCR tendon and all structures but the radial artery are reclined ulnarly, a plate is inserted under the pronator quadratus just proximal to the "watershed line." The distal epiphyseal screws are put in place, and the proximal part of the plate is exposed by flexion of the wrist to put in place the proximal screws. No drainage or postoperative immobilization is used. It offers the advantage of preserving ligamentotaxis which facilitates the reduction, and the small size of the scar improves the esthetic result of the procedure. It is indicated for extra-articular fractures of the distal radius. In the case of an intraarticular fracture, an arthroscopy may be associated. In the case of a proximal diaphyseal extension of the fracture, a second proximal approach can be added in order to use a longer plate. Relative contraindications are comminuted articular fractures in elderly osteoporotic patients. Functional and radiological results are comparable to those obtained with the extented flexor carpi radialis approach. A conversion of the procedure for a larger incision is always possible in the case of a difficult reduction.

BACKGROUNDS: The goal of this study was to compare trapeziectomy alone to trapeziectomy associated with a technique of suspensionplasty using the abductor pollicis longus with a minimum of 6 years of follow-up in the treatment of osteoarthritis of the first carpometacarpal joint. METHODS: Our series included 46 trapeziectomies in 35 patients, aged 69 years in average, among which 27 were women. In 27 cases group I, a trapeziectomy alone was performed, and in 19 cases group II, a trapeziectomy associated with an abductor pollicis longus suspensionplasty was performed. RESULTS: At the last follow-up, the difference between pre- and postoperative Quick DASH was - 36.104 in average in group I and - 38.877 in average in group II p = 0.7717. The difference between pre- and postoperative pain was - 3.926 in average in group I and - 4.368 in average in group II p = 0.35. The difference of pre- and postoperative thumb opposition was 0.630 in group I and 0.421 in group II p = 0.3033. The average difference of pre- and postoperative key pinch was 0.389 in group I and 0.842 in group II p = 0.3303. No complication was reported. CONCLUSIONS: Our results suggest that it is unnecessary to perform a ligamentoplasty when a trapeziectomy is indicated.

This study describes a new total wrist implant (Prosthelast®) designed to reduce the risk of distal migration of the carpal component. The Prosthelast® implant consists in a one-block radial implant replacing the metaphysis and the articular surface fixed to a radial elastic centromedullar wire and a carpal component in titanium with an articular condylar surface in polyethylene. We operated on five patients (three male patients and two female patients) and followed them up for 12 months on average. Two of the patients presented with rheumatoid arthritis of the wrist, and an ulnar osteotomy (Darrach procedure) was carried out at the same time of the arthroplasty. All clinical variables improved postoperatively (Quick DASH score, pain score, range of motion) except from wrist flexion which was reduced. No patients underwent revision surgery. Two patients presented with a periprosthetic radiolucent loosening around the radial component, but no implant migration was observed. Overall, the preliminary results of our case series show that the new Prosthelast® implant presents comparable short-term results to those described in the literature. We will follow up the patients to verify that long-term results are as satisfactory as the short-term results.

The goal of this study was to evaluate the role of conservative treatment of distal radius fractures in the elderly since 2000. Our series included 22 articles, of which 3 reviews, 2 meta-analyses, 6 descriptive series of conservative treatment, 10 series comparing surgical and conservative treatments and 1 series comparing different types of conservative treatments. The reduction was performed by closed reduction in 7 cases and by traction in 3 cases. A short-arm cast was used in 8 cases and a long-arm cast in 4 cases for an average length of 6 weeks. The indications of conservative treatment were varied: AO type A fractures in 2 cases, type C fractures in 1 case, types A and C in 8 cases and types A, B and C in 3 cases. No article reported any significant difference between surgical and conservative treatments in terms of DASH or PRWE, whereas strength was sometimes improved when surgical treatment was used. Radiological results were always better after surgical treatment. The rate of complications varied greatly, greater, equally or lower compared to conservative treatment depending on the series. All in all, there is no consensus to assert that a conservative treatment is more justified than a surgical treatment to treat distal radius fractures after the age of 65. Most authors consider that the long-term clinical outcome is identical and that the radiological result is better after surgical treatment. The fact that the fixation of a distal radius fracture by a volar locking plate after 65 years speeds up the recovery remains to be proven.
