RESUMO

Methods of flap and combined plastics in dorsal adduction cicatricial contracture of the thumb are discussed. Different variants of plastics are substantiated on the basis of study of the anatomical features of the contracture. The formation of a commissure by a trapeziform flap of skin and fatty tissue of the palmar layer and palm is the foundation of the operation. Depending on the degree of deficiency of the cicatricial layer of the fold in the first interdigital space, the wounds to the sides of the main flap are closed with local tissues or by free dermatoplasty. The operative methods were approved in more than 200 patients and good functional and cosmetic results were produced in all of them.

RESUMO

Defects in the soft tissues of the lower extremities after burns and traumas in 10 patients were managed by plastic surgery using ++cutaneo-fascial grafts formed from the leg of the same or contralateral side. The operation was performed in two or three stages. ++Cutaneo-fascial grafts formed on the posteromedial or posterolateral surface of the leg may have a width:length ratio of 1:3. In cross plastics the proximal one third of the graft is folded like a tube. Inclusion of the vena saphena magna in the graft and measures aimed at prevention of separation of the fascia from the layer of fat increase the viability of the graft. Secondary deformity is significantly reduced by modelling the edges of the donor wound and closing it with an unperforated graft of split skin.

RESUMO

The authors sum up their experience with the surgical treatment of postburn deformations of the dorsal surface of the hand in 50 patients. They describe the technique of radical dissection of all the scars of the dorsal surface of the hand and fingers and plasty of the interdigital commissures by palmar trapezoid whole thickness grafts. Wounds after excision of the scars were closed by nonperforated grafts of 0.3-0.4 mm thick splitted skin. Long-term results were followed up in 21 patients. All of them showed good functional and cosmetic results.

RESUMO

A method for elimination of the flexion cicatrical contracture of fingers used in 46 patients (109 fingers) is described. The operations included plasty of the flexion surface of the fingers by contrary trapezoid flaps cut from the palmar and lateral surfaces of the finger, displacement of the flaps towards each other and their suturing. In 43 patients (103 fingers) flexion cicatrical contractures have been completely eliminated. Long-term results were followed in 17 patients. No recidivations were noted.

RESUMO

Most optimal methods of plasty with the maximum use of intact skin of a healthy leaf and adjacent area as a trapezoid cellulo-cutaneous flap are proposed. Different variants of the operation are proposed as one-, two-, three- and many-flaps plasty with local tissues and one- and two-flaps combined plasty. The choice is dependent on the localization and degree of contracture. Plasty by the methods proposed allows to adequately eliminate marginal contractures of large vessels, syndactylia, microstomia and narrowing of the lid slit.
