The bacterium Escherichia coli is one of the main causes of urinary tract infections. The formation of bacterial biofilms, especially associated with the use of urinary catheters, contributes to the establishment of recurrent infections and the development of resistance to treatment. Strains of E. coli that produce extended-spectrum beta-lactamases (ESBL) have a greater ability to form biofilms. In addition, there is a lack of drugs available in the market with antibiofilm activity. Promethazine (PMZ) is an antihistamine known to have antimicrobial activity against different pathogens, including in the form of biofilms, but there are still few studies of its activity against ESBL E. coli biofilms. The aim of this study was to evaluate the antimicrobial activity of PMZ against ESBL E. coli biofilms, as well as to assess the application of this drug as a biofilm prevention agent in urinary catheters. To this end, the minimum inhibitory concentration and minimum bactericidal concentration of PMZ in ESBL E. coli strains were determined using the broth microdilution assay and tolerance level measurement. The activity of PMZ against the cell viability of the in vitro biofilm formation of ESBL E. coli was analyzed by the MTT colorimetric assay and its ability to prevent biofilm formation when impregnated in a urinary catheter was investigated by counting colony-forming units (CFU) and confirmed by scanning electron microscopy (SEM). PMZ showed bactericidal activity and significantly reduced (p < 0.05) the viability of the biofilm being formed by ESBL E. coli at concentrations of 256 and 512 µg/ml, as well as preventing the formation of biofilm on urinary catheters at concentrations starting at 512 µg/ml by reducing the number of CFUs, as also observed by SEM. Thus, PMZ is a promising candidate to prevent the formation of ESBL E. coli biofilms on abiotic surfaces.

The diagnosis of human leptospirosis is mainly based on serological assays. Since the extraction by N-butanol has only been studied as an antigen for the diagnosis of cattle leptospirosis, this study aimed to investigate the feasibility of the N-butanol preparation for the diagnosis of human leptospirosis and compare it with sonicated and thermo-resistant antigens in IgM dot-blot test. Paired serum samples from 147 laboratory-confirmed leptospirosis cases were tested. The control group consisted of 148 serum samples from healthy individuals and nonleptospirosis cases. N-butanol antigens from serovar Copenhageni (ButC3) and serovar Patoc (ButP3) showed reactivity with antileptospiral antibodies from patients with confirmed leptospirosis. In the acute phase, sensitivities of IgM dot-blot assay with ButC3 and ButP3 antigens were 47.6% and 51.0%, respectively. In the convalescent phase, sensitivities were 95.9% (ButC3) and 93.2% (ButP3), and no significant differences were observed among the IgM dot-blot tests with other antigens. The specificity of the IgM dot-blot test with ButC3 antigen was good (92.6%), but with ButP3 (83.1%), it was significantly lower than with the other tests. The IgM dot-blot test described in this study is simple to perform and presents reliable visual results. Antigens prepared by N-butanol proved to be valuable diagnostic markers of leptospirosis.

BACKGROUND: Acute pain during vaccine administration is an expected event; however, some strategies, such as the use of high frequency vibration devices associated with cryotherapy, may minimize it. AIM: Evaluate the effect of high frequency vibration associated with cryotherapy on the level of pain related to the administration of influenza vaccine by intramuscular route in adults. METHOD: A randomized clinical trial was conducted with 350 adults who received the influenza vaccine. Participants allocated to the intervention group used a high-frequency vibration device associated with cryotherapy during vaccination, and those in the control group administered the vaccine according to the service routine. RESULTS: Mean self-reported pain after vaccination of participants who used the device was lower (1.5 ±1.7) when compared to those who did not use it (1.9 ±1.9) (p = .041). There was an association of higher levels of pain after vaccination with participants in the control group (p = .011), females (p = .042), with higher level of pain expected with vaccination (p < .001) and higher level of anxiety before (p < .001) and after vaccination (p = .001). CONCLUSIONS: The use of high frequency vibration associated with cryotherapy has been shown to be a viable non-pharmacological intervention for the reduction of pain associated with influenza vaccination in adults.

PURPOSE: Poliovirus has been nearly eliminated as part of a world-wide effort to immunize and contain circulating wild-type polio. Nevertheless, poliovirus has been detected in water supplies and represents a threat to patients with humoral immunodeficiencies where infection can be fatal. To define the risk, we analyzed antibodies to poliovirus 1, 2, and 3 in serum samples collected over a year from patients with primary immunodeficiency diseases (PID) on regular intravenous immunoglobulin (IVIG) replacement. METHODS: Twenty-one patients on regular IVIG replacement therapy were evaluated: Twelve patients with common variable immune deficiency (CVID), six with X-linked agammaglobulinemia (XLA), and three with hyper IgM syndrome (HIGM). Over 1 year, four blood samples were collected from each of these patients immediately before immunoglobulin infusion. One sample of IVIG administered to each patient in the month before blood collection was also evaluated. Poliovirus antibodies were quantified by seroneutralization assay. RESULTS: All IVIG samples had detectable antibodies to the three poliovirus serotypes. Despite that, only 52.4, 61.9, and 19.0% of patients showed protective antibody titers for poliovirus 1, 2, and 3, respectively. Only two patients (9.5%) had protective antibodies for the three poliovirus serotypes on all samples. Most patients were therefore susceptible to all three poliovirus serotypes. CONCLUSIONS: This study demonstrates the need for ongoing vigilance regarding exposure of patients with PID to poliovirus in the community.

Introduction. Candida albicans and Staphylococcus aureus are recognized for their development of resistance and biofilm formation. New therapeutic alternatives are necessary in this context.Hypothesis. Etomidate shows potential application in catheters against mixed biofilms of fluconazole-resistant C. albicans and methicillin-resistant S. aureus (MRSA).Aim. The present study aimed to evaluate the activity of etomidate against mixed biofilms of fluconazole-resistant C. albicans and MRSA.Methodology. The action of etomidate against mature biofilms was verified through the evaluation of biomass and cell viability, and its ability to prevent biofilm formation in peripheral venous catheters was determined based on counts of colony forming units (c.f.u.) and confirmed by morphological analysis through scanning electron microscopy (SEM).Results. Etomidate generated a reduction (P<0.05) in biomass and cell viability starting from a concentration of 250 µg ml-1. In addition, it showed significant ability to prevent the formation of mixed biofilms in a peripheral venous catheter, as shown by a reduction in c.f.u. SEM revealed that treatment with etomidate caused substantial damage to the fungal cells.Conclusion. The results showed the potential of etomidate against polymicrobial biofilms of fluconazole-resistant C. albicans and MRSA.

OBJECTIVE: to validate a Standard Operating Procedure on the intramuscular vaccine administration technique in adults using high frequency vibration associated with cryotherapy. METHODS: a literature review on intramuscular vaccination practice using a vibration device associated with cryotherapy. Then, a form was created to validate the instrument, detailing the items that were assessed by judges following recommendations in the literature. Judges' answers were assessed using the Content Validity Index, with items whose index was greater than or equal to 0.80 being validated. RESULTS: twenty-five nurses participated in validity, identifying judges' opinion regarding item relevance, clarity and accuracy. Judges validated the instrument, according to the values that remained between 0.88 and 1.0. CONCLUSIONS: the instrument developed and validated is a tool capable of guaranteeing safety and standardizing immunization practice in vaccine rooms.

The infestation of Rhipicephalus microplus (Acari: Ixodidae) (Canestrini, 1887) represents a significant health problem for cattle. Up until today, no consistent control has been recommended for the free-living larval stage of R. microplus. The objective of the present study was to determine the larvicidal effect of an Allium sativum (garlic) aqueous extract against R. microplus using the larval package test, LPT. Tick larvae were obtained from adult female ticks from naturally infected animals, and 200 larvae were exposed to each A. sativum extract concentration (5, 7, 10, 16, 25, and 50%). All garlic groups received the extract in three different volumes (2, 5, and 7 ml) to determine the levels of humidity that would affect the free-living stage. Larvae from the control group were treated with distilled water and the positive control used cypermethrin in a 5-ml volume. Live and dead larvae were determined after 24 h of exposure. There was a significant statistical difference (P = 0.041) at 5% between 2 and 5 ml and between 7 and 10% of the extract using 2 ml (P = 0.034). There was a significant larvicidal activity of 100% mortality for the 16, 25, and 50% concentrations for all volumes. We observed a clear concentration-dependent effect for the extracts with a correlation coefficient of R2 > 0.92. The lethal concentration of 50% (LC50) was 13.6, 9.8, and 7.6% for the 2, 5, and 7 ml. The present A. sativum solutions may be considered a viable biofriendly alternative to reduce the number of the free-living larval stage of R. microplus on pasture and the consequent reduction of cattle infestation due to its high larval mortality.

BACKGROUND: Vaccination is one of the most effective strategies for prevention and eradication of immunopreventable diseases, but community acceptance of vaccination can be influenced by different factors, such as pain and anxiety. The use of high-frequency vibration associated with cryotherapy has been used to manage pain and anxiety during the vaccination process in children, but studies with adults are still scarce. This study aims to evaluate the effect of high-frequency vibration associated with cryotherapy on the levels of self-reported pain and anxiety related to administration of the Influenza vaccine intramuscularly in adults. METHODS: A two-arm, parallel, randomized clinical trial conducted in a Brazilian Primary Health Care Unit is proposed. A sample of 350 adults will be randomly assigned to participate in the control group, receiving the vaccine intramuscularly according to the standard protocol of the service, or in the intervention group, receiving the vaccine by the same route and using a portable device of high frequency vibration associated with cryotherapy for 30 s before and during administration. The primary endpoints will be self-reported levels of pain, assessed before and after vaccine administration. Secondary endpoints will be levels of anxiety, satisfaction with vaccine administration, and discomfort caused by high frequency vibration and temperature of the frozen bag in contact with the skin. Self-reported levels of pain and anxiety will be compared before and after vaccination as well as between the control and intervention groups. DISCUSSION: By evaluating the effect of high-frequency vibration associated with cryotherapy on pain and anxiety levels, we expect to find evidence that will support nursing practice, in order to promote greater comfort and safety in the vaccination process and, consequently, greater compliance by the population, by minimizing its undesirable effects. The Northeast of Brazil has experienced a triple epidemic, with the simultaneous circulation of dengue virus (DENV), chikungunya virus (CHIKV) and Zika virus (ZIKV), which may have contributed to the observed increase across this region of atypical forms of disease and deaths. In view of this fact, non-congenital neurological disorders related to arboviruses were compared with other etiologies, mortality and survival rates of patients admitted to referral neurology hospitals in Pernambuco State, Northeast Brazil, from 2015 to 2018. Blood and cerebrospinal fluid samples were collected and tested using molecular and serological assays. The arbovirus-exposed groups were compared with respect to epidemiological, clinical and neurologic characteristics by using the Pearson's chi-square test. For the survival analysis, the Kaplan-Meier and Hazard Ratio (HR) tests were used, with a 95% confidence interval (CI). Encephalitis and encephalomyelitis were more frequent in arboviruses, while myelitis predominated in the neurological disorders of other etiologies. Guillain-Barré Syndrome (GBS) was similarly distributed amongst the groups. Exposure to one of the arboviruses caused a six-fold increase in the risk of death (HR: 6.37; CI: 2.91 - 13.9). Amongst the arbovirus-exposed groups, infection (DENV/CHIKV) increased nine times the risk of death (HR: 9.07; CI: 3.67 - 22.4). The survival curve indicates that have been exposed to some arbovirus decreased the likelihood of survival compared to those with other etiologies (Log-Rank: p<0.001). Within this scenario, neurologic manifestations of DENV, CHIKV and ZIKV have the potential to increase mortality and decrease survival, and concomitant infection (DENV/CHIKV) is an aggravating factor in reducing the likelihood of survival when compared to monoinfections.

Chikungunya, a viral infection that presents with fever, rash and polyarthritis, is usually an acute febrile illness. Uncommon neurological manifestations include meningoencephalitis, encephalitis, myelitis, Guillain-Barre syndrome, myelopathy and neuropathy. During an outbreak of the disease in La Reunion Island, abnormalities were observed in the magnetic resonance imaging (MRI) of patients with encephalitis and acute disseminated encephalomyelitis, showing bilateral, frontoparietal, white matter lesions with restricted diffusion, similar to our case. We report a 57-year-old male patient with comorbidities, admitted with high fever, arthralgia, asthenia, vomiting, psychomotor agitation, behavioral changes and seizures. Cerebrospinal fluid (CSF) values revealed pleocytosis (98 cells/mm3 with 68% lymphocytes and 12% monocytes) and high levels of protein (161 mg%). Brain MRI showed hyperintense lesions in the temporal and frontal lobes and bilaterally in the posterior thalamus. CSF serology was positive for IgM antibodies to Chikungunya virus. Encephalitis due to an acute viral infection by Chikungunya was diagnosed. The patient's clinical condition worsened and he died on the twenty-fourth day of admission to our hospital.

PURPOSE: To cross-culturally adapt and verify the measurement properties of the Brazilian version of the Motor Assessment Scale (MAS). METHODS: The process of cross-cultural adaptation followed standardized procedures. Construct validity of the MAS was investigated using Rasch analysis (n = 100), whereas inter-rater and test-retest reliabilities were evaluated using Kappa coefficients and Bland and Altman plots (n = 52). RESULTS: The MAS demonstrated satisfactory measurement properties. The Kappa coefficients for the inter-rater and test-retest reliabilities were 0.73 (ranging from 0.79 to 1.00) and 0.82 (ranging from 0.86 to 1.00), respectively. The Bland and Altman plots showed adequate inter-rater and test-retest agreements. No ceiling or floor effects were observed and only one item exhibited misfit to the Rasch model expectations. Item 4 "sitting to standing" exhibited marginal misfit (infit MnSq = 1.44; Zstd = 2.6), but it did not affect the unidimensionality of the scale. CONCLUSIONS: The MAS demonstrated good indicators of validity and reliability to be used for the assessment of motor function of individuals with stroke within clinical and research contexts. Implications for rehabilitation The Motor Assessment Scale is a performance-based scale for the assessment of motor function of individuals with stroke, based on a task-oriented approach. The Motor Assessment Scale has shown good clinical utility indicators dues to its quick administration, objectivity, and clinical relevance as a functional predictor. The Motor Assessment Scale showed good indicators of validity and reliability to be used within clinical and research contexts for the evaluation of motor function of individuals after stroke.

Resumo Objetivo Analisar o modelo de gestão adotado nos Prontos Atendimentos dos municípios que integram um Departamento Regional de Saúde. Métodos Estudo de métodos mistos, com desenho incorporado concomitante, realizado em 2017, com seis gestores e 223 trabalhadores de saúde (médicos, enfermeiros e técnicos/auxiliares de enfermagem) de dez Prontos Atendimentos de oito municípios do Departamento Regional de Saúde XIII, São Paulo. Foi entregue o instrumento de coleta de dados composto por seis questões fechadas e quatro questões abertas. Os dados quantitativos foram analisados por meio de estatística descritiva; os dados qualitativos, desenvolvidos por meio da técnica do Discurso do Sujeito Coletivo, foram analisados com o apoio do software DSCSoft®. Resultados Predominaram profissionais do sexo feminino 173 (75,2%) e que ocupavam o cargo de auxiliar de enfermagem 78 (33,9%). Enquanto os gestores avaliaram o modelo de gestão da instituição como contemporâneo e participativo, possibilitando que os trabalhadores participassem da tomada de decisão e dando autonomia aos mesmos para decidirem sobre questões do dia a dia, os trabalhadores avaliaram como gestão tradicional, destacando-se por sua estrutura formal. Conclusão Foi possível observar que gestores e trabalhadores têm visão destoante sobre o modelo de gestão das instituições de saúde. Os pressupostos do modelo de gestão compartilhada englobam concepções valiosas, como comunicação dialógica, descentralização do poder e maior autonomia para a tomada de decisões. Contudo, ao analisar a opinião da maior parte dos trabalhadores que participaram deste estudo, observa-se que o modelo hierarquizado ainda é muito utilizado nas instituições.

Métodos Estudio de métodos mixtos, con diseño incorporado simultáneo, realizado en 2017, con seis gerentes y 223 trabajadores de la salud (médicos, enfermeros y técnicos/auxiliares de enfermería) de diez servicios de emergencias de ocho municipios del Departamento Regional de Salud XIII, São Paulo. Se entregó el instrumento de recopilación de datos, compuesto por seis preguntas cerradas y cuatro preguntas abiertas. Los datos cuantitativos se analizaron mediante estadística descriptiva. Los datos cualitativos, elaborados mediante la técnica del discurso del sujeto colectivo, se analizaron con ayuda del software DSCSoft®. Resultados Predominaron profesionales de sexo femenino 173 (75,2 %), que ocupaban el cargo de auxiliar de enfermería 78 (33,9 %). Mientras los gerentes evaluaron el modelo de gestión de la institución como contemporáneo y participativo, que permite a los trabajadores participar de la toma de decisiones y les da autonomía para decidir sobre cuestiones del día a día, los trabajadores la evaluaron como una gestión tradicional, con énfasis en su estructura formal. Conclusión Se pudo observar que los gerentes y los trabajadores tienen visiones discordantes sobre el modelo de gestión de las instituciones de salud. Las premisas del modelo de gestión compartida engloban concepciones valiosas, como comunicación dialógica, descentralización del poder y mayor autonomía para la toma de decisiones. Sin embargo, al analizar la opinión de la mayoría de los trabajadores que participaron en este estudio, se observa que el modelo jerárquico aún es muy utilizado en las instituciones.


Abstract Objective To analyze the management model adopted in Emergency Services in municipalities that are part of a Regional Health Department. Methods Mixed methods study with a concomitantly incorporated design. It was conducted in 2017 with six managers and 223 health workers (physicians, nurses and nursing technicians/assistants) from ten Emergency Care Units in eight municipalities of the Regional Health Department XIII, São Paulo. The data collection instrument composed of six closed questions and four open questions was delivered to participants. Quantitative data were analyzed using descriptive statistics; qualitative data were developed using the Collective Subject Discourse technique and analyzed using the DSCSoft® software. Results There was a predominance of female professionals, 173 (75.2%), in the position of nursing assistants, 78 (33.9%). Managers evaluated the institution's management model as contemporary and participatory, enabling workers to participate in decision making and giving them autonomy to decide on day-to-day issues, while workers evaluated it as traditional management, highlighting its formal structure. Conclusion It was observed that managers and workers have a different view of the management model of health institutions. The assumptions of the shared management model encompass valuable concepts, such as dialogic communication, decentralization of power and greater autonomy for decision making. However, when analyzing the opinion of most workers who participated in this study, the wide use of a hierarchical model in institutions is still observed.

RESUMO

ABSTRACT Objective: to validate a Standard Operating Procedure on the intramuscular vaccine administration technique in adults using high frequency vibration associated with cryotherapy. Methods: a literature review on intramuscular vaccination practice using a vibration device associated with cryotherapy. Then, a form was created to validate the instrument, detailing the items that were assessed by judges following recommendations in the literature. Judges' answers were assessed using the Content Validity Index, with items whose index was greater than or equal to 0.80 being validated. Results: twenty-five nurses participated in validity, identifying judges' opinion regarding item relevance, clarity and accuracy. Judges validated the instrument, according to the values that remained between 0.88 and 1.0. Conclusions: the instrument developed and validated is a tool capable of guaranteeing safety and standardizing immunization practice in vaccine rooms.


RESUMEN Objetivo: validar un Procedimiento Operativo Estándar sobre la técnica de administración de vacunas intramusculares en adultos mediante vibración de alta frecuencia asociada a crioterapia. Métodos: revisión de la literatura sobre la práctica de la vacunación intramuscular mediante un dispositivo de vibración asociado a la crioterapia. Luego, se elaboró un formulario para validar el instrumento, detallando los ítems que fueron evaluados por los jueces siguiendo las recomendaciones de la literatura. Las respuestas de los jueces fueron evaluadas mediante el Índice de Validez de Contenido, siendo validados los ítems cuyo índice fuera mayor o igual a 0,80. Resultados: 25 enfermeros participaron de la validación, captando la opinión de los jueces sobre la pertinencia, claridad y precisión de los ítems. Los jueces validaron el instrumento según los valores, que se mantuvieron entre 0,88 y 1,0. Conclusiones: el instrumento desarrollado y validado es una herramienta capaz de garantizar la seguridad y estandarizar la práctica de inmunización en salas de vacunas.

Métodos: revisão de literatura sobre a prática de vacinação intramuscular utilizando dispositivo de vibração associado à crioterapia. Em seguida, elaborou-se um formulário para validação do instrumento, com detalhamento dos itens que foram avaliados por juízes seguindo recomendações da literatura. As respostas dos juízes foram avaliadas pelo Índice de Validade de Conteúdo, sendo validados os itens cujo índice foi maior ou igual a 0,80. Resultados: 25 enfermeiros participaram da validação, captando o parecer dos juízes quanto à relevância, clareza e precisão dos itens. Os juízes validaram o instrumento segundo os valores, que permaneceram entre 0,88 e 1,0. Conclusões: o instrumento desenvolvido e validado é uma ferramenta capaz de garantir a segurança e padronizar a prática de imunização nas salas de vacina.

OBJECTIVE:: The goal of this study is to analyze nurses' leadership in intensive care units at hospitals in the state of São Paulo, Brazil, in the face of positive and negative critical incidents. METHOD:: Exploratory, descriptive study, conducted with 24 nurses by using the Critical Incident Technique as a methodological benchmark. RESULTS:: Results were grouped into 61 critical incidents distributed into categories. Researchers came to the conclusion that leadership-related situations interfere with IC nurses' behaviors. Among these situations they found: difficulty in the communication process; conflicts in the daily exercise of nurses' activities; people management; and the setting of high quality care targets. FINAL CONSIDERATIONS:: Researchers identified a mixed leadership model, leading them to the conclusion that nurses' knowledge and practice of contemporary leadership theories/styles are crucial because they facilitate the communication process, focusing on behavioral aspects and beliefs, in addition to valuing flexibility. This positively impacts the organization's results. OBJETIVO:: Analisar a liderança do enfermeiro em Centros de Terapia Intensiva de hospitais localizados no interior do estado de São Paulo, diante de incidentes críticos positivos e negativos. MÉTODO:: Estudo exploratório, descritivo, realizado com 24 enfermeiros, que utilizou a Técnica do Incidente Crítico como referencial metodológico. RESULTADOS:: Os resultados foram agrupados em 61 incidentes críticos distribuídos em categorias. Identificou-se que situações relacionadas à liderança interferem no comportamento do enfermeiro de Terapia Intensiva, dentre elas: dificuldade no processo de comunicação, conflitos existentes no dia a dia do exercício profissional, gerenciamento de pessoas e estabelecimento de metas para o alcance da assistência qualificada. CONSIDERAÇÕES FINAIS:: Encontrou-se um modelo misto de liderança, o que permite concluir que o conhecimento e a prática dos enfermeiros acerca de teorias/estilos contemporâneos de liderança tornam-se fundamentais, pois facilitam o processo de comunicação, focando nos aspectos comportamentais e crenças, e valorizam a flexibilidade, impactando positivamente os resultados da organização.

Visceral leishmaniasis, associated with HIV/AIDS coinfection, is becoming a more aggressive disease, complicating an accurate prognosis. A 21-year-old HIV-positive female presenting with clinical features of visceral leishmaniasis was enrolled in this study. Bone marrow cytology, Novy-MacNeal-Nicolle culture and kDNA PCR of peripheral blood were all positive. Typing methods, multilocus enzyme electrophoresis and ITS1-RFLP PCR of peripheral blood confirmed infection by Leishmania (L.) infantum chagasi . PCR has proved to be safer and more affordable than other characterization methods; ITS1-RFLP PCR can diagnose and type Leishmania spp. in both endemic and non-endemic areas, favoring the prognosis and allowing the appropriate treatment of patients.

ABSTRACT Poliomyelitis is still an endemic disease in Afghanistan, Nigeria, and Pakistan despite the efforts to eradicate the disease. Therefore, there is a potential risk of international spread. Since the start of the polio eradication program by the Global Polio Eradication Initiative in 1988, the incidence of polio has been reduced by 99%. In the last decade, wild poliovirus type 2 (WPV2) was eliminated and declared eradicated in 2015. Wild poliovirus type 3 (WPV3) was last reported in November 2012. These changes have allowed the removal of Sabin poliovirus type 2 from the oral poliovirus vaccine (OPV) in April 2016 and countries either introduced bivalent OPV (bOPV) containing Sabin types 1 + 3 poliovirus or added at least one dose of inactivated poliovirus vaccine (IPV) into their routine immunization schedule. Many efforts are needed to eradicate polio, and new strategies should be implemented such as the development and approval of new genetically stable OPV, and vaccines that do not require infectious processes for virus growth, such as virus-like particles (VLPs), or packing-cell technology. IPV will increasingly be produced from Sabin strains, and further attenuated or genetically modified strains. Furthermore, there is also a need for the development of antiviral drugs to treat immunodeficient patients who are long-term excretors infected with poliovirus, thus avoiding contamination of individuals susceptible to polioviruses, due to reversal of pathogenicity. If all these measures are successfully implemented, the world will be close to the global


RESUMEN La poliomielitis sigue siendo una enfermedad endémica en Afganistán, Nigeria y Pakistán a pesar de los esfuerzos por erradicar la enfermedad. Por lo tanto, existe un riesgo de propagación mundial. Desde el inicio del programa de erradicación de la poliomielitis por la Iniciativa de Erradicación Mundial de la Poliomielitis [Global Polio Eradication Initiative (GPEI)] en 1988, la incidencia de la poliomielitis se ha reducido en un 99%. En la última década, el poliovirus salvaje tipo 2 (WPV2) fue eliminado y declarado erradicado en 2015. El poliovirus salvaje tipo 3 (WPV3) se informó por última vez en noviembre de 2012. Estos cambios han permitido la eliminación del poliovirus Sabin tipo 2 de la vacuna antipoliomielítica oral (VPO) en abril de 2016, y los países introdujeron la VPO de tipo bivalente (bVPO), que contiene poliovirus Sabin tipos 1 y 3, o agregaron al menos una dosis de vacuna antipoliomielítica inactivada (VPI) al programa de inmunización de rutina. Se necesitan muchos esfuerzos para erradicar la poliomielitis y se deben implementar nuevas estrategias, como el desarrollo y aprobación de nuevas VPO genéticamente estables y vacunas que no requieren procesos infecciosos para el crecimiento del virus, como partículas pseudovirales (VLP) o tecnología de células empaquetadas (packing-cell). La VIP se producirá cada vez más a partir de cepas Sabin y otras cepas más atenuadas o modificadas genéticamente. Además, también es necesario desarrollar fármacos antivirales para tratar a pacientes inmunodeficientes que son excretores a largo plazo, evitando así la contaminación de individuos susceptibles a poliovirus, debido a la reversión de la patogenicidad. Si todas estas medidas se implementan con éxito, el mundo estará cerca de la interrupción global de la transmisión del WPV y la erradicación de la poliomielitis.


RESUMO A poliomielite ainda é uma doença endêmica no Afeganistão, na Nigéria e no Paquistão, apesar dos esforços para erradicá-la. Portanto, há risco de propagação mundial. Desde o início do programa de erradicação da poliomielite pela Iniciativa de Erradicação Global da Pólio [Global Polio Eradication Initiative (GPEI)], em 1988, a incidência da doença foi reduzida em 99%. Na última década, o poliovírus selvagem do tipo 2 (WPV2) foi eliminado e declarado erradicado em 2015. O poliovírus selvagem do tipo 3 (WPV3) foi reportado pela última vez em novembro de 2012. Essas mudanças promoveram a remoção do poliovírus Sabin tipo 2 da vacina oral antipólio (VOP) em abril de 2016, e os países introduziram a vacina oral bivalente (VOPb), que contém os poliovírus Sabin tipos 1 + 3, ou adicionaram pelo menos uma dose da vacina inativada contra o poliovírus (VIP) no calendário de imunização. É necessário muito empenho para erradicar a poliomielite. Novas estratégias devem ser implementadas, como o desenvolvimento e a aprovação de novas VOPs geneticamente estáveis e vacinas que não requerem processos infecciosos para o crescimento do vírus, como partículas pseudovirais (VLP), ou tecnologia de células de empacotamento (packing-cell). A VIP será cada vez mais produzida a partir de cepas Sabin, de outras cepas atenuadas ou geneticamente modificadas. Além disso, é imprescindível o desenvolvimento de medicamentos antivirais para tratar os pacientes imunodeficientes que são excretores de longo prazo, evitando assim a contaminação de indivíduos suscetíveis aos poliovírus, devido à reversão da patogenicidade. Se todas essas medidas forem implementadas com sucesso, o mundo estará próximo da interrupção global de transmissão do WPV e da erradicação da poliomielite.

BACKGROUND: While there are several instruments in Brazil that measure motor function in patients after stroke, it is unknown whether the measurement properties of these instruments are appropriate. OBJECTIVE: To identify the motor function instruments available in Brazil for patients after stroke. To assess the methodological quality of the studies and the results related to the measurement properties of these instruments. METHOD: Two independent reviewers conducted searches on PubMed, LILACS, CINAHL, Web of Science, and Scopus. Studies that aimed to cross-culturally adapt an existing instrument or create a Brazilian instrument and test at least one measurement property related to motor function in patients after stroke were included. The methodological quality of these studies was checked by the COSMIN checklist with 4-point rating scale and the results of the measurement properties were analyzed by the criteria developed by Terwee et al. RESULTS: A total of 11 instruments were considered eligible, none of which were created in Brazil. The process of cross-cultural adaptation was inadequate in 10 out of 11 instruments due to the lack of back-translation or due to inappropriate target population. All of the instruments presented flaws in the measurement properties, especially reliability, internal consistency, and construct validity. CONCLUSION: The flaws observed in both cross-cultural adaptation process and testing measurement properties make the results inconclusive on the validity of the available instruments. Adequate procedures of cross-cultural adaptation and measurement properties of these instruments are strongly needed.

Introdução: O desperdício é um grande problema enfrentado pelo Brasil, em função dos hábitos alimentares, sendo o aproveitamento de cascas de frutas, folhas e talos de hortaliças ainda baixo. Sabe-se, no entanto, que a utilização dessas partes tradicionalmente não aproveitadas pode contribuir para a produção de alimentos mais saudáveis, nutritivos e com menor impacto negativo para o meio ambiente. Objetivo: Aproveitar os vegetais na sua forma integral ou de suas partes alimentícias não convencionais, e caracterizar as farinhas obtidas, com vistas à alimentação humana. Métodos: As farinhas foram obtidas de banana verdes (casca e polpa), casca de tangerina, casca e polpa de berinjela e bagaço de uva oriundo da produção de suco de uva, que foram secas em estufa ventilada a 40°C por 48 horas. Foram realizadas análises físico-químicas e microbiológicas. Resultados: As farinhas apresentaram alto teor de fibras (17,7 a 48,94 g/100g) e compostos fenólicos (778,81 a 2708,11 mg EAG/100g), e baixo teor de lipídeos (0,70 a 3,40 g/100g). Para a quantidade de proteínas, a farinha de berinjela foi a que mais se destacou (13,81 g/100g), podendo ser uma alternativa aos indivíduos que não consomem produtos de origem animal. Conclusões: Todas as farinhas são seguras do ponto de vista microbiológico, de acordo com a legislação vigente, e portanto, podem ser utilizadas na alimentação humana para aumentar o aporte de nutrientes e diminuir os impactos da poluição, contribuindo para a manutenção do meio ambiente e gerando renda. Introduction: Waste is a great problem faced by Brazil, due to dietary habits, with a low use of fruit peels, leaves and stems of vegetables. However, it is known that the use of these parts traditionally not used can contribute to the production of healthier and more nutritious food with less negative impact on the environment. Objective: Take advantage of vegetables in their entirety or from their non-conventional food parts, and to characterize the flours obtained, for human consumption. Methods: Flours were obtained from green bananas (peel and pulp), tangerine peel, eggplant peel and pulp and grape marc from grape juice production, which were dried in a ventilated oven at 40 ° C for 48 hours. Physical-chemical and microbiological analyzes were realized. Results: Flours showed a high fiber (17.7 to 48.94 g/100g) and phenolic compounds (778.81 to 2708.11 mg EAG/100g) content, and low lipid content (0.70 to 3.40 g/100g). For the amount of proteins, eggplant flour was the one that stood out the most (13.81g/100g), it can be an alternative to individuals who do not consume animal products. Conclusions: All flours are safe in a microbiological point of view, according to current legislation, and therefore they can be used in food to increase the supply of nutrients and reduce the impacts of pollution, contributing to the environment maintenance and income generating. Atualmente, indivíduos buscam uma alimentação mais saudável, destacando-se, os alimentos orgânicos. O objetivo desse trabalho foi avaliar o conhecimento de produtos orgânicos dos moradores do Estado do Rio de Janeiro. Foi utilizado um questionário estruturado com 14 perguntas; on-line. Foram entrevistadas 147 pessoas, sendo 108 do sexo feminino e 39 do sexo masculino. Sobre o conhecimento de orgânicos, 133 pessoas responderam conhecer e consumir. Em relação a vantagem de consumir produtos orgânicos, 109 das respostas afirmaram consumir por não conter substâncias químicas nocivas. O estudo observou que o uso excessivo de pesticidas na produção de alimentos é uma das grandes preocupações, e o custo dos produtos o principal entrave para a sua compra.

Plantas alimentícias não convencionais (PANC) são aquelas que possuem uma ou mais partes com importância alimentar. São fontes de diversos nutrientes mesmo que atualmente em desuso pela maior parte da população. O Brasil é um dos países com maior biodiversidade de fauna e flora no mundo, entretanto a agricultura dominante é dependente de grande extensão de terras e práticas nocivas à saúde. Com esse perfil agrícola observamos o desaparecimento de algumas espécies de plantas na dieta da população. Com o objetivo de avaliar o conhecimento sobre as plantas alimentícias não convencionais na população, foi realizado questionário de perguntas fechadas por meio de redes sociais. Os dados indicaram que a população era na maioria do sexo feminino e apesar do grau de instrução, pouco conhecimento em relação às PANC, foi observado. Os indivíduos que possuíam algum conhecimento adquiriram de mães e avós. Quanto à identificação através de imagens, destacou-se a hortelã pimenta e a taioba, devido em especial a utilização em preparações. Observou-se pouco conhecimento e consumo de PANC pelos entrevistados, apesar das plantas nativas e exóticas naturalizadas serem importantes como suplemento da dieta alimentar, fonte de renda complementar, além de contribuir para a conservação da biodiversidade. Portanto, estudos sobre essas plantas precisam ser realizados.
