RESUMO

Puerperal ovarian vein thrombophlebitis is a rare pathology. We present the case of a 23-year-old female who presented with fever and cough that occurred after vaginal delivery and persisted after several courses of antibiotics between the 2nd and 20th day postpartum. CT pulmonary angiography revealed right subsegmental pulmonary embolism. An abdominal ultrasonographic examination led to the diagnosis of ovarian vein thrombosis. She was treated with warfarin for 2 weeks with a good response. Our case highlights the importance of prompt ultrasonographic diagnosis and clinical treatment of ovarian vein thrombosis to prevent morbidity and mortality.

Pemetrexed (PEM) is an effective chemotherapeutic drug used for the treatment of clinical non-small-cell lung cancer (NSCLC) and is reported to induce severe hepatotoxicity. Exploring potential drugs which could counteract the side effects of PEM is of great clinical interest. Here, we aim to examine the beneficial effects of Montelukast, a novel anti-asthma drug, against PEM-induced cytotoxicity in hepatocytes, and to explore the underlying mechanism. We found that Montelukast reduces cytotoxicity of PEM in hepatocytes, confirmed by its increasing cell viability and reducing lactate dehydrogenase (LDH) release. In addition, Montelukast attenuated PEM-induced oxidative stress by reducing mitochondrial reactive oxygen species (ROS), increasing reduced glutathione (GSH), and downregulating NADPH oxidase 4 (NOX-4) expression. Importantly, Montelukast suppressed PEM-induced activation of the nucleotide oligomerization domain-like receptor protein 3 (NLRP3) inflammasome and mitigated endoplasmic reticulum (ER) stress by reducing NLRP3, growth arrest, and DNA damage-inducible protein 34 (GADD34), CEBP-homologous protein (CHOP), and also blocking the eukaryotic initiation factor 2 (eIF-2α)/activating transcription factor 4 (ATF4) signaling pathway. Lastly, we found that Montelukast inhibited the transcriptional activity of nuclear factor kappa-B (NF-κB). Montelukast exerted a protective action against PEM-induced cytotoxicity in hepatocytes by mitigating ER stress and NLRP3 activation.

The aim of our study was to establish an artificial intelligence tool for the diagnosis of breast disease base on ultrasound (US) images. A deep learning algorithm Efficient-Det assisted US diagnosis method was developed to determine breast suspicious lesions as benign, malignant, or normal. Totally 1181 US images from 487 patients of our hospital and 694 publicly accessible images were employed for modeling, including 558 benign images, 370 malignant images, and 253 normal tissue images. The actual diagnosis results for the patients were determined by the biopsy or surgery. Efficient-Det was first retrained using an exclusive public breast cancer US dataset with transfer learning techniques. A blind test set consisting of 50 benign, 50 malignant, and 50 normal tissue images was randomly picked from the patients' images as the independent test set to test its searching ability on suspicious tumor regions. Furthermore, the confusion matrix and classification accuracy were employed as evaluation metrics to select the optimal classification models. Efficient-Det has demonstrated remarkable progress in general image recognition tasks with specific advantages of locating and identifying tumor areas simultaneously. Compared to the manual method (mean accuracy: 95.3% and 60 s per image) and traditional feature engineering method (mean accuracy: 90% and 15 s per image), our Efficient-Det has the capability of providing a competitive (mean accuracy: 92.6%) and fast (0.06 s per image) classification result. The deployment of Efficient-Det in our local breast cancer discrimination task exhibits specific applicability within real clinical workflows.

BACKGROUND: Only a few studies have investigated the risk factors for the prognosis of progressive cerebral infarction (PCI) and the relationship between PCI and cerebral infarction (CI) recurrence. OBJECTIVE: The objective of this study is to analyze the risk factors for PCI and PCI prognosis and evaluate the relationship between PCI and CI recurrence, mortality, short-term outcomes. METHODS: The retrospective study included 221 CI patients. PCI and non-PCI patients were divided into the observation (91) and control (130) groups, respectively. Patients' clinical data, including diabetes history, laboratory blood indices, National Institutes of Health Stroke Scale (NIHSS) scores at admission, and presence of carotid and intracranial artery stenoses, were retrospectively analyzed. Instances of CI recurrence, adverse short-term outcomes, and death within 1 year postanalysis were recorded. RESULTS: Diabetes, homocysteine, NIHSS score at admission, fibrinogen, and intracranial artery stenosis were associated with PCI. Age, NIHSS score at admission, and pneumonia were associated with PCI prognosis. By 12-month follow-up, the mortality and adverse outcome rate in the observation group were significantly increased than those of the control group. There was no diference in CI recurrence rates between the groups. Adverse outcomes were associated with PCI, age, and homocysteine. CONCLUSIONS: PCI risk factors included diabetes, homocysteine, NIHSS score at admission, fibrinogen, and intracranial artery stenosis. The independent risk factors for PCI prognosis included age, high NIHSS score at admission, and pneumonia. PCI did not affect CI recurrence but may affect adverse short-term outcomes.

Two nanocomposites with a hierarchical structure (GO@CuSilicate@Fe3O4 and GO@Fe3O4@CuSilicate) were fabricated in this paper. These as-synthesized nanocomposites were analyzed for their structural, compositional, and morphological features by X-ray diffraction, scanning electron microscopy (SEM), Raman spectroscopy, and Brunauer-Emmett-Teller methods. SEM images showed that both nanocomposites had a core-shell structure, and their shells were composed of CuSilicate nanoneedle arrays. Further, their total electromagnetic shielding efficiency was measured and compared in a wide frequency range of 8-12 GHz (X-band). Because of the "antenna" role of CuSilicate nanoneedle arrays and the polarization at the interface between graphene oxide (GO) and Fe3O4, GO@Fe3O4@CuSilicate showed higher electromagnetic shielding performance than that of GO@CuSilicate@Fe3O4. With 1 mm thickness, GO@Fe3O4@CuSilicate showed a high electromagnetic shielding efficiency (over 40 dB) in the whole X-band (8.2-12.4 GHz) and reached a maximum value (41.8 dB) at 8.2 GHz. Its total electromagnetic shielding efficiency was mainly contributed by absorption rather than reflection. This study provided an idea for the structural design of high-performance electromagnetic shielding materials in the GHz frequency range (X band).

Polymer foams are promising for sound absorption applications. In order to process an industrial product, a series of polystyrene (PS) composite foams were prepared by continuous extrusion foaming assisted by supercritical CO2. Because the cell size and cell density were the key to determine the sound absorption coefficient at normal incidence, the bio-resource lignin was employed for the first time to control the cellular structure on basis of hetero-nucleation effect. The sound absorption range of the PS/lignin composite foams was corresponding to the cellular structure and lignin content. As a result, the maximum sound absorption coefficient at normal incidence was higher than 0.90. For a comparison, multiwall carbon nanotube (MWCNT) and micro graphite (mGr) particles were also used as the nucleation agent during the foaming process, respectively, which were more effective on the hetero-nucleation effect. The mechanical property and thermal stability of various foams were measured as well. Lignin showed a fire retardant effect in PS composite foam.

Excitation wavelength-dependent photoluminescence (PL) in two-dimensional (2D) transition-metal chalcogenides enables a strong excitonic interaction for high-performance chemical and biological sensing applications. In this work, we explore the possible candidates in the domain of post-transition-metal chalcogenides. Few-layered 2D p-type tin monosulfide (SnS) nanoflakes with submicrometer lateral dimensions are synthesized from the liquid phase exfoliation of bulk crystals. Excitation wavelength-dependent PL is found, and the excitonic radiative lifetime is more than one order enhanced compared to that of the bulk counterpart because of the quantum confinement effect. Paramagnetic NO2 gas is selected as a representative to investigate the exciton-driven chemical-sensing properties of 2D SnS. Physisorption of NO2 results in the formation of dipoles on the surface of 2D SnS, causing the redistribution of photoexcited charges in the body and therefore modifying PL properties. For practical sensing applications, 2D SnS is integrated into a resistive transducing platform. Under light irradiation, the sensor exhibits excellent sensitivity and selectivity to NO2 at a relatively low operating temperature of 60 °C. The limit of detection is 17 parts per billion (ppb), which is significantly improved over other previously reported 2D p-type semiconductor-based NO2 sensors.

The application area of a sound insulation material is highly dependent on the technology adopted for its processing. In this study, thermoplastic rubber (TPR, polypropylene/ethylene propylene diene monomer) composites were simply prepared via an extrusion method. Two microscale particles, CaCO3 and hollow glass microspheres (HGW) were chosen to not only enhance the sound insulation but also reinforced the mechanical properties. Meanwhile, the processing capability of composites was confirmed. SEM images showed that the CaCO3 was uniformly dispersed in TPR matrix with ~3 µm scale aggregates, while the HGM was slightly aggregated to ~13 µm scale. The heterogeneous dispersion of micro-scale fillers strongly affected the sound transmission loss (STL) value of composites. The STL values of TPR composites with 40 wt % CaCO3 and 20 wt % HGM composites were about 12 dB and 7 dB higher than that of pure TPR sample, respectively. The improved sound insulation performances of the composites have been attributed to the enhanced reflection and dissipate sound energy in the heterogeneous composite. Moreover, the mechanical properties were also enhanced. The discontinued sound impedance and reinforced stiffness were considered as crucial for the sound insulation.

To improve the mechanical strength and antipollution properties of membranes, this research presents a facile method to prepare crosslinked polyacrylonitrile (PAN) membranes. This was achieved firstly by radical copolymerization with acrylonitrile, allyl methacrylate and sulfobetaine methacrylamide. Then, the copolymer was crosslinked by a thiol-ene click reaction under UV irradiation. Finally, the crosslinked membranes were prepared by traditional immersion precipitation phase inversion. These prepared membranes showed excellent water-pressure resistance and solvent swelling, owing to their crosslinked structure. This research will help in preparing crosslinked membranes through facile crosslinking under mild reaction conditions. The betaine structure also considerably improved the antifouling properties of the membranes.

Human hepatobiliary cystadenoma is a rare benign cystic tumor of the liver, and is extremely rare in the caudate lobe. We herein present a case of a 70-year-old male with a hepatobiliary cystadenoma originating from the caudate lobe.
