[Possible decrease of nonspecific immunity in wounds contaminated by copper ore minerals]. / Mozliwosc obnizenia odpornosci nieswoistej ustroju przy zanieczyszczeniu ran mineralami rud miedzi.

Continuing the research--started in 1973--on the influence of wounds contamination by copper ore minerals upon the living organism the authors conducted experiments on 30 animals. The experiments covered: phagocytic and metabolic activity of peripheral blood phagocytes, oxidoreductase activity of granulocytes, lysosyme level in sera of the examined animals and total hemolytic activity of the complement. The experiments demonstrated--since the 20th day after their start--a considerable decrease in phagocytic and metabolic activity of peripheral blood granulocytes in the animals with wounds contaminated by copper ore minerals. Those disturbances may decrease nonspecific cellular immunity of the organism, which entails organism's susceptibility to infections.

[Algodystrophy--Sudeck syndrome]. / Algodystrofia--zespól Sudeck'a.

Modern views are presented on the development of algodystrophy illustrated with own material with a detailed description of characteristic cases. In the whole material of 175 patients, four age groups, and two groups according to trauma site (hand and foot) were isolated. All patients were subjected to clinical, X-ray, and some to radioisotope examinations. Greatest number of cases of algodystrophy was found in the age group over 60 years which confirmed the observations of other authors. On the basis of the presented material the authors think that of greatest importance are early diagnosis, treatment with of greatest importance are early diagnosis, treatment with analgesics, blood flow improving drugs, and salmon calcitonin, and rehabilitation treatment. It seems also that treatment with magnetic field may be beneficial--however, too small material of patients makes general conclusions impossible.

[Clinical evaluation of hydrogel-type dressing materials after their 8-year use]. / Kliniczna ocena hydrozelowych materialów opatrunkowych po 8-letnim ich zastosowaniu.

The subject to assessment were hydrogel dressings (in 42 patients) compared with the classical gauze dressing with an addition of various pharmaceuticals (in 65 patients). We found out that the gauze dressings get dry 24 hours after they have been applied, their replacement being very painful. The hydrogel dressings, on the other hand, do not cause any pain when replaced after the 24 hours. The hygroscopic properties of hydrogel dressings allow a quick cleaning of the wound from microorganisms. Permitting an easy absorption of antibiotics and other drugs, they contribute to an acceleration of the healing process and epidermis development.

On the basis of the many years usage of Gentamycin-Septopal in treatment of blood-derived and traumatic inflammation of bones we can say that in both forms of inflammation fully satisfying results were achieved. In chronic traumatic inflammations of bones with active stomias where the inflammatory process lasted many weeks, and from the purulent matter two or more tribes with various sensitiveness to antibiotics, associated treatment was also used with application of large doses cephalosporin antibiotics of Glaxo-Zinacef of Fortum firms. It should be stressed that in treatment of a patient with that disease correct radioisotopic diagnostic of the focus of inflammation and the evaluation of the immunity state of the organism of the patient, especially during long-lasting disease, is, among others, important.

[Cephalosporin antibiotics from the firm Glaxo in treatment of patients with complications after trauma]. / Antybiotyki cefalosporynowe firmy Glaxo w leczeniu chorych z powiklaniami po urazach.

The most serious complications in the treatment of open fractures and after procedures on bone tissue are infections. The broad spectrum antibiotics are usually used in the treatment of soft-tissue and bone infections. Effect of the compound treatment with cephalosporins produced by Glaxo administrated intravenously and with Septopal-locally were estimated basing on radioisotopic bone examination. In compound treatment cephalosporins and gentamicin in form of Septopal renal function evaluation is demanded and recovery process should be documented by the radioisotopic image.

[Radiologic evaluation of changes occurring in bone under the influence of gentamycin-septopal]. / Radiologiczna ocena zmian zachodzacych w kosci pod wplywem Gentamycyny-Septopalu.

On the basis of several-year experience it was acknowledged that in treatment of chronic blood-derived and traumatic inflammations of bones the introduction of chains of Septopal to treatment was an obvious progress. The changes occurring in the bones of the patients for whom Septopal was left for good in the marrow cavity under the influence of antibiotic were evaluated. The observation included 89 patients for whom X-ray examinations were made many times in various intervals. Initially the chains of antibiotic were surrounded with a thin layer of connective tissue, and after several new-generating osseous tissue grew into them. In none of the observed cases any reaction indicating recoil of the carrier of the antibiotic was noticed.

[Effect of septopal on kidney function during treatment of traumatic chronic inflammation of bones]. / Wplyw Septopalu na funkcje nerek w leczeniu pourazowego przewleklego zapalenia kosci.

Treatments included 20 patients for whom in the marrow cavity 1 chain of Septopal was inserted, and 10 patients for whom 2-4 chains were inserted. Before the planned operation the mark of the level of urea and of creatinine in the blood serum was made for the patients, and the clearance of creatinine was calculated. It was stated that usage of Gentamycin-Septopal for people who do not let know about the suffering from a kidney disease in the past, and the laboratory tests before the operation do not show deviations from the standard is a safe treatment recommended in prophylaxis and in treatment of blood-derived and traumatic inflammations of bones. Because of nephrotoxicity of Gentamycin the function of kidneys should be monitored before and during treatment with that medicine. Gentamycin can be connected with other antibiotics in order to improve the efficiency of its work.

Possibilities of vital dyeing to determine blood supplied from non blood supplied tissues are subject of many years' investigations. They may play an important role in traumatology, reconstructive and septic surgery. In acute traumatic changes one of the methods known is the so called immediate surgical treatment with delayed operation. In chronic and septic pathological changes there exists a great difficulty distinguishing necrotic parts of bone or soft tissues, which are the cause of long lasting inflammation and external fistulas. This process can last for months or even years if not treated by a proper and precise surgical procedure. Very helpful may be vital dying of tissues which was introduced by K. Klemm in 1970 for the surgical treatment of septic bone necrosis using the Disulphine Blue, produced by Imperial Chemical Industries Limited, Pharmaceuticals, Division, Macclesfield Cheshire, G. B. The tissue dyes are used widely for subcutaneous injections in lymphography, i.e. in about 30,000 patients in USA yearly. Disulphine Blue is a water soluble and partly spirit soluble powder giving a blue solution, which can be sterilized in autoclave. After an intravenous application of 0.25-0.5 ml/kg body of 6.2% solution a green colour of skin and conjunctivas appears in 5 minutes. The dye can appear in bile, bronchial tree, feces and in intestine secretions. It is evacuated with urine. Green colour of the skin disappear after 36 hours in children and after 48 hours in adults. This dye has been used as a direct visual test to investigate normal blood-supplied and necrotic soft and bone tissues. This phenomenon can have a special significance for defining the vitality in inflammation of bone tissue, in burns and in necrosis of soft tissues a.a. of the Achilles tendon. A demonstrative case report. Patient K. J., 32 years old, a worker and sportsman was hospitalized in our clinic because of chronic inflammation and purulent fistulas 10 weeks after two operations in one of hospitals because of traumatic right Achilles tendon rupture. During the first operation the tendon was sutured and the leg immobilized for a period of 6 weeks. After that time the plaster dressing was removed and during the rehabilitation was stated a secondary Achilles tendon rupture. The tendon was resutured. The course after the operation was complicated with haematoma and purulent fistulas. Antibiotics were used but without effect. In our Clinic 10 weeks after the primary injury and unsuccessful local treatment we performed a subsequent operation after slow intravenous injection of 10 ml Disulphine Blue.(ABSTRACT TRUNCATED AT 400 WORDS)

[Local treatment of burn injuries and traumatic wounds with a hydrogel Geliperm dressing]. / Miejscowe leczenie ran oparzeniowych i urazowych hydrozelowym opatrunkiem Geliperm.

Hydrogel Geliperm dressings of BYK Goldbin - Konstanz and Geistlich Sons Ldt. firms were used in patients in case of skin burns and some traumatic wounds. During a four-year observation it was discovered that those materials are safe for the patient, their removing is painless and their are fully useful in the treatment of burns and traumatic wounds in surgical departments and in the ambulatory treatment. Geliperm does not have allergic action. Because of its physicochemical and biological properties Geliperm may have also usage for example in the treatment of leg ulcerations, of bed-sores and of radiation skin defect.

[Experimental testing of Dexon, Vicryl and Gore-Tex mesh used to complete the abdominal wall]. / Badania doswiadczalne wchlanialnych i niewchlanialnych siatek Dexon, Vicryl i Gore-Tex do uzupelniania powlok brzusznych.

Injuries of abdominal integuments, operations and reoperations of hernias, injuries of a spleen, defects in the wall of the chest and diaphragm cause that the need for substitute materials and alloplastic materials for completing those defects is large. To satisfy this tests have been made experiments with absorbable--Dexon, made by Davis and Geck enterprise, Ethicon and non-absorbable--Teflon made by Gore-Tex enterprise according to the proposition of H. Kus. The experimental tests were made on 30 rabbits of the white New Zealand breed which weighted 2-3 kg. On the hypogastrium in the distance of 3 cm from the white line on its both sides defects of the size 2 x 3 cm were made. On the right side both fascial-muscular layers of the external and internal oblique muscle of abdomen were cut out but the peritoneum was left while on the left side only one layer was cut out. The defects were completed with nets and fixed with interrupted sutures and then with continuous suture. During the operation the behaviour of the nets in the operation area and the difficulties in connecting the nets with the animals' tissues were tested. Sections of the animals and macroscopic tests were made 7, 14, 21 and 90 days after the implantation of each surgical net estimating the process of healing and resorption of the absorbable nets. The state of the organs of the abdominal cavity was also estimated. Microscopic tests consisted of estimation of the histological specimens made from the tissues taken during the sections of the animals. At the estimation of the surgical convenience of the tested nets it turned out that each of them is very convenient. The differences were in the fact that the Dexon and Vicryl nets had little elasticity and extensibility and after cutting there was a little shredding of their edges. The Gore-Tex net, on the other hand, due to its specific spongy structure, had little extensibility, good elasticity and a complete lack of shredding of the edges after cutting. The macroscopic tests carried out in the planned terms did not showed any deformation or handicap of the functions of the abdominal integuments. The nets implantation did not cause displacement of the internal organs nor intraperitoneal adhesions. On the basis of the microscopic investigation it was noticed that the absorbable surgical nets underwent progressing resorption in the tissues in the period of 21 to 90 days after the implantation.(ABSTRACT TRUNCATED AT 400 WORDS)

[Usefulness of selected parameters from the diagnostic laboratory for evaluating absorbed grafting materials]. / Przydatnosc wybranych parametrów z diagnostyki laboratoryjnej do oceny wchlanialnych materialów implantacyjnych.

The results of hematologic, biochemical research and research of clotting system after intraperitoneal implantation of absorbed synthetic threads Dexon S are presented in this work. The research was made on rats of Wistar type. Blood for the research was taken 3, 7, 14, 30 and 60 days after the implantation. Morphology (Ht, Hb, MCH, WBC, the number of platelets), activity of aspartic and alanine aminotransferase and antithrombin activity, concentration of comolete protein and its fraction and concentration of glucose, urea, creatinine and also concentration of ions Na+, K+, Mg2+, Ca2+ and fibrinogen, protein C3 and C4 of complement system as well as kaolin-kephalin time and prothrombin time of plasma were marked. On the basis of the obtained results of the research it was noticed that the used methods of research constitute supplement of biological estimation of absorbed grafting materials.

[Changes in the level of selected laboratory parameters in blood after implantation of chitosan fibers]. / Zmiany w poziomie wybranych parametrów laboratoryjnych krwi po implantacji wlókien chitozanowych.

The tests were carried out on rats of the Wistar tribe to the peritoneal cavity of which chitosan fibres were implanted. The blood for the tests was taken in 3, 7, 14, 30 and 60 days after the implantation. In the blood hematological, biochemical and clotting system parameters were marked. Moreover they made pathomorphological tests of the tissues surrounding the implanted material and laboratory and biological tests of aqueous extracts from chitosan fibres. The shown quantitative changes in the level of the marked parameters of blood to the 14th day of observation are connected mainly with toxicity of chitosan fibres, whereas to the 30th and 60th day-with the process of biodegradation and resorption of collagenous fibres.

Evaluation of biomaterials with the help of routine methods not always allow full and explicit stating of their toxicity. That is why we still seek new methods of evaluation of biocompatibility on the cellular level. Usage of changes in activity of interleukin after biomaterials implantation creates such a possibility. The aim of this work as use of changes in activity of cytokines IL-1 beta and cytokines IL-6 for evaluation of biocompatibility of chosen medical materials. The tests were made on mice to whom polyester and aramid fibres as well as discs of rubber drains were implanted into the peritoneal cavity. Evaluation of the changes of the level of interleukin-1 beta and interleukin-6 was made in the fluid from the peritoneal cavity 3, 7, 14 and 21 days after implantation. Introductory tests of changes in the level of IL-1 beta and IL-6 after implantation of biomaterials with different degree of biocompatibility allow to assume that testing their activity may be useful in evaluation of their toxic effect.
