[COMPARISON OF INTRAOPERATIVE SCLEROOBLITERATION AND ECHOSCLEROOBLITERATION EFFICIENCY OF VARICOSE DISEASES OF THE LOWER EXTREMITIES C6 CLASS IN PATIENTS BY TYPE II DIABETES MELLITUS].

The experience of surgical treatment of 50 patients for varicose disease of lower extremities, complicated by trophic ulcers, in the presence of diabetes mellitus type II were analysed. During surgery in patients of the 1st group performed a combined phlebectomy, group 2--scleroobliteration and echoscleroobliteration. Using fleboscleroobliteration method helped reduce the frequency of early postoperative complications in (6.5 +/- 1.3) times.

[Studying of sclerosing action of the foam form of polidokanol in experiment in vivo].

The results of investigation on impact of the foam form of sclerosant in experiment on 80 mongrel white rats, were presented. Sclerosing action of 0.5%, 2%, 3% solution of polydocanol in a microfoam form was studied after its infusion into lateral caudal vein of the rat. Quantity of preparation 0,5 cm3 was determined in accordance to the vein volume. Clinical and macroscopic estimation of the preparation action on internal organs and caudal rat's vein was conducted on the 10th min, every day during 1 week and on the 30th day. When the foam form of sclerosant is introduced intravenously there was not observed thrombotic complications of posterior vena cava and specific macroscopic changes in internal organs, not depending from its concentration, what witnesses security of the method, and selection of the sclerosant solution concentration must depend on the vessel diameter.

[Subfascial endoscopic dissection of perforant veins in ambulatory surgery of varicose disease].

The results of ambulatory application of subfascial endoscopic dissection of perforant veins (SEDPV) in 47 patients, suffering C4-C5 functional class varicose disease (according to CEAP classification) were presented. In all the patients SEDPV was performed in combination with miniinvasive methods of vertical reflux and varicosely changed tributaries elimination. The complications have had occurred in 6 (12.77%) patients, but they were not severe, did not need secondary hospitalisation of the patients, did not impact essentially restoration of ability to work and did not worsened the treatment outcome. Duration of inability to work period have lasted 12 - 25 days after the operation, 14.7 days at average. There was proved, that SEDPV permits to perform radical surgical intervention in patients, suffering chronic venous insufficiency class C4-C5 in ambulatory conditions.

[Algorithm of modern technologies for outpatient surgical treatment of varicose disease of the lower extremities].

The results of ambulatory surgical treatment for the 2007 - 2012 yrs period of 736 patients, suffering varicose disease of lower extremities (VDLE) with chronic venous insufficiency Class C0-C6, according to CEAP (Clinical Etiological Anatomical Pathophysiological) classification, were analyzed. In 258 patients intraoperative truncal scleroobliteration was used and in 124 -endovenous laser coagulation, in 47 - subfascial endoscopic dissection of perforant veins, in 307 - microsclerotherapy, compressive treatment, medicinal therapy. The treatment algorithm of ambulatory surgery of VDLE was elaborated, basing on the analysis of the results of treatment.

[Crossectomy conduction under local anesthesia in conjunction with mini-invasive methods of a varicose disease treatment].

Experience of treatment of 135 patients for varicose disease (VD) of lower extremities (VLE) was analyzed. The investigation objective was the elimination of adverse reactions of general anesthesia and minimization of surgical trauma without loss of the operation radicalism. In 112 (83%) patients a crossectomy (upper and/or lower) was conducted under local infiltrating anesthesia. The criterion for the patients choice for the operation in conditions of local anesthesia was the presence of a VD, independent of its stage. In the investigation were not included the patients, suffering a VD recurrence and its decompensated forms. Our experience witness, that in a certain level of a surgeon qualification present, in a majority of patients, suffering VDLE, including its complicated forms, crossectomy may be performed in conditions of a local anesthesia.

Character of microflora in patients with infection complications of diabetic foot syndrome was analyzed. The efficacy of modern antibacterial preparations for empirical and specific therapy was studied up.

An attempt was made to develop a new method for separate measurement of sterols of similar chemical structure--desmosterol, cholesterol, 7-dehydrocholesterol--by means of thin-layer and gas-liquid chromatography. Fluoroanalogues of the sterols were well separated in the adsorbent thin layer. This facilitated their further identification, quantitation and accumulation. Similar results were obtained with the aid of gas liquid chromatography.

The sterol composition of Palaemon adspersus is studied. It is established that the nonsaponifiable fraction of Palaemon adspersus contains cholesterol, 7-dehydrocholesterol, desmosterol, substance "240" and inconsiderable amounts of other still nonidentified sterols.

[Alterations in composition of sterols and in properties of erythrocyte membranes in rats with Shvetz experimental leukemia and after UV irradiation]. / Izmenenie sostava sterinov i svoistv membran éritrotsitov u krys s éksperimental'nym leikozom Shvetsa pri UF-obluchenii.

Amount of cholesterol was distinctly decreased but content of scvalene, lanosterol, 7-dehydrocholesterol and 7-beta-hydroxycholesterol--increased in erythrocyte membranes of rats with Shvetz experimental leukosis. At the same time, osmotic stability of erythrocytes and a pattern of acidic erythrogramms were altered. After UV irradiation of rats amount of cholesterol was increased as well as other unidentified substances appeared in erythrocyte membranes. These alterations appear to be one of factors responsible for development of body resistance to leukosis.

Contents of scvalene, sterols and their derivatives were studied in lymphoid tissues as well as in liver and kidney tissues of normal rats and the animals with Shvetz experimental leukosis. Amount of cholesterol and of its precursor scvalene was distinctly decreased simultaneously with an increase in total content of nonsaponified substances in thymus, spleen, bone marrow and liver tissue of rats with experimental leukosis. Scvalene was increased in kidney tissue. Simultaneous accumulation of hydroxycholesterol derivatives, among which 7-beta-hydroxycholesterol was identified, was observed in these tissues under conditions of leukosis. Role of cholesterol hydroxyderivatives in biosynthesis of sterols is discussed.

Ionizing radiation causes considerable changes in the content of sterols and squalene in the lymphoid system tissues and erythrocyte membranes which is in accordance with the concept of high radiosensitivity of haemopoietic tissue. The processes of cholesterol conversion to its oxy-derivatives are increased under the effect of radiation. The content of some lipid components in the lymphoid system tissues and erythrocyte membranes is changed depending on the dose and time after irradiation. There is a relationship between the changes in the sterol composition and in the properties of erythrocyte membranes.

[Isolation and identification of desmosterol and its concentration in animal brain tissue]. / Vydilennia ta identyfikatsiia desmosterynu i ioho vmist u tkanyni mozku tvaryn

A method is developed for isolating desmosterol fro the brain of some animals. Sterols with Rf 0.37 and 0.51 are identified. Desmosterol is found in the brain unsaponifiable fraction of calves, piggies, chickens and rats. Its content in the mentioned animals brain is compared. Desmosterol content is shown to rise sharply in the rat brain during the first 12 days after birth. Interrelation between the level of sterols in the brain and formation of the nerve tissue myelin sheath is discussed.

[Puncture drainage of abscess of the liver and abdominal cavity controlled by echotomoscopy and laparoscopy]. / Punktsionnoe drenirovanie abstsessov pecheni i briushnoi polosti pod kontrolem ekhotomoskopii i laparoskopii.

In 13 patients, the diagnosis of abscesses and subsequent puncture treatment were performed under echotomoscopic guidance, in 4--simultaneous laparoscopy was used. In 2 patients, the puncture method of treatment has proved to be ineffective, and they required the abscess opening. One female patient died from abscessed primary hepatic cancer.

Comparative data are presented on the effect of hypolipidemic agents on the content and composition of sterol-fibrinogen complexes of the blood in patients with ischemic heart disease. Inclusion of heparin, miscleron, complamin in the complex treatment was accompanied by a distinct reduction of the concentration of sterol-fibrinogen complexes, normalization of the ratio of polar sterols. The employment of miscleron and heparin furthered a reduction of the load of sterols on fibrinogen. The use of diosponin did not essentially effect the above-mentioned values.
