RESUMO

An understanding of how arthropods use energy is fundamental to explaining their diverse life histories and adaptation to specific environments. It is also of importance when attempting to predict the impacts of environmental change on patterns of development and phenology. Here, lipid use by the economically important agent of ovine myiasis, Lucilia sericata (Diptera: Calliphoridae), was quantified at a range of temperatures. During pupation, at temperatures above the minimum temperature required for development (9 °C), pupae depleted an average of 30% of their total lipid over the course of pupation regardless of temperature. There was no detectable loss of lipid during pupation at temperatures below 9 °C. In general, larger individuals had the same relative amounts of lipid as smaller individuals. Newly emerged adults metabolized about 16% of the lipid reserves with which they emerged in the first 24 h during flight-related activity. Starved adults, with access to water but without sucrose or protein, depleted their lipid reserves and died within about 4 days of emergence. However, adults with access to protein and/or carbohydrate were able to maintain a stored lipid content of about 2.38% of their total body mass for at least 14 days after emergence, irrespective of sex. This finding is similar to that in field-caught individuals, in which lipid content was found to be a mean of 3% of body mass. The data suggest that warmer environmental conditions, within the temperature limits tested here, although shortening the time required for development and altering the patterns of seasonal abundance of L. sericata, are unlikely to impact on fly survival because of greater metabolic demands during non-feeding stages of the lifecycle.

Devic's neuromyelitis optica (NMO) associates optic neuritis and myelitis without any other neurological signs. Many patients with NMO may be diagnosed as having multiple sclerosis (MS), optic neuritis and myelitis being the inaugural symptom in 20% and 5% of MS cases, respectively. The aim of our study was to compare a new NMO cohort with recent studies and to try to determine the place of NMO in the spectrum of MS. We retrospectively studied 13 patients with a complete diagnostic workup for NMO. We compared our data with the most recent studies on NMO and with the criteria proposed by Wingerchuck et al. [Neurology 53 (1999) 1107]. We also determined whether these patients fulfilled the diagnostic criteria for MS. Thirteen patients (10 women and three men, with a mean age of 37.4 years) were included in the study. We found similar results to previously published data, except for an association with vasculitis in 38% of our cases. All but three of the patients fulfilled the clinical criteria for MS and two patients fulfilled both clinical and MRI criteria for MS. However, if we applied more restrictive criteria concerning spinal cord and brain MRI and CSF, none of our NMO patients fulfilled the MS diagnostic criteria. NMO might therefore be differentiated from MS by the application of more stringent criteria. Furthermore, all NMO patients should be investigated for vasculitis, even those with no history of systemic disease.

During a period of one year, 6,863 employees underwent routine medical check-ups: 3,223 ground staff, 3,129 cabin staff, and 511 cockpit crew. One hundred and fifty permanent hypertensives were recorded and their status and evolution analysed in a survey. For every patient data were collected on illness, treatment, heredity, aetiology, risk factors, type of work and life style. The 150 patients surveyed were divided into four groups: (A) 70 satisfactorily treated hypertensives (57 men, 13 women); (B) 31 unsatisfactorily treated hypertensives (24 men, 7 women); (C) 33 neglected hypertensives (31 men, 2 women); and (D) 16 newly discovered hypertensives (14 men, 2 women). The incidence of hypertension was higher in ground staff (3.9%) than in cockpit crew (1.76%) and very low in cabin staff (0.41%). The neglected and unsatisfactorily treated hypertensives showed a clear improvement in their blood pressure through ongoing surveillance. In conclusion, while the incidence of hypertension in the population surveyed is quite low, this study shows a significantly lower rate in cabin staff and a large proportion of neglected and unsatisfactorily treated hypertension. It also shows how an occupational medical practitioner with no power of prescription can be effective in detecting the disease and referring the employee to his general practitioner, with a view towards achieving better collaboration.

Before contract negotiations in 1999, the author served on an "issue-based" health benefits committee of faculty union representatives and university administrators. Although the committee solicited estimates from health insurers regarding the impact of higher copayments on monthly premiums, in subsequent negotiations, the projected cost savings did not lead to changes in coverage or copayments. The explanations offered are (1) national or regional employers may be reluctant to raise employees' health benefit copayments when labor markets are tight; (2) collective bargaining, particularly when other, nonmonetary issues are being bargained, may lead to results different from those from a strictly competitive model; and (3) employers with market power in the product market may shift these highest costs to consumers through higher prices.

RESUMO

Consecutive sections of certain neurons in the central ganglia of the pond snail Lymnaea stagnalis appear to be immunoreactive to anti-dopamine and anti-serotonin. The Cerebral Giant Neurons stain in addition with anti-vasotocin. The observations indicate the presence of two biogenic amines within the same neuron and in addition their co-existence with a biologically active peptide.

RESUMO

A case of neonatal anterior rectal duplication is reported. Abdominal ultrasonography revealed a prerectal cystic mass. The different types of rectal duplications and the main differential diagnosis are described. Mechanical obstruction and neoplastic risk make early surgery necessary.
