BACKGROUND: Acute appendicitis (AA) is the most common emergency general surgical condition worldwide. Diagnosis is challenging and incorporates clinical, biochemical and radiological investigations. Our aim was to provide data from routine practice investigating widely utilised diagnostic methods from a single centre within the United Kingdom. METHODS: We conducted a retrospective observational cohort study of patients who underwent a laparoscopic appendicectomy for AA between April 2022 and March 2023. AA was defined as the presence of transmural polymorphonuclear leukocytes in histology. Subgroup analysis was performed on paediatric patients. Factors associated with AA were investigated, and the diagnostic utility of biochemical and radiological investigations was examined. RESULTS: A total of 330 appendicectomies were analysed. We found an overall negative appendicectomy rate (NAR) of 38% and 48% in paediatric patients. Independent factors associated with AA on the multivariate analysis included elevated neutrophil counts (>7 × 109/L) (OR 4.04), elevated CRP (>5 mg/L) (OR 3.04) and a radiological diagnosis (OR 8.0). Computerised tomography (CT) and ultrasound had sensitivity/specificity of 98%/47% and 35%/86%, respectively. The positive-predictive values were 85% for CT and 50% for ultrasound, and the negative-predictive values were 86% for CT and 77% for ultrasound. CONCLUSION: Our study has highlighted the importance of utilising a combination of factors to improve the diagnostic certainty of AA. However, our routine practice data have shown different sensitivities and specificities of imaging in comparison to existing literature, resulting in a high NAR. Further real-world data are needed to understand whether these differences from the existing data are seen in other clinical settings.

PURPOSE: To determine whether perfusion computed tomography (CT)-derived vascular parameters-namely, blood flow, mean transit time (MTT), volume transfer constant (K(trans)), permeability-surface area product (PS), extracellular extravascular space volume, and vascular volume-correlate with the immunohistologic markers of angiogenesis in colorectal tumors. MATERIALS AND METHODS: This prospective study was approved by the Regional Ethics and Research and Development Committees. The perfusion CT protocol was incorporated in the staging CT after informed consent in 29 patients (14 men, 15 women; mean age, 70 years; age range, 55-94 years). The perfusion parameters were calculated over two regions of interest (ROIs), at the invasive and luminal site defined by two radiologists independently. Accurate representative data were captured manually by correcting for motion artifacts and were analyzed by using Matlab software. The vascular heterogeneity between ROIs was assessed by using the Wilcoxon signed rank test. Perfusion CT parameters were correlated with the microvessel density (MVD) count at both corresponding sites obtained by means of immunohistochemical staining of the selected histologic slide with factor VIII and CD105 antigens by using Spearmen rank coefficient. RESULTS: There was no statistically significant difference found between perfusion CT vascular parameters at the two ROIs by either of the radiologists. The Pearson coefficient for blood flow, MTT, K(trans), and PS at the two ROIs demonstrated good to moderate interobserver variability (for the two ROIs, 0.46 and 0.44; 0.67 and 0.64; 0.41 and 0.72; and 0.86 and 0.56, respectively). None of these parameters correlated with MVD count at the invasive or the luminal site for either of the two antigens. CONCLUSION: Perfusion CT measurements may measure vascularity of colorectal tumors, however, correlation with MVD, which is a morphologic measure, appears inappropriate. © RSNA, 2013.

RESUMO

Trichobezoars are rare, but most commonly found in young women with trichophagia and trichotillomania. Complications can include iron deficiency anaemia and gastric erosion or, rarely, perforation. A 19-year-old woman presented with epigastric pain, vomiting and lethargy. Initial investigations revealed a palpable abdominal fullness on examination and iron deficiency anaemia. Oesophagogastroduodenoscopy found a large trichobezoar associated with gastric erosions, polyps and an ulcer. Subsequently, the patient reported previous consumption of artificial hair extensions, which ceased 5 years previously. Attempts to remove the trichobezoar by endoscopy were ineffective and in line with current literature, laparotomy was successful. This case describes a rare cause of trichobezoar and emphasises the importance of appropriate initial investigations and definitive management.
