BACKGROUND: Erythroderma is a clinical skin syndrome shared by patients with cutaneous disorders of distinct aetiologies as a result of the combined actions of chemokines, adhesion molecules, and cytokines, such as vascular endothelial growth factor (VEGF). OBJECTIVE: To evaluate the profile of serum levels of VEGF and soluble vascular endothelial growth factor receptor 1 (sVEGFR-1) in pemphigus foliaceus (PF) patients with erythroderma. METHODS: We conducted a retrospective study, which included (i) a chart review of all PF patients from the Autoimmune Blistering Clinic, University of Sao Paulo, Brazil, from January 1991 to December 2014, together with an evaluation of demographic variables, hospitalization duration and complications and (ii) analysis of the circulating VEGF and sVEGFR-1 levels in PF patients with erythroderma by ELISA. The controls included patients with pemphigus vulgaris or psoriasis. RESULTS: We observed higher serum VEGF levels in PF patients during erythroderma than during the non-erythrodermic phase. PF patients showed increased serum levels of sVEGFR-1 during the erythrodermic phase in comparison to controls. Interestingly, the sVEGFR-1 and antidesmoglein-1 levels were positively correlated during the non-erythrodermic period. CONCLUSION: Erythroderma, which represents one clinical form of PF, implies more severe outcomes. The circulating levels of VEGF, a potent endothelial activator, are increased in PF patients with erythroderma; this result suggests the contribution of the blood vessel endothelium to the pathogenesis of this clinical syndrome. Interestingly, our findings showed a positive correlation between the sVEGFR-1 and antidesmoglein-1 antibody levels, indicating a suppressive response to VEGF augmentation during the erythrodermic phase of PF.

The capability for actual measurements-not just simulations-of the dynamical behavior of THz electromagnetic waves, including interactions with prevalent 3D objects, has become increasingly important not only for developments of various THz devices, but also for reliable evaluation of electromagnetic compatibility. We have obtained real-time visualizations of the spatial evolution of THz electromagnetic waves interacting with a single metal micro-helix. After the micro-helix is stimulated by a broadband pico-second pulse of THz electromagnetic waves, two types of anisotropic re-emissions can occur following overall inductive current oscillations in the micro-helix. They propagate in orthogonally crossed directions with different THz frequency spectra. This unique radiative feature can be very useful for the development of a smart antenna with broadband multiplexing/demultiplexing ability and directional adaptivity. In this way, we have demonstrated that our advanced measurement techniques can lead to the development of novel functional THz devices.

RESUMO

We report on the operation of a highly sensitive terahertz imaging system relying on a planar metallic plasmonic crystal as a terahertz surface plasmon resonant (THz-SPR) sensor. The terahertz imaging is based on the resonantly enhanced transmission phenomenon of a periodically perforated metal film. The detection sensitivity and the imaging contrast for small amounts of substance are considerably better than those of the conventional terahertz transmission imaging without a THz-SPR sensor. As a demonstration, a high contrast image of a fingerprint recorded on a thin film can be achieved by using this system.

RESUMO

A surface-wave sensor based on the resonant transmission characteristics of metal hole arrays is demonstrated in the terahertz (THz) region. Since the frequency of the transmission peak of a metal hole array, which corresponds to the resonant frequency of the surface waves, is particularly sensitive to the refractive index in the vicinity of the metal surface, a very small change in the substances attached to the surface can be detected by monitoring the transmission spectrum. By attaching a layer of substance (thickness t < 5 microm) much thinner than the wavelength of the THz wave (lambda(THz) = 1 mm at 0.3 THz) to the surface of a metal hole array, we demonstrated that the existence of such a small amount of substance can be detected more easily than without the metal hole array. This demonstration of THz sensing with metal hole arrays indicates the possibility of realizing THz surface-wave sensors for biochemical molecules in the THz region.

Abstract A 17-year-old woman who was being treated with prednisolone for cutaneous vasculitis developed recurrent massive melena and abdominal pain. An emergency resection was performed because of uncontrollable melena, revealing many small intestinal ulcers with cytomegalic inclusion bodies, which were found by immunopathological staining. However, the cytomegalovirus (CMV) antigenemia (CMV-Ag) assay and the IgM antibody titer for CMV were negative on admission. This case indicates that a high state of alertness for CMV infection in immunocompromised patients with gastrointestinal bleeding is required even if the CMV-Ag assay and IgM antibody are both negative.

Criterion validity of a two-stage Cross-Cultural Cognitive Examination (CCCE) designed for epidemiologic use was evaluated in Japanese subjects by comparison with a physician's DSM-III-R diagnosis of dementia and the Hasegawa Dementia Rating Scale (the standard Japanese instrument similar to the Mini-Mental State Exam). We report on 188 subjects tested in three locations in Japan: Tokyo area, Ise, and Osaka. Subjects ranged in age (50-93 years) and education (1-22 years) and included neurology outpatients, community volunteers, and inpatients. The CCCE was 97.4% specific for dementia, with sensitivity of 88%. The correlation with the Hasegawa scale was significant (r(175) = .8230, p less than .0001). Diagnosis using the CCCE showed good validity when compared with Japanese criteria for dementia. If the instrument could be shown to be reliable and more "culture fair" than the currently available tests, it may be useful in cross-cultural epidemiologic studies of dementia.

Sera from two patients with chronic active paracoccidioidomycosis yielded negative double immunodiffusion results with a culture filtrate antigen from Paracoccidioides brasiliensis routinely used in our laboratory. Complement fixation tests were positive for both sera using a polysaccharide-rich antigen. This study reports the results of a more extensive serological investigation of these two sera. Both a somatic antigen and a saline extract from the fungus yielded positive results in the double immunodiffusion. However, the immunodominant 43 kDa glycoprotein antigen showed negative results, although it was recognized by both sera in the Western blot assay. The value of the double immunodiffusion as a single serological test in paracoccidioidomycosis diagnosis is discussed.
