BACKGROUND: Ultrashort echo time (UTE) T2* is sensitive to molecular changes within the deep calcified layer of cartilage. Feasibility of its use in the hip needs to be established to determine suitability for clinical use. PURPOSE: To establish feasibility of UTE T2* cartilage mapping in the hip and determine if differences in regional values exist. MATERIAL AND METHODS: MRI scans with UTE T2* cartilage maps were prospectively acquired on eight hips. Hip cartilage was segmented into whole and deep layers in anterosuperior, superior, and posterosuperior regions. Quantitative UTE T2* maps were analyzed (independent one-way ANOVA) and reliability was calculated (ICC). RESULTS: UTE T2* mean values (anterosuperior, superior, posterosuperior): full femoral layer (19.55, 18.43, 16.84 ms) (P=0.004), full acetabular layer (19.37, 17.50, 16.73 ms) (P=0.013), deep femoral layer (18.68, 17.90, 15.74 ms) (P=0.010), and deep acetabular layer (17.81, 16.18, 15.31 ms) (P=0.007). Values were higher in anterosuperior compared to posterosuperior regions (mean difference; 95% confidence interval [CI]): full femur layer (2.71 ms; 95% CI 0.91-4.51: P=0.003), deep femur layer (2.94 ms; 95% CI 0.69-5.19; P=0.009), full acetabular layer (2.63 ms 95% CI 0.55-4.72; P=0.012), and deep acetabular layer (2.50 ms; 95% CI 0.69-4.30; P=0.006). Intra-reader (ICC 0.89-0.99) and inter-reader reliability (ICC 0.63-0.96) were good to excellent for the majority of cartilage layers. CONCLUSION: UTE T2* cartilage mapping was feasible in the hip with mean values in the range of 16.84-19.55 ms in the femur and 16.73-19.37 ms in the acetabulum. Significantly higher values were present in the anterosuperior region compared to the posterosuperior region.

PURPOSE: Assess feasibility of ultrashort echo time (UTE) T2* cartilage mapping in sacroiliac (SI) joints. METHODS: Prospective magnetic resonance imagings with UTE T2* cartilage maps obtained on 20 SI joints in 10 subjects. Each joint was segmented into thirds by 2 radiologists. The UTE T2* maps were analyzed; reliability and differences in UTE T2* values between radiologists were assessed. RESULTS: Mean UTE T2* value was 10.44 ± 0.60 ms. No difference between right/left SI joints (median, 10.52 vs 10.45 ms; P = 0.940), men/women (median, 10.34 vs. 10.57 ms; P = 0.174), or different anatomic regions (median range 10.55-10.69 ms; P = 0.805). Intraclass correlation coefficients were 0.94 to 0.99 (intraobserver) and 0.91 to 0.96 (interobserver). Mean bias ± standard deviation on Bland-Altman was -0.137 ± 0.196 ms (limits of agreement -0.521 and 0.247) without proportional bias (ß = 0.148, P = 0.534). CONCLUSIONS: The UTE T2* cartilage mapping in the SI joints is feasible with high reader reliability.

OBJECTIVE: To assess MR features following MPFL reconstruction and determine their influence on post-operative pain, progressive arthritis, or graft failure. MATERIALS AND METHODS: Retrospective study on 38 patients with MPFL reconstruction and a post-operative MRI between January 2010 and June 2019. Two radiologists assessed MPFL graft signal, graft thickness, femoral screw, femoral tunnel widening, and patellofemoral cartilage damage. The third performed patellofemoral instability measurements. All three assessed femoral tunnel position with final result determined by majority consensus. Imaging findings were evaluated in the setting of post-operative pain, patellofemoral arthritis, and MPFL graft failure including need for MPFL revision. Statistics included chi-square, Fisher's exact test, t test, and kappa. RESULTS: Mean graft thickness was 6.0 ± 1.8 mm; 24% of the grafts were diffusely hypointense. Mean femoral tunnel widening was 2.5 ± 1.8 mm; 34% of the femoral screws were broken or extruded. Fifty-two percent of the patients had no interval cartilage change. Non-anatomic femoral tunnels were found in 66% of patients, including in all 9 patients requiring revision MPFL reconstruction (p = 0.013). Revised MPFL grafts had more abnormal femoral screws compared to those that did not (67% vs. 24%) (p = 0.019). Other MR features did not significantly influence the evaluated outcomes. CONCLUSION: The need for revision MPFL reconstruction occurs more frequently when there is a non-anatomic femoral tunnel and broken or extruded femoral screws. The appearance of the MPFL graft itself is not an influencing factor for post-operative pain, progression of patellofemoral arthritis, or graft failure.

OBJECTIVE: To determine the prevalence of athletic pubalgia imaging findings on MRI in patients with femoroacetabular impingement and assess for correlative risk factors. MATERIALS AND METHODS: A retrospective search identified 156 hips with femoroacetabular impingement and a control group of 113 without femoroacetabular impingement that had an MRI performed between January 1, 2015, and January 1, 2018. Two fellowship-trained musculoskeletal radiologists reviewed studies for the presence of acute osteitis pubis, chronic osteitis pubis, adductor tendinosis, and tendon tear; rectus abdominis tendinosis and tendon tear; and aponeurotic plate tear. Findings were correlated with various clinical and imaging risk factors. Univariate and multivariate statistical analyses were performed. RESULTS: Imaging findings of adductor tendinosis (p = 0.02) and chronic osteitis pubis (p = 0.01) were more prevalent in FAI patients than controls. Univariate analyses in FAI patients showed that an alpha angle ≥ 60° had a higher prevalence of aponeurotic plate tears (p = 0.02) and adductor tendinosis (p = 0.049). Multivariate analyses showed that an alpha angle ≥ 60° had a higher prevalence of chronic osteitis pubis (OR = 2.27, p = 0.031), sports participation had a higher prevalence of adductor tendon tears (OR = 4.69, p = 0.013) and chronic osteitis pubis (OR = 2.61, p = 0.0058), and males had a higher prevalence of acute osteitis pubis (OR = 5.17, p = 0.032). CONCLUSION: Sports participation, alpha angle ≥ 60°, and male sex predict a higher prevalence of athletic pubalgia imaging findings in patients with femoroacetabular impingement.

To use deep learning with advanced data augmentation to accurately diagnose and classify femoral neck fractures. A retrospective study of patients with femoral neck fractures was performed. One thousand sixty-three AP hip radiographs were obtained from 550 patients. Ground truth labels of Garden fracture classification were applied as follows: (1) 127 Garden I and II fracture radiographs, (2) 610 Garden III and IV fracture radiographs, and (3) 326 normal hip radiographs. After localization by an initial network, a second CNN classified the images as Garden I/II fracture, Garden III/IV fracture, or no fracture. Advanced data augmentation techniques expanded the training set: (1) generative adversarial network (GAN); (2) digitally reconstructed radiographs (DRRs) from preoperative hip CT scans. In all, 9063 images, real and generated, were available for training and testing. A deep neural network was designed and tuned based on a 20% validation group. A holdout test dataset consisted of 105 real images, 35 in each class. Two class prediction of fracture versus no fracture (AUC 0.92): accuracy 92.3%, sensitivity 0.91, specificity 0.93, PPV 0.96, NPV 0.86. Three class prediction of Garden I/II, Garden III/IV, or normal (AUC 0.96): accuracy 86.0%, sensitivity 0.79, specificity 0.90, PPV 0.80, NPV 0.90. Without any advanced augmentation, the AUC for two-class prediction was 0.80. With DRR as the only advanced augmentation, AUC was 0.91 and with GAN only AUC was 0.87. GANs and DRRs can be used to improve the accuracy of a tool to diagnose and classify femoral neck fractures.

OBJECTIVE. The objective of this study was to determine how use of analytics-driven worklists for MRI based on relative individual interpretation time affects the overall group interpretation time in an academic musculoskeletal practice. SUBJECTS AND METHODS. In this prospective study, interpretation times for all MRI studies signed by three musculoskeletal fellowship-trained radiologists during 2016 were calculated from initial study view and report signing times. Custom worklists were made for each radiologist with body parts ordered from the fastest to the slowest based on relative interpretation time. These worklists were then used for a trial period of 7 consecutive days. The difference in mean interpretation times between the trial period and baseline and the differences in volume distribution were calculated. Changes in individual interpretation time were assessed by z-score with statistical significance set at ≤ 0.05. RESULTS. Across all readers, total interpretation time decreased by a mean of 29.5 minutes per day during the trial period. Only two types of studies were read with an individual interpretation time significantly different from baseline (wrist studies for reader 1 were 10 minutes slower [p = 0.01] and cervical spine studies for reader 3 were 9 minutes faster [p < 0.01]). Volume distributions changed across various body parts (-3% to 4% for reader 1, -13% to 14% for reader 2, and -24% to 10% for reader 3). CONCLUSION. Analytics-driven worklists for MRI may decrease overall group interpretation time without significant alteration in individual speed, though a change in volume distribution is required.

OBJECTIVE: The objective of our study was to compare anterior inferior iliac spine (AIIS) morphology in symptomatic hips with femoroacetabular impingement (FAI) and in asymptomatic hips, determine the prevalence of impingement morphology in patients with a radiographic "crossover" sign, and identify potential risk factors for having impingement morphology. MATERIALS AND METHODS: For this retrospective study, we identified consecutive symptomatic hips with FAI (n = 54) and asymptomatic hips (n = 35) in patients who underwent CT from 2015 to 2017. Two radiologists blindly and independently evaluated 3D CT images of each hip and graded the AIIS morphology according to the Hetsroni classification scheme. The prevalence of AIIS morphology types was calculated. Associations of AIIS morphology types with symptoms and the crossover sign were evaluated with a chi-square test. A multivariable logistic regression determined risk factors for abnormal AIIS morphology (i.e., type 2 or 3). RESULTS: There was no difference in the prevalence of AIIS morphology types for symptomatic hips with FAI versus asymptomatic hips (p = 0.44) or for hips with a positive versus those with a negative crossover sign (p = 0.21). There was moderate interobserver agreement (κ = 0.44) and good-to-excellent intraobserver agreement (κ = 0.67 and 0.90) for grading AIIS morphology. Age, sex, femoral version, acetabular version, alpha angle, lateral center edge angle, and the crossover sign were not significant risk factors for abnormal AIIS morphology in patients with FAI (p = 0.11-0.79). CONCLUSION: There is no difference in AIIS morphology between symptomatic hips with FAI versus asymptomatic hips or between hips with and those without the radiographic crossover sign. Age, sex, and other FAI parameters are not risk factors for developing AIIS impingement morphology.

OBJECTIVE: To determine if a Bennett lesion and its size are associated with additional MRI shoulder abnormalities in an overhead athlete. MATERIALS AND METHODS: An IRB-approved retrospective review of our database from 1 January 2012 to 1 April 2018 identified 35 overhead athletes with a Bennett lesion on MRI. A control group consisting of 35 overhead athletes without a Bennett lesion were matched for age, level of play (professional vs non-professional), and type of study (arthrogram vs non-arthrogram). Each study was assessed independently by two MSK fellowship trained radiologists. The sizes of the Bennett lesions were measured. Each MRI was assessed for the presence of a labral tear, posterior glenoid cartilage abnormality, humeral head notching or cysts, and fraying or tear of the supraspinatus or infraspinatus tendons. Statistical analyses were performed using Student's t test, Fisher's exact test, and Chi-squared test. RESULTS: There was an increased incidence of posterior glenoid cartilage abnormalities in athletes with Bennett lesions vs those without (23% vs 3%, p value = 0.01). There was no difference in any other MRI abnormalities, including labral tears and findings of internal impingement between these two groups (p value range = 0.09-0.46). There was no association between the size of a Bennett lesion and the presence of glenoid cartilage lesions, labral tears, internal impingement, age, professional status, or need for surgery (p value range = 0.08-0.96). CONCLUSION: Symptomatic overhead athletes with Bennett lesions have an increased frequency of posterior glenoid cartilage abnormalities, but not labral tears or findings of internal impingement compared to those without Bennett lesions.

Deep learning for MRI detection of sports injuries poses unique challenges. To address these difficulties, this study examines the feasibility and incremental benefit of several customized network architectures in evaluation of complete anterior cruciate ligament (ACL) tears. Two hundred sixty patients, ages 18-40, were identified in a retrospective review of knee MRIs obtained from September 2013 to March 2016. Half of the cases demonstrated a complete ACL tear (624 slices), the other half a normal ACL (3520 slices). Two hundred cases were used for training and validation, and the remaining 60 cases as an independent test set. For each exam with an ACL tear, coronal proton density non-fat suppressed sequence was manually annotated to delineate: (1) a bounding-box around the cruciate ligaments; (2) slices containing the tear. Multiple convolutional neural network (CNN) architectures were implemented including variations in input field-of-view and dimensionality. For single-slice CNN architectures, validation accuracy of a dynamic patch-based sampling algorithm (0.765) outperformed both cropped slice (0.720) and full slice (0.680) strategies. Using the dynamic patch-based sampling algorithm as a baseline, a five-slice CNN input (0.915) outperformed both three-slice (0.865) and single-slice (0.765) inputs. The final highest performing five-slice dynamic patch-based sampling algorithm resulted in independent test set AUC, sensitivity, specificity, PPV, and NPV of 0.971, 0.967, 1.00, 0.938, and 1.00. A customized 3D deep learning architecture based on dynamic patch-based sampling demonstrates high performance in detection of complete ACL tears with over 96% test set accuracy. A cropped field-of-view and 3D inputs are critical for high algorithm performance.

BACKGROUND: The purpose of this study was to determine the utility of 18F-FDG-PET for evaluating the presence and the extent of malignant peritoneal mesothelioma (MPM), for disease surveillance/recurrence detection and for evaluating response to therapy. METHODS: We retrospectively analyzed clinical and imaging data of 60 MPM patients (34 women and 26 men, mean age 53.6 y, range 18-80 y) who had multiple 18F-FDG-PET/CT or PET scans (18F-FDG scans) at various stages of the disease. RESULTS: Eleven patients had baseline pretreatment scans and all 11 scans showed 18F-FDG avid diffuse, nodular or mixed disease distribution patterns characteristic of MPM. Four patients out of eleven had an early post-treatment 18F-FDG scan (<6 months) and all scans were accurate in determining response to treatment. Forty-nine patients with a history of treated MPM without baseline scans had multiple disease surveillance 18F-FDG scans. Their initial 18F-FDG scans had an accuracy of 82% and positive predictive value of 83% and negative predictive value of 80% for the detection of disease presence and disease-free state, respectively. For fifteen patients with a true negative 18F-FDG scan, a second follow-up scan accurately detected disease recurrence or absence of recurrence in all cases. Metastatic or remote nodal disease was more common in the biphasic histopathologic subtype group while pleural disease was predominantly seen in the epithelial MPM group. No relationship was found between the uptake pattern and the histopathologic subtype. CONCLUSION: 18F-FDG-PET is a valuable imaging modality in the pre-surgical evaluation and management of MPM and further prospective studies are warranted.

BACKGROUND: The purpose of this study was to determine the prevalence of glenohumeral articular cartilage lesions in patients with rotator cuff tendinopathy and to assess the accuracy of noncontrast magnetic resonance imaging (MRI) in detecting these defects compared with the "gold standard" of arthroscopy. METHODS: Noncontrast MRI studies obtained in 84 consecutive patients undergoing shoulder arthroscopy for rotator cuff tendinopathy (mean age, 54.8 years; range, 17-82 years) were prospectively evaluated for glenohumeral cartilage lesions. Two fellowship-trained, experienced musculoskeletal radiologists were blinded from the arthroscopic findings and independently evaluated the glenoid and humeral head cartilage on 2 separate occasions. RESULTS: At arthroscopy, cartilage lesions of the humeral head were detected in 23 patients (frequency, 27.4%), and glenoid cartilage lesions were found in 20 patients (frequency, 23.8%). For detection of a humeral lesion on MRI, the radiologists' combined accuracy was 78%, sensitivity was 43%, and specificity was 91%. The combined accuracy for detection of glenoid lesions on MRI was 84%, sensitivity was 53%, and specificity was 93%. Combining the readers, low-grade lesions (International Cartilage Repair Society grades 1 and 2) of the glenoid and humerus were read as negative on MRI in 63% and 86% of cases, respectively. CONCLUSION: Overall accuracy of noncontrast MRI for detection of glenohumeral articular cartilage lesions is good; however, interpretation is reader dependent, and accuracy is significantly reduced for detection of low-grade lesions. On the basis of these findings, we recommend that patients with rotator cuff tendinopathy undergoing arthroscopy be informed that the presence and severity of cartilage lesions may be underestimated on MRI.

Ankle sprains are among the most common musculoskeletal injuries and can lead to ankle ligament and cartilage injuries. Imaging plays an important role in differentiating ligament injuries from other causes of ankle pain such as fractures, osteochondral lesions or tendon injuries that helps guide further management. Chronic untreated ankle ligamentous and cartilage injuries can further progress to ankle osteoarthritis, hence the need for timely diagnosis and treatment. Surgical treatment is often required in patients not responding to conservative treatment and ranges from repair and reconstruction procedures for ligament injuries to arthroscopic debridement and repair procedures for cartilage injuries. Cartilage defects and deficiency may be augmented depending on the extent of cartilage loss and associated subchondral changes on MRI. Awareness of operative techniques utilized is essential to interpret imaging findings in postoperative settings.

OBJECTIVE: To quantify the radial and lateral extents of femoral cam lesions in FAI patients relative to the alpha angle and correlate with clinical data. METHODS: Retrospective study of 81 hips with femoral cam morphology that underwent arthroscopic surgery between 2017 and 2019. At each hour over the clockface, the alpha angle (α) (abnormal defined as > 55°), radial extent, and lateral extent of cam lesions were measured on CT. These measurements were correlated with clinical and arthroscopic data. Statistics included independent samples t-test and chi-squared test with Bonferroni correction and multivariate logistic regression. RESULTS: Larger α at 12:00-4:00 in males vs females (56.6-63.4° vs 44.3-58.5°, P < 0.001) and at 2:00-4:00 with elite sports participation vs without (56.7-70.9° vs 49.6-61.1°, P ≤ 0.004). Independent risk factors for radial extent beyond 12:00-3:00 were: male sex (OR 4.82, 95% CI [1.46, 15.85], P = 0.010), BMI > 25 (OR 4.74, 95% CI [1.61, 14.00], P = 0.005), and elite sports participation (OR 3.28, 95% CI [1.09, 9.82], P = 0.034). Lateral extent increased at 1:00-4:00 in males vs females (7.8-18.6 mm vs 1.6-9.1 mm, P < 0.0001). A 16% prevalence of distal cam lesions was found at locations with normal α, resulting in underestimation of radial extent by an average of 1.7 hours. CONCLUSION: There is a positive correlation between the alpha angle, lateral extent, and radial extent of cam lesions. FAI patients who were male, participated in elite level sports, and had a BMI > 25 had larger cam lesions. A larger alpha angle is a risk factor for cartilage damage. Patients may have distal cam lesions at locations with normal alpha angles, though their significance is unknown.

PURPOSE: To compare a 2-view radiograph series (AP of the pelvis and 45° Dunn of the hip) with a 5-view radiograph series for sensitivity in identifying femoral cam morphology. MATERIALS AND METHODS: This is a retrospective review of consecutive patients with a 5-view radiograph series (AP pelvis and AP, 45° Dunn, frog lateral, and false profile of the affected hip) from 2016 to 2017. Three fellowship trained radiologists blindly and independently evaluated 2 views (AP pelvis and Dunn) for a femoral cam lesion, acetabular rim calcification, Tonnis grade, and important incidental findings. Two weeks later, the same assessment was made on all 5 views. A noninferiority test of the 2-view series vs the 5-view series for sensitivity in identifying femoral cam morphology was conducted. Individual reader sensitivity calculations were performed and agreement was determined with the kappa statistic. RESULTS: The 2-view series was noninferior to the 5-view series for cam identification (P value = 0.010). In comparing the 2-view vs 5-view series for individual readers, there was no difference in the sensitivities (84%-100% vs 85%-98%, P = 0.85-1.0) or specificities (11%-56% vs 7%-56%, P = 0.58-1.0) for cam identification. There was fair to excellent 2-view intrareader agreement (k = 0.38-0.93) and similar inter-reader agreement between the 2-view and 5-view (k = 0.33 vs 0.37). CONCLUSIONS: A 2-view radiograph series (AP pelvis and Dunn hip) is noninferior to a 5-view radiograph series for sensitivity in identifying femoral cam morphology.

RESUMO

Complications from diabetic foot infections are a leading cause of nontraumatic lower-extremity amputations. Nearly 85% of these amputations result from an infected foot ulcer. Osteomyelitis is present in approximately 20% of diabetic foot infections. It is imperative that clinicians make quick and successful diagnoses of diabetic foot osteomyelitis (DFO) because a delay in treatment may lead to worsening outcomes. Imaging studies, such as plain films, bone scans, musculoskeletal ultrasound, computerized tomography scans, magnetic resonance imaging, and positron emission tomography scans, aid in the diagnosis. However, there are several mimickers of DFO, which present problems to making a correct diagnosis.
