BACKGROUND: Whether fetal cardiothoracic ratio (CTR) is constant or increasing with gestational age (GA) is controversial. The majority of the fetal CTR data has been obtained through ultrasound. PURPOSE: To retrospectively analyze CTR of diameter, area, and circumference on prenatal MR images in a low-risk population of singleton pregnancies, and to clarify its diagnostic value. STUDY TYPE: Retrospective. SUBJECTS: 1024 low-risk singleton pregnancies undergoing MRI. FIELD STRENGTH: Balanced steady state free precession sequence and single shot-fast spin echo sequence at 1.5 Tesla. ASSESSMENT: Pregnancy clinical data were recorded and diameter, area, and circumference of the fetal heart and thorax were measured by two researchers with 6 and 7 years of radiology experience, respectively, and their variation with GA was investigated. The relationship between CTRs with GA was also investigated. Finally, the value of CTR in the diagnosis of fetuses with abnormal development was explored by using receiver operating characteristic (ROC) curves. STATISTICAL TESTS: Linear regression and ROC curves. A P value <0.05 was considered significant. RESULTS: There were significant positive linear correlations (R2 > 0.7, P < 0.0001) between the diameter, area, and circumference of the heart and thorax with GA. The CTRs remain constant values and do not change with GA. The 5th, 50th, and 95th percentiles of the CTR in 21-38 weeks GA were 0.32, 0.39, and 0.48 respectively. The corresponding percentiles for the area ratio were 0.15, 0.21 and 0.27, respectively, and for the circumference ratio were 0.40, 0.46, and 0.52, respectively. Based on ROC curves of CTR with three methods, the area under curves (AUCs) were up to 0.95, the sensitivity and the specificity were more than 88%. DATA CONCLUSION: Reference ranges of fetal CTR were established using MRI, which remain constant. These may be helpful in making a definitive diagnosis in fetuses with abnormal development. Cardiac teratomas are very rare primary tumors; most are intrapericardial, while a few are intracardiac. Furthermore, most reported intracardiac teratomas are in the pediatric population, with few cases of secondary metastases from testicular teratomas reportedly manifesting in adults. Here, we report a rare case of a mature cystic teratoma in the right ventricle complicated by a bicuspid aortic valve (BAV) in an adult. Echocardiography and enhanced computed tomography (CT) were performed, and the mass was surgically excised. A pathological examination confirmed the diagnosis of a mature cystic teratoma. Meanwhile, mechanical valve replacement of the aortic valve was performed. No tumor recurrence or symptoms occurred in the 2-year follow-up. This is the first report of an adult primary intracardiac teratoma with solid hyperechoic findings on echocardiography and a BAV.

Intravenous leiomyomatosis (IVL) is a rare condition characterized by a tumor that originates within a pelvic vein wall or the uterine smooth muscle, with extension into the right heart (referred to as intracardiac leiomyomatosis) in approximately 10% of all IVL cases. Usually, computed tomography (CT) or magnetic resonance imaging (MRI) is performed for diagnostic imaging of IVL. Notably, this neoplasm presents with characteristic ultrasonographic findings. In this report, we present the case of a 49-year-old woman with IVL, which extended into the right heart. Echocardiography combined with abdominal ultrasonography was useful to outline the course of the tumor from the right heart to the uterus. Our findings suggest that in addition to CT or MRI, ultrasonography shows high diagnostic value in cases of IVL, and ultrasonography combined with CT or MRI can further improve the preoperative diagnosis rate of IVL.

OBJECTIVE: To evaluate the association of asymmetrical cortical vein sign (ACVS) and asymmetrical medullary vein sign (AMVS) on susceptibility-weighted imaging (SWI) with 90-day poor outcomes in patients with unilateral middle cerebral artery acute ischemic stroke (AIS) after conservative drug treatment. METHODS: Clinical data for the participants included age, sex, smoking, alcohol, hypertension, diabetes, hyperlipidemia, coronary heart disease, NHISS-admission, and NHISS-discharge scores. Participants underwent magnetic resonance imaging (MRI) within 12 h of hospital admission, including conventional scan sequences and a SWI sequence. Poor prognosis was defined as a modified Rankin scale (mRS) ≥ 3 at 90 days. RESULTS: A total of 108 patients were included from January 2021 to March 2022. Twenty-seven (25%) patients had a poor outcome at 90 days. Univariate analysis indicated that diabetes, NHISS-admission, NHISS-discharge, DWI-ASPECTS, SWI-ASPECTS, FLAIR-ASPECTS, and AMVS + were associated with 90-day poor outcome. Multivariate regression analysis showed that AMVS + was associated with 90-day poor outcome from the three models (OR = 3.57, P = 0.006; OR = 3.74, P = 0.005; OR = 5.14, P = 0.0057). However, no significant association was found between ACVS + and 90-day poor outcome. CONCLUSIONS: AMVS might be a helpful neuroimaging predictor for poor outcome at 90 days compared to ACVS in drug-conserving treatment of patients with unilateral middle cerebral artery ischemic stroke.

OBJECTIVE: To investigate the influence of improved exposure parameters on the image quality of multi-slice spiral computed tomography in nasal bone fracture imaging. METHODS: Fifty patients with optimised parameters combined with coronal scanning were allocated to the modified group and 50 patients with routine scanning parameters to the routine group. The image quality and nasal bone display of the two groups were assessed and statistically analysed, and the quality of scanned images before and after parameter optimisation was compared. RESULTS: The optimised image quality was better than that of conventional scanning parameters. The parameters used were 120 kv, 180 mA, a layer thickness of 0.625 mm, a layer spacing of 0.312 mm, a pitch of 0.516:1, a frame speed of 1 s, a scanning field of 12 cm and a reconstructed layer thickness for scanning of 0.625 mm; the scanned image was clear, and the parameter optimisation was achieved. This ensures that the annotation data in ITK labelling is more accurate. CONCLUSION: The optimised parameters and scanned coronal plane show the nasal bone and its surrounding structures more comprehensively, which is of high diagnostic value for nasal bone fractures. The three-dimensional annotation data based on ITK is more standardised, laying a foundation for the subsequent research of artificial intelligence modelling.

PURPOSE: To evaluate and determine age-related changes in rat brains by studying the diffusion kurtosis imaging results among different age groups of rats. METHODS: Sprague-Dawley (SD) rats underwent conventional magnetic resonance imaging (MRI) and diffusion Kurtosis Imaging (DKI). Two diffusion values of mean kurtosis (MK) and kurtosis (K⊥ ) were measured and analyzed based on laterality, brain regions and age groups. The MK and K⊥ data were plotted against different age groups. RESULTS: No laterality was found for the MK or K⊥ values in the cerebral cortex (CT), external capsule (EC), or caudate putamen (CPu) regions. In contrast, significant changes in these values were observed among different age groups. Changes of the MK and K⊥ values were significant in both hemispheres in the EC, the CT, and the CPu brain regions. The changes in the MK and K⊥ values showed a parabolic relationship with ages in all the brain regions. CONCLUSION: No laterality in the MK and K⊥ values was observed for the EC, CT, or CPu regions of the rat brain. Significant changes in MK and K⊥ values were both observed among different age groups, thus suggesting diffusion kurtosis imaging as an efficient tool for studying brain aging in rats.

PURPOSE: Currently, the diagnosis of neural antibody-mediated epilepsy/seizure (NAME/S)relies heavily on neural antibody testing, which is time-consuming, costly and introduces diagnostic delays. A statistical tool to predict the probability of a patient with NAME/S is lacking. We aimed to construct a predictive model to help clinicians expedite the diagnostic process. METHODS: We retrospectively recruited subjects (206 in the development group and 62 in the validation group) with new-onset seizures or established epilepsy suspected to have presented with antibody-mediated seizures between January 2014 and December 2019. We collected data about demographics, medical history, clinical manifestations and follow up. Binary logistic regression was used to select potential predictors for the construction of a predictive model. Five-fold cross and bootstrap validation were applied to avoid overfitting. Concordance index, calibration plots and decision curve analysis were used to assess its performance. RESULTS: The model, incorporating presence/absence of tumour, psychiatric/cognitive/emotional changes, language disturbances, sensory auras, tonic-clonic seizures, multiple seizure events, hyponatremia and MRI inflammation, was visualized as a nomogram. The crude and adjusted concordance indices were both 0.88 with a cut-off value of 0.62, sensitivity of 83.2 % and specificity of 77.4 %. The slope and intercept of the calibration curve were 0 and 1, respectively. The model also showed good performance in the validation group with a concordance index of 0.82, cut-off value of 0.33, sensitivity of 75.5 % and specificity of 73.1 %. The slope was 0.86 and the intercept was 0.039. Decision curve analysis showed that the model was useful with an optimal threshold probability of >4 % in both groups. CONCLUSIONS: Despite limitations such as sample volume and selection bias in subject enrolment, this model may be used to estimate the individualized probability of having NAME/S, deserving further exploration and validation.

Cell senescence, an irreversible cell cycle arrest, reflects a safeguard program that limits the capacity of uncontrolled cell proliferation. Treatment of tumor cells with certain chemotherapeutic agents activates premature senescence to decrease the tumorigenecity. Here we show that sublethal concentrations of adriamycin could induce premature senescence in lung cancer cells. Adriamycin treatment resulted in the up-regulation of BMP4, which is underexpressed in NSCLC (non-small cell lung cancers). Moreover, the BMP4-Smad pathway played a key role in mediating adriamycin-induced senescence. Overexpression of BMP4 was able to induce premature senescence in lung cancer cells and this process required the participation of cyclin/cyclin-dependent kinase (cdk) inhibitors p16(INK4a) and p21(WAF1/cip1). We also show that increases of p16(INK4a) and p21(WAF1/cip1) expression in response to BMP4 were mediated by the Smad signaling pathway. Furthermore, our data revealed that p300 was recruited to P16(INK4a) and P21(WAF1/cip1) promoters by Smad1/5/8 to induce the hyperacetylation of histones H3 and H4 at the promoters. The present study provides useful clues to the evaluation of the potentiality of BMP4 as a responsive molecular target for cancer chemotherapy.
