RESUMO

A female patient with the IV grade cervical carcinoma without distant metastases is presented. The patient underwent a radical surgical intervention. An anterior pelvic exenteration with ileocystoplasties was successfully performed. The patient is alive 14 months after the initial treatment. No greater complications were observed during the operation and after it, but in spite of a satisfactory general status, the patient has had nocturnal incontinence all the time. It is believed that this complication is the sequel of the newly formed urinary bladder, as the convolution of the ileum is only 30 cm long which has proved insufficient.

The relative function of the obstructed kidney (RFOK) was assessed in 43 adult patients with upper urinary tract obstruction (UUTO) using 99mTc-DTPA and 131I-ortho-iodohippurate (OIH) dynamic studies and 99mTc-DMSA dynamic and static studies. The patients were divided into five groups according to the duration and degree of obstruction. Findings were as follows: a) in patients with the first occurrence of acute severe obstruction (group 1), the relative glomerular filtration rate (GFR) was significantly less than the relative effective renal plasma flow (ERPF); b) in patients with chronic severe obstruction and long term uroinfection, the relative ERPF decreased significantly compared with the relative GFR; c) the RFOK calculated from the DMSA dynamic study was the same as both the relative GFR or ERPF in any group; d) the RFOK calculated from the DMSA static study seemed to parallel the relative ERPF more closely than the relative GFR, but in group one it was significantly higher than any of three other estimates. It is concluded that 99mTc-DTPA is the radiopharmaceutical of choice in obstructive uronephropathy, but when interpreting the RFOK, the time course and severity of obstruction, the presence or absence of uroinfection should also be considered.

RESUMO

Results of 140 combined first-pass and dynamic functional renal studies were clinically evaluated in 50 patients with a transplanted kidney. The renal handling of three different nephroaffine radionuclides, namely the glomerular agent (99m Tc-DTPA), the predominantly tubular agent (99m TcMAG3) and combined glomerulo-tubular agent (99m Tc-PAHIDA) was also analysed in various dysfunctions of renal transplants. It has been concluded, that the clinically most useful information at all levels of the overall renal function has been given by 99m TcMAG3. However, the behaviour of glomerular and tubular agents in postischaemic acute tubular lesion was found to be discordant which in turn could be valuable differential-diagnostic information, and out of this reason their successive application is essential. The combined firstpass and dynamic functional renal studies have been judged as a helpful diagnostic tool in the follow-up of patients who have undergone kidney transplantation, provided that its results are interpreted in the light of clinical and other relevant findings.
