RESUMO

Objective: To present the first laparoscopsic ureteroneotsistostomies we have performed by the Boari methods - operative technique and results. Materials and Methods: We investigated 9 patients for a period of two years -from 2015 to 2016 year, 9 had had previous surgery, in 2 of them followed by radiotherapy (one man and one woman). The average age of the patients was 45 (29-61) years. Women in the study were 7, men - 2. We performed 9 laparoscopic operating procedures following the open operative technique of Boari. Results: All patients we have operated and who have no previous radiotherapy had excellent results - no hydronephrosis, recovering the kidney parenchyma and absence of reflux. In two patients who had undergone radiotherapy we had to perform an open procedure because of severe adhesions. Conclusions: The experience we have gained with open operative technique and the development of laparoscopic techniques allow us to look for more - often and less invasive methods of surgery. We have best results when indications for operative technique are strictly followed.

RESUMO

Phlegmon of the serotum is a severe form of suppurative inflammatory process, which causes are extremely various. For the 2012 - 2015 year period at the department of urology we had five cases of phlegmon of the scrotum, where incision was performed as well as debridement and drainage and the wound was closed with the available tissues of the scrotum. Aggressive debridement of all necrotic tissues and all fascial planes that were separating easily during the blunt dissection were excised until well vascularized tissue was reached. The wounds were left wide opened until the appearance of healthy granulation tissue and then assessment of their secondary closing. Tissues were sent for histological and microbiological assessment.

RESUMO

PURPOSE: To share our experience about partial resection (nephron sparing) in localized (T1aN0M0) renal hypernephroid cancer as organ preserved operation. Opportunities and longterm results. MATERIAL AND METHOD: For last 10 years we have done 89 partial resections in patients with localized renal hypernehroid cancer. In all cases we have done lymphatic dissection of relevant lymph region of the affected side. RESULTS AND DISCUSSING: All operated patients had five years of survival. We observed developing contralateral hypernefroid carcinoma in one patient that necessary performing a partial resection in this place. In two patients we performed a partial resection of a solitarykidney. The results of surgery are very good when we stage properly cancer process that allows organ preservation.

RESUMO

AIM: To introduce our experience, opportunities, complications and conclusions with our colegues about first twelve cases of laparoscopic transperitonealis radical nephrectomy. MATERIAL AND METHODS: For the period from 16.04.08 to 16.09.08 we made twelve laparoscopic transperitoneal radical nephrectomies. Seven males and five females age 48 to 81 have been operated. RESULTS: In two of these cases we made revision for bleeding. The other cases were without complications. CONCLUSION: The laparoscopic transperitoneal radical nephrectomy is the most perfect technic for the moment--because of the low bleeding and short postoperative period in the hospital it is a good method of choice.

RESUMO

PURPOSE: To perform our 5-years experience in 40 patienets with orthotopic bladder substitution in Studer's technic follow radical cystectomy. MATERIALS AND METHODS: We created orthotopic neobladder using segment of terminal ileum in Studer's fashion in 40 patients. To decrease peristaltic and respectively bowel pressure, isolated segment was opened longitudinal and crossed sutured. This way we created a low pressure urinary reservoir. RESULTS: All patients were completely continent. Bladder capacity was 400-700 ml. At 4 years followup the patients didn't require correction in their therapy for metabolic disorder. Eight of the patients undergone this procedure died in 3-50 monts postoperatively for pelvic recurrence. CONCLUSION: Studer's orthotopic neobladder is an exellent alternative for patients undergone radical cystectomy. Thechnical ability decreases the operating time and surgical complications significantly.

RESUMO

After radical cystoprostatovesikulectomy, an ileal bladder substitute had been performed in 14 male patients à modo Studer. In order to avoid peristalsis with high pressure peaks resulting in incontinence, the antimesenteric border was transsected, and an ileal low-pressure pouch was created. One to two months after operation, the continence for urine was good or excellent. After a follow-up time of 12 months, no metabolic disorders requirling any substitution, nor pyelonephritic changes were observed.
