RESUMO

The use of silicone rubber implants for repair of retinal detachments has become a common mode of therapy in recent years. Medical-grade silicone rubber is nonallergenic, easily cut to an appropriate size with scissors, and does not support bacterial growth. Several patients with these implants were studied recently by computed tomography. The characteristic computed tomographic appearances of these implants are discussed.

RESUMO

Digital intravenous angiography was used for the documentation and evaluation of cerebral vasospasm secondary to acute ruptured aneurysm. Attenuation of vessel caliber and/or generalized poor visualization of intracranial arteries and prolongation of circulation time were observed in seven patients with the acute clinical diagnosis of cerebral vasospasm. Posttreatment intravenous angiography demonstrated improvement in both arterial caliber and circulation time in six patients who responded to medical therapy for vasospasm. Digital intravenous angiography allowed repeat investigations of the intracranial vasculature safely and quickly without undue risk to the critically ill patient.

RESUMO

Noninvasive procedures (ultrasound, CT, and MRI) are increasingly valuable in the diagnosis of diseases of the head and neck. Angiography, however, is still essential in the demonstration of vascular pathology, including the preoperative evaluation of vascular neoplasms. Through angioplasty and embolization techniques, angiography also plays a leading role in the treatment of arteriosclerotic vascular disease, vascular malformations, and vascular neoplasms.

The magnetic resonance (MR) imaging characteristics of five primary intradural epidermoid tumors are described. At 0.35 T, the most consistent finding on spin echo imaging was a tumor signal intensity that differed from brain and CSF. On T1-weighted images [repetition time (TR) 0.5 s, echo time (TE) 30-40 ms] all tumors exhibited a signal intensity intermediate between brain and CSF. On moderately T2-weighted images (TR 2.0 ms, TE 60-80 ms) the tumor signal intensity was greater than brain and CSF in all cases. The tumor signal consistency was mixed in four of five lesions and homogeneous in one. The tumor margins were well defined in all cases; in three cases the tumor margins were irregular, in one case smooth, and in the last case, one margin was irregular and the remaining margins were smooth. These findings are contrasted with the MR appearance of arachnoid cysts. Using identical imaging factors, four arachnoid cysts were studied and exhibited a homogeneous signal intensity that was identical to CSF on all pulse sequences; their margins were smooth and well defined in each case. These MR findings contribute to the characterization of extraaxial lesions.

A case of brain stem infarction after chiropractic manipulation of the cervical spine is presented. Proposed mechanisms and sites of possible arterial injury are discussed. A diagnosis of vertebral artery occlusion was made using conventional brachial angiography. Digital intravenous angiography, a relatively new and less invasive vascular imaging technique which was used as an adjunct for evaluating the remainder of the cervicocephalic vessels, documented the vertebral occlusion. Chiropractic manipulation, which is increasing in popularity, may be a cause of potentially devastating neurologic disease.
