RESUMO

Efforts to confront the challenges of environmental change and uncertainty include attempts to adaptively manage social-ecological systems. However, critical questions remain about whether adaptive management can lead to sustainable outcomes for both ecosystems and society. Here, we make a contribution to these efforts by presenting a 16-y analysis of ecological outcomes and perceived livelihood impacts from adaptive coral reef management in Papua New Guinea. The adaptive management system we studied was a customary rotational fisheries closure system (akin to fallow agriculture), which helped to increase the biomass of reef fish and make fish less wary (more catchable) relative to openly fished areas. However, over time the amount of fish in openly fished reefs slowly declined. We found that, overall, resource users tended to have positive perceptions about this system, but there were negative perceptions when fishing was being prohibited. We also highlight some of the key traits of this adaptive management system, including 1) strong social cohesion, whereby leaders played a critical role in knowledge exchange; 2) high levels of compliance, which was facilitated via a "carrot-and-stick" approach that publicly rewarded good behavior and punished deviant behavior; and 3) high levels of participation by community actors.

Cystic echinococcosis (CE) is a zoonotic disease caused by a complex of species known as Echinococcus granulosus sensu lato. CE is endemic in Argentina, Chile, Peru, Uruguay and the South part of Brazil. In contrast, little is known regarding the presence of CE in Bolivia. In this study, 35 cysts isolated from livestock (mostly from the Department of La Paz) and 3 from humans (La Paz, Oruro and Potosi) were genetically characterized analysing the sequence of the cox1 gene (1609 bp). In total, 30 cysts (from La Paz, Cochabamba and Beni) were characterized as E. granulosus sensu stricto (3 fertile and 4 non-fertile cysts from sheep, 8 fertile and 12 non-fertile cysts from cattle and 3 fertile cysts from humans). A detailed analysis of the cox1 haplotypes of E. granulosus s.s. is included. Echinococcus ortleppi (G5) was found in 5 fertile cysts from cattle (from La Paz and Cochabamba). Echinococcus intermedius (G7) was identified in 3 fertile cysts from pigs (from Santa Cruz). Additionally, E. granulosus s.s. was detected in 4 dog faecal samples, while E. ortleppi was present in other two dog faecal samples. The implications of these preliminary results in the future implementation of control measures are discussed.

Echinococcus granulosus (sensu lato) is now recognized as an assemblage of cryptic species, which differ considerably in morphology, development, host specificity (including infectivity/pathogenicity for humans) and other aspects. One of these species, E. granulosus sensu stricto (s.s.), is now clearly identified as the principal agent causing cystic echinococcosis in humans. Previous studies of a small section of the cox1 and nadh1 genes identified two variants of E. granulosus s.s. to be present in Australia; however, no further work has been carried out to characterize the microdiversity of the parasite in its territory. We have analysed the sequence of the full length of the cox1 gene (1609 bp) from 37 isolates of E. granulosus from different hosts and geographic regions of Australia. The analysis shows that seven haplotypes of E. granulosus s.s. not previously described were found, together with five haplotypes known to be present in other parts of the world, including the haplotype EG01 which is widespread and present in all endemic regions. These data extend knowledge related to the geographical spread and host range of E. granulosus s.s. in a country such as Australia in which the parasite established around 200 years ago.

Cystic echinococcosis caused by Echinococcus granulosus remains an important and neglected issue in public health. The study of the likely efficacy of the currently available EG95 vaccine against other genotypes of the parasite is important to improve the vaccine as a potential tool to be used in control programmes. The recombinant vaccine EG95-1G1 was developed based on the G1 genotype of E. granulosus. Characterization of the eg95 gene family in the G6 genotype by genomic DNA cloning previously produced the first unequivocal information about the composition of the gene family in a different genotype. The information was used in this study to predict and express two EG95-related proteins from the G6 genotype as recombinants, for assessment of their capacity to bind antibodies raised in sheep vaccinated with the EG95-1G1 vaccine. The proteins (EG95-1G6 and EG95-5G6) from the G6 genotype of E. granulosus were unable to bind all the antibodies raised by sheep vaccinated with EG95-1G1. Differences in the amino acid sequence of EG95-related proteins from G6 and likely the differences in the encoded FnIII domain may be responsible for changes in the conformation of these epitopes.

Echinococcus granulosus sensu lato comprises a number of recognized species which cause cystic echinococcosis (CE) in humans and intermediate hosts. These species have particular geographic distributions, with E. granulosus sensu stricto (genotypes G1/2/3 and micro variants) being most widely spread. In Lithuania, E. intermedius (G7) is known to be the only species circulating between pigs and dogs but is also infecting cattle and humans. In fact, recent reports showed a rise of the incidence to 1.13 human cases/100,000 inhabitants/year. Most of the pigs reared on the backyard farms in Lithuania are slaughtered on site during the cold season (October-April) and are used for own consumption. Therefore, in this study, we examined the impact on taeniid transmission of treating dogs with baits containing an oral formulation of praziquantel every two months during the pig slaughtering season in endemic villages in Lithuania. This study started in November 2006 and ended in January 2011; the first dog treatment was administered in February 2007. The results show that the prevalence of E. intermedius, E. multilocularis and Taenia spp. decreased significantly in treated dogs from the second year of the study when compared to untreated dogs. The treatment of dogs also had an impact on reducing the incidence of CE in fattener pigs from 17.6% (2006-2007) to 3.8% (2008; P < 0.05) and in sows from 26.9% (2006-2007) to 3.6% (2008), and eventually to zero in 2010 (P < 0.05) in fatteners and sows as compared to animals from "control" areas (30.7% and 63.7%, respectively). The results document a significant decrease in the transmission of E. intermedius (G7) after treatment of dogs with praziquantel in a relatively short time on farm level in Lithuania. Taeniid prevalence in dogs remained low in 2017 in the areas where anthelmintic intervention was performed until 2010 and, surprisingly, it was also strongly reduced in control areas. Reduction of taeniid transmission is likely associated with a decrease in the number of dogs in the villages as well as an overall decline in backyard pig farming after 2014 due to the outbreaks of African swine fever in Lithuania.

BACKGROUND AND PURPOSE: As the standard of care for the evaluation of the cervical spine shifts from plain radiographs to multidetector row CT (MDCT), a re-examination of the normal anatomic relationships of the occipitovertebral articulations is needed. We aimed to define the normal anatomic relationships of craniocervical articulations on MDCT and address any discrepancies with currently accepted ranges of normal on plain radiographs. MATERIALS AND METHODS: A total of 200 patients underwent an MDCT scan of the cervical spine with multiplanar reconstructions (MPR). We measured the basion-axial interval (BAI), basion-dens interval (BDI), Powers ratio, atlantodental interval (ADI), and atlanto-occipital interval (AOI) in each patient. After statistical analysis, we compared these values to previously accepted data on plain radiographs. RESULTS: Ninety-five percent of the population was found to have a BDI less than 8.5 mm compared with 12 mm on data from plain radiographs. The Powers ratio demonstrated no significant difference compared with data obtained by plain radiographs. Ninety-five percent of the population was found to have an ADI less than 2 mm, compared with 3 mm previously accepted. The AOI demonstrated 95% of the population ranged between 0.5 mm and 1.4 mm. The BAI was difficult to reproduce on CT images. CONCLUSION: Normal values for the craniocervical relationships on MDCT are significantly different from the accepted ranges of normal on plain radiographs. We propose these values as normal for the adult population.

Alveolar echinococcosis (AE) and cystic echinococcosis (CE) are severe helminthic zoonoses. Echinococcus multilocularis (causative agent of AE) is widely distributed in the northern hemisphere where it is typically maintained in a wild animal cycle including canids as definitive hosts and rodents as intermediate hosts. The species Echinococcus granulosus, Echinococcus ortleppi, Echinococcus canadensis and Echinococcus intermedius are the causative agents of CE with a worldwide distribution and a highly variable human disease burden in the different endemic areas depending upon human behavioural risk factors, the diversity and ecology of animal host assemblages and the genetic diversity within Echinococcus species which differ in their zoonotic potential and pathogenicity. Both AE and CE are regarded as neglected zoonoses, with a higher overall burden of disease for CE due to its global distribution and high regional prevalence, but a higher pathogenicity and case fatality rate for AE, especially in Asia. Over the past two decades, numerous studies have addressed the epidemiology and distribution of these Echinococcus species worldwide, resulting in better-defined boundaries of the endemic areas. This chapter presents the global distribution of Echinococcus species and human AE and CE in maps and summarizes the global data on host assemblages, transmission, prevalence in animal definitive hosts, incidence in people and molecular epidemiology.

With the availability of genetic sequencing data, quantitative reverse transcription PCR (RT-qPCR) is increasingly being used for the quantification of gene transcription across species. Too often there is little regard to the selection of reference genes and the impact that a poor choice has on data interpretation. Indeed, RT-qPCR provides a snapshot of relative gene transcription at a given time-point, and hence is highly dependent on the stability of the transcription of the reference gene(s). Using ovine efferent lymph cells and peripheral blood mono-nuclear cells (PBMCs), the two most frequently used leukocytes in immunological studies, we have compared the stability of transcription of the most commonly used ovine reference genes: YWHAZ, RPL-13A, PGK1, B2M, GAPDH, HPRT, SDHA and ACTB. Using established algorithms for reference gene normalization "geNorm" and "Norm Finder", PGK1, GAPDH and YWHAZ were deemed the most stably transcribed genes for efferent leukocytes and PGK1, YWHAZ and SDHA were optimal in PBMCs. These genes should therefore be considered for accurate and reproducible RT-qPCR data analysis of gene transcription in sheep.

The repellency and insecticidal efficacy of Nopikex, a soap formulation containing 20% diethyl toluamide and 0.5% permethrin, was evaluated against a laboratory colony of phlebotomine sand flies (Lutzomyia longipalpis). The repellency of Nopikex soap was also compared with that of a placebo soap against another species (Lu. youngi) in a forest near Tulua, Valle del Cauca, Colombia. In laboratory trials of the soap, no reduction in repellency was seen 4 hr after application, but within 8 hr, repellency decreased significantly to 67.0% of the initial value (P < 0.05) based on calculations of the coefficient of protection (CP). Under field conditions, the soap gave up to 100% protection immediately after application, but within 4 hours its CP value had decreased to a median value of only 44.3%. The placebo soap was also found to be somewhat repellent when compared with no treatment, giving a median CP value of 67.7 immediately after application. No significant mortality was seen in sand flies within 24 hr of exposure to the soap in the laboratory, even in those that had fed on protected volunteers.

This study examined acute-convalescent changes in diagnostic anti-streptococcal antibodies by the anti-streptolysin O (ASO) and anti-DNAase B (ADAB) tests among patients (n 28) with lymphedema and recurrent erisipela of the lower limb, comparing them with endemic normal control residents (n=25). The study was based in Villa Francisca, an urban focus of Bancroftian filariasis in eastern Santo Domingo, capital of the Dominican Republic. The acute signs and symptoms of erisipela were consistent with a diagnosis of bacterial cellulitis. The ASO test was especially successful at demonstrating a rise in mean titer during convalescence, whereas the ADAB produced about the same frequency of significant increases (0.2 log titer) as did the ASO. When subjects were scored as responders if mounting a minimal titer increase by either test, patients were found more frequently positive than were controls (chi2=5.3, P=0.02). About half (54%) of all patients mounted at least a minimal antibody increase. Filaria-specific IgG4 antibodies were absent from all sera of 20 residents of a nonendemic Dominican mountain town but appeared in about two-thirds of the sampled residents of the endemic barrio. Notably however, levels did not change between the acute phase and convalescence. These findings are consistent with the hypothesis that recurrent streptococcal invasion of the lymphatics may be a significant factor triggering or amplifying lymphedema and elephantiasis in patients with chronic filariasis.

A study was conducted during 1996-1997 in 20 villages of Tumaco, Colombia, to evaluate the effectiveness of personal protective measures against cutaneous leishmaniasis (CL). The intervention was effective, but the high costs of the preventive measures and the lack of a more holistic approach hampered the intervention's sustainability. This paper analyzes the results using an ecosystem approach to human health. Using this approach, we found that CL has been present in the study area for a long time and affects farmers and those living closest to the forest. The forest constitutes the habitat for insect vectors (sandflies) and parasite reservoirs (wild mammals). Four spatial scales were identified in this ecosystem: residential, village, regional, and global. From the ecosystem perspective, three interventions are proposed to prevent CL in the 20 villages: improve housing construction, organize village housing in clusters, and make diagnosis and treatment of CL more accessible. The design and implementation of these interventions require active involvement by people with the disease (village inhabitants) and decision-makers (local authorities).

BACKGROUND AND PURPOSE: Analysis of the prevertebral soft tissue (PVST) is helpful in detecting osseous and ligamentous injuries of the cervical spine. Because the standard of care has shifted from radiographs to multidetector CT (MDCT), a re-examination of the PVST on MDCT images is needed to establish normal values for thickness appropriate for this imaging technique. MATERIALS AND METHODS: Thickness of the PVST was measured in 192 patients undergoing screening cervical spine MDCT with multiplanar reconstructions as part of a trauma protocol. Patients included in the study were not intubated, had an immobilized cervical spine, had normal findings on cervical spine CT, and did not have a diagnosis of osseous or soft-tissue cervical injury. Exclusion criteria included patients with a congenital or acquired (nondegenerative) abnormality of the cervical spine or PVST. RESULTS: The upper limits of normal for the thickness of the PVST were 8.5 mm at C1, 6 mm at C2, 7 mm at C3, 18 mm at C6, and 18 mm at C7. The upper limit of normal was not determined for C4 and C5 levels due to variable position of the esophagus and larynx. The smallest variability and calculated SDs were found at C2 and C3. CONCLUSIONS: The thickness of the PVST is important in the detection of underlying injuries to the cervical spine. We propose the obtained values as the upper limits of normal for PVST thickness on MDCT images in the adult population.

A survey of cigarette smoking in Santo Domingo was conducted among residents of 499 households in two neighborhoods. Residents over 14 years old were classified as current smokers (34.6%), former smokers (11.3%), and never smokers (54.1%). Most of the 200 current smokers interviewed (83%) had started smoking by age 20, but over half (59%) were relatively light smokers, consuming < or = 10 cigarettes per day. Remarkably for Latin America, smoking was nearly as common among women (32.9%) as among men (36.3%). Over 90% of the interview subjects disapproved of smoking by adults, the practice being considered undesirable by most because of its effects on health. Most of the adolescents interviewed viewed smoking as a mark of maturity (82.3%) and as "in" among their friends (53.7%). Even though few respondents understood the specific nature of health threats posed by smoking, health was the single most important factor motivating cessation. Most of the current smokers (87.0%) wanted to quit, and over two-thirds (67.5%) reported making at least one serious attempt to do so. Tobacco industry figures show that between 1962 and 1988 the total apparent cigarette consumption of the Dominican Republic rose 3.7-fold. Most of that increase was the result of promotion by Philip Morris, which now dominates the Dominican market. At present cigarettes are heavily advertised without health warnings and are sold to minors without significant restrictions. Such activities should be of particular concern in this and other developing countries with high fertility rates and young populations.

RESUMO

The purpose of this study was to examine the accidents and incidents that take place in the workplace and their relationship with the occupational health and safety climate. The sample for this study was chosen by convenience and was composed of 154 professional nurses. The Occupational Health and Safety Climate Scale. (Roduguez et al., 2000) was used for the research. The results of the study reveal that the variables that contribute to understanding the determinants of the occupational health and safety climate are the individual's relationships with peers and supervisors, the number of trainings in the area of occupational health and safety and the number of accidents that the individual has experienced.
