The present study was undertaken to estimate effect of various genetic and non-genetic factors on persistency of milk production and to identify the most appropriate persistency method that fits best in our environment. In the present study, effects of different non-genetic factors, viz. year, season, days to attain peak yield, and genetic group based on the level of exotic inheritance on persistency of milk yield in crossbred cattle were studied. Data comprised of 686 first lactation daily milk yield records of crossbred cattle that were maintained at GADVASU dairy farm over a period of 25 years from 1991 to 2015 were utilized to calculate persistency coefficients by four methods, viz., Ludwick and Peterson method (P1), Mahadevan method (P2), ratio method (P3), and Prasad et al. method (P4). Overall least squares means for persistency by Ludwick and Peterson method (P1), Mahadevan method (P2), ratio method (P3), and Prasad et al. method (P4) were 0.896 ± 0.096, 1.385 ± 0.224, 187.207 ± 26.398, and 0.621 ± 0.098, respectively. Effect of sires was significant (P < 0.05) on P2 and P4 methods. Effect of genetic group on all four methods was non-significant. Period of calving had significant (P < 0.01) effect on persistency of milk yield (P2, P3, and P4 methods). Effect of season of calving on persistency of milk yield was found to be significant in all estimates obtained by the four methods. Summer and autumn calvers were most persistent whereas spring and winter calvers were least persistent for (P2, P3, and P4 methods). Persistency of milk yield was significantly (P < 0.05) affected by days to attain peak yield in P1 and P2 methods. Maximum persistency was obtained in animals attaining peak at 41-57 days of lactation and minimum in < 41 days for Mahadevan method and ratio method. The highest heritability of persistency and minimum value of standard error was estimated as 0.275 ± 0.11 for the Mahadevan method followed by the Prasad method (0.197 ± 0.10) by half sib correlation method. The maximum coefficient of variation which indicates available variability was estimated as 20.788% for persistency by the Mahadevan method followed by 18.969% for the Prasad method. The highest correlation was also observed between P1 and P3 methods by Spearman's and Pearson's correlation for least squares breeding value of the sires. On the basis of heritability, standard error of heritability, and coefficient of variation, it can be concluded that the Mahadevan method followed by the Prasad method suits best to our environment for animals in first lactation as well as they can be utilized for effective selection for higher persistency in crossbred animals of Punjab.

Vulvar carcinoma is a rare and aggressive gynecological malignancy. It affects elderly females, with the mean age at diagnosis being 55-60 years. Regional metastasis to inguinal lymph nodes is common. There is a high incidence of pelvic node involvement, especially in those with pathologically positive inguinal nodes. Surgery appears to be the only curative treatment option in the early stages of the disease. But in most patients, surgery is associated with considerable morbidities and psychosexual issues. Hence, in the quest for a less morbid form of treatment, multimodality approaches with various combinations of surgery, chemotherapy, and radiation therapy have been suggested for advanced vulvar cancers. Due to the low incidence of the disease, the level of evidence for the success of these treatment modalities is poor. In countries like India, a heterogeneous incidence of vulvar carcinoma exists across the country, with patients presenting at advanced stages when the option of surgery is often supplemented or replaced by chemotherapy and radiotherapy. In this review, we attempt to study the available published literature and trials and discuss the treatment options in various stages of vulvar carcinoma.

AIM: To study feasibility of simultaneous integrated boost by intensity modulated radiotherapy (SIB-IMRT) in patients undergoing breast conserving surgery and its impact on cosmesis and dosimetry. PATIENTS AND METHODS: Between January 2014 and June 2017, all breast cancer patients fulfilling inclusion and exclusion criteria were enrolled in a prospective study conducted at a tertiary cancer centre in North India. All patients received adjuvant radiotherapy by simultaneous integrated boost technique following breast conserving surgery. Clinical information including patient and pathological characteristics, observed acute and chronic toxicities along with cosmesis using Harvard score were recorded and analysed. Univariate analysis and multivariate logistic regression analysis were performed for those variables which were found to be significant (p < 0.050) to study the influence of clinicopathological and dosimetric factors on toxicity and cosmetic outcome. RESULTS: Maximum acute skin toxicity during treatment was Grade 0-1 in 68.2% and Grade 2-3 in 31.8% of cases, respectively. Fibrosis was the commonest late toxicity with ≥Grade II fibrosis being noted in 16.3% of cases. Assessment of global cosmesis at 12 months follow-up showed good/excellent cosmesis in 88.4% of cases. Mean age, tumor size and homogeneity index (HI) were the significant factors associated with fair or poor cosmetic outcome and ≥Grade 2 fibrosis on multivariate analysis. Telengectasia and breast edema were more frequent in patients with larger tumor size/GTV volume. There were 5 recurrences including 1 ipsilateral local breast tumor recurrence. CONCLUSION: SIB-IMRT is a dosimetrically feasible option in patients undergoing breast conserving surgery and provides good/excellent cosmetic outcome.

BACKGROUND: Cyclin dependent kinase-4 (CDK4) encoded by CDK gene, is a heterodimer protein of cell cycle in G1-S transition. This study aimed to characterize the CDK4 immunoreactivity in different histological grades of oral leukoplakias (OLs) and oral squamous cell carcinomas (OSCCs) and also aims to discuss its probable role in the tumor biogenesis. MATERIALS AND METHODS: Expression of CDK4 was investigated in total of 52 samples including OL (15), OSCCs (30) and normal oral tissues (07). A labeled Streptavidin-Biotin immunohistochemistry assay was performed and staining intensity was evaluated. RESULTS: The staining pattern was similar in all tissues and was located in both nuclei and cytoplasm. Dysplastic epithelium displayed a progressive increase in nuclear expression of CDK4 when compared to normal tissues. Also, positive staining cytoplasm was highly evident in OSCC with loss of differentiation. CONCLUSION: Our study indicated a progressive over expression of CDK4 from normal to leukoplakias (various histological grades of dysplasias) and OSCCs.
