RESUMO

The effect of Bacillus subtilis on the immune responses and morphometry of the immune organs was evaluated in broilers challenged with S. gallinarum. For this purpose, Salmonella-free birds (n = 240) were split into four groups with six replicates of ten birds each. Groups included an NC (negative control, non-infected + non-medicated), a PC-S (positive control, Salmonella-infected + non-medicated), an AT-S (Salmonella-infected + medicated with enrofloxacin), and a BS-S (Salmonella-infected + B. subtilis (2.0 × 1010 cfu/g; 0.1 g/kg) group. On day 21, the thickness of the thymus cortex and medulla, germinal centre area of the spleen, bursal follicular length and bursal follicular area increased (P < 0.05) in the BS-S when compared to the NC and PC-S groups. On day 35, the BS-S group exhibited a higher (P < 0.05) antibody titre against the Newcastle disease virus (NDV), and cortex of the thymus was thicker (P < 0.05) compared to the other groups. A decrease in the thymus medulla thickness, germinal area of the spleen and bursal follicular number were noted in the PC-S group when compared to the other treatment groups. In conclusion, the prophylactic use of B. subtilis type probiotics alleviated the stress resulting from a Salmonella gallinarum infection and improved the immune organs development and function in infected broilers.

RESUMO

Severe acute respiratory syndrome (SARS CoV-2/COVID-19) is a highly contagious and deadly disease caused by a virus belonging to the coronaviridae family. Researchers working in histopathology laboratories, dealing with morbid samples, are particularly vulnerable to infection unless they have very strong immunity. Hence, a proper precautionary protocol is required for the safety of the laboratory staff. The current review highlights the biological and physical agents that can be used to inactivate the virus and disinfect the surrounding environment in the laboratory.

RESUMO

Postpartum infective endocarditis is a rare disease, especially in people with no risk factors (ie, intravenous drug use), that can be followed by severe morbidity and mortality. Here, we report a case of postpartum infective endocarditis with an unusual acute coronary syndrome-like presentation in a patient with minimal risk factors. In addition to lesions on the aortic valve causing severe aortic insufficiency, the patient's case was also complicated by left main coronary artery embolization, which was subsequently aspirated during surgery. Repeat angiography demonstrated complete removal, with no evidence of downstream embolization.


L'endocardite infectieuse du post-partum est une maladie rare, en particulier en l'absence de facteur de risque (comme l'utilisation de drogues intraveineuses), qui peut être une cause de morbidité grave ou de mortalité. Nous décrivons ici un cas d'endocardite infectieuse du post-partum caractérisé par des symptômes inhabituels s'apparentant à un syndrome coronarien aigu, chez une patiente présentant peu de facteurs de risque. En plus de lésions de la valve aortique entraînant une insuffisance aortique grave, le cas de cette patiente a été compliqué par une embolie de l'artère coronaire gauche principale, traitée ultérieurement par aspiration au cours du traitement chirurgical. Une angiographie post-intervention a révélé la disparition complète des lésions sans signe d'embolie en aval.

BACKGROUND: The objective of our study was to analyze the outcome of surgical shunts for the management of variceal bleeding associated with portal hypertension. METHODS: This was a retrospective analysis carried out at The Aga Khan University Hospital, Karachi of medical records from Jan 1991--Dec 2001. The main outcome measures included morbidity and mortality associated with the surgical procedure, and the long term outcome in terms of recurrent bleeding. RESULTS: A total of 30 patients underwent a shunt procedure during the study period. The mean age was 35+/-13.75 years, with 22 (73%) males and 8 (27%) females. The indication for surgery was recurrent bleeding in 23 (77%) patients, and active bleeding refractory to endoscopic therapy in 7 (23%) patients. According to Child-Pugh classification, 19 (63%) patients were classified as Childs' A, 7 (23%) as Childs' B, and 4 (13%) as Childs' C. The surgical procedure included distal splenorenal shunt in 25 (83%), central splenorenal shunt in 3 (10%), and portocaval shunt in 2 (7%) cases. Five patients expired within 30 days of surgical intervention with mortality rate of 16%. Three of these patients were Childs' C, as compared to one each in Childs' A and B, the difference being statistically significant. Similarly, the frequency of encephalopathy and recurrent bleeding was also significantly higher in patients with Childs' class C. CONCLUSIONS: Surgical shunts may be considered as a reasonable alternative for long term control of recurrent variceal bleeding in patients with good hepatic reserve.

RESUMO

Concomitant subclavian artery pseudoaneurysm and arteriovenous (AV) fistula has been rarely reported. We report a case of late concomitant pseudoaneurysm and AV fistula of the subclavian artery caused by a gunshot wound. Open surgical approach with an infraclavicular approach was used; subclavian artery was repaired with a Gortex interposition graft and subclavian vein was ligated. The postoperative course was uneventful. In developing countries, where the endovascular expertise is unavailable or if the lesion is large, open surgical approach provides similar results as an endovascular approach.

Life expectancy has increased during recent decades leading to a growing number of older population. The objective of this study was to evaluate the outcomes of coronary artery bypass grafting (CABG) in octogenarians and to compare the outcomes of the emergent CABG with elective surgery. Prospectively collected data from 31 consecutive octogenarian patients who underwent CABG between 1 January 2006 and 31 December 2008 were analyzed. Main outcomes of interest included mortality, length of ICU stay, length of hospital stay, priority of surgery, postoperative complications and functional status on follow-up. Fifteen patients were operated on an urgent basis. Patients operated on an urgent basis were in NYHA class III or IV preoperatively (P=0.0016). There were no significant differences in operative and postoperative variables. There were three in-hospital deaths and 23 patients (82%) were alive on follow-up and 19 were in functional class I or II. Quality of life assessment was performed using Seattle Angina Questionnaire and patients reported remarkable improvement in quality of life. Overall, 90% patients were not or slightly disabled in their daily activity. Satisfaction with their current quality of life was reported by 95% of patients. CABG may be performed in octogenarians with remarkable outcomes and improvement in quality of life.

Pharyngeal perforations are uncommon in children and are usually secondary to instrumentation or external penetrating injuries. A delay in management can lead to life-threatening complications such as retropharyngeal abscess, mediastinitis, and airway compromise. We report three children who had pharyngeal perforation due to apparently innocuous injury and who developed serious complications. A high index of suspicion for a significant pharyngeal injury, use of lateral soft-tissue x-ray films of the neck, and flexible endoscopy in the emergency room can assist in early diagnosis when evaluating patients with oropharyngeal and penetrating neck injury. Prompt administration of broad-spectrum antibiotics, local drainage, and debridement can avert life-threatening complications.
