RESUMO

Veterans transitioning from the military to civilian life may encounter difficulties in different domains of functioning. Most research in this area comes from the US and Israel, with Veterans in Northern Ireland (NI) in the United Kingdom, remaining an understudied population. This qualitative study aimed to examine the nature of transition experiences of NI Veterans by analyzing responses (N = 252) to an open-ended question related to the transition process, in a self-report survey. Thematic analysis highlighted both positive and negative experiences across high-level themes. These were related to (1a) how good the military life had been, (1b) the transition had been easy for some Veterans, and (1c) the skills gained in the military have been valuable; (2) it was hard to adjust to civilian life/still adjusting; (3) negative employment experiences; (4) lack of trust; (5) transitioning is hard in NI; and (6) inadequate support, post-service. The findings highlight that NI Veterans share some of the same challenges as other Veterans; however, the challenges in NI are compounded by ongoing security concerns and political tensions, which means living under the radar is a reality for many, making finding meaningful work and community integration difficult. The findings indicate that preparation for civilian life and the acculturation process needs to start many months before discharge. Perhaps more crucially, regiments should work closely with and support civilian employers to equip them to recognize and value the skills ex-Services Veterans can offer, and find a good fit for their skills within their organizations.

RESUMO

BACKGROUND: In the UK, smoking prevalence in people with depression (34%) and anxiety (29%) is more than double that of the general population (13%). People who stop smoking improve their mental health with comparable effect sizes found for antidepressants. In England, online psychological therapy is a standard treatment for depression and anxiety. Online therapy is an acceptable setting for smoking cessation support; however, integrated smoking and mental health support is not available. This novel study aims to assess the acceptability and feasibility of an online smoking cessation intervention, and trial procedures, offered alongside online mental health treatment as it offers increased reach to people with common mental health difficulties who smoke. METHODS: A two-armed; Intervention (Integrated SilverCloud smoking cessation support) and control group (SilverCloud usual care), pragmatic, randomised controlled feasibility trial. We aim to recruit 500 adult smokers eligible for online mental health treatment. Follow-up will be conducted at 3-months and 6-months. We will assess the acceptability and feasibility of the trial procedures (i.e., recruitment, data completeness, self-reported acceptability and satisfaction) and the intervention (i.e., self-reported quit attempt, engagement with the smoking cessation and mental health programs, smoking cessation medicine and e-cigarette use, self-reported acceptability and satisfaction) and pilot clinical outcomes (i.e., biologically validated smoking abstinence, anxiety, depression, quality of health). CONCLUSION: If the Trial is successful, a randomised controlled effectiveness trial will follow to examine whether integrated smoking cessation and mental health treatment increases smoking abstinence and improves depression and anxiety compared to usual care. TRIAL REGISTRATION: ISRCTN10612149 (https://doi.org/10.1186/ISRCTN10612149), 02/02/2023.

RESUMO

OBJECTIVE: The primary and secondary impacts from the COVID-19 pandemic are claimed to have had a detrimental impact on health professional retention within the UK National Health Service (NHS). This study set out to identify priorities for intervention by scaling the relative importance of widely cited push (leave) influences. DESIGN: During Summer/Autumn 2021, a UK-wide opportunity sample (n=1958) of NHS health professionals completed an online paired-comparisons exercise to determine the relative salience of work-related stress, workload intensity, time pressure, staffing levels, working hours, work-homelife balance, recognition of effort and pay as reasons why health professionals leave NHS employment. SETTING: The study is believed to be the first large-scale systematic assessment of factors driving staff exits from the NHS since the COVID-19 pandemic. RESULTS: All professions gave primacy to work-related stress, workload intensity and staffing levels. Pay was typically located around the midpoint of the respective scales; recognition of effort and working hours were ranked lowest. However, differences were apparent in the rank order and relative weighting of push variables between health professions and care delivery functions. Ambulance paramedics present as an outlier, notably with respect to staffing level (F-stat 4.47, p=0.004) and the primacy of work-homelife balance. Relative to staffing level, other push variables exert a stronger influence on paramedics than nurses or doctors (f 4.29, p=0.006). CONCLUSION: Findings are relevant to future NHS health professional retention intervention strategy. Excepting paramedics/ambulance services, rankings of leave variables across the different health professional families and organisation types exhibit strong alignment at the ordinal level. However, demographic differences in the weightings and rankings, ascribed to push factors by professional family and organisation type, suggests that, in addition to signposting universal (all-staff) priorities for intervention, bespoke solutions for different professions and functions may be needed.

RESUMO

Intellectual humility, which entails openness to other views and a willingness to listen and engage with them, is crucial for facilitating civil dialogue and progress in debate between opposing sides. In the present research, we tested whether intellectual humility can be reliably detected in discourse and experimentally increased by a prior self-affirmation task. Three hundred and three participants took part in 116 audio- and video-recorded group discussions. Blind to condition, linguists coded participants' discourse to create an intellectual humility score. As expected, the self-affirmation task increased the coded intellectual humility, as well as participants' self-rated prosocial affect (e.g. empathy). Unexpectedly, the effect on prosocial affect did not mediate the link between experimental condition and intellectual humility in debate. Self-reported intellectual humility and other personality variables were uncorrelated with expert-coded intellectual humility. Implications of these findings for understanding the social psychological mechanisms underpinning intellectual humility are considered.

RESUMO

Despite significant investment to increase recycling facilities and kerbside collection of waste materials, plastic packaging is frequently discarded as litter, resulting in significant environmental harm. This research uses qualitative methods to explore the contextual and psychological factors that influence plastic waste disposal behaviour from the perspectives of consumers. This research also reports key results from a brief online survey exploring consumer perspectives toward plastics and plastic recycling. A total of N = 18 adults living in Northern Ireland (NI) participated in a semi-structured interview and N = 756 adults living in NI took part in an online survey. Interview data was analysed via a semi-directed content analysis approach, using the COM-B behaviour change model as a guiding framework. Survey data underwent descriptive and frequency analysis. Collectively, the findings suggest that environmental concern exists among consumers generally, but there is a degree of ambivalence toward recycling that reflects a gap between intentions to recycle and actual recycling behaviour. Plastic recycling behaviour is hindered by three common barriers: 1. confusion and uncertainty about which plastic materials can be recycled (exacerbated by the abundance of plastic products available) 2. perceiving plastic recycling to be less of a personal priority in daily life 3. perceiving that local government and manufacturers have a responsibility to make plastic recycling easier. As recycling is simply not a priority for many individuals, efforts should instead be placed on providing greater scaffolding to make the process of recycling less tedious, confusing, and more habitual. Visual cues on product packing and recycling resources can address ambiguity about which plastic materials can/cannot be recycled and increasing opportunities to recycle (via consistent availability of recycling bins) can reduce the physical burden of accessing recycling resources. Such interventions, based on environmental restructuring and enablement, may increase motivations to recycle by reducing the cognitive and physical burden of recycling, supporting healthier recycling habits.

RESUMO

Because of a perceived decline in staff morale, the UK National Health Service has begun to routinely assess the extent to which commitment to the National Health Service may aid staff retention. While a number of studies have investigated the role of employee commitment in relation to staff turnover, no research to date has empirically tested if staff commitment to the NHS could protect job satisfaction from the effects of high job demands, and if this varies according to age. Using latent variable path analysis, this novel study examined this question among a national sample of Healthcare Professionals Allied to Medicine in the National Health Service. The results indicate that the negative effects of high job demands on job satisfaction were fully mediated by commitment to the National Health Service, but age mattered. Among the over 45s and over 55s, commitment to the National Health Service acted as an effective buffer against the negative effects of job demands on job satisfaction, but this effect was not as strong among the 35-44 age group. The broader policy implications of these findings are that age sensitive policies to support NHS workforce retention are needed. Also, pro-social institutions who employ Healthcare Professionals Allied to Medicine should develop policies for inspiring commitment to that institution, as it could help them with the demands of the job, and may even encourage more skilled workers to work longer.

RESUMO

Once a nurse author writes a quality manuscript, the article is submitted to an appropriate journal, reviewed by the editor, and sent to review panel members with expertise consistent with the topic of the article or the methodology of research articles. After review, panel members recommend the article be published, accepted for publication after revision, revised without promise of publication, or rejected. Nurse authors have options as to how they handle the recommendation. To successfully publish in a peer-reviewed journal, nurse authors should communicate with the editor and realize that the editor and author have the same goals.

RESUMO

Writing for publication is a responsibility of all a profession's members; however, nurses may not have had the opportunity to learn the process of writing and publishing manuscripts in professional journals. The process of writing for publication begins with an idea that is focused on and slanted toward a particular audience. Nurse authors must then create space in their fast-paced day for writing in an environment that supports their concentration and creativity. Several strategies can be used, including reading well-written manuscripts, to assist nurses in writing quality manuscripts that are likely to be published.

RESUMO

OBJECTIVES: To explore and portray the perspectives of National Health Service Ambulance personnel related to the latest rise in the National Health Service occupational pension age. METHODS: Data gathering took the form of 35 in-depth interviews. A thematic analysis was used to characterise and articulate key concepts and meanings. The analysis applied interpretive techniques, as views expressed were from personal experiences, and allowed for an in-depth analysis of shared meanings. RESULTS: The themes reported captured the desire of many Ambulance personnel to exit their employment well in advance of their retirement age, despite satisfaction gained from patient care. This early exit is being driven by increased worry that the work demands of the job are unsustainable, especially for older workers, as clinical responsibilities increase and their social support diminishes. Also, Ambulance personnel feel betrayed by their employers, because their retirement is being delayed further by another change in their pensionable age. CONCLUSION: There is an increased orientation for 'living for today' and indications of a willingness to sacrifice salary and pension income in order to protect their health in older age, which has implications for long-term financial and general well-being in retirement.

RESUMO

OBJECTIVE: Prevention of diabetes demonstrated in the diabetes prevention program has not been thoroughly evaluated in workplace settings. METHODS: Glycemic control was assessed by glycated hemoglobin (HbA1c) and its impact on 5-year incidence of diabetes and prediabetes prevalence, in wellness program participants (WPP) of (Group A, with) or (Group B, without) health coaching and monetary incentives. RESULTS: HbA1c fell in Group A WPP (5.52 ±â€Š0.60 vs 5.58 ±â€Š0.36 SD, P = 0.04) as did prediabetes. Incident diabetes was less than expected (24 vs 74, P < 0.001). HbA1c increased in Group B WPP (5.37 ±â€Š0.40 SD vs 5.58 ±â€Š0.54, P < 0.001) and prediabetes increased from 28 to 36 (P < 0.001). CONCLUSIONS: Health coaching and monetary incentives improved glycemic control over 5 years, an improvement not observed in WPP without these interventions.

RESUMO

Occupational health nurses work in a variety of settings and some have chosen to practice the profession as consultants. The process of consulting is similar to that of other disciplines but only two small descriptive studies offer functions and roles specific to occupational health nurse consultants. This article provides an overview of consulting practice, the process, the functions, and the roles of occupational health nurse consultants, and presents some common characteristics of successful consultants.

RESUMO

Qualifications such as "global warming hysteria" and "energy policy schizophrenia" put forward by some climate change skeptics, usually outside the academic arena, may suggest that people who seriously worry about the environment suffer from psychological imbalance. The present study aimed to refute this thesis. While habitual worrying in general is strongly associated with psychopathological symptoms, in a survey a near-zero correlation was found between habitual ecological worrying and pathological worry. Instead, habitual ecological worrying was associated with pro-environmental attitudes and behaviors, and with a personality structure characterized by imagination and an appreciation for new ideas. The study had sufficient statistical power and measures were valid and reliable. The results confirm that those who habitually worry about the ecology are not only lacking in any psychopathology, but demonstrate a constructive and adaptive response to a serious problem. In the public domain, these findings may contribute to a more rational and less emotional debate on climate change and to the prevention of stigmatization of people who are genuinely concerned about our habitat and are prepared to do something about it ("habitual worriers are not crazy"). In the academic arena this study may contribute to environmental psychology ("habitual worrying is part of a green identity"), as well as to the literature on worry and anxiety ("habitual worrying can be a constructive response").

RESUMO

Many students struggle with statistical concepts such as interaction. In an experimental group, participants took a paper-and-pencil test and then were given training to establish equivalent classes containing four different statistical interactions. All participants formed the equivalence classes and showed maintenance when probes contained novel negative exemplars. Thereafter, participants took a second paper-and-pencil test. Participants in the control group received two versions of the paper-and-pencil test without equivalence-based instruction. All participants in the experimental group showed increased paper-and-pencil test scores after forming the interaction-indicative equivalence classes. Class-indicative responding also generalized to novel exemplars and the novel question format used in the paper-and-pencil test. Test scores did not change with repetition for control group participants. Implications for behavioral diagnostics and teaching technology are discussed.
