[Splenic resection for the spleen cyst].

Tactics and results of surgical treatment of 5 patients for splenic cyst were analyzed. The original method of splenic cyst resection, using blood-preserving technologies (ultrasonic aspirator-destructor, the argon laser and hemostatics) was applied. The data obtained permit to consider the splenic resection performance an adequate procedure for the focal organ pathology treatment.

[The use of Sandostatin in the combined treatment of surgical diseases of the pancreas]. / Primenenie sandostatina v kompleksnom lechenii khirurgicheskikh zabolevanii podzheludochnoi zhelezy.

The paper analyzes experience gained with the use of Sandostatin in patients with a surgical operation-induced pathology of pancreas. Sandostatin is known to be capable of suppressing the exocrine activity of pancreas, which fact made for decline in the recordable postoperative complications and improvement of results of surgical treatment. The maiden experience gained with its use showed high efficacy of Sandostatin in a multimodality treatment of patients presenting with surgical pathology of pancreas.

[Delayed occlusion of the pancreatic duct]. / Otsrochennaia okkliuziia protoka podzheludochnoi zhelezy.

Seventy six delayed occlusions of the main pancreatic duct were fulfilled during the period from 1978 to 1985. No severe complications or lethal outcomes due to performing a delayed occlusion of the duct were noted.

[Intervention on the portal vein in pancreatic resection]. / Vmeshatel'stvo na vorotnoi vene pri rezektsii podzheludochnoi zhelezy.

The authors consider that resection of the pancreas with the intervention portal vein must be fulfilled by a specially trained team of surgeons--masters technique of operations on the vessels. The operative method should be chosen dually and depends on the localization and size of the vessel injury.

[Partial resection of the head of the pancreas]. / Partsial'naia rezektsiia golovki podzheludochnoi zhelezy.

Data of performing partial resections of head of the pancreas in 29 patients are described. Depending on the state of the pancreas it is possible to perform external drainage of the major pancreatic duct of the distal stump followed by its delayed occlusion, to form pancreatoenteroanastomosis or recanalization of the duct using a small intestine segment. The partial resections are less traumatic operations and give less amount of postoperative complications.

[Partial resection of the pancreas in the treatment of chronic pancreatitis]. / Partsial'naia rezektsiia podzheludochnoi zhelezy v lechenii khronicheskogo pankreatita.

On the basis of an experience with 1215 operations for chronic pancreatitis the authors came to a conclusion concerning expediency of partial resection (up to 40% of the volume) of the pancreas with a local damage of the organ. Such operations were fulfilled in 33 patients. Good and satisfactory results were obtained in 28 of 33 patients. There were no lethal outcomes. In all the operated patients subjected to selective suppression of the exocrine function of the retained part of the gland no recurrencies of pancreatitis were noted.

[Direct thermometry of the pancreas in the postoperative period]. / Priamaia termometriia podzheludochnoi zhelezy v posleoperatsionnom periode.

Direct thermometry of the pancreas in 44 patients after operations for carcinoma of the pancreato-duodenal zone or chronic pancreatitis has shown a relationship between temperature of the pancreas stump and the fermentemia degree. Characteristic changes of temperature regimens are observed 5-17 hours earlier than manifestations of clinical symptoms of postoperative pancreatitis make their appearance.

[Occlusion of the pancreatic duct]. / Primenenie okkliuzii pankreaticheskogo protoka.

Results of the treatment of 81 patients by occlusion of the pancreatic duct are presented. Good nearest and long-term results are noted in patients with chronic pancreatitis. The results obtained show good prospects in using the method of occlusion.

[Treatment method in external pancreatic fistulae]. / Sposob lecheniia naruzhnykh pankreaticheskikh svishchei.

The authors' experience includes 62 patients with pancreatic fistulas. Complex conservative treatment was first used in all the patients and proved to be effective in 11 of them. In 42 patients with fistulas resistant to conservative therapy the surgical treatment was fulfilled. Postoperative lethality was 7,1%. During 1980 the authors were using their original operative method of closure of pancreatic fistulas with good and stable effect.
