BACKGROUND: Acute respiratory failure (ARF) has become one of the most prevalent serious pathologies encountered in the emergency medical service (EMS). In hospital settings, noninvasive ventilation (NIV) therapy prevents complications from more aggressive treatments for that condition. However, the scarce evidence on the benefits of NIV in prehospital EMS (i.e., during transport to the hospital) is inconclusive. OBJECTIVES: To determine whether the administration of NIV during prehospital EMS in cases of ARF reduces in-hospital mortality compared with starting NIV on arrival to in-patient EMS. METHODS: This is a multicentre, observational, prospective cohort study. We recruited a total of 317 patients from the Madrid region (Spain) who were prescribed NIV for their ARF using a nonprobabilistic consecutive sampling method. Analyses of the main outcome (in-hospital mortality) and secondary outcomes (length of hospital stay, readmissions, percentage of intensive care unit admissions, and cost-effectiveness) will include descriptive analyses of patients' characteristics, as well as bivariate and multivariate analyses and cost-effectiveness analysis. DISCUSSION: This study will provide data on NIV management in prehospital and in-patient EMS in patients with ARF. Results will contribute to the existing evidence on the benefits of NIV in the context of prehospital EMS while underlining the importance of a standardized formal training for physicians and nurses working in prehospital and in-patient EMSs. CONCLUSION: The VentilaMadrid study will provide valuable data on the clinical factors of patients receiving NIV in prehospital EMS. Further, were our hypothesis to be confirmed, our results would strongly suggest that the administration of NIV in prehospital EMS by medical and nursing profesionals formally trained in the technique reduces mortality and improves prognoses.

OBJECTIVE: To analyse the prognosis and mortality of patients hospitalised for acute coronary syndrome before and after the implementation of a coronary unit, haemodynamics room and the Código corazón primary angioplasty programme. METHODS: We conducted an observational and retrospective study that analysed the epidemiological characteristics, reperfusion strategies, adverse cardiovascular events and mortality for 5 years of follow-up. The results of the post-code period (March 1 - December 31, 2012; n=471) were compared with those of the pre-code stage (March 1 - December 31, 2009; n=432). RESULTS: There were no differences in the baseline characteristics of the 2 groups; however, an increase in ST-segment elevation acute coronary syndrome (STE-ACS) from 17.6% to 34.8% (P<.001) was observed during the post-code phase. The use of percutaneous coronary intervention was made widespread at the hospital, achieving rates of 64.8% in non-ST-segment elevation acute coronary syndrome (NSTE-ACS) and of 95.5% in STE-ACS. Readmissions were reduced (from 38.2% to 25.1% for NSTE-ACS [P=.001] and from 23.7% to 11.0% for STE-ACS [P=.018]), the combined prognostic variable of adverse cardiovascular events and mortality at 5 years of follow-up was reduced (from 58.7% to 45% [P=.001] for NSTE-ACS and from 40.8% to 23.8% [p=.009] for STE-ACS), and 30-day mortality was decreased for STE-ACS (from 11.8% to 3.7%; P=.021). CONCLUSIONS: With the structural changes in the hospital, the use of percutaneous coronary intervention was made widespread and improved the prognosis of patients with acute coronary syndrome, decreasing admissions, adverse cardiovascular events and mortality.

An increasing awareness about novel medical applications of smaller, inorganic-based nanoparticles, possessing unique properties at the nanoscale, has led to a burst of research activities in the development of "nanoprobes" for diagnostic medicine and agents for novel, externally activated therapies. In this research field magnetic nanoparticles are prominent due to fundamental peculiar properties particularly appealing for their use in materials and biomedical applications. Aiming to study the relationship between the topology of the magnetic nanoparticles and their efficacy as MRI contrast agents (relaxometric properties), we prepared three different stable colloidal suspension (ferrofluid) of magnetic nanobeads (MNBs) constituted by a discrete number of maghemite nanoparticles, arranged in disordered clusters or ordered in a polymeric matrix. An accurate morpho-dimensional and magnetic characterization displays the close correlation between the magnetic fundamental properties and the topology of our spin systems. The NMR relaxometry profiles confirmed the nature of the physical mechanisms inducing the increase of nuclear relaxation rates at low (magnetic anisotropy) and high (Curie relaxation) magnetic fields. Moreover the transverse relaxivity (r2) values for all the MNBs are higher than those of common contrast agents and the differences between the three MNBs are suggested to be due to the spin topology effect.

OBJECTIVE: To analyze the severity of the oral lesions in low-risk oral mucous membrane pemphigoid (OMMP) measured according to the size of the bullous areas and the number of simultaneously affected oral locations. MATERIAL AND METHODS: A total of 100 cases of low-risk OMMP were studied. The symptoms and location of OMMP in the oral cavity were analyzed. The bullous areas were measured, establishing three grades according to the greatest bullous lesion size (grade 1: < 3 cm in size; grade 2: 3-6 cm; and grade 3: > 6 cm). RESULTS: The mean age of the patients was 66.07 ± 13.55 years, with a clear predominance of females (84%). Desquamative gingivitis was the most common presentation (97%). A single oral location was found in 67% of the cases, two in 18% and three in 15%. The most common presentation corresponded to grade 1 (the greatest bullous lesion size < 3 cm; 51.6% of the cases). CONCLUSIONS: Most cases of low-risk OMMP are restricted to a single site in the oral cavity, the gingiva being the most common location-the predominant grade corresponding to the greatest bullous lesion size < 3 cm.

RESUMO

Penile carcinoma is a rare disease that has a wide range of pathology and morbidity. Occurs commonly in the 6th and 7th decades of life. Squamous cell carcinoma (SCC) is the predominant histological type. We present a case of a penile lesion of exophytic papillary morphology accompanied by multiple bilateral mobile inguinal lymph nodes.

WHAT IS KNOWN AND OBJECTIVE: Medication is the main treatment option for patients with chronic atrial fibrillation. However, medication can have negative effects. We aimed to detect negative outcomes associated with medication that led to patients with chronic atrial fibrillation presenting themselves to hospital emergency departments. We assessed the severity of those outcomes and comment on whether they could have been avoided. METHODS: This descriptive, cross-sectional study included all patients with chronic atrial fibrillation who attended the emergency department of our tertiary hospital. We used the Dader method to identify and evaluate the negative outcomes associated with medication through interviews with patients and scrutiny of the clinical charts. RESULTS AND DISCUSSION: Of the 198 eligible patients who presented at the emergency department, 134 (67·7%) did so because of negative outcomes associated with medication (41% related to necessity, 32·1% to effectiveness and 26·9% to safety); 67·9% of those negative outcomes could have been avoided. In terms of severity, 6·7% were mild, 31·3% moderate, 51·5% severe and 10·4% fatal. The Anatomical Therapeutic Chemical Classification anatomical group most frequently associated with negative outcomes was the cardiovascular system, followed by blood/blood-forming organs. WHAT IS NEW AND CONCLUSION: A high percentage of patients with chronic atrial fibrillation presenting at hospital emergency departments had negative outcomes associated with medication. Some led to deaths. More than half of these were severe, and most could have been avoided.

The Tgf-ß(3) null mutant mouse palate presents several cellular anomalies that lead to the appearance of cleft palate. One of them concerns the cell proliferation of both the palatal medial edge epithelium and mesenchyme. In this work, our aim was to determine whether there was any variation in the presence/distribution of several cell proliferation-related molecules that could be responsible for the cell proliferation defects observed in these palates. Our results showed no difference in the presence of EGF-R, PDGF-A, TGF-ß(2), Bmp-2, and Bmp-4, and differences were minimal for FGF-10 and Shh. However, the expression of EGF and Msx-1 changed substantially. The shift of the EGF protein expression was the one that most correlated with that of cell proliferation. This molecule is regulated by TGF-ß(3), and experiments blocking its activity in culture suggest that EGF misexpression in the Tgf-ß(3) null mutant mouse palate plays a role in the cell proliferation defect observed.

A reduction was observed in readmissions (from 38.2% to 25.1% for NSTE-ACS (p=.001) and from 23.7% to 11.0% for STE-ACS (p=.018)), the composite prognostic variable of adverse cardiovascular events and 5-year mortality (from 58.7% to 45% (p=.001) for NSTE-ACS and from 40.8% to 23.8% (p=.009) for STE-ACS), and a decrease in 30-day mortality in STE-ACS (from 11.8% to 3.7%; p=.021). CONCLUSIONS: With the structural changes in the hospital, the use of percutaneous coronary intervention was made widespread and improved the prognosis of patients with acute coronary syndrome, decreasing admissions, adverse cardiovascular events, and mortality.

BACKGROUND: Erythema multiforme (EM) is an acute disorder of the skin and mucosal membranes manifesting in the oral cavity (60-70% of all patients) as polymorphic erosive, ampullar, and bloodstained crusts. The etiology is unclear, although an autoimmune mechanism is involved. Infections and drugs have been implicated in the etiopathogenesis. With the exception of corticosteroids, no specific treatment for EM is available. METHODS: Data were collected on the clinical manifestations, antecedents of viral infection, and the use of drugs substances as possible etiological factors, treatment, and response to topical and systemic corticotherapy. RESULTS: A total of 22 patients were studied (14 males and 8 females), with a mean age of 47±20.4 years. A relationship was clearly suspected between drug use and lesion outbreak in 6 patients (27.2%). On the contrary, in 7 patients (31.8%) the triggering factor could have been herpes virus infection (herpes labialis). One half of the patients (11 cases) were classified as presenting minor EM, 36.4% (8 cases) presented major forms of the disease, and 13.6% (3 cases) were classified as corresponding to Stevens-Johnson syndrome. Systemic and/or topical corticosteroids proved effective in controlling the outbreaks in all of our patients. CONCLUSION: The oral mucosa is the most affected mucosal region in EM, with a predilection for the lip mucosa, erosive forms and bloodstained crusts. Systemic corticosteroids are effective in controlling the outbreaks, although their use as maintenance therapy is not clearly indicated.

Many effective medium theories are designed to describe the macroscopic properties of a medium (the rock, or reservoir in this case) in terms of the properties of its constituents (the background matrix of the rock and the inclusions, for our scenario). A very well known effective medium theory is the Eshelby-Cheng model, which was studied by us in previous work, being tested for the case where the background medium was weakly-anisotropic and porous. The analysis was done testing elastic velocities and Thomsen parameters - as a function of crack density for fixed values of aspect ratio - predicted by the model with data acquired from synthetic rock samples. In this work, we aim to complete the analysis of the Eshelby-Cheng model capabilities when applied to rocks with porous and vertical transversely isotropic (VTI) backgrounds, testing the model for the elastic velocities as functions of aspect ratio - for fixed values of crack density - against experimental data. The data used to test the model were obtained from 17 synthetic rock samples, one uncracked and 16 cracked, the latter divided into four groups of four samples each, each group with cracks having the same aspect ratio, but with the samples having different crack densities. In these samples, ultrasonic pulse transmission measurements were performed to obtain the experimental velocities used to test the model. As was not possible to acquire data for velocity as a function of aspect ratio for fixed values of crack density, we performed interpolations of the experimental data to estimate these velocities. Eshelby-Cheng model effective velocities and Thomsen parameters were calculated using three formulations proposed for the crack porosity: one proposed by Thomsen, the second one proposed in our previous work (which depends only on the crack density) and the third one proposed in this work (which depends on the crack porosity and the aspect ratio, just like Thomsen's proposal). The comparisons between elastic velocities and Thomsen parameters - as function of crack aspect ratio, for fixed values of crack density - predicted by the model and estimated from the data via interpolation showed that the third formulation produced better fittings (lower root-mean-square errors) between model and experimental data for all ranges of aspect ratio and crack density.

Pasture established on polluted soil may pose a risk to grazing livestock creating a requirement for mechanical management which may affect biodiversity and expend energy. The risk associated with managing pasture by grazing horses (non-edible livestock) is being assessed in the Guadiamar Valley (SW Spain), where soils are polluted with trace elements following a major pollution incident. Soil pollution does not affect biomass production or floristic composition of pasture, although both variables influence trace element accumulation in herbage. Element concentrations in herbage are below maximum tolerable limits for horses. Faecal analysis showed regulated absorption of essential elements, while non-essential elements seemed preferentially excreted. Elemental content of horse hair did not differ in animals from polluted and control pastures. If pastures are managed by grazing, periodic monitoring is recommended in view of the long-term chronic trace element exposure in these systems.

The Domingo Rubio tidal channel (Palos de la Frontera, Huelva, Spain) is an estuary located in the mouth of the Tinto River. The estuary is affected by different sources of pollution (waters of the Tinto River, contaminated with trace elements from the Iberian Pyrite belt, and effluent from the Huelva chemical industrial area). Soil and the most frequent plant species were collected in 2004 and 2006 at six different locations on the estuary. In general, N-Kjedahl, Total Organic Carbon values, salinity and contamination (total trace elements up to 1,000 mg kg(-1) As, 6 mg kg(-1) Cd, 2,500 mg kg(-1) Cu, 1,900 mg kg(-1) Pb and 1,300 mg kg(-1) Zn) tended to increase downstream of the tidal channel. Soil biochemical properties were not negatively affected either by the high salinity or by trace element contamination. Despite the high values of the trace elements, analysed plant samples showed that Cu was the only metal that could be a serious risk for the food chain.

The study of fractures in subsurface is very important since they are, in some cases, the main conduits for hydrocarbon flow in a reservoir. There are many ways to study the behavior of seismic waves in different fracturing conditions, including the use of physical modeling. This method allows, among other approaches, the analysis of the behavior of seismic wave properties in complex fractured media, such as media with orthorhombic symmetry. In this work we performed ultrasonic measurements on fractured physical models with orthorhombic symmetry from which we analyzed the behavior of elastic velocities and anisotropy parameters for different number of fractures. The presented results show the efficiency of the construction methodology used in the study by presenting P- and S- wave velocity values consistent with the theory for an orthorhombic medium. It was observed that for the direction perpendicular to the fracture system the values of P and S-wave velocities were the smallest for each model, and that the velocities decreased as the number of fractures increased in all models. Furthermore, most of the ∊ and γ values show a decreasing behavior as a function of the decreasing number of cracks, being the trend curves of ∊ linear and most of the trend curves of γ quadratic. Additionally, all the ∊ parameters presented a high correlation with the γ parameters for a small number of fractures, lower than 5.

INTRODUCTION: Increasing life expectancy in the general population has led to a rise in the incidence of cancer and new challenges with regard to the diagnosis, therapy and prognosis of this disease. AIM: To assess prognostic factors in the initial work-up of patients ultimately diagnosed with cancer in an Internal Medicine Service, particularly those related with age. PATIENTS AND METHODS: A prospective study was undertaken with 224 patients ultimately diagnosed with cancer, as confirmed by histological or cytological study. The neoplasms included respiratory, gastrointestinal, genitourinary, metastatic adenocarcinoma of unknown origin, gynaecological, hepatobiliary and others. Before reaching the diagnosis, the following factors were investigated in all patients: functional status [Karnofsky Performance Status (KPS)], comorbidity (Charlson scale), body mass index (BMI), serum cholesterol and albumin concentrations, cognitive level (Mini-mental test), quality of life (Short Form 36 questionnaire), and extension of the disease according to established criteria. Survival at 1 year was analysed. Statistical analyses were done with spss 11.0 for Windows, using a forward stepwise (likelihood ratio) method to construct the model and a Cox multivariate model for the survival analysis. RESULTS: A total of 224 patients, 167 men (74.5%) and 57 women (25.5%), with a mean age of 66.1 +/- 12.3 years were studied. KPS was >or= 70 in 84% and comorbidity was zero or one in 74%. BMI was 24.25 +/- 4.3, cholesterol 180.7 +/- 4.3, albumin 3.32 +/- 0.5 and Mini-mental score 25.4 +/- 3.7. Metastasis was seen in 131 patients (58.5%) and local disease in 93 cases (41.5%). One-year survival was 38.8% (87 patients) with a mean of 203.8 +/- 143 days. In the Cox analysis, the independent predictive factors for survival were KPS [hazard ratio (HR) = 0.951; 95% CI = 0.930-0.974; p < 0.01], metastatic dissemination (HR = 2.422; 95% CI = 1.643-3.571; p < 0.01), physical quality of life (HR = 0.978; 95% CI = 0.962-0.995; p < 0.01) and albumin (HR = 0.653; 95% CI = 0.455-0.936; p < 0.01). CONCLUSIONS: In the initial work-up of patients ultimately diagnosed with cancer in an Internal Medicine Service, functional status, dissemination, the physical component in the quality of life scale and serum albumin levels were independent prognostic factors for survival. Age was not an independent prognostic factor and should not be used as a basis for adopting diagnostic or therapeutic decisions in these patients.

In this study, the effect of a partial substitution of peat for compost on the growth and nutrition of a native shrub (Pistacia lentiscus L.) was tested. Composts were prepared from pruning and municipal solid wastes or pruning waste and sewage sludge. For preparing growing media each compost was added at a rate of 40%, fresh pine bark at 20% or 40% and peat at 20%, 40% or 60%. Aqueous extracts from the substrates did not impair germination of cress (germination bioassay). In relation to plants growing in peat-based substrate (used as a control), plants of the compost-based substrates reached better growth and nutrition, especially when using the sewage sludge-based compost, and the P uptake was notably enhanced. The concentrations of trace elements were far lower than the ranges considered phytotoxic for vascular plants. Detrimental effect derived from using fresh pine bark was not observed.

The Aznalcóllar accident (28th April 1998) occurred because the collapse of the tailing-dam dike of the Aznalcóllar-Los Frailes mines. Soils were affected by a slurry of acidic water loaded with trace elements, finely divided metal sulphides, and materials used in the refining /floating process. Studies carried out before and after the soil restoration activities (sludge removal, amending, tilling, and afforestation) showed severe trace-element contamination (mainly As, Cd, Cu, Pb, Tl and Zn) in the superficial layer of the sludge-affected soils. Despite Hg being an important component of the Los Frailes ore and therefore of the contaminant sludge, data on Hg content of sludge-affected soils are scarce and sometimes controversial. The aim of this study was to determine the effect of the spill and of restoration measures on the Hg content of soils and how this related to other elements. Concentration of Hg immediately after the spill was 8-fold above background (0.061+/-0.012 mg kg(-1); mean+/-SD) at the surface (0-5 cm) and 3-4-fold greater in deeper layers (0-20; 0-50 cm). After the remediation measures, mean values of Hg and other elements (As, Cd, Cu, Pb and Zn) were very variable and remained above background values. These anomalies are due to the sludge left on the soil surface or buried during restoration operations, resulting in an irregular distribution of trace elements. The highest values for the less mobile elements (up to 176 mg kg(-1) As, 2.36 mg kg(-1) Hg and 1556 mg kg(-1) Pb) were observed in the area 1 km downstream of the tailings dam.

High levels of phosphorus and pathogens in runoff are 2 major concerns following manure applications to fields. Phosphorus losses from fields following manure applications have been linked to the solubility of phosphorus in manure; therefore, by decreasing manure phosphorus solubility, a decrease in phosphorus loss in runoff should be apparent. The objective of this research was to develop a process using quicklime that would result in reduced phosphorus solubility and bacteria counts in broiler litter. The 4 litter treatments evaluated were T1, new wood shavings without the addition of quicklime; T2, used, untreated broiler litter; T3, used litter with 10% quicklime (based on the weight of the litter); and T4, used litter with 15% quicklime (based on the weight of the litter). Body weight, cumulative feed consumption, and feed conversion (feed:BW) were determined on a weekly basis through 42 d of age. Mortality was recorded daily. Carcass weights and percentages of carcass yield without giblets were determined prechill. Litter pH, total phosphorus, nitrogen, soluble phosphorus, litter moisture (%), and total plate counts were measured for each litter treatment on d 7 and 42 after bird placement. No significant differences were found for BW, feed consumption, feed conversion, mortality, carcass weight, or carcass yield. No breast or footpad blisters were observed. On d 7, 15% quicklime had higher (P < 0.001) pH (11.2) when compared with the other treatments. Percentages of phosphorus and nitrogen were lower (P < 0.001) for new wood shavings in comparison with the used litter treatments. Soluble phosphorus (ppm) was lower (P < 0.001) for 15% quicklime (2.75) when compared with new wood shavings (42.2), untreated broiler litter (439.2), and 10% quicklime (35.0). Although not significant, 15% quicklime had lower total plate counts (cfu/g) in comparison with the other treatments on d 1 and 10 postmixing and at 7 d after bird placement. Litter conditions on d 42 after bird placement were similar. We concluded that the use of quicklime as a treatment for broiler litter would initially reduce nitrogen and soluble phosphorus and bacteria counts without negatively affecting bird productivity.

We describe a case of a 10 yr old boy, who suffered from gastroesophageal reflux in the neonatal period and from recurrent abdominal pain, epigastric distension and vomiting since birth, but with an increase in this clinic in the last two years, especially in the last five months, without suffering weight loss, having operations or other illnesses. After the diagnosis of superior mesenteric artery syndrome was confirmed by a superior barium series and a CT-scan, a laparotomy was performed, carrying out the Koecher manoeuvre, cutting the Treitz ligament near of the 4th part of the duodenum until descend that and freeing the aorto-mesenteric space. The follow up after 5 years showed an excellent clinical evolution. In our opinion, this case should be considered to be of congenital origin, but without other associated abdominal anomalies. The possible different interventions are also discussed.

Desertification is occurring throughout the mountainous areas of the Mediterranean. These processes lead to reduced soil fertility, increased soil loss, and reduced vegetation cover and species richness. To prevent further damage, it is recommendable to use low-cost approaches that are compatible with the European Strategy of Circular Economy guidelines. We investigated the systemic benefits from recycling of forest clearance residue by adding it to a dry Mediterranean mountainous area. More specifically, we performed afforestation without addition of residue in two control plots (C plots), and afforestation with addition of 10Mgha-1 of clearance residue from a nearby region dominated by Aleppo pine (Pinus halepensis Mill.) in two other plots (PM plots). We conducted the experiments throughout 30months after the afforestation process. Eighteen months after the intervention, the PM plots had significant increases in the soil organic carbon (SOC), and related increases in ecosystem productivity and stability. More generally, addition of clearance residues improved soil and vegetation recovery, and contributed to more successful afforestation. The improvements may be explained by an increase of infiltration process due to the physical changes in the soil following bio-waste addition. Addition of the forest residues increased the formation of soil macrochannels, and also increased the sink area, thereby improving the hydrodynamics of the ecosystem. Thus, soil loss was reduced by 98.2% in the PM plots relative to the C plots. Our study indicates that application of forest clearance residues to Mediterranean mountainous areas is an effective land management practice that produces very little waste, and it is in accordance with European policy.

Thallium is a scarce, highly toxic element. There are several investigations that report Tl accumulation in plants of the family Brassicaceae. These plants could pose a risk in areas where Tl is present at higher concentrations than normal soils. The present study reports analyses of two wild Brassicaceae, Hirschfeldia incana and Diplotaxis catholica, growing spontaneously at five sampling sites moderately polluted with Tl and other trace elements in the Green Corridor of the Guadiamar river, Seville, S. Spain. In general, trace element content was unremarkable in all part plants, despite the concentrations present in soil. Thallium was the only element whose concentration in both plant species was above normal for plants (maximum values of 5.00 mgkg(-1) in H. incana flowers). There were significant positive correlations between total Tl in soil and Tl in both plant species. Transfer Coefficients (TC) for all elements were, in general, <1 for both species, except for Tl in flowers and fruits at some sites. The highest Enrichment Factor (EF) was found for Tl in H. incana fruits (EF = 607) and D. catholica flowers (EF = 321). H. incana was studied in a previous growing season (2004) in the same area, although the rainfall was 3 times more than in the year of the present study (2005), giving a maximum Tl content of 46.5 mgkg(-1) in H. incana flowers. The data presented here show that Tl content of plants growing in semi-arid conditions can be significantly influenced by precipitation. In dry years, plant Tl accumulation may be significantly reduced.
