BACKGROUND: The main aspect of concern for upper GI procedures has been the retraction of the liver especially large left lobes as commonly encountered in Bariatric surgery. Not doing so would compromise the view of the hiatus, hence theoretically reducing the quality of the surgery and increasing the possibility of complications. The aim of this study was to review the various liver retraction techniques in single incision surgery being done at our institute and analyze them. MATERIAL AND METHODS: A retrospective study of the various techniques and a subsequent analysis was made based on advantages and disadvantages of each method. Objectively a quantitative measure of hiatal exposure was done using a scoring system based on the grade of exposure after reviewing the surgical videos. From January 2011 to January 2013 total 104 patients underwent single incision surgery with the various liver retraction techniques with following grades of exposure -liver suspension tube technique with naso gastric tubing (2.11) and with corrugated drain (2.09) needlescopic method (1.2), Umbilical tape sling (1.95), crural stitch method (2.5). Needeloscopic method has the best grade of exposure and is the easiest to start with. The average time to create the liver retraction was 2.8 to 8.6 min.There was no procedure related morbidity or mortality. CONCLUSIONS: The mentioned liver retraction techniques are cost effective and easy to learn. We recommend using these techniques to have a good exposure of hiatus, without compromising the safety of surgery in single incision surgery.

OBJECTIVE: Our objective was to evaluate the diagnostic accuracy and reliability of MRI and its ability to depict responsiveness to treatment for the evaluation of the axial joints (temporomandibular joint [TMJ], spinal joints, and sacroiliac joints) in juvenile idiopathic arthritis (JIA). CONCLUSION: There is fair (grade B) evidence that MRI is an accurate diagnostic method for evaluating early and intermediate changes in the TMJ in JIA and insufficient evidence to indicate MRI is an accurate diagnostic method for detecting JIA in the spinal (grade I) and sacroiliac (grade I) joints.

RATIONALE AND OBJECTIVES: The aim of this study was to determine whether resident performance in head ultrasound on neonates improves following brain phantom simulation training. MATERIALS AND METHODS: Ten junior radiology residents with at least one year of radiology training were divided into two equal groups. Both groups received a detailed head ultrasound protocol sheet and observed a technologist perform a head ultrasound on a neonatal patient at the beginning of their first pediatric radiology rotation. Both groups of residents also received teaching with a brain phantom model. Group A residents independently performed one head ultrasound exam, subsequently received phantom simulation training, and then performed a post-training head ultrasound exam. Group B residents received phantom simulation training prior to their first head ultrasound exam. Three pediatric radiologists independently and blindly reviewed the ultrasound images of each head ultrasound exam for proficiency of image acquisition using a validated scoring system. Scores of Group A residents prior to phantom training were compared to their scores after phantom training as well as to scores of Group B residents using simple linear regression. RESULTS: There was a statistically significant improvement in the performance of head ultrasound on neonates when comparing the same residents pre- and postphantom training (p = 0.003). Residents who initially trained with the phantom performed significantly better on their first head ultrasound examination on a neonate than those residents who did not (p = 0.005). CONCLUSION: Our novel head ultrasound phantom training model significantly improves radiology resident performance of head ultrasound on neonates and may, therefore, be beneficial for residency education.

Magnetic resonance cholangiopancreatography (MRCP) has not been assessed as a surrogate biomarker in pediatrics. We aimed to determine the inter-rater reliability, prognostic utility, and construct validity of the modified Majoie endoscopic retrograde cholangiopancreatography classification applied to MRCP in a pediatric primary sclerosing cholangitis (PSC) cohort. This single-center, retrospective, cohort study included children with PSC undergoing diagnostic MRCP between 2008 and 2016. Six variations of the Majoie classification were examined: 1) intrahepatic duct (IHD) score, 2) extrahepatic duct (EHD) score (representing the worst intrahepatic and extrahepatic regions, respectively), 3) sum IHD-EHD score, 4) average IHD score, 5) average EHD score, and 6) sum average IHD-EHD score. Inter-rater reliability was assessed using weighted kappas and intraclass correlation coefficients (ICCs). Ability to predict time to PSC-related complications (ascites, esophageal varices, variceal bleed, liver transplant [LT], or cholangiocarcinoma) (primary outcome) and LT (secondary outcome) was assessed with Harrell's concordance statistic (c-statistic) and univariate/multivariable survival analysis. Construct validity was further assessed with Spearman correlations. Forty-five children were included (67% boys; median, 13.6 years). The inter-rater reliability of MRCP scores was substantial to excellent (kappas/ICCs, 0.78-0.82). The sum IHD-EHD score had the best predictive ability for time to PSC complication and LT (c-statistic, 0.80 and SE, 0.06; and c-statistic, 0.97 and SE, 0.01, respectively). Higher MRCP scores were independently associated with a higher rate of PSC-related complications, even after adjusting for the PSC Mayo risk score (hazard ratio, 1.74; 95% confidence interval, 1.14-2.). MRCP sum scores correlated significantly with METAVIR fibrosis stage, total bilirubin, and platelets (r = 0.42, r = 0.33, r = -0.31, respectively; P < 0.05). Conclusion: An MRCP score incorporating the worst affected intrahepatic and extrahepatic regions is reliable and predicts meaningful outcomes in pediatric PSC. Next steps include prospective validation and responsiveness assessment.

Here, we discuss an unusual case of a pararectal tail gut cyst, initially misdiagnosed to be an ischiorectal abscess which was presented to us after being operated for incision and drainage. It was excised by a laparoscopic assisted approach. In such cases, a digital examination, transrectal ultrasound, CT scan, or MRI can help in the diagnosis. A combined approach is useful to locate and remove the cyst intact; however, a lower approach is useful for low-lying lesions. Histopathology can differentiate between a rectal duplication cyst and a tail gut cyst.

The destabilization of endothelial nitric-oxide synthase (eNOS) mRNA in hypoxic endothelial cells may be important in the etiology of vascular diseases, such as pulmonary hypertension. Recently, an overlapping antisense transcript to eNOS/NOS3 was implicated in the post-transcriptional regulation of eNOS. We demonstrate here that expression of sONE, also known as eNOS antisense (NOS3AS) or autophagy 9-like 2 (APG9L2), is robustly induced by hypoxia or functional deficiency of von Hippel-Lindau protein. sONE is also up-regulated in the aortas of hypoxic rats. In hypoxic endothelial cells, sONE expression negatively correlates with eNOS expression. Blocking the hypoxic induction of sONE by RNA interference attenuates the fall in both eNOS RNA and protein. We provide evidence that the induction of sONE primarily involves transcript stabilization rather than increased transcriptional activity and is von Hippel-Lindaubut not hypoxia-inducible factor 2alpha-dependent. We also demonstrate that sONE transcripts are enriched in the nucleus of normoxic cells and that hypoxia promotes an increase in the level of cytoplasmic and polyribosome-associated, sONE mRNA. The finding that eNOS expression can be regulated by an overlapping cis-antisense transcript in a stimulus-dependent fashion provides evidence that sense/antisense interactions may play a previously unappreciated role in vascular disease pathogenesis.

The Dicer enzyme is a key component of the RNA interference pathway and also responsible for the processing of micro RNAs, non-coding RNA molecules which regulate the activity of mRNAs by antisense base pairing. Little is known about the structure and regulation of human Dicer mRNA. A comprehensive characterization of Dicer 5'-untranslated region (5'-UTR) RNA structure revealed important diversity within human Dicer mRNA transcripts. Three exon 1 variants were defined, some of which exhibited very restricted patterns of tissue distribution. A number of alternatively spliced 5'-leader exons were also noted, revealing the potential for complex post-transcriptional regulation. Surprisingly, this diversity all occurred within the 5'-UTR of Dicer mRNAs and did not affect the coding region. The Dicer mRNA 5'-UTR variants had profound effects on translational efficiency both in vitro and in transiently transfected cells. A number of major Dicer RNA species are inefficient substrates for the translational machinery.
