RESUMO

OBJECTIVE: To describe the frequency of incomplete histological margins following planned narrow excision (PNE) of mast cell tumors (MCTs) and soft tissue sarcomas (STSs), and to assess the residual tumor classification (R) scheme for reporting histological margins in clinical cases. STUDY DESIGN: Retrospective clinical study. SAMPLE POPULATION: Forty-four client-owned dogs with 47 masses. METHODS: Medical records of dogs undergoing planned narrow excision of STSs and MCTs were reviewed (2016-2019). Histologic specimens were reviewed by a single pathologist and assigned R scoring (histologically incomplete/R1 margins defined as "tumor on ink"). RESULTS: Six out of 23 (26%) MCT PNEs and 10/42 (42%) of STS PNEs resulted in R1 margins. R1 margins were more likely when performing PNE with 6-10 mm lateral measured surgical margins (LMSMs) versus 0-5 mm LMSM for MCTs (1/14 vs 5/9), but not STSs (3/7 vs 7/17) (P = .049). The R scheme resulted in higher retrospective percentage agreement in histological reporting than defining incomplete histological margin as tumor cells within ≤1 mm of the margin (83% vs 68% agreement). Complications occurred in 12/47 surgeries, with none requiring additional surgery. Tumors recurred in 3/18 (17%) STSs and 2/18 (11%) MCTs. CONCLUSION: Fewer R1 margins were obtained when PNE with LMSM of 6-10 mm was performed for mast cell tumors. The use of the R scheme increased agreement in histopathological margin assessment. CLINICAL SIGNIFICANCE: Planned narrow excision is a viable technique for histopathological diagnosis of appendicular soft tissue sarcomas and mast cell tumors for limb salvage.

RESUMO

A 7-year-old neutered male Staffordshire bull terrier dog was presented for investigation of chronic profuse urethral hemorrhage. A vascular mucosal mass lesion was identified in the proximal penile urethra on ultrasound examination; prescrotal urethrotomy was performed to allow rigid urethroscopy and mass removal. Histopathological changes were consistent with proliferative urethritis. Key clinical message: Prescrotal urethrotomy to facilitate rigid urethroscopy has not been previously described and is a useful technique to allow visualization of the male canine proximal penile urethra distal to the pelvic flexure. Proliferative urethritis is an important differential diagnosis for dogs presenting with profuse urethral hemorrhage.


Urétrotomie préscrotale pour l'ablation urétroscopique d'une masse mucosale urétrale hémorragique. Un Staffordshire bull terrier mâle castré de 7 ans a été présenté pour exploration d'une hémorragie urétrale chronique profuse. Un examen échographique a permis d'identifier une masse mucosale vascularisée dans la partie proximale de l'urètre pénien; une urétrotomie préscrotale a été réalisée pour permettre une urétroscopie rigide et le retrait de la masse. Les changements histopathologiques étaient compatibles avec une urétrite proliférative.Message clinique clé:L'urétrotomie préscrotale pour faciliter la réalisation d'une urétroscopie rigide n'a pas été décrite précédemment et constitue une technique utile pour permettre la visualisation de la partie proximale de l'urètre pénien du chien mâle distalement à la courbure pelvienne. L'urétrite proliférative est un diagnostic différentiel important à considérer chez les chiens présentant une hémorragie urétrale profuse.(Traduit par les auteurs).

OBJECTIVES: The aim of this study is to describe the presentation, surgical management and perioperative outcome of cats with acetabular fractures. METHODS: Case records and radiographs of cats with acetabular fractures were reviewed from presentation to the end of follow-up. Surgical technique, complications, radiographic reduction and the presence of neurological deficits were recorded. RESULTS: Sixteen cats with 17 acetabular fractures met the inclusion criteria. All fractures were associated with concurrent orthopaedic injuries. All cats were either moderately or severely lame on presentation. Five fractures were stabilised using screws, wire, pins and polymethylmethacrylate, nine were repaired using a straight dynamic compression plate and three were repaired using a locking plate. Two of seven cats that were neurologically normal prior to surgery developed transient neuropraxia following surgery. There were three major complications and no minor complications. At the end of follow-up 3/16 cats had full function, whereas 13/16 had acceptable function. CONCLUSIONS AND RELEVANCE: All cats undergoing surgical stabilisation of acetabular fractures returned to full or acceptable function by the end of the follow-up period and there was a low number of intraoperative and short-term complications. Neurological deficits were common preoperatively, but the majority of these deficits had resolved by follow-up.
