RESUMO

Groundwater drawn daily from shallow alluvial sands by millions of wells over large areas of south and southeast Asia exposes an estimated population of over a hundred million people to toxic levels of arsenic. Holocene aquifers are the source of widespread arsenic poisoning across the region. In contrast, Pleistocene sands deposited in this region more than 12,000 years ago mostly do not host groundwater with high levels of arsenic. Pleistocene aquifers are increasingly used as a safe source of drinking water and it is therefore important to understand under what conditions low levels of arsenic can be maintained. Here we reconstruct the initial phase of contamination of a Pleistocene aquifer near Hanoi, Vietnam. We demonstrate that changes in groundwater flow conditions and the redox state of the aquifer sands induced by groundwater pumping caused the lateral intrusion of arsenic contamination more than 120 metres from a Holocene aquifer into a previously uncontaminated Pleistocene aquifer. We also find that arsenic adsorbs onto the aquifer sands and that there is a 16-20-fold retardation in the extent of the contamination relative to the reconstructed lateral movement of groundwater over the same period. Our findings suggest that arsenic contamination of Pleistocene aquifers in south and southeast Asia as a consequence of increasing levels of groundwater pumping may have been delayed by the retardation of arsenic transport.

BACKGROUND: As an important quality measure, the rates of recommended immunizations among immunocompromised inflammatory bowel disease (IBD) patients in community practice have not been well studied. AIMS: This study sought to investigate the rates and predictors of recommended immunizations and screening tests among IBD patients receiving anti-tumor necrosis factor (TNF) therapy in a large integrated healthcare organization. METHODS: We conducted a retrospective cohort study of 1401 IBD patients on anti-TNF therapy between 2010 and 2013 within the Kaiser Permanente Northern California healthcare system. The rates of vaccinations and screening tests were quantified, and the associated predictors were investigated. RESULTS: Vaccination rates for influenza and pneumococcus were 43.5 and 24.1%, respectively. The majority of patients (73.7%) received hepatitis B screening and/or vaccine. Patients receiving infliximab had higher rates of pneumococcal vaccine (P = 0.002), hepatitis B screening (P < 0.001), and tuberculin skin test (P < 0.001) compared with patients receiving adalimumab. Older patient age (≥50 years) was associated with higher likelihood of having HBsAg test (odds ratio [OR] 1.5, 95% confidence interval [CI] 1.2-2.0, P = 0.002), influenza vaccine (OR 2.6 [2.1-3.4], P < 0.001), and pneumococcal vaccine (OR 4.0 [3.0-5.3], P < 0.001). In contrast, older providers (≥50 years) were associated with significantly lower likelihood of their patients' having hepatitis A and B screening tests, and pneumococcal vaccination. CONCLUSIONS: The rates of immunizations for IBD patients receiving anti-TNF treatment were lower than recommended. Structured reminders for vaccinations and education for both patients and providers (older physicians in particular) may prove beneficial in improving immunization rates among immunocompromised IBD patients.

Here, we investigated the effects of the probiotic strain Lactobacillus casei Shirota (LcS) on immune profiles and intestinal microbial translocation among children infected with human immunodeficiency virus (HIV). This prospective study included 60 HIV-infected children-including 31 without antiretroviral therapy (ART) (HIV(+)) and 29 who received ART for a median of 3.5 years (ART(+)) and 20 children without HIV infection (HIV(-)). Participants were recruited in Vietnam. All children were given fermented milk containing LcS (6.5 × 108 cfu) daily for 8 weeks. Before and after LcS ingestion, blood samples were collected for virological, immunological, and bacteriological analyses. After LcS ingestion, peripheral CD4⁺ T-cell and Th2 (CXCR3-CCR6-CD4⁺) counts significantly increased in both HIV-infected groups; Th17 (CXCR3-CCR6⁺CD4⁺) counts increased in all three groups; regulatory T-cell (CD25highCD4⁺) counts decreased in the ART(+) and HIV(-) groups; activated CD8⁺ cells (CD38⁺HLA-DR⁺CD8⁺) decreased from 27.5% to 13.2% (p < 0.001) in HIV(+) children; and plasma HIV load decreased slightly but significantly among HIV(+) children. No group showed a significantly altered frequency of bacterial 16S/23S rRNA gene detection in the plasma. No serious adverse events occurred. These findings suggest that short-term LcS ingestion is a safe supportive approach with immunological and virological benefits in HIV-infected children.

CD4⁺ T-lymphocyte destruction, microbial translocation, and systemic immune activation are the main mechanisms of the pathogenesis of human immunodeficiency virus type 1 (HIV) infection. To investigate the impact of HIV infection and antiretroviral therapy (ART) on the immune profile of and microbial translocation in HIV-infected children, 60 HIV vertically infected children (31 without ART: HIV(+) and 29 with ART: ART(+)) and 20 HIV-uninfected children (HIV(-)) aged 2-12 years were recruited in Vietnam, and their blood samples were immunologically and bacteriologically analyzed. Among the HIV(+) children, the total CD4⁺-cell and their subset (type 1 helper T-cell (Th1)/Th2/Th17) counts were inversely correlated with age (all p < 0.05), whereas regulatory T-cell (Treg) counts and CD4/CD8 ratios had become lower, and the CD38⁺HLA (human leukocyte antigen)-DR⁺CD8⁺- (activated CD8⁺) cell percentage and plasma soluble CD14 (sCD14, a monocyte activation marker) levels had become higher than those of HIV(-) children by the age of 2 years; the CD4/CD8 ratio was inversely correlated with the plasma HIV RNA load and CD8⁺-cell activation status. Among the ART(+) children, the total CD4⁺-cell and Th2/Th17/Treg-subset counts and the CD4/CD8 ratio gradually increased, with estimated ART periods of normalization being 4.8-8.3 years, whereas Th1 counts and the CD8⁺-cell activation status normalized within 1 year of ART initiation. sCD14 levels remained high even after ART initiation. The detection frequency of bacterial 16S/23S ribosomal DNA/RNA in blood did not differ between HIV-infected and -uninfected children. Thus, in children, HIV infection caused a rapid decrease in Treg counts and the early activation of CD8⁺ cells and monocytes, and ART induced rapid Th1 recovery and early CD8⁺-cell activation normalization but had little effect on monocyte activation. The CD4/CD8 ratio could therefore be an additional marker for ART monitoring.

Arsenic contamination of shallow groundwater is among the biggest health threats in the developing world. Targeting uncontaminated deep aquifers is a popular mitigation option although its long-term impact remains unknown. Here we present the alarming results of a large-scale groundwater survey covering the entire Red River Delta and a unique probability model based on three-dimensional Quaternary geology. Our unprecedented dataset reveals that ∼7 million delta inhabitants use groundwater contaminated with toxic elements, including manganese, selenium, and barium. Depth-resolved probabilities and arsenic concentrations indicate drawdown of arsenic-enriched waters from Holocene aquifers to naturally uncontaminated Pleistocene aquifers as a result of > 100 years of groundwater abstraction. Vertical arsenic migration induced by large-scale pumping from deep aquifers has been discussed to occur elsewhere, but has never been shown to occur at the scale seen here. The present situation in the Red River Delta is a warning for other As-affected regions where groundwater is extensively pumped from uncontaminated aquifers underlying high arsenic aquifers or zones.

In this research, the analytical method was developed and evaluated for determining phenol and its nine derivatives belong to the US EPA priority pollutant list in water samples by using dual-channeled capillary electrophoresis (CE) coupled with two types of optical detectors, namely LED-induced fluorescence (LEDIF) and ultraviolet (UV) detectors. The optimal background electrolytes for the first and second CE channels were 20 mM borate (pH 9.80) with 400 µM fluorescein and 55 mM borate (pH 11.75), respectively. The two-step liquid-liquid extraction (LLE) was used for sample preparation and enrichment, in which phenol and its derivatives were extracted from the aqueous phase using 10 mL of n-hexane/1-octanol (60/40, v/v) and then were back extracted into a 0.1 M NaOH as a final acceptor phase. Under the optimal CE and two-step LLE conditions, the enrichment factors of 10 phenols were 184 - 1120-fold, and the method detection limits were lowered to 0.02-0.60 µg/L. The obtained intra-day and inter-day precisions in terms of relative standard deviations (RSD) were between 4.0 and 7.3 % and 6.7 and 14 %, respectively. This approach was used to determine phenols in water samples, with recoveries ranging from 82.0 to 108.9 %. In combination with sample enrichment by two-step LLE extraction, this is the first CE study conducted to determine phenols in the EPA list using two detector approaches, specifically CE-LEDIF/CE-UV.

Although Eichhornia crassipes, commonly known as water hyacinth, has been widely used in wastewater treatment, further investigations are still needed to explore the removal efficiency of perfluoroalkyl acids (PFAAs) from the aqueous environment using this floating aquatic plant. In this study, a hydroponic experiment was conducted to assess accumulation, bioconcentration factors (BCFs), translocation factors (TFs), and removal rates of eight PFAAs by water hyacinth. The obtained results indicated that all PFAAs, including five perfluoroalkyl carboxylic acids (PFCAs) with chain lengths C4-C8 and three perfluoroalkyl sulfonic acids (PFSAs) with C4, C6, and C8, were readily accumulated in water hyacinth. Throughout the duration of the experiment, there was a noticeable increase in PFAA concentrations and BCF values for different plant parts. For the root, PFAAs with more carbon numbers showed a higher uptake than the shorter homologues, with PFSAs being more readily accumulated compared to PFCAs with the same carbon number in the molecules. In contrast, the levels of long-chain PFAAs were comparatively lower than those of short-chain substances in the stem and leaf. Notably, PFAAs with less carbon numbers, like PFPeA, PFBA, and PFBS, showed a remarkable translocation from the root to the stem and leaf with TFs >1. For the whole plant, no significant correlation was found between BCFs and organic carbon-water partition coefficients (Koc), octanol-water partition coefficients (Kow), membrane-water distribution coefficients (Dmw), or protein-water distribution coefficients (Dpw). The removal rates of PFAAs ranged from 40.3 to 63.5 % throughout the three weeks of the experiment while the removal efficiencies varied from 48.9 % for PFHxS to 82.6 % for PFPeA in the last week.

A portable capillary electrophoresis instrument featuring an automated, robust, valve-based injection system was developed. This significantly facilitates operation in the field compared to previous injection approaches. These generally required delicate manual operations which are difficult to perform outside the laboratory environment. The novel system relies on pressurized air for solution delivery and a micromembrane pump for sample aspiration. Contactless conductivity detection was employed for its versatility and low power requirement. The instrument has a compact design, with all components arranged in a briefcase with dimensions of 45 × 35 × 15 cm (w × d × h) and a weight of about 8 kg. It can operate continuously for 9 h in the battery-powered mode. Depending on the task at hand, the injection system allows easy optimization for high separation efficiency, for fast separations, or for low limits of detection. To illustrate these features, the separation of four anions within 16 s is demonstrated as well as the determination of nitrite below 1 µM. The determination of phosphate at a sewage treatment plant was carried out to demonstrate a field application.

Vaccines against two high-risk human papillomavirus (HPV) types, HPV-16, and HPV-18, are in use currently, with high efficacy for preventing infections with these HPV types and consequent cervical cancers. However, circulating HPV types can vary with geography and ethnicity. The aim of this study was to investigate the prevalence of HPV types and the association between HPV types and abnormal cervical cytology among female sex workers in Northern Vietnam. Cervical swabs and plasma samples were collected from 281 female sex workers at two health centers in Hanoi and Hai Phong in 2009. The HPV L1 gene was amplified by PCR using original and modified GP5(+)/6(+) primers. Amplified PCR products were genotyped by the microarray system GeneSquare (KURABO) and/or clonal sequencing. Of the 281 women, 139 (49.5%) were positive for HPV DNA. Among the HPV-positive samples, 339 strains and 29 different types were identified. Multiple-type and high risk-type HPV infections were found in 85 (61.2%) and 124 (89.2%) women, respectively. The most common genotype was HPV-52, followed by HPV-16, HPV-18, and HPV-58. Abnormal cervical cytology was detected in 3.2% (9/281) of the women, and all of these samples were positive for HPV-DNA. Age ≤25 years and infection with human immunodeficiency virus were associated positively with HPV infection among the women while ever smoking was associated negatively. These results show that HPV-52 is most prevalent among female sex workers in Northern Vietnam, most of whom had normal cervical cytology. This information may be important for designing vaccination strategies in Vietnam.

Human papillomavirus (HPV) has several intragenotypic variants with different geographical and ethnic distributions. This study aimed to elucidate the distribution patterns of E6 and E7 (E6/E7) intragenotypic variants of HPV type 16 (HPV-16), which is most common worldwide, and HPV-52, which is common in Asian countries such as Japan, the Philippines, and Vietnam. In previous studies, genomic DNA samples extracted from cervical swabs were collected from female sex workers in these three countries and found to be positive for HPV-16 or HPV-52. Samples were amplified further for their E6/E7 genes using type-specific primers and analyzed genetically. Seventy-nine HPV-16 E6/E7 genes were analyzed successfully and grouped into three lineages: European (Prototype), European (Asian), and African-2. The prevalences of HPV-16 European (Prototype)/European (Asian) lineages were 19.4%/80.6% (n = 31) in Japan, 75.0%/20.8% (n = 24) in the Philippines, and 0%/95.8% (n = 24) in Vietnam. The 109 HPV-52 E6/E7 genes analyzed successfully were grouped into four lineages, A-D; the prevalences of lineages A/B/C/D were, respectively, 5.1%/92.3%/0%/2.6% in Japan (n = 39), 34.4%/62.5%/0%/3.1% in the Philippines (n = 32), and 15.8%/73.7%/7.9%/2.6% in Vietnam (n = 38). The distribution patterns of HPV-16 and HPV-52 lineages in these countries differed significantly (P < 0.000001 and P = 0.0048, respectively). There was no significant relationship between abnormal cervical cytology and either HPV-16 E6/E7 lineages or specific amino acid mutations, such as E6 D25E, E6 L83V, and E7 N29S. Analysis of HPV-16 and HPV-52 E6/E7 genes can be a useful molecular-epidemiological tool to distinguish geographical diffusion routes of these HPV types in Asia.

BACKGROUND: This study aimed to evaluate the effectiveness of gancyclovir (GCV) treatment for severe cytomegalovirus (CMV)-associated pneumonia in immunocompetent children. METHOD: We enrolled patients with CMV-associated severe pneumonia admitted to the Vietnam National Hospital of Pediatrics, Hanoi, Vietnam, from January 2010 to December 2011. On admission, though respiratory bacteria and viruses were not detected in tracheal aspirates, more than 5 × 10(3) copies/mL of CMV-DNA were detected in both tracheal aspirates and in blood plasma. GCV was given intravenously at a dose of 10 mg/kg/24 h for a duration of 14 days at most. The dose was then reduced to 5 mg/kg/24 h until CMV-DNA was not detected in plasma. The main study variables included clinical symptoms, complete blood count, hepatic and renal function, chest X-ray, CMV viral load, duration of GCV treatment and outcome. RESULTS: Forty-three patients were enrolled in the study. The median age of patients was 57 (interquartile range [IQR] 45-85) days. Clinical and laboratory findings included anemia (67.4%), leukocytosis (90.7%), hepatosplenomegaly (60.5%), elevated liver enzymes (74.4%), decreased ratio of CD4: CD8-positive T lymphocytes (69.4%), and decreased serum IgG concentration (25.7%). The median duration of GCV treatment was 12 days (IQR 7-21). Thirty-seven patients (86.0%) showed normal chest X-rays at the end of treatment. One infant died (2.3%); the other children (97.7%) were discharged in good condition. There was no severe toxicity associated with GCV treatment. CONCLUSION: GCV is safe and effective for the treatment of severe CMV-associated pneumonia in children.

In this study, two analytical procedures were developed and validated using dual-channel capillary electrophoresis-coupled contactless conductivity detection (CE-C4D) followed by solid-phase extraction (SPE) for simultaneous determination of glyphosate (GLYP), glufosinate (GLUF), and their two major metabolites, aminomethylphosphonic acid (AMPA) and 3-(methylphosphinico) propionic acid (MPPA), respectively, in a popular beverage such as tea infusions. GLYP, GLUF, and AMPA were analyzed in the first channel using background electrolyte (BGE) of 1 mM histidine (His) adjusted to pH 2.75 by acetic acid (Ace). In contrast, MPPA was quantified in the second channel with a BGE of 30 mM His adjusted to pH 6.7 by 3-(N-morpholino) propanesulfonic acid (MOPS) and 10 µM of cetyltrimethylammonium bromide (CTAB). In addition, the samples of tea infusions were treated using SPE with 10 mL of 0.5 mM HCl in methanol as eluent. At the optimized conditions, the method detection limit (MDL) of GLYP, GLUF, AMPA, and MPPA is 0.80, 1.56, 0.56, and 0.54 µg/l, respectively. The methods were then applied to analyze four target compounds in 16 samples of tea infusions. GLYP was found in two infusion samples of oolong tea with concentrations ranging from 5.34 to 10.74 µg/L, and GLUF was recognized in three samples of green tea infusion in the range of 45.1-53.9 µg/L.

This study focuses on determination of quantity, shape, size and polymer types of microplastics in Pacific oysters (Crassostrea gigas) cultured in Danang Bay, Vietnam. Visual verification and chemical identification of microplastics was carried out by µFT-IR spectra using Nicolet iN10 MX Infrared Imaging Microscope. The average abundance of microplastic in oysters was 1.88 ± 1.58 particles/g (wet weight) and 18.54 ± 10.08 particles/individual. The prevalent shape of microplastics included three main forms as fragment, fiber and bead corresponding to 73.71; 25.84 and 0.45%, respectively. Additionally, the size of microplastics was a range of 22.4-1318.8 µm, and the most common size was less than 100 µm, accounted for 77.30%. Fifteen polymer types were detected while Nylon was the most abundant polymer type with 50.56%. In this study, microplastics accumulation with their abundance and polymeric structures could be considered as reliable evidence for further studies on assessment of their potential risk to human health.

Residue concentrations of fifteen antibiotics including sulfonamides, quinolones, macrolides, ß-lactams, and trimethoprim in lakes from Hanoi metropolitan area, Vietnam, were analyzed using ultra-performance liquid chromatography coupled with tandem mass spectrometry (UPLC/MS-MS) to elucidate their occurrence and behavior in urban environment. For surface water, the average concentrations of five antibiotic classes decreased in the order: sulfonamides (117.9 ng/L) > ß-lactams (31.28 ng/L) > quinolones (20.19 ng/L) > macrolides (17.74 ng/L) > trimethoprim (8.93 ng/L). While the highest concentration of SMX was detected at 806.5 ng/L in surface water, those obtained in sediment were only at 1.35 ng/g because of their high solubility in water. Quinolones were found at a maximal concentration of 158.7 ng/L for OFL in water phase whereas those in sediment phase were 4,017 ng/g due to their great affinity in sediment. These findings revealed the different fate and release mechanisms of each antibiotic group in the environment. The ecological risk assessment implied some targeted compounds, and in particular, OFL and AZM could pose high risks to algae in the aquatic ecosystem.

The ethylacetate extracts produced from the leaves of Stixis suaveolens (Roxb.) was characterized on the basis of NMR spectra combined with extensive mass spectroscopic techniques. The chemical characterization revealed presence of two new phenolic amides which were named as stixilamides A and B.

Per- and polyfluoroalkyl substances (PFASs) have attracted great concern because of their great recalcitrant nature and harmful environmental health effects. Eight PFASs in wastewater from craft villages and industrial environments of Vietnam were analyzed using liquid chromatography triple quadrupole mass spectrometry (LC-MS/MS) with negative electrospray ionization interface. For analysis of PFASs, percent recoveries ranged from 87 to 112, and MQL varied from 0.19 ng/L to 0.49 ng/L. Treated wastewater samples from eight metal-plating and eight textile-dyeing factories were collected for analysis of PFASs. Concentrations of PFOS in wastewater samples obtained from metal-plating factories with decorative plating stage were found at a range of 0.73-18.91 ng/L. For textile-dyeing factories, PFOA and/or PFHxA, which were present in all effluent wastewater samples, varied from 0.37 to 15.96 ng/L and 1.07 to 43.58 ng/L, respectively. Sixty surface water samples in four locations of the textile dyeing craft villages, a recycling plastic village, a paper recycling village, and 10 river water samples in the control area (a rural area without specific waste sources) were collected and analyzed for PFASs. The total concentrations of eight PFASs in surface water samples of craft villages ranged from 0.83 to 58.2 ng/L, which were significantly higher than those in the control area. PFOA, PFHxA, and PFOS are the three most dominant congeners in wastewater taken from craft villages with the highest concentrations of 27.4, 23.8, and 7.36 ng/L, respectively. The environmental risks posed by PFASs in surface water from craft villages were mainly in a range of extremely low to low level, particularly a few points have high ecological risks of PFDoA.

OBJECTIVE: We investigated the impact of human immunodeficiency virus (HIV) infection and anti-retroviral therapy (ART) on the gut microbiota of children. DESIGN: This cross-sectional study investigated the gut microbiota of children with and without HIV. METHODS: We collected fecal samples from 59 children with HIV (29 treated with ART [ART(+)] and 30 without ART [HIV(+)]) and 20 children without HIV [HIV(-)] in Vietnam. We performed quantitative RT-PCR to detect 14 representative intestinal bacteria targeting 16S/23S rRNA molecules. We also collected the blood samples for immunological analyses. RESULTS: In spearman's correlation analyses, no significant correlation between the number of dominant bacteria and age was found among children in the HIV(-) group. However, the number of sub-dominant bacteria, including Streptococcus, Enterococcus, and Enterobacteriaceae, positively correlated with age in the HIV(-) group, but not in the HIV(+) group. In the HIV(+) group, Clostridium coccoides group positively associated with the CD4+ cell count and its subsets. In the ART(+) group, Staphylococcus and C. perfringens positively correlated with CD4+ cells and their subsets and negatively with activated CD8+ cells. C. coccoides group and Bacteroides fragilis group were associated with regulatory T-cell counts. In multiple linear regression analyses, ART duration was independently associated with the number of C. perfringens, and Th17 cell count with the number of Staphylococcus in the ART(+) group. CONCLUSIONS: HIV infection and ART may influence sub-dominant gut bacteria, directly or indirectly, in association with immune status in children with HIV.

This study reports on the development of a novel instrument for capillary electrophoresis (CE) coupled with laser induced fluorescence (LIF) detection that is inspired by the Lego-toy concept. The Lego CE-LIF design is an evolution of purpose-made CE instrumentation, allowing the users to construct their own analytical device with a high degree of standardization (i.e. a "standard" setup) without requirement of mechanical and electronic workshop facilities. To allow instrument reproduction outside the original fabrication laboratory, which is not trivial for in-house-built CE systems, the new design is based on unprecedent 'plugging' hyphenation of various off-the-shelf parts available for microfluidics, optics and electrophoresis. To render the operation with Lego CE-LIF optimal, we developed a new background electrolyte (BGE), using for the first time extremely high concentrations of zwitterionic and large weakly charged species for much improvement of detection sensitivity. The Lego CE-LIF was demonstrated for separation and detection of oligosaccharides labelled with 8-aminopyrene-1,3,6-trisulfonic acid (APTS). The new gel-free BGE for oligosaccharide analysis also allowed simplification of the conventional CE-LIF protocol used with commercial instruments while keeping satisfactory separation performances. Furthermore, the new BGE is fully compatible with a non-thermostatted Lego CE instrument thanks to low current and therefore low heat generation under application of a high voltage.

BACKGROUND: Gag protein of human immunodeficiency virus (HIV) has been reported to play a crucial role in establishing infection, viral replication, and disease progression; thus, gag might be related to treatment response. The objective of this study was to investigate molecular genotypes of the gag gene, particularly the important functional binding domains in relation to treatment outcomes. METHODS: HIV-infected children enrolled and treated at Vietnam National Children's Hospital were recruited in the study. A total of 25 gag sequences were generated and used to construct phylogenetic trees and aligned with a reference sequence comparing 17 functional domains. RESULTS: We found that all patients in a treatment failure (TF) group belonged to one cluster of the phylogenetic tree. In addition, the rate of mutations was significantly higher in TF compared with a treatment success (TS) group, specifically the PIP2 recognition motif, and the nucleocapsid basic and zinc motif 2 domains [median and (interquartile range (IQR): 12.5 (6.25-12.5) versus 50 (25-50), p < 0.01; 0 (0-0) versus 0 (0-21.43), p = 0.03 and 0 (0-7.14) versus 7.14 (7.14-7.14), p = 0.04, respectively]. When analyzing gag sequences at different time points in seven patients, we did not observe a consistent mutation pattern related to treatment response. CONCLUSION: Gag mutations in certain domains might be associated with increased viral load; therefore, studying the molecular genotype of the gag gene might be beneficial in monitoring treatment response in HIV-infected children.

Iron oxides control the mobility of a host of contaminants in aquifer systems, and the microbial reduction of iron oxides in the subsurface is linked to high levels of arsenic in groundwater that affects greater than 150 million people globally. Paired observations of groundwater and solid-phase aquifer composition are critical to understand spatial and temporal trends in contamination and effectively manage changing water resources, yet field-representative mineralogical data are sparse across redox gradients relevant to arsenic contamination. We characterize iron mineralogy using X-ray absorption spectroscopy across a natural gradient of groundwater arsenic contamination in Vietnam. Hierarchical cluster analysis classifies sediments into meaningful groups delineating weathering and redox changes, diagnostic of depositional history, in this first direct characterization of redox transformations in the field. Notably, these groupings reveal a signature of iron minerals undergoing active reduction before the onset of arsenic contamination in groundwater. Pleistocene sediments undergoing postdepositional reduction may be more extensive than previously recognized due to previous misclassification. By upscaling to similar environments in South and Southeast Asia via multinomial logistic regression modeling, we show that active iron reduction, and therefore susceptibility to future arsenic contamination, is more widely distributed in presumably pristine aquifers than anticipated.
