RESUMO

Alloantibody-mediated and autoantibody-mediated immune destruction are common causes of early neonatal thrombocytopenia. The authors report a case of severe, early-onset thrombocytopenia with mucocutaneous bleeding in an otherwise well-appearing full-term neonate. Recurrence of thrombocytopenia following initial treatment and its persistence after 2 weeks of life suggested a dual immune mechanism. This is a rare case of immune thrombocytopenia caused by human platelet antigen-5b alloimmunization and passive transfer of maternal antiplatelet antibodies. Appropriate, timely treatment and absence of severe bleeding complications, namely intracranial hemorrhage, conferred a good overall prognosis.

RESUMO

OBJECTIVES: To analyze and assess Lee Geropaliative Caring Model according to Fawcett's criteria. METHOD: A theoretical-reflective study about the Geropaliative Caring Model. The analysis resulted in a detailed review of the scope, context and content of a situation-specific theory, in order to determine aspects related to nursing practice and research, and assessment addressed the concrete concepts developed in theory, their significance, internal consistency, parsimony, testability and empirical and pragmatic adequacy in nursing as a discipline. RESULTS: Based on the analysis, a situation-specific theory was used based on the science of care and aimed at caring for older adults undergoing palliation and their families, structured into four fields, namely: (1) aligning care; (2) keeping safe; (3) comforting body/mind/spirit; and (4) facilitating transitions. FINAL CONSIDERATIONS: The analyzed and assessed theory leads to a structure that includes well-defined, delimited and interrelated concepts, based on the science of care as a grand theory that allowed Geropaliative Care Model derivation.

RESUMO

OBJECTIVE: To reflect on the contributions of representing nursing practice elements in the ISO 18.104:2023 standard. METHOD: This is a theoretical study with standard analysis. Categorical structures were described to represent nursing practice in terminological systems and contributions identified in the parts of the version were analyzed. RESULTS: There is innovation in the inclusion of nurse sensitive outcomes, nursing action, nursing diagnosis explanation as an indicator of nursing service demand and complexity of care, representation of concepts through mental maps and suggestion of use of restriction models for nursing actions. It describes that the Nursing Process is constituted by nursing diagnosis, nursing action and nurse sensitive outcomes. FINAL CONSIDERATIONS: Indicating a nursing diagnosis as an indicator will bring benefits for knowledge production and decision-making. Although care outcomes are not exclusive responses to nursing action, the modifiable attributes of a nursing diagnosis generate knowledge about clinical practice, nursing action effectiveness and subjects of care' health state. There is coherence in understanding the Nursing Process concept evolution.

RESUMO

OBJECTIVES: To analyze the concept of men's health care and identify its essential, antecedent and consequent attributes in the health context. METHODS: This is a concept analysis structured in the theoretical-methodological framework of the Walker and Avant Model. An integrative review was carried out between May and July 2020, using keywords and descriptors: Men's Care and Health. RESULTS: The concept of men's health care is structured by 240 attributes, 14 categories, 82 antecedents and 159 consequents, from the selection of 26 published manuscripts. The design was evidenced from the dimensions: Intrapersonal, psychological and behavioral related to masculinities, interpersonal, organizational and structural, ecological, ethnoracial, cross-cultural and transpersonal. CONCLUSIONS: The concept of men's health care revealed the male specificities regarding the recognition of the place of health care and the daily exercise in the lived experience.

RESUMO

The spectrum of the clinical presentation and severity of malaria infections is broad, ranging from uncomplicated febrile illness to severe forms of disease such as cerebral malaria (CM), acute lung injury (ALI), acute respiratory distress syndrome (ARDS), pregnancy-associated malaria (PAM) or severe anemia (SA). Rodent models that mimic human CM, PAM and SA syndromes have been established. Here, we show that DBA/2 mice infected with P. berghei ANKA constitute a new model for malaria-associated ALI. Up to 60% of the mice showed dyspnea, airway obstruction and hypoxemia and died between days 7 and 12 post-infection. The most common pathological findings were pleural effusion, pulmonary hemorrhage and edema, consistent with increased lung vessel permeability, while the blood-brain barrier was intact. Malaria-associated ALI correlated with high levels of circulating VEGF, produced de novo in the spleen, and its blockage led to protection of mice from this syndrome. In addition, either splenectomization or administration of the anti-inflammatory molecule carbon monoxide led to a significant reduction in the levels of sera VEGF and to protection from ALI. The similarities between the physiopathological lesions described here and the ones occurring in humans, as well as the demonstration that VEGF is a critical host factor in the onset of malaria-associated ALI in mice, not only offers important mechanistic insights into the processes underlying the pathology related with malaria but may also pave the way for interventional studies.

RESUMO

OBJECTIVES: to analyze the concept of cardiovascular risk to support nursing practice. METHODS: this is an analysis to define the concept of cardiovascular risk, through the use of eight steps of Walker's and Avant's framework, using a literature review in indexed scientific journals. RESULTS: cardiovascular risk is defined in a broad and original way as a context of health and nursing care that makes it possible to identify modifiable (cardiometabolic, behavioral, psychosocial, cultural and occupational) and non-modifiable (biological) risk factors for cardiovascular diseases that act as early and interrelated markers, of multiple and heterogeneous etiology, predisposing to cardiovascular vulnerability. CONCLUSIONS: with the analysis and definition of the concept of cardiovascular risk, we realized that it will be possible to base nursing practice, with implications in clinical practice for identifying and reducing risk factors for cardiovascular diseases, with nursing relevance in the care of these subjects.

RESUMO

OBJECTIVE: to map the diverse scientific evidence available about the repercussions of the COVID-19 pandemic for people with Autism Spectrum Disorder and their family members. METHOD: a scoping review conducted in the following databases: MEDLINE, CINAHL, SciELO, SCOPUS, EMBASE and Wiley Online Library, in August and September 2021. The protocol of this review was registered at the Open Science Framework under DOI 10.17605/OSF.IO/JYTVD. The data were analyzed and synthesized in a narrative way. RESULTS: a total of 46 publications identified indicate that the pandemic period brought about countless repercussions for the population with autism and their family members when experiencing serious difficulties in the changes or irregularities of the scheduled routines and limited access to education, therapies and social support. CONCLUSION: the diverse evidence suggests that the COVID-19 pandemic intensified the inequalities already experienced by individuals with autism and their family members, with negative consequences in the emotional, psychological, behavioral and social aspects, interfering with the quality of life and mental health of this population segment. Future studies on individuals with autism and their families during public health crisis periods are of fundamental importance for planning psychiatric, psychosocial and educational interventions.

RESUMO

Metabolic syndrome (MS) is a clinical condition and a relevant risk factor in the development of cardiovascular diseases; it occurs as a result of lifestyle factors, e.g., work. The aim of this research was to estimate the interaction between work and MS among primary health care (PHC) nursing professionals in the state of Bahia, Brazil. A sectional multicentered study carried out in 43 municipalities in Bahia, whose study population consisted of nursing professionals. The exposure variables were occupation, professional exhaustion, and working time, and the outcome variable was MS. Interaction measures based on the additivity criteria were verified by calculating the excess risks due to the interactions and according to the proportion of cases attributed to the interactions and the synergy index. The global MS prevalence is 24.4%. There was a greater magnitude in the exposure group regarding the three investigated factors (average level occupation, professional exhaustion, and working time in PHC for more than 5 years), reaching an occurrence of 44.9% when compared to the prevalence of 13.1% in the non-exposure group (academic education, without professional burnout, and working time in PHC for up to 5 years). The study's findings showed a synergistic interaction of work aspects for MS occurrence among PHC nursing professionals.

RESUMO

To analyze the implications of the Nursing Care Systematization in Brazilian professional practice. Theoretical and reflective study, which listed as analytical categories the regulatory framework of systematization, the implications for professional practice, the limits and possibilities. In the legal frameworks, an evolution of understanding about Systematization was identified, which resulted in an update in the resolution that deals with the theme. It is emphasized that the implications for professional practice can be elucidated when the concepts, method, personnel and instruments are better understood, the three pillars of systematization, since they do not have conceptual and operational definitions in the current resolution. The lack of conceptual clarity about the term, as well as the lack of conceptual and operational definitions of its pillars and constituent elements, have made the understanding of the Nursing Care Systematization difficult, impacting the perception of its contribution to professional practice, since it is not rarely used as a synonym for the Nursing Process. The production of knowledge about Systematization must consider its limits and possibilities. Thus, contributing to the demarcation of the production of knowledge that is representative of this epistemological object.

OBJECTIVE: to build a specialized nursing terminology for the prevention of falls in the elderly in primary healthcare, based on the ICNP®. METHOD: this is a methodological study developed in two stages: (1) identification of relevant terms for the prevention of falls in the elderly in official documents; (2) cross mapping of the identified terms with the terms contained in the ICNP® Seven-Axis Model, version 2019/2020. RESULTS: a total of 13,408 terms was extracted from official documents, which were submitted to manual screening, resulting in the inclusion of 391 relevant terms. Cross mapping revealed 283 constant terms (67.8% with level of equivalence 1; 32.2% with level of equivalence 2) and 108 non-constant terms (88.9% with level of equivalence 5; 6.5% with level of equivalence 4; 4.6% with level of equivalence 3). In the set of constant terms, the terms of the Focus (43.1%), Action (26.1%), and Means (13.1%) axes stood out; in non-constant terms there was a predominance of the Focus (38.0%), Means (23.1%), and Judgment (15.7%) axes. CONCLUSION: we obtained a specialized nursing terminology that will support the development of nursing diagnoses, outcomes, and interventions to contribute to the prevention of falls in the elderly in primary healthcare.

RESUMO

OBJECTIVE: To create specialized nursing terminology for the care of people with COVID-19. METHODS: Methodological study, carried out based on the identification of concepts related to the care of the person with the infection, present in the official guidelines of the Brazilian Ministry of Health; the concepts were validated by three specialist nurses; and a cross-mapping of the extracted concepts was done with CIPE® 2019 primitive concepts. RESULTS: Out of 436 unique concepts, being 399 of these validated; of these, 70.9% were in the Classification and referred to the nursing care of people with COVID-19. In the Axis, the concepts related to Focus, Action, and Environment stood out. As for the non-validated concepts, 78.4% were not included in the 2019 version of CIPE®. CONCLUSION: We were able to structure a specialized CIPE® terminology, with the potential to generate nursing care indicators for people with COVID-19.

RESUMO

OBJECTIVE: to analyze the metabolic syndrome concept and to identify its essential features, antecedents, and outcomes within the context of nursing. METHOD: conceptual analysis, based on the methodological steps of a model. We carried out an integrative review by accessing four databases online: Medical Literature Analysis and Retrieval System Online, Scientific Electronic Library Online, Latin American and Caribbean Health Sciences Literature, and Índice Bibliográfico Español en Ciencias de la Salud. RESULTS: the essential features most frequently involved the diagnostic criteria of metabolic syndrome. Inadequate nutrition and physical inactivity were highlighted as the most common antecedents of the syndrome, and the outcomes were occurrences of cardiovascular disease and diabetes mellitus type 2. As implication, we highlight relevant empirical data to the broad definition of the concept. CONCLUSION: we could analyze the concept under study regarding essential features, antecedents, and outcomes, operationally defining it as a potential nursing phenomenon, which demands health care focusing on reducing risks and morbidity and mortality for cardiovascular diseases.

RESUMO

In crisis scenarios, the professions that have the essence of caring for are highlighted. In this sense, during the COVID-19 pandemic, nursing played a key role in the identification, testing, care and rehabilitation of patients.(1) It was through this profession that many clients were fully assisted in the process of illness/rehabilitation, revealing the importance of the category for the health system; a fact that also persists in a post-COVID scenario. COVID-19 led to the collapse of global health with high rates of mortality and hospital morbidity, generating an estimated 18.2 million deaths around the world.(2) Nevertheless, it is known that COVID-19 is a respiratory disease with vascular implications that interact with cardiometabolic factors such as oxidative stress, endothelial dysfunction, insulin resistance, atherosclerosis, overweight, increased body fat and alterations in the microbiome, leading to systemic complications and death.(3)
